복합부위통증증후군 환자에서 척수자극술과 교감신경 고주파열응고술 -증례 보고- (Thoracic Spinal Cord Stimulation and Radiofrequency Thermocoagulation of Lumbar Sympathetic Ganglion in a Patient with Complex Regional Pain Syndrome in the Lower Extremity -A case report-)

  • Herein is described the successful treatment of complex regional pain syndrome type II with the combination treatment of spinal cord stimulation and radiofrequency thermocoagulation of the lumbar sympathetic ganglion. A 62 years old male patient, suffering from CRPS type II in his left lower extremity, visited our pain clinic. Medication and nerve blockade produced only slight improvement in his symptoms and signs. Therefore, a linear type spinal cord simulator was inserted into the thoracic epidural space, using a non-surgical percutaneous approach, with the cephalad lead located at the T11 level. Two months later, the repositioning of the electrode to the T12 level for more effective pain control, with radiofrequency thermocoagulation of lumbar sympathetic ganglion also performed at the left L2 and 3 levels for the control of trophic change. These resulted in significant pain relief and decreased trophic change, with no complications, after which the patient was able to resume a normal life.

  • 경락에서의 침구효과 원리는 침 자극이 인체의 저하된 기능상태로부터 각성상태, 응급상태, 국부적 쇼크 상태로 만들어줌으로써 인체의 전반적 생리기능이 새로운 평형을 이룰 수 있도록 하는데 있다. 그러나, 서양의학에서는 침구치료효과를 시술자의 생체에너지(기) 전달에 의한 효과보다는 단순한 침자극에 의한 신경-내분비-면역 계통의 작용과 반응으로 간주하고있다. 따라서 침구치료의 효과가 침 자극에 의한 경락의 작용임을 확인하고, 시술방법에 따른 전위 변화를 분석하기 위해서 경혈과 비경혈, 절연자침과 비절연자침, 보법과 사법으로 자극했을 때의 전위변화를 측정한 결과, 각각 다른 반응이 나타났다. 이는 동일 경락에서 침구의 작용효과는 단순한 침 자극에 의한 효과뿐만 아니라, 시술자의 생체에너지 전달에 의해 복합적으로 반응할 수 있음을 의미한다. 임상실험을 분석한 결과, 시술방법에 따른 경락에서의 전위 변화는 해당 경락의 허실과 해당장기의 건강상태에 따라 다르게 나타나고 있어서, 경락의 허실상태를 진단할 수 있는 진단 파라메터로서의 유의성을 확인하였다.

Caudal Neuromodulation with the Transforaminal Sacral Electrode (InterStim): Experience in a Pain Center Regarding 12 Implants

  • Background: Sacral nerve stimulation is a therapeutic option with demonstrated efficacy for conditions presenting with perineal pain caused by different etiologies. We aimed to assess whether a sacral electrode ($Interstim^{(R)}$, Medtronic, Minneapolis, MN, USA) inserted through the caudal pathway is able to offer an acceptable level of sacral stimulation and rate of catheter migration. Methods: We present 12 patients with pelvic pain who received sacral neuromodulation via the sacral hiatus with the InterStim electrode. We evaluated patient satisfaction as well as migration and removal of the electrode, if necessary. Results: Our experience included 12 patients, 10 women and two men, with a mean age of 60 years. In eight of the 12 patients, the initial therapy was effective, and the final system implantation was performed. During subsequent follow-up, patient satisfaction was good. To date, there have been no cases of electrode displacement or migration. Conclusions: The caudal insertion of the InterStim electrode, with its own fixation system, and initially designed for transsacral insertion, appears in our experience to be a satisfactory option which can minimize electrode displacements, achieving similar results in therapeutic efficacy and causing no difficulties in removal.

인체에 적용한 초음파 및 침전극 저주파자극이 콜린에스테라제의 활성에 미치는 영향 (Effect of stimulus in the Acupuncture point with Ultrasound and Electro-acupuncture on human's Cholinesterase activity)

