Long-term Chemotherapy with Lomustine Plus Hydroxyurea for Choroid Plexus Papilloma in a Schnauzer Dog

  • A 10-year-old male Schnauzer dog presented with a 1-week history of ataxia, right-sided hemiparesis, and right-sided head tilt. On the basis of magnetic resonance imaging (MRI) with neurological examination and cerebrospinal fluid analysis results, a primary brain tumor was suspected. Therapy with imatinib mesylate plus hydroxyurea for 7 weeks was not effective and clinical signs worsened. Chemotherapy was then changed to lomustine plus hydroxyurea. Although the existing clinical signs continued, they did not deteriorate. No change in mass size was observed in subsequent MRI. The subject suddenly died from dyspnea 388 days after initial presentation. In this case, choroid plexus papilloma was definitively diagnosed based on histopathological findings.

Lomustine Plus Hydroxyurea Chemotherapy for Primary Intramedullary Spinal Cord Tumor in a Maltese Dog

  • A 7-year-old, male Maltese dog with a body weight of 2.8 kg was presented with a history of hind limbs ataxia that progressed to tetraparesis over a one-month period. Based on physical and neurological examinations, tetraparesis with concomitant UMN signs, kyphosis and severe neck pain were identified. On MRI scan, we tentatively diagnosed this patient as a primary intramedullary spinal cord tumor. Therapy with lomustine plus hydroxyurea and prednisolone was initiated and the clinical signs rapidly improved. The patient was regularly checked by MRI scan and the range of the mass was gradually reduced to complete remission for 11 months. About 19 months after treatment, the patient showed anemia and hematochezia which suspected as adverse effects of chemotherapy. The condition was getting worse over 2 months and the patient suddenly expired 657 days after initial presentation. On histopathological examination, the spinal cord sample was identified as a neuronal atrophy without evidence of tumor cell.

Neoadjuvant Chemotherapy and Subsequent Adjuvant Chemotherapy with Hydroxyurea after Craniotomy in a Cat with a Meningioma

  • An 11-year-old neutered male domestic short-haired cat presented with neurological symptoms that developed over a three-month period. These included mental dullness, vocalization, ataxia, and visual impairment. The patient was diagnosed with a primary intracranial tumor at a local animal hospital. After the first diagnosis, the cat was administered hydroxyurea, prednisolone, omeprazole, and gabapentin for 3 months. After the initiation of medical treatment, the patient's clinical symptoms did not improve and the size of the tumor was static on the second magnetic resonance imaging (MRI). The dosage of hydroxyurea and prednisolone was increased for two weeks. The patient's clinical signs improved, and subsequently, a craniotomy was performed. The clinical signs completely resolved six days after surgery. Adjuvant chemotherapy with hydroxyurea was continuously administered after the craniotomy. The patient demonstrated a good clinical status during the nine-month follow-up period. Neoadjuvant chemotherapy has not yet been reported for meningiomas in cats. Further clinical trials with longer follow-up periods and larger patient cohorts will be required to confirm the effectiveness of neoadjuvant chemotherapy with hydroxyurea in feline meningioma.

Feline Infectious Peritonitis associated Neuropathy in a Cat (고양이에서 발생한 고양이전염성복막염에 의한 신경병증 증례)

  • A 8-month-old, spayed female, Domestic shorthair cat lived in a shelter was presented with pelvic limbs ataxia and dysuria. Serum biochemical profile abnormalities were hyperproteinemia and decreased albumin/globulin (A:G) ratio (0.70). Results of cerebrospinal fluid (CSF) analysis were mixed cells pleocytosis with predominance neutrophils and an increase in protein concentration. In addition, feline coronavirus was detected by realtime RT-PCR in CSF. Magnetic resonance imaging (MRI) findings revealed lesions of the lumbar spinal cord. Based on clinical signs, MR finding, CSF analysis and realtime RT-PCR result in CSF, this case was diagnosed as feline infectious peritonitis (FIP) associated meningomyelitis. Although prednisolone and mycophenolate mofetil were administrated, clinical signs were not resolved and progressed to tetraplegia and coma status. This case presentation describes that feline infectious peritonitis virus could affect the lumbar spinal cord only and cause meningomyelitis with pelvic limbs ataxia without other neurological signs.

Combined Cytosine Arabinoside and Prednisolone Therapy for Meningoencephalitis of Unknown Etiology in 2 Dogs (원인불명의 수막뇌염을 보인 개에서 Cytosine Arabinoside와 Prednisolone으로 치료한 2례)

  • A Maltese (5-years old, castrated male) and a Yorkshire Terrier (8-years old, intact female) were referred to Seoul National University Hospital for Animals with ataxia, circling, depression and anorexia. It was suspected to meningoencephalitis of unknown etiology (MUE) based on neurological signs, magnetic resonance imaging findings, cerebrospinal fluid analysis and canine distemper virus antigen test. Immunosuppressive dose of corticosteroid was initially chosen to treat in two dogs. However, the clinical signs were relapsed and side effects had arisen including polyuria, polydipsia, calcinosis cutis, liver enzyme elevation, weight gain and abdominal distension. Combination therapy of cytosine arabinoside (CA) and prednisolone (PDS) was tried. With this combination, the clinical signs were resolved and the side effects were alleviated. The dose of PDS was tapered to 0.5 mg/kg q48h in one case and the PDS was discontinued in the other case. These cases suggested that combined CA and PDS therapy may be significantly useful for the treatment of MUE uncontrolled by PDS alone.

