Use of Neurologic Assessment Performance and Education Needs of Neurologic Ward Nurses (신경계 간호사의 신경학적 검진 수행정도와 교육요구도에 대한 조사연구)

  • Purpose: The purpose of this study was to describe performance and education needs related to neurologic assessment by neurologic ward nurses. Methods: The study was conducted with a cross-sectional descriptive design. Participants, 297 neurologic ward nurses working in ten major hospitals in Seoul and Gyeonggi were surveyed from September 7 to October 1, 2015. Results: The mean score for performance of neurologic assessments was 3.04 points. Performances performed with high quality were orientation exam, mental status documentation, and muscle strength of extremities in that order. Performances performed with low quality were assessment of accessory nerves, hypoglossal nerves, and cerebellum function in that order. There were significant differences according to whether there was ward education programs on neurologic assessment and teaching experiences within a year. These variables significantly affected the quality of neurologic assessment performance. The need for neurologic assessment training was 3.14 points. Conclusion: Amount of performance and demand for education on neurologic assessment, which are required in the clinical setting, were significantly high. To perform neurologic assessments with high quality, development of education program for neurologic assessment and continuous education courses are required at either the department or neurological nursing organization level.

Comparative Study of Risk Factors for Ischemic Stroke in Koreans : Multi-center Study (한국인의 허혈성 뇌졸중의 위험요인에 대한 비교고찰 - 다기관 연구)

  • Objective : This study is about stroke risk factors which may differ between countries. Methods : We recruited 1725 ischemic stroke patients from multiple-centers. We analyzed stroke risk factors: hypertension, diabetes mellitus, dyslipidemia, depression, cardiac source, smoking, waist-to hip ratio, alcohol intake, physical activity, and psychosocial stress. We then compared risk factor results from other countries (the INTERSTROKE study) with this study. Results : 10 risk factors are related to stroke incidence. They were somewhat different between this study and the INTERSTROKE study. Conclusions : Hypertension is the most important risk factor of stroke, and other 9 risk factors also should be managed to prevent stroke incidence. Frequency of risk factors were different from the INTERSTROKE study. I think it could be partly due to differences of ethnicity and age. More studies are needed to describe characteristics of risk factors of Koreans.

  • Objectives : This study was conducted to assess the effect of Uhwangchungsimwon on the autonomic nervous system. Methods : Eligible subjects were enrolled from healthy people in their 20s-30s. They were divided into two groups, the UC group (n=22) and the control group (n=20). We monitored ECG from 14:00 to 18:00 in both groups. In the UC group, subjects were administered UC at 15:00. HRV analysis was conducted at 14:00, 16:00, and 17:00 for 512 seconds. Results : The degree of LF/HF ratio change was significantly higher in the UC group at 1 hour after administration. Conclusion : We suggest Uhwangchungsimwon may be useful for stabilization of the autonomic nervous system by activating parasympathetic nerve in healthy people.

  • Object : The purpose of this study is to observe the correlation between change in pattern identification symptoms and scandinavian stroke scale in ischemic stroke patient who had receive traditional Korean medical treatment and conservative treatment. Methods : 43 subjects were recruited from patients with ischemic stroke within 30 days of onset. We chose the subjects who had at least one follow up session and had checked the score between last follow up session and first session in pattern identification and scandinavian stroke scale. We also assessed the correlation between pattern identification and scandinavian stroke scale. Results : There were significant negative correlation between pattern identification and scandinavian stroke scale in Fire-heat pattern and positive correlation in Dampness-phlegm pattern. Conclusions : This study provides evidence that collaborative treatment maybe effective in improving neurologic symptoms in ischemic stroke patients diagnosed as Fire-heat pattern. Further studies with larger scale and longer observation period, more neurologicscales scales, control group would be required.

  • Patients with cerebellar infarction often complain of many symptom-vertigo, gait disturbance, nausea, vomiting. Especially, Vertigo is the main symptom, and patients cannot achieve normal activity in daily life. When we prescribed Banhabakchulchunma-tang and Gastrodiae Rhizoma extract to a 61year-old female stroke patient with vertigo, we could observe that the symptom was improved rapidly after treatment. Therefore we suggest that Banhabakchulchunma-tang which increase Chunma in quantity is very useful for vertigo after stroke. Further case studies of herbal treatment of this ailment are needed.

