Clinical Report of two TGA cases (일과성 전기억상실증으로 진단된 환자 치험 2례)

  • Transient global amnesia(TGA) is a clinical syndrome characterized by sudden, temporary dysfunction of anterograde and recent retrograde memory without other neurologic deficits. Different hypotheses have been considered for its etiology, but it still remains obscure. Recently the diffusion-weighted magnetic resonance imaging(DWI), has been considered as the sensitive tool to detect small punctate hyperintense lesions in the hippocampus in transient global amnesia(TGA). We report two TGA cases, all of them answers to TGA clinical criteria, and one of them showed two dot like high signal intense foci in Rt. hippocampus on DWI.

  • Traumatic blunt aortic injury especially on proximal ascending aorta is a rare injury with a few reports. Generally emergency surgical management was performed. In this case, however, in multiple trauma with brain injury, emergency surgical management of aortic injury might result in unexpected secondary injury of the brain. Herein, we report a case of a 33-year-old man who was driving a truck was injured in a head-on collision. Evaluation revealed a pseudoaneurysm on his ascending aorta concomitant with epidural hemorrhage. He was treated by surgical management of his ascending aorta after 3 days from accident. There were no postoperative and neurologic complications and the patient was discharged after 18 days.

Spinal Lymphoma in a Cat (고양이에서 척수림프종의 발생례)

  • A 3.5kg, 3-year-old castrated male Japanese domestic cat withpelvic limb ataxia and dysuria was referred to the Veterinary Medical Center of the Tokyo University. On the neurologic examination findings, both pelivic had a LMN paresis. The cat was FeLV positive and FIV negative. Radiographic findings did not identify the spinal lesions. In magnetic resonance images(MRI) of the lumbar spinal cord, the cat had the lesions in the lumbar(L) 1 and L3, characterized by hyperintensity on a transverse T2-weighted and T1-weighted images, and contrast enhancement was evident. The mass removed by dorsal laminectomy. Histopathological examination of the mass revealed spinal lymphoma. Three weeks after the surgery, the cat administered chemotherapy protocol for lymphoma by current protocol. Two weeks after chemotherapy, the cat had a metastasis to bone marrow and died.

Single Stage Replacement of Entire Thoracic Aorta for Chronic Aortic Dissection (만성 대동맥 박리 환자에서 전 흉부 대동맥 동시 치환)

  • Operations for extensive aortic aneurysm are generally performed as staged operations with or without elephant trunk technique. However, we must consider single stage replacement in cases that are unsuitable for elephant trunk technique. We report a case of successful sing1e stage replacement of the entire thoracic aorta from the aortic valve to the level of diaphragm. The patient was a 35-year-old male who had Marfanoid features and had previously undergone replacement of the ascending aorta for aortic dissection. He recovered without neurologic complication and was discharged on 29th day after the operation. .

Spinal Intradural Extramedullary Cavernoma Presenting with Intracranial Superficial Hemosiderosis

  • A case of intradural extramedullary cavernous angioma is presented with headache, dizziness, and bilateral sensorineural hearing loss caused by an intracranial superficial hemosiderosis. It was incidentally found in a patient with a 3-month history of sustained headache, dizziness and a 3-year history of hearing difficulty. The neurological examination was unremarkable in the lower extremity. MR images showed an intracranial superficial hemosiderosis mostly in the cerebellar region. Myelography and MR images of the thoracolumbar spine revealed an intradural extramedullary mass, which was pathologically proven to be a cavernous angioma. T12 total laminoplastic laminotomy and total tumor removal were performed without any neurologic deficits. The patient's symptoms, including headache and dizziness, have been absent for three years. Intradural extramedullary cavernous angioma can present with an intracranial superficial hemosiderosis as a result of chronic subarachnoid hemorrhage.

Surgical Management of Coronary Artery Disease Combined with Carotid Artery Stenosis -A Report of Two Cases- (경동맥 협착을 동반한 관상동맥 질환의 수술적 치료 -2례 보고-)

  • The optimal surgical approach to the patients with coronary artery disease combined with carotid artery stenosis is controversial. We report two cases of successful surgical management of the patients with combined obstructive coronary and carotid artery disease. The first case was a 69-year-old female who had unstable angina pectoris and a past medical history of left carotid endarterectomy. She was revealed to have triple vessel coronary disease and nearly total occlusion of right internal carotid artery. She was undergone staged right carotid endarterectomy 10 days before coronary bypass surgery. The second case, a 54-year-old male with a past medical history of left hemiparesis and dysarthria, was admitted due to unstable angina pectoris. He was revealed to have triple-vessel coronary disease and more than 90% stenosis of left internal carotid artery and 50% stenosis of right internal carotid artery. In the latter case, a combined coronary bypass surgery and left carotid endarterectomy was done. In both cases, postoperative neurologic complications were not observed.

Repair of intracardiac defect under simple deep hypothermia in infancy without cardiopulmonary bypass: report of 3 cases (단순 초저온법에 의한 개심술: 3례 보)

  • Although the conventional methods of cardiopulmonary bypass for open heart surgery have been employed, it has been usual method to repair of congenital heart disease in infancy using deep hypother-mia and circulatory arrest technique. In 1980, we reported total correction of congenital heart disease using surface induced hypothermia-total circulatory arrest and rewarming with limited cardiopulmonary bypass. in 1981, three patients below 10 kilogram, who had ASD and PDA, and two of VSD with pulmonary hypertension were operated on using simple deep hypothermia without cardiopulmonary bypass. During surface cooling, there were no ventricular fibrillation and arrhythmia. There were no difficulties to resuscitate the heart. Postoperative respiratory and neurologic complication were not occurred. Follow up examination for two to three years gave no evidence of cerebral damage due to circulatory arrest.

Understanding of Rett Syndrome (레트 증후군의 이해)

  • Purpose : The purpose of this study is understanding of Rett Syndrome. Rett Syndrome is a common developmental - neurologic disorder that has been reported almost exclusively in female. Recently mutations in the gene encoding X-linked methyl-CpG binding protein 2 (MECP2) have been identified as the cause of Rett syndrome. Consistent with the diagnostic criteria, hand skills, verbal or non - verbal communication skills and common motor skills were lost during regression. Regression most commonly occurred between 12 and 18 months of age. Methods : This is a literature study with books, articles, web site for Rett syndrome international association. Results : There is a continuing need to further elucidate the pre- and post - regression features of Rett syndrome. Rett syndrome need to physical therapy, musical therapy, special education and medical interventions. Conclusion : There has not been therapeutic method to the root of Rett syndrome but our goal is relaxation of symptom and physical therapist's study of Rett syndrome.

  • Hemolysis, elevated liver enzymes and low platelets (HELLP) syndrome, a variant presentation of severe preeclampsia/eclampsia is associated with high maternal morbidity and mortality. Despite several advantages to the use of epidural analgesia for the management of labor pain in preeclamptic parturients, this procedure is relatively contraindicated in the presence of severe thrombocytopenia. Determining the pain management of choice depends on the parturient's condition, fetal well-being, and the urgency of the situation. We report a safe case of epidural analgesia in a HELLP syndrome parturient with severe thrombocytopenia for labor pain management without any neurologic complications or epidural hematoma.