Relationship Between Neurologic Soft Signs and Neuroleptic Treatment in Patients with Schizophrenia (정신분열증에서의 연성 신경학적 증상과 항정신병 약물 치료의 관련성)

  • This study was performed to examine the role of neuroleptics may in the development of neurologic soft signs in patients with schizophrenia. Neurologic soft signs were evaluated in 28 neuroleptic naive patients with schizophrenia or schizophreniform disorder and 31 neuroleptic non-naive patients with schizophrenia using a structured tool for measuring neurologic abnormalities, Neurological Evaluation Scale-Korean version(NES-K). Relationship to dose, duration and neurological side effects of neuroleptic treatment were also evaluated. Total scores of NES-K in neuroleptic naive group were significantly higher than those of non-naive group. Scores of motor coordination, sequencing of complex motor acts and others items in functional subcategories were also significantly higher in drug-naive patients. The sensory integration item was not different between two groups. After controlling covariates such ac dose of neuroleptics, age and sex, total scores, motor coordination and others items of NES-K were significantly higher in neuroleptic naive group. However there was no difference between drug naive and non-naive group in the sequencing of complex motor acts item due to effects of these covariates. In neuroleptic non-naive group the dosage of neuroleptics correlated with the motor coordination item, nor were there relationships between duration and side effects of neuroleptic treatment and neurologic soft signs. These findings suggest that neuroleptic treatment may play a only relative role in the development of neurologic soft signs in patients with schizophrenia and these abnormalities may be one of possible trait markers of schizophrenia. To elucidate this opinion, well-controlled, prospective study in same subjects will be helpful.

Idiopathic Vestibular Disease in a Beagle Dog: Clinical Findings and Outcome (비글 견에서 발생한 특발성 전정계 질병 증례: 임상적 발견과 결과)

  • A 10-year-old, intact female Beagle dog was presented for examination of acute onset of right-sided head tilt and moderate ataxia. Clinical signs were acutely presented 12 days ago and had been progressively alleviated until the admission day. The dog was bright, alert, and responsive. On neurological examinations, mild head tilt to the right and mild ataxia were noted, thus vestibular disorders were suspected. Central vestibular disease was ruled out based on the clinical signs, magnetic resonance imaging(MRI), and cerebrospinal fluid(CSF) analysis. Otoscopic examination showed that the tympanic membranes were intact and normal in appearance. On radiographic and MR examinations, abnormalities were not found in the tympanic bulla and the petrous portion of the temporal bone. Hypothyroidism was ruled out by thyroid-stimulation hormone(TSH) stimulation test. Clinical signs were completely disappeared at 2 weeks after discharge without any therapy. Thus, the dog was definitively diagnosed as idiopathic vestibular disease based on the clinical signs, excluding other causes of vestibular dysfunction, and the alleviation of clinical signs with time.

Changes of the Extracellular Concentrations of Striatal Dopamine and Its Metabolites by MK-801 in Morphine-Dependent Rats (MK-801 투여에 의한 몰핀의존성랫드 뇌선초체중 도파민신경절달물질의 변화)

  • The roles of dopamine(DA) and N-methyl-D-aspartate(NMDA) system in the development and expression of morphine dependence were investigated by monitoring the concentrations of extracellular DA and its metabolites by in vivo microdialysis and simultaneous observation of behavioral changes in morphine dependent rats. Extracellular DA level in caudate putamen of morphine-dependent rat was decreased and the concentrations of its metabolites, dihydroxy phenylacetic acid(DOPAC) and homovanillic acid(HVA), were increased during naloxone-precipitated withdrawal. DA contents were recovered to normal levels by pretreatment of MK-801, a noncompetitive NMDA receptor antagonist, which may explain the mechanism of diminishing effect of MK-801 on withdrawal symptoms in morphine-dependent rats. MK-801(0.3 mg/tg, i.p.) induced the untoward hamful neurological signs such as ataxia and severe rotations, which may be produced by hyperactivation of dopaminergic system. These results suggest that MK-801 may inhibit the expression of mophine dependence by altering the dopamine release.

Intrahepatic portosystemic shunt with a second degree atrioventricular block fixed by transvenous coil embolization in a dog

  • A 2-year-old female Pekingese dog was presented with primary complaints including exercise intolerance and neurological sign associated with hepatic encephalopathy. The major findings in clinical examination included an intermittent seizure, a slow heart rate with pulse deficit, leukocytosis and anemia in hemogram, elevated pre- and post-prandial serum bile acid and hepatic enzymes, hypoproteinemia, coagulopathy, ammonium urate crystaluria and bilirubinuria. Diagnostic tests revealed an intrahepatic portosystemic shunt complicated with a second degree atrioventricular block and QT prolongation. The case was successfully treated with a transvenous coil embolization. Clinical signs were gradually improved and cardiac bradyarrhythmia disappeared. This case is a rare case of intrahepatic portosystemic shunts complicated with a cardiac bradyarrhythmia in a small breed dog fixed by a transvenous coil embolization.

High Signal Intensities on T1-Weighted MRI as a Biomarker of Manganese

  • Increased signal in T1-weighted images was observed in the experimental manganese (Mn) poisoning of the non-human primate and a patient with Mn neurointoxication. However, our study showed that the increased signals in magnetic resonance images (MRI) were highly prevalent (41.6%) in Mn-exposed workers. Blood Mn concentration correlated with pallidal index. These changes in MRI tend to disappear following the withdrawal from the source of Mn accumulation, despite permanent neurological damage. Thus increased signal intensities on a T1-weighted image reflect exposure to Mn, but not necessarily manganism. Our study also showed that the concentration of Mn required to produce increased signal intensities on MRI is much lower than the threshold necessary to result in overt clinical signs of manganism. Increased signal intensities in the globus pallidus were determined by manganese accumulation in the animal experiment. Reanalysis of the previous data with the structural equation model revealed that pallidal index (Pl) on MRI reflects target organ dose of occupational Mn exposure