  • When we prescribed Shiryungtang to a 65 years-old male patient with ARF after stroke, we could observe that his symptoms, I/O balance and laboratory findings were all improved. Therefore, we suggest that shiryungtang might be useful for ARF in the patient with stroke.

  • The object of this study was to examine the relationship between arteriosclerosis and oriental medical diagnosis of the patients who had suffered ischemic stroke. The degree of arteriosclerosis of the patients was assessed by cardio-ankle vascular index(CAVI), and diagnostic criteria used for all patients were deficiency of ki, deficiency of blood, blood stasis and stagnation of water. One hundred four patients were included and their general characteristics, CAVI and oriental diagnosis were evaluated. Results showed a significantly strong correlation between CAVI and blood stasis. These results suggest a close relationship between arterioslcerosis and blood static syndrome as defined by oriental medicine. If so, diagnosis of blood static syndrome is a reilable predictor of arteriosclerosis.

  • Objectives: Uwhangchungsim-won (DC) has been used in various medical fields such as stroke, hypertension, atherosclerosis, autonomic imbalance and mental instability, etc. The aim of this study was to evaluate the effect of UC on cerebral hemodynamics and estimate the appropriate dose of UC. Methods: We studied changes of hyperventilation-induced cerebrovascular reactivity and mean blood flow velocity of middle cerebral arteries (MCA) using transcranial Doppler. We observed the changes of mean blood pressure, pulse rate and expiratory CO2 using S/5 Compact Anesthesia Monitor from 10 healthy young volunteers who were administered UC twice a day in the 1 st section and then once a day in the 2nd section. Results: Mean blood pressure tended to decrease at 1 hour and pulse rate tended to decrease at 2 hours after second administration. After 2 hours, mean blood pressure rose to state before administration, but pulse rate maintained from 2 hours to 4 hours. The changes were not statistically significant. Cerebral blood flow velocity in middle cerebral artery was not statistically significant after second administration. Cerebrovascular reactivity increased from 2 hours to 4 hours after second administration. Conclusions: This study provides that administration of UC twice a day is more effective on hyperventilation-induced cerebrovascular reactivity than administration of UC once a day.

  • Object : The purpose of this study was to evaluate the relationship between pattern identification (PI) and stroke risk factors, such as hypertension, diabetes mellitus, dyslipidemia, stroke history, obesity, abdominal obesity and metabolic syndrome. Methods : 46 patients with acute ischemic stroke were recruited from May 2012 to November, 2012. We analyzed the data of 32 patients, and pattern identification was identified by resident and specialist of Korean medicine. We analized patient's PI and risk factor by Fisher's exact test. Results : We found that Dampness-phlegm group was more related with patient's metabolic syndrome than non Dampness-Phlegm group. And Yin deficiency group had less relationship with patient's metabolic syndrome, obesity, abdominal obesity and dyslipidemia than non Yin deficiency group. Conclusions : According to the analysis, these results provide evidence for relationship between the Dampness-phlegm group, Yin deficiency and metabolic syndrome.

  • This study was aimed to evaluate the effectiveness of moxibustion on dysphagia after stroke. We conducted his study in the Department of Cardiovascular and Neurologic Diseases (Stroke Center), Hospital of Oriental Medicine, Kyung Hee Medical Center, from May 1, 2005 to August 1, 2006. After recruiting dysphagia patients after stroke, we applied the Direct Moxibustion ($1.2\;{\times}\;1.4\;cm$) to the Chon-Jung acupoint 5 times, and assessed its effect on swallowing reflex by Swallowing Provocation Test (SPT) before and 30 min after treatment. Effectiveness of the swallowing reflex was assessed by the Latency Time of Swallowing Reflex (LTSR) which was checked from test starting to the onset of swallowing reflex. Thirty four subjects with dysphagia were treated with moxibustion. The mean of the latency time in swallowing reflex was shortened from $2.2\;{\pm}\;0.9$ to $1.5\;{\pm}\;0.5$ seconds after moxibustion, which had statistical significance (P < 0.001). We suggest that moxibustion at Chon-Jung could be useful for dysphagia after stroke. Further study with a randomized controlled trial compared to placebo is needed to confirm this suggestion.
