소형견 15예의 흉요추추간판질환에 대한 보존치료 (The Effect of Conservative Therapy on Thoracolumbar Intervertebral Disc Disease on 15 dogs)

  • Fifteen dogs with clinical signs of paraparesis, paraplegia and urinary dysfunction were referred to Veterinary Medical leaching Hospital, College of Veterinary Medicine, Konkuk University. According to physical and neurologic examination, survey radiograph and myelography, these patients were diagnosed as thoracolumbar intervertebral disc disease. All of them were treated with medical (prednisolone or carprofen, antibiotics), acupuncture (Ji Zhong, Bai Hui, Zhong Shu, Pang Guang Shu, Zu San Li, Huan Tiao) and physical therapy (hydrotherapy, thermotherapy, massage, manipulation, swimming). Of twelve patients were recovered of neurologic deficits within 21 days after the onset of treatment. However, three dogs were not resolved, which were found to have other spinal cord disease concomitantly. All of patients showed normal urination after treatment. Until 21 months after treatment, there were no recurrence in twelve patients. This study indicated that the conservative therapy could be effectively managed in paraparesis or paraplegia and urinary dysfunction which caused by intervertebral disc disease.

FCST의 음양균형장치를 활용한 후천성 사경증 증례보고 (A Torticollis Case Managed by Balance Appliance of FCST for the Meridian and Neurologic Balance)

  • Objectives : Therapeutic effect of Balance Appliance of functional cerebrospinal therapy (FCST) for meridian and neurologic yinyang balance was observed in a refractory torticollis case. Methods : A post-traumatic severe torticollis case with 18 years of duration was managed by the Balance Appliance on temporomandibular joint (TMJ), combined with acupuncture and manual medicine. Results : Assessment was made by self assessment of subjective symptoms and clinical observacon. The patient reported over-90% remission and returned to ordinary daily life after 20months of therapy, which effect was reported to maintain for 9 months. Conclusions : An impressive effect was observed and further clinical and biological research on FCST is expected.

  • The diagnosis of peripheral neuropathy had been performed by electrophysiologic studies and neurologic examination. However, Magnetic Resonance Imaging (MRI) has recently been proposed as a supplementary tool for its diagnosis. A 55-year-old woman presented with back pain and painful proximal weakness of the right leg. Neurologic examination and electrophysiologic studies suggested an upper lumbar plexopathy. MRI disclosed the signal change in lumbar plexus with the atrophy of the innervating muscles. We report a patient with idiopathic lumbar plexopathy confirmed by MRI.

  • Acute event in cerebrovascular disease is the second most common cause of death in Korea following cancer, and it can also cause serious neurologic deficits. Understanding of perfusion status is important for clinical applications in management of patients with cerebrovascular diseases, and then the attacks of ischemic neurologic symptoms and the risk of acute events can be reduced. Therefore, the normal vascular anatomy of brain, various clinical applications of acetazolamide-enhanced brain perfusion SPECT, including meaning and role of assessment of vascular reserve in carotid stenosis before procedure, in pediatric Moyamoya disease before and after operation, in prediction of development of hyperperfusion syndrome before procedure, and in prediction of vasospasm and of prognosis in subarachnoid hemorrahge were reviewed in this paper.

  • 무수 알코올로 내장신경차단 완료후 양측 서혜부 상하의 지각 탈실, 이상감감이 합병되었다, 차단 6일 후부터 지속적 경막외차단을 시작하여 점차 이상감각 및 통증은 조절되었으며 지각탈실 부위도 점차 축소되었다.

Newly Developed Urinary Retention and Motor Weakness of Lower Extremities in a Postherpetic Neuralgia Patient

  • During the early stage of postherpetic neuralgia, an epidural block on the affected segment is helpful in controlling pain and preventing progression to a chronic state. The main neurologic complication following an epidural block is cord compression symptom due to an epidural hematoma. When neurologic complications arise from an epidural block for the treatment of postherpetic neuralgia, it is important to determine whether the complications are due to the procedure or due to the herpes zoster itself. We report a case of a patient who was diagnosed with herpes zoster myelitis during treatment for postherpetic neuralgia. The patient complained of motor weakness in the lower extremities after receiving a thoracic epidural block six times. Although initially, we believed that the complications were due to the epidural block, it was ultimately determined to be from the herpes zoster myelitis.

  • Hand-Foot-Mouth disease, which has a various enanthem-exanthem complex at the tongue, buccal mucosa, hands and feets and buttock area with febrile illness, is usually caused by Coxscakie virus type A(16). Generally, this disease shows self limited course and good prognosis without neurologic manifestations. However, enterovirus 71, which was newly discovered and reported in 1974, can cause the striking features of Hand-Foot-Mouth disease outbreaks and has neuropathogenic potentials of polio-like paralytic illness including aseptic meningitis, meningoencephalitis and respiratory disease. We experienced a case of Hand-Foot-Mouth disease with polyradiculitis manifestations, and a case of Hand-Foot-Mouth disease with meningoencephalitis. Therfore, we report these cases with brief review of related literatures.

Intradural Lumbar Disc Herniation

  • Intradural lumbar disc herniation[ILDH] is a rare pathology. The pathogenesis of ILDH is not known with certainty. Adhesions between the ventral wall of the dura and the posterior longitudinal ligament[PLL] could act as a preconditioning factor. Diagnosis of ILDH is difficult and seldom suspected preoperatively. Prompt surgery is necessary because the neurologic prognosis appears to be closely related to preoperative duration of neurologic symptoms. Despite preoperatively significant neurological deficits, the prognosis following surgery is relatively good. We report on case of ILDH at L3/4 with differential diagnoses, and the possible pathogenic factors are discussed.

  • The generation of human induced pluripotent stem cells (iPSCs) from somatic cells using gene transfer opens new areas for precision medicine with personalized cell therapy and encourages the discovery of essential platforms for targeted drug development. iPSCs retain the genome of the donor, may regenerate indefinitely, and undergo differentiation into virtually any cell type of interest using a range of published protocols. There has been enormous interest among researchers regarding the application of iPSC technology to regenerative medicine and human disease modeling, in particular, modeling of neurologic diseases using patient-specific iPSCs. For instance, Parkinson's disease, Alzheimer's disease, and spinal cord injuries may be treated with iPSC therapy or replacement tissues obtained from iPSCs. In this review, we discuss the work so far on generation and characterization of iPSCs and focus on recent advances in the use of human iPSCs in clinical setting.

Clinical and Radiologic Characteristics of Caudal Regression Syndrome in a 3-Year-Old Boy: Lessons from Overlooked Plain Radiographs

  • Caudal regression syndrome (CRS) is a rare neural tube defect that affects the terminal spinal segment, manifesting as neurological deficits and structural anomalies in the lower body. We report a case of a 31-month-old boy presenting with constipation who had long been considered to have functional constipation but was finally confirmed to have CRS. Small, flat buttocks with bilateral buttock dimples and a short intergluteal cleft were identified on close examination. Plain radiographs of the abdomen, retrospectively reviewed, revealed the absence of the distal sacrum and the coccyx. During the 5-year follow-up period, we could find his long-term clinical course showing bowel and bladder dysfunction without progressive neurologic deficits. We present this case to highlight the fact that a precise physical examination, along with a close evaluation of plain radiographs encompassing the sacrum, is necessary with a strong suspicion of spinal dysraphism when confronting a child with chronic constipation despite the absence of neurologic deficits or gross structural anomalies.