  • The purpose of this study was to investigate the Effect of stimulus in the Acupuncture point with Ultrasound and Electro-acupuncture on human's Cholinesterase activity. Pain is physiological symptoms in human body. Pain stimulation transference is according to Acetylcholine that is a matter of nerve stimulation. Cholinesterase analyze the Acetylcholine and Cholin acetylase compose the Acetylcholine. To identify of Cholinesterase activity, this study attempted to Electroacupuncture and Ultrasound for 20 person. Twenty subjects participating in this consisted of A group(difference of Cholinesterase's figure make use of Electroacupuncture with Zi-Shil), B group(difference of Cholinesterase's figure make use of Ultrasound with Zi-shil), C group(difference of Cholinesterase's figure make use of Eltroacupuncture and Ultrasound with Zi-shil). As a result the following conclusion was draun: 1. A group showed significantly decreased of Cholinesterase's figure make used of Electroacupuncture with Zi-Shil(the period is between pre-application and 20minute after application, between pre-application and mean of 20minute-30minute after application). 2. B group showed significantly decreased of Cholinesterase's figure make used of Ultrasound with Zi-Shil(the period is between pre-application and 3minute after application, between 3minute after application and 7minute after application, between preapplication and 7minute after application, between pre application and mean of 3minute-7minute after application). 3. C group not showed significantly decreased of Cholinesterase's figure make used of Elcetroacupuncture and Ultrasound with Zi-Shil(the period is between pre-application, between 20minute-Eletroacupuncture and 3minute-Ultrasound after application, between 30minute-Eletroacupuncture and 7minute-Ultrasound after application). The results of this study showed that decrease of Cholinestese' s figure make used of Elctroacupuncture and Ultrasound Zi-Shil. The result mean stimulation of Elctroacupuncture and Ultrasound decreased Acetylcholin' quantity. And decrease of Acetylcholin' quantity decreased Cholinesterase' figure.

c-Fos Expression in the Nucleus of the Solitary Tract in Response to Salt Stimulation in Rats

  • Salt signals in tongue are relayed to the nucleus of the solitary tract (NST). This signaling is very important to determine whether to swallow salt-related nutrition or not and suggests some implications in discrimination of salt concentration. Salt concentration-dependent electrical responses in the chorda tympani and the NST were well reported. But salt concentration-dependency and spatial distribution of c-Fos in the NST were not well established. In the present study, NaCl signaling in the NST was studied in urethane-anesthetized rats. The c-Fos immunoreactivity in the six different NST areas along the rostral-caudal axis and six subregions in each of bilateral NST were compared between applications of distilled water and different concentrations of NaCl to the tongue of experimental animals. From this study, salt stimulation with high concentration (1.0 M NaCl) induced significantly higher c-Fos expression in intermediate NST and dorsal-medial and dorsal-middle subregions of the NST compared to distilled water stimulation. The result represents the specific spatial distribution of salt taste perception in the NST.

신경병증성 통증에 대한 전침자극 효과의 연구 (Effects of Electroacupuncture on Neuropathic pain in Rats)

  • Objcetive : Neuropathic pain sometimes arises from a partial peripheral nerve injury. This kind of pain is usually accompanied by spontaneous burning pain, allodynia and hyperalgesia. It has been well known that acupuncture is effective to the pain control from ancient time in Asia. However, it is not clear whether acupuncture can control neuropathic pain. The aim of the present study is to examine if acupuncture stimulation may be effective to the mechanical allodynia in a rat model of neuropathic pain. Methods : To produce neuropathic pain, under sodium pentobarbital anesthesia, the right superior caudal trunk was resected between the S3 and S4 spinal nerves. After the neuropathic surgery, we examined if the animals exhibited the behavioral signs of mechanical allodynia. The mechanical allodynia was assessed by stimulating the tail with von Frey hair (bending force : 2.0g). three or 6 weeks after the neuropathic surgery, acupuncture stimulation was delivered to Houxi (SI 3) as the following parameters (2HZ frequency, 0.07mA intensity and 3msec duration) for 30 minutes. Results : The stimulation of Houxi (SI 3) acupoint relieved the behavioral signs of mechanical allodynia. Conclusion : Our results suggest that acupuncture can control the mechanical allodynia of neuropathic pain.

가압훈련의 혈류 압박 정도에 따른 복합근 활동전위의 변화 (Changes in Compound Muscle Action Potential Depending on Pressure Level of Blood Flow During KAATSU Training)

  • Purpose: In recent years, there has been increasing interest in using blood flow-restricted exercise (BFRE) or KAATSU training. The KAATSU training method, which partially restricts arterial inflow and fully restricts venous outflow in the working musculature during exercise at reduced exercise intensities, has been proven to result in substantial increases in both muscle hypertrophy and strength. The purpose of this study was to investigate the proper level of pressure for KAATSU training using compound muscle action potential (CMAP) analysis. Methods: Twenty-two healthy adults voluntarily participated in this study. CMAP was conducted by measuring the terminal latency and amplitude using a motor nerve conduction velocity test. For reference-line, supramaximal electrical stimulation was applied to the median nerves of the participants to obtain CMAP for the abductor pollicis brevis. For baseline, the intensity of the electrical stimulation was decreased to a level at which the CMAP amplitude was about a third of the CMAP amplitude obtained by the supramaximal electrical stimulation. The pressure levels for the KAATSU were set as a systolic blood pressure (strong pressure), the median values of systolic and diastolic blood pressure (intermediate pressure), and diastolic blood pressure (weak pressure). In the KAATSU condition, CMAP was performed under the same conditions as baseline after low-intensity thumb abduction exercises were performed at the subjects' own pace for one minute. Results: As the pressure increased, the CMAP amplitude was significantly increased, signifying that more muscle fibers were recruited. Conclusion: This study found that KAATSU training recruited more muscle fibers than low-intensity exercise without the restriction of blood flow.

