Objective : Electrooculography (EOG) records eyeball movements as changes in the potential difference between the negatively charged retina and the positively charged cornea. We aimed to investigate whether reliable EOG waveforms can be evoked by electrical stimulation of the oculomotor and abducens nerves during skull base surgery. Methods : We retrospectively reviewed the records of 18 patients who had undergone a skull base tumor surgery using EOG (11 craniotomies and seven endonasal endoscopic surgeries). Stimulation was performed at 5 Hz with a stimulus duration of 200 μs and an intensity of 0.1-5 mA using a concentric bipolar probe. Recording electrodes were placed on the upper (active) and lower (reference) eyelids, and on the outer corners of both eyes; the active electrode was placed on the contralateral side. Results : Reproducibly triggered EOG waveforms were observed in all cases. Electrical stimulation of cranial nerves (CNs) III and VI elicited positive waveforms and negative waveforms, respectively, in the horizontal recording. The median latencies were 3.1 and 0.5 ms for craniotomies and endonasal endoscopic surgeries, respectively (p=0.007). Additionally, the median amplitudes were 33.7 and 46.4 μV for craniotomies and endonasal endoscopic surgeries, respectively (p=0.40). Conclusion : This study showed reliably triggered EOG waveforms with stimulation of CNs III and VI during skull base surgery. The latency was different according to the point of stimulation and thus predictable. As EOG is noninvasive and relatively easy to perform, it can be used to identify the ocular motor nerves during surgeries as an alternative of electromyography.
Purpose : The purpose of this study was to investigate the effect of applying intensity on the muscle activity and muscle hardness of the upper trapezius muscle in adult males when TENS (transcutaneous electrical nerve stimulation) was applied. In addition, this study intends to prepare the scientific basic data of TENS for the purpose of relaxation based on the experimental results. Methods : Eighty-seven healthy adult males participated in the experiment, and they were randomly divided into an experimental group and a control group. All subjects in this study were healthy subjects without musculoskeletal or nervous system damage. All subjects were subjects who voluntarily consented to the purpose and method of the experiment. All subjects were provided with a load by typing for 20 minutes, and muscle activity and muscle hardness of the upper trapezius muscle were measured immediately. Afterwards, TENS was given to each groups for 15 minutes, and the experimental group received stimulation at the motor threshold level, and the control group received a placebo stimulation. After stimulation, muscle activity and muscle hardness of the upper trapezius muscle were measured in the same method. The measured data were compared between groups through an independent t-test and dependent t-test. The statistical significance level was set at .05. Results : The application of TENS statistically significantly decreased the muscle activity and muscle hardness of the trapezius muscle in the experimental group, and the results showed a significant difference from the control group. Conclusion : Application of TENS significantly decreased the muscle activity and muscle stiffness of the upper trapezius muscle. The application of TENS of applying intensity that induces muscle contraction may induce relaxation by reducing the muscle activity and muscle hardness of the trapezius muscle.
Objective: To evaluate the changes in bladder capacity and storage through non-invasive neuromodulation by application of repetitive Trans magnetic stimulation (rTMS) and genital nerve stimulation (GNS) in traumatic spinal cord survivors. Design: A Single Case Study. Method: The Patient was registered in trail with the clinical trial registry of India (CTRI/2022/05/042431). The Patient was interposed with rTMS on lumbar area, from T11-L4 vertebrae with 1 Hz and the intensity was 20% below that elicited local paraspinal muscular contraction for 13 minutes. GNS was placed over dorsum of the penis with the cathode at the base and anode 2 cm distally at 20 Hz, 200 microseconds, Continuous and biphasic current was delivered and amplitude of stimulation necessary to elicit the genito-anal reflex. For assessment, Neurological examination was done for peri-anal sensation (PAS), voluntary anal contraction (VAC) and bulbocavernous reflex (BCR), deep anal pressure (DAP), and American Spinal Injury Association Impairment Scale (ASIA scale). Outcome assessment was done using Urodynamics, Spinal Cord Independence Measure Scale Version-III (SCIM-III), American Spinal Injury Association Impairment Score (ASIA Score), Beck's Depression Inventory Scale (BDI). The baseline evaluation was taken on Day 0 and on Day 30. Results: The pre-and post-data were collected through ASIA score, SCIM-III, BDI and Urodynamics test which showed significant improvement in bladder capacity and storage outcomes in the urodynamics study across the span of 4 weeks. Conclusion: rTMS along with GNS showed improvement in bladder capacity & storage, on sensory-motor score, in functional independence of individual after SCI.
Objective : The purpose of this study is to evaluate neuroprotective effect of sacral neuromodulation in rat spinal cord injury (SCI) model in the histological and functional aspects. Methods : Twenty-one female Sprague Dawley rats were randomly divided into 3 groups : the normal control group (CTL, n=7), the SCI with sham stimulation group (SCI, n=7), and the SCI with electrical stimulation (SCI+ES, n=7). Spinal cord was injured by dropping an impactor from 25 mm height. Sacral nerve electrical stimulation was performed by the following protocol : pulse duration, 0.1 ms; frequency, 20 Hz; stimulation time, 30 minutes; and stimulation duration, 4 weeks. Both locomotor function and histological examination were evaluated as scheduled. Results : The number of anterior horn cell was $12.3{\pm}5.7$ cells/high power field (HPF) in the CTL group, $7.8{\pm}4.9$ cells/HPF in the SCI group, and $6.9{\pm}5.5$ cells/HPF in the SCI+ES group, respectively. Both the SCI and the SCI+ES groups showed severe loss of anterior horn cells and myelin fibers compared with the CTL group. Cavitation and demyelinization of the nerve fibers has no significant difference between the SCI group and the SCI+ES group. Cavitation of dorsal column was more evident in only two rats of SCI group than the SCI+ES group. The locomotor function of all rats improved over time but there was no significant difference at any point in time between the SCI and the SCI+ES group. Conclusion : In a rat thoracic spinal cord contusion model, we observed that sacral neuromodulation did not prevent SCI-induced myelin loss and apoptosis.
