One Case of paralytic strabismus(oculomotor nerve palsy) which was treated electroacupuncture at oculomotor muscles (외안근 전침요법을 이용한 소아의 마비성 사시 치험 1례)

  • Objectives : Strabismus refers to the oculomotor muscles imbalance that results from improper alignment of the visual axes of the two eyes. It may be divided into paralytic and nonparalytic strabismus. Paralytic strabismus is primarily result from neurologic problem and nonparalytic strabismus is more strictly opthalmologic problem. At first we used this method(electroacupuncture at oculomotor muscles) and gained good recoveries for adult paralytic strabismus cases or Miller-Fisher syndrome, and I tried to know the effective and safety for children cases. Methods : We treated the case by using the electroacupuncture at paralytic oculomator muscles. The case was treated almost daily and every treatment was enforced 10 minutes. We use the PG-306 electra-acupuncture products(Suzki Iryoki Co. Japan) and apply the low consequence wave of I-8Hz Results : We found that the case was recovered perfectly in short term and appealed any sides effect or compliant. So I try to apply this treatment in many children cases and make the treating protocol for them.

  • The plant Korean rosebay are members of the Ericaceae(Heath) family and may contain grayanotoxins, which exert toxic effects by binding to sodium channels in cell membranes. We had experience of 2 cases of Korean rose bay intoxication, who exposed to Rhododendron mucronulatum. Patients were presented to our ED with nausea and vomiting, hypotension, and dizziness or depressed mentation. The patients were performed monitoring and supportive treatment and were fully recovered within 48hours. Korean rose bay intoxication should be considered in patients with clinical evidence of gastrointestinal irritation, cardiac arrhythmias, and/or neurologic signs without identifiable causes, especially in spring or early summer.

  • A schwannoma is a benign soft tissue tumor arising from the nerve sheath of a Schwann cell. Clinically, a schwannoma is an asymptomatic mass rarely causing neurologic deficits. However, it can cause discomfort as well as motor and sensory disturbances by compressing the nerve of its origin. The authors encountered a huge schwannoma arising from the median nerve at the proximal forearm, which caused symptoms mainly in the ulnar nerve. The tingling sensation along the ulnar nerve disappeared completely after enucleation of the schwannoma originating from the median nerve.

  • A 4-year-old Yorkshire terrier was presented with hindlimb paresis and urinary incontinence after accidental ingestion of an herbicide. Based on neurologic examinations, decreased hindlimb proprioception with flaccid paresis were revealed. Other possible causes of the clinical signs were excluded. The clinical signs gradually improved after administration of anti-inflammatory and antioxidant therapy. This case report is the first to describe the long-term outcome of hindlimb paresis and urinary incontinence induced by glyphosate surfactant herbicide (GPSH) poisoning in a dog.

  • Primary canine lung tumor is very uncommon and account for only about 1% of all cancers diagnosed [1]. The average age at diagnosis is about 10 years and no breed or sex predilection has been confirmed. Almost all primary cancers of the lungs are carcinomas, the most common being adenocrcinoma. This study describes primary lung tumor in a dog that is accompanied by cerebellar metastasis with related neurologic signs. (omitted)

  • A 50 year old man with acute aortic dissection DeBakey type I, involving right coronary artery and aortic valve, underwent replacement of the ascending aorta and aorto-right coronary bypass grafting. The operative findings showed a large transverse intimal tear was at about 4cm above the aortic valve. The dissection extended out into the proximal right coronary artery. And we found that the right coronary artery originated from the left sinus of Valsalva, run transversally in the aortic wall, with partial rupture. Postoperatively he had no ischemic cardiac symptoms and neurologic complications. He was discharged on postoperative 9th day with good result.

  • Subarachnoid-pleural fistula after routine thoracotomy is a rare complication but a very serious problem. Twenty one cases have been reported in the literature. We report a care of subarchnoid-pleural fistula that dveloped after the esecation of posterior mediastinal neurogenic tumor. The patient presented with large amount of clear pleural fluid with mild headache and dizziness. Surgical intervention following a trial of conservative therapy was undertaken because we strongly suspected subarachnoid-pleural fistula. A dural tear was found at the level of resected intercostal nerve root. The dura was closed by way of direct suture and fibrin glue. In this case, the recognition of subarachnoid-pleural fistula formation is difficult because the patient had not presented any neurologic deficit.

  • Enterovirus commonly causes neurologic diseases (aseptic meningitis, encephalitis, etc.), hand-foot-mouth disease, herpangina, and acute hemorrhagic conjunctivitis. However, it rarely causes pneumonia in immunocompetent adults. In Korea, no case has been reported about pneumonia caused by enterovirus in healthy adults. We can cite the case of a 20-year-old woman who presented severe community-acquired pneumonia caused by enterovirus. The diagnosis was based on reverse transcriptase polymerase chain reaction (RT-PCR) of a respiratory specimen.

  • Acute disseminated encephalomyelitis (ADEM) and acute hemorrhagic leukoencephalitis (Hurst's disease) are rare autoimmune demyelinating disorders, which show a monophasic illness with preceding infection. We report a 42-year-old woman presented with multiphasic and progressive neurologic deterioration without definite evidences of infection. She developed hypesthetic ataxia, followed by ipsilateral weakness after a weak, and finally encephalopathy after a month. In contrast to the first MRI showing a small longitudinal lesion, the next images revealed massive bilateral frontal lesions with hemorrhagic necrosis and biopsy unveiled inflammatory demyelination.

Bilateral Femoral Neuropathy Combined with Multifocal Abscesses (다발성 농양과 동반한 양측성 대퇴신경 마비)

  • We report a 29-year old female who developed bilateral femoral neuropathy combined with multiple abscesses in both thigh muscles. She was present with weakness in both lower extremities for 15 days and intermittent chilling sense for 3 months. Nerve conduction study showed complete absence of compound muscle action potentials in bilateral femoral nerves when stimulated at inguinal area. Electromyographic examination revealed no motor unit action potentials in both rectus femoris and vastus medialis muscles. CT revealed multifocal abscesses in bilateral thigh muscles. After antibiotic treatment, the patient's neurologic symptoms were improved.