Effect of Non-invasive Transcutaneous Electrical Stimulation in Women With Stress Urinary Incontinence: A Prospective Study

  • Background: Stress urinary incontinence (SUI) impacts the social, physical, and psychological well-being and quality of life of the patient. Several techniques exist for its management, including transcutaneous electrical stimulation (TES). Objects: We aimed to demonstrate the effects of TES on ultrasonographic variables and quality of life in women with SUI. Methods: This prospective study recruited 21 women who had been diagnosed with grade 1 or 2 SUI between July 2018 and March 2019. The exclusion criteria were pregnancy and a history of urogenital surgery. All participants were assessed at baseline and 8 weeks after intervention initiation. The bladder neck position (BNP), length of the urethra (LU), funneling index (FI), and rhabdosphincter thickness (RT) were measured. The Incontinence-Quality of Life (I-QOL) was used to assess incontinence-specific quality of life. Statistical significance level was set at p < 0.05. Results: Twenty-one patients with SUI used TES for 8 weeks. BNP and FI significantly decreased after intervention (p < 0.05). LU, anterior and posterior RT (indicators of external sphincter hypertrophy) significantly increased post-intervention (p < 0.05). The total I-QOL score increased from 64.81 to 71.86 after the intervention (p < 0.05). Conclusion: This intervention improved BNP, LU, FI, RT, and subjective indicators such as quality of life in women with SUI. Therefore, TES can be an effective non-surgical treatment method for improving SUI symptoms and quality of life in these patients.

뇌 SPECT와 SPM을 이용한 합곡-곡지 TENS 치료의 뇌혈류에 대한 효과 (Effect of LI4-LI11 Transcutaneous Electrical Nerve Stimulation on Regional Cerebral Blood Flow in Healthy Human Using SPECT and Statistical Parametric Mapping)

  • Background and purpose: So far it was reported that acupuncture increased cerebral blood supply and stimulated the functional activity of brain nerve cells. A previous study demonstrated a correlation between LI4-11 electro-acupuncture (EA) and rCBF increase in frontal lobe. However, there remained a need to study further using various controls in acupuncture research. Transcutaneous electrical nerve stimulation (TENS) has been used as a non-invasive control in acupuncture study. This study was to evaluate the effect of LI4-LI11 TENS on regional cerebral blood flow (rCBF) in normal volunteers using single photon emission computed tomography (SPECT) and statistical parametric mapping (SPM). Methods: In the resting state, $^{99m}Tc-ECD$ brain SPECT scans were performed on 10 normal volunteers (9 males, 1 female, mean age 26.6$\pm$0.5 years; age range from 26 to 27 years). On the other day, 7 days after the resting examination, 15 minute TENS were applied at LI 4 and LI 11 on the right side of the subjects. Immediately after LI4-LI11 TENS, the second SPECT images were obtained in the same manner as the resting state. Significant increases and decreases of regional cerebral blood flow after LI4-LI11 TENS were estimated by comparing their SPECT images with those of the resting state using paired t statistics at every voxel, which were analyzed by statistical parametric mapping with a threshold of p = 0.001, uncorrected (extent threshold: k=100 voxels). Results: TENS applied at right LI4-LI11 increased rCBF in the left somatosensory association cortex (Brodmann area 5, 7). However there was no area where LI4-11 TENS decreased rCBF. Conclusion and suggestions: These results demonstrate that right LI4-LI11 TENS increased rCBF only in corresponding somatosensory association cortex, which was different from the previous results using LI4-11 EA. It is suggested that there be a different mechanism between TENS and EA.

Functional Changes of Spinal Sensory Neurons Following Gray Matter Degeneration

  • Excitatory amino acids (EAA) are thought to play an important role in producing cell death associated with ischemic and traumatic spinal cord injury. The present study was carried out to determine if the response characteristics of spinal sensory neurons in segments adjacent to degeneration sites induced by EAA are altered following these morphological changes. Intraspinal injections of quisqualic acid (QA) produced neuronal degeneration and spinal cavitation of gray matter. The severity of lesions was significantly attenuated by pretreatment with a non-NMDA antagonist NBQX. In extracellular single unit recordings, dorsal horn neurons in QA injected animal showed the increased mechanosensitivity, which included a shift to the left in the stimulus-response relationship, an increased background activity and an increase in the duration of after-discharge responses. Neuronal responses, especially the C-fiber response, to suprathreshold electrical stimulation of sciatic nerve also increased in most cases. These results suggest that altered functional states of neurons may be responsible for sensory abnormalities, e.g. allodynia and hyperalgesia, associated with syringomyolia and spinal cord injury.