Purpose: This study examined the effects of changes in the intensity of Functional Electrical Stimulation (FES) on CorticoMuscular Coherence (CMC) during action observation. This paper presents a neurophysiological basis for the effective intensity of FES. Methods: Twenty-seven healthy volunteers were asked to observed a video with FES. The FES was provided with a sensory stimulation level, nerve stimulation level, and motor stimulation level. Simultaneously, an electroencephalogram (EEG) of the sensorimotor cortex and electromyogram (EMG) from the wrist extensor muscle were recorded. The peak CMC and average CMC were analyzed to compare the differences caused by the FES intensity. Results: The peak CMC showed a significant increase in the alpha band during motor stimulation (p<0.05). The average CMC showed a significant increase in the beta band during motor stimulation (p<0.05). Conclusion: The intensity of FES, which causes actual movement, increased the CMC during action observation. These results show that the intensity of the FES can affect the functional connection between the sensorimotor cortex and muscle.
Background: The purpose of this study was influence of change of electrical stimulation time on body regions affects on electrical current threshold. Methods: The present study is to examine the effect of silver spike point (SSP) electrical stimulation (1Hz), transcutaneous electrical nerve stimulation (TENS, 100Hz), and interferential current therapy (ICT, 50Hz) from the low back and scapulodorsal and knee joint regions on stimulation-induced current thresholds from the elderly (over sixty) people (male:72, female:91) in senior welfare center. Result: The low back region, but not scapulodorsal and knee joint region, significantly increased the TENS and ICT, but not SSP, electrical stimulation, significantly increased the time-dependent current thresholds in elderly patients. Conclusion: Therefore, these results, in part, suggest that the TENS and ICT were shown to be a more adaptable method of stimulation, and that needed of the development of senile specialized physical therapy and the utilization of senior leisure facilities such as senior welfare center the others.
Purpose: The purpose of this study was to examine the effect on plasma beta endorphin concentration level and the influences on pain score of transcutaneous electrical nerve stimulation (TENS) mediation to patients During a prostate needle biopsy. Methods: TENS was administered to only the experimental group. The electric current was given in high frequency (40-100 pps) and low intensity ($2-50{\mu}s$) from the waiting room stage until the end of the procedure. The average time spent was 35 minutes. Following 10 minutes of retention in the rectum, there was a biopsy. In two groups, the pain score was assessed twice when vas pain penetrated into the rectum, during the needle biopsy. The Beta endorphin concentration level was assessed through blood gathering 2 times in the Nuclear Medicine Labs before and after the test. Results: There was not much difference in pain levels from both groups when a microscope probe penetrated into the rectum and in the time when tissues were collected. However, the average overall pain level was reduced during those two procedures. The plasma beta endorphin level was increased in the TENS medicated group compared with the unmedicated group after the procedures were completed. Conclusion: The research indicates that TENS was desirable to be considered as a non-invasive method for controlling pain.
Objective : Vagus nerve stimulation (VNS) has been used in epilepsy patients refractory to standard medical treatments and unsuitable candidates for resective or disconnective surgery. In this study, we investigated the efficacy of VNS to patients who had refractory result to epilepsy surgery and patients with post-traumatic epilepsy. Methods : We analyzed the effect of VNS in 11 patients who had undergone previous epilepsy surgery and patients with intractable post-traumatic epilepsy associated with brain injury. All patients underwent VNS implantation between October 2005 and December 2006. Results : We evaluated seizure frequency before and after implantation of VNS and maximum follow up period was 24 months. In the first 6 months, 11 patients showed an average of 74.3% seizure reduction. After 12 months, 10 patients showed 85.2% seizure reduction. Eighteen months after implantation, 9 patients showed 92.4% seizure reduction and 7 patients showed 97.2% seizure reduction after 24 months. Six patients were seizure-free at this time. Conclusion : We conclude that the VNS is a helpful treatment modality in patients with surgically refractory epilepsy and in patients with post-traumatic epilepsy due to severe brain injury.
Background: The purpose of this study was to evaluate the effect of dexamethasone or dexmedetomidine added to ropivacaine on the onset and duration of ultrasound-guided axillary brachial plexus blocks (BPB). Methods: Fifty-one ASA physical status I-II patients with elective forearm and hand surgery under axillary brachial plexus blocks were randomly allocated to receive 20 ml of 0.5% ropivacaine with 2 ml of isotonic saline (C group, n = 17), 20 ml of 0.5% ropivacaine with 2 ml (10 mg) of dexamethasone (D group, n = 17) or 20 ml of 0.5% ropivacaine with 2 ml ($100{\mu}g$) of dexmedetomidine (DM group, n = 17). A nerve stimulation technique with ultrasound was used in all patients. The onset time and duration of sensory blocks were assessed. Results: The duration of the sensory block was extended in group D and group DX compared with group C (P < 0.05), but there was no significant difference between group D and group DX. However, there were no significant differences in onset time in all three groups. Conclusions: Dexamethasone 10 mg and dexmedetomidine $100{\mu}g$ were equally effective in extending the duration of ropivacaine in ultrasound-guided axillary BPB with nerve stimulation. However, neither drug has significantly effects the onset time.
