Anesthetic considerations for a patient with myasthenia gravis undergoing deep sedation in an outpatient oral surgery setting

  • Myasthenia gravis (MG) is a neuromuscular autoimmune disorder which clinically presents as muscular weakness and fatigue due to autoantibody formation against acetylcholine receptors (AChR), leading to their subsequent destruction. Due to the neuromuscular implications of MG, certain considerations must be taken into account when providing anesthesia to MG patients. In the following case report, we have outlined procedural considerations for the anesthetic management of a patient with MG undergoing deep sedation for an elective oral surgery in an outpatient setting, as well as a discussion of relevant literature.

  • Benign monomelic amyotrophy (BMA) is a benign motor neuron disease in which amyotrophic change is confined to either the upper or the lower extremities. Numerous cases of BMA have been reported from Japan and India. However, only a few cases have been reported from other regions, including South Korea. Here we report a rare case of late-onset BMA in Korean male using conventional diagnostic approach with magnetic resonance imaging and electromyography. The patient received ten sessions of manual therapy, which focused on strengthening of the left ankle. At two-month follow up, weakness was still isolated to the patient's left ankle. There were no signs of disease progression.

  • Multifidus cervicis plane block has been effectively used to provide analgesia during and after cervical spine surgery, but not for any other purpose. Here, we report three cases of chronic axial neck pain unresponsive to medical treatment. We performed multifidus cervicis plane block bilaterally, which lowered numerical rating scale (NRS) score within 2 weeks. Thus, multifidus cervicis plane block is one of the novel options that may be used for chronic axial neck pain.

  • Bovine spastic paresis (BSP) is a neuromuscular disorder characterized by hypertension and stiffness of hindlimb. Two Korean native cattle (Hanwoo) calves developed BSP or BSP-like symptoms, and a tenotomy of superficial tendon of medial head and deep tendon of lateral head of gastrocnemius muscle was performed for treatment. A cast was applied postoperatively to prevent muscle rupture and was removed three weeks later. The prognosis was evaluated at 3 weeks, 6 and 18 months postoperatively. Neither calf showed any other postoperative sequelae. This is the first case study to report the diagnosis, treatment, and prognosis of BSP in Hanwoo.

  • Sugammadex has shown faster reversal of steroidal neuromuscular blockade (NMB) than neostigmine, a traditional reversal agent for NMB, even in the intense block phase. This efficiency is possible because of the unique mechanism of action by encapsulating the NMB molecules. Therefore, with the use of sugammadex, we can also expect to avoid direct interactions with the cholinergic system and its subsequent side effects, which are disadvantages of traditional drugs. However, despite these benefits and US Food and Drug Administration (FDA) approval in 2015, rare adverse events associated with sugammadex have been reported. Herein, we report a case of bronchospasm that developed immediately after sugammadex administration.

  • Delayed onset muscle soreness (DOMS) is a common problem that can interfere with rehabilitation as well as activities of daily living. The purpose of this study was to determine the effects of both transcutaneous electrical nerve stimulation (TENS) and microcurrent electrical neuromuscular stimulation (MENS) on DOMS, Twenty-seven untrained and male volunteer subjects were randomly assigned to one of three treatment groups: 1) a group that received TENS (7 Hz), 2) MENS (60 ${\mu}A$, .3 pps) or 3) a control group that received no treatment. Subjects performed repeated eccentric exercise of the non-dominant forearm flexor muscle with submaximal intensity by the simply designed eccentric exercise devices. Treatments were applied after 24 hours and 48 hours. Subjects attended on two consecutive days for treatment and measurement of elbow flexion, extension, resting angle (universal goniometer), and pain (visual analogue scale: VAS) on a daily basis. Measurements were taken after treatment. Analysis of results were as follows; 1) There were no significant differences between TENS and MENS by one-way repeated ANOVA, 2) The t-test for pain, resting, flexion and extension angle revealed significant differences within TENS group, 3) The t-test for resting angle revealed significant differences within MENS group.

고유수용성신경근촉진법이 뇌졸중 환자의 연하기능에 미치는 효과 (The Effect of Proprioceptive Neuromuscular Facilitation(PNF) on Swallowing Function of the Stroke Patients)

  • 본 연구는 뇌졸중 환자를 대상으로 고유수용성신경근촉진법이 연하기능에 미치는 효과를 알아보고자 하였다. 이를 위해 뇌졸중 환자 중 실험군 9명, 대조군 9명을 대상으로 주 3회 30분~50분씩 총 12회 동안 고유수용성신경근 촉진법을 이용한 프로그램으로 중재하였다. 연구 결과 첫째, 연하곤란 정도의 변화는 고유수용성신경근촉진법 중재 후에 실험군의 연하곤란 척도 점수가 감소하여 유의한 차이가 있었다. 둘째, 구강 인두기능 점수는 고유수용성신경근 촉진법 중재 후에 실험군에서 유의한 차이를 나타내었다. 셋째, 연하속도의 변화는 실험군에서 중재 후에 1회 연하시간, 총소요시간이 감소하여 유의한 차이를 보였다. 따라서 고유수용성신경근촉진법이 뇌졸중 환자의 연하기능을 향상시키는데 효과가 있는 것으로 나타났으며, 앞으로도 다양한 중재가 결합된 운동프로그램을 개발하여 뇌졸중 환자의 연하기능 향상에 기여할 수 있는 연구들이 진행되어져야 할 것으로 사료된다.

Relationship between Myosin Isoforms and Meat Quality Traits in Pig Semitendinosus Neuromuscular Compartments

  • The aim was to determine the relationship between muscle structure and meat quality traits in neuromuscular compartments (NMCs: R1, R2, R3, R4) of pig semitendinosus muscle. Barrows from the INTA-MGC genetic line (Argentina) were slaughtered at 100 kg body weight. In each NMC the following parameters were determined: the fibre types I, IIA, IIX and IIB by immunohistochemistry, the fibre cross sectional area (FCSA), the pH of meat after 24 h post-mortem ($pH_{24}$), instrumental meat tenderness (WB) and colour ($L^*$, $a^*$, $b^*$). There were significant differences in the following: $L^*$ (R1 = R4$a^*$ (R1>R4>R2 = R3), $b^*$ (R1 = R4R1 = R3 = R4), $pH_{24}$ (R1 = R4>R2 = R3). The relative percentages of FCSA were as follows: I (R4>R1>R3>R2), IIA (R1>R4>R3>R2), IIX (R1 = R2 = R3 = R4) and IIB (R2>R3>R1>R4). The correlation values were statistically significant between IIB and WB (R1 and R4, $r_s$ = 0.66), (R2 and R3 $r_s$ = 0.74), IIB and $L^*$ (R1 and R4 $r_s$ = 0.84), IIX and $L^*$ without discriminating NMCs. Our data suggest that the NMC where the sampling takes place is important for determining meat quality traits because of the heterogeneity of the whole muscle.
