Bickerstaff's Brainstem Encephalitis Treated with Immunoglobulin (면역글로불린으로 치료한 Bickerstaff 뇌간뇌염 1예)

  • Bickerstaff's brainstem encephalitis (BBE) is a rare disorder with acute ophthalmoparesis, ataxia, consciousness disturbance, and pyramidal signs of suspected autoimmune origin. A-62-year-old woman developed acute diplopia, dysarthria, gaze-paretic nystagmus and gait ataxia. Her mental status fell subsequently into stupor. Brain MRI and nerve conduction study showed no significantly abnormal findings. Electroencephalography revealed diffuse low voltage slowings. After treating with intravenous immunoglobulin, she demonstrated rapid clinical recovery. This case suggests that immunoglobulin can be an alternative option in BBE treatment, especially when plasmapheresis and corticosteroids are difficult or contraindicated.

Morphological and Functional Outcomes after Vertical Hemilaryngeal Transplantation in the Canine (개에서 후두수직절반 이식 후 형태 및 기능에 대한 연구)

  • In this study, the author developed a new animal model to examine morphological changes and functional recoveries after vertical hemilaryngeal transplantation in the canine. Seven vertical hemilaryngeal transplantations were carried out in the canine. After preparing the host dog removing right sided hemilarynx, hemilarynx of the donor dog was transplanted by hooking up the arteries, veins, nerves and hypopharyngeal mucosa. Especially, recurrent laryngeal nerve was anastomosed at the branch level(anterior and posterior) respectively. After 7 days, for the first evaluation of the transplantation, four out of seven dogs were considered successful. Three dogs survived more than one month, which is the critical period to evaluate the functional recovery after transplantations. After EMG examination, two dogs(#3, #5 dog) showed some functional recoveries. The five-transplanted hemilarynges were sectioned at the arytenoid cartilage region to examine the morphological changes. The results showed that the transplanted hemilarynx appeared normal as control in #5 dog. In addition, #2 dog showed fairly good condition even though died from asphyxia after 9 days out of transplantation. The other. three dogs(#3, #6, #7) showed various levels of atrophy and disappearance of the muscles and cartilages in their larynges. It can be suggested that this model could contribute an advance to preparing human laryngeal transplantation in the future.

Effect of Exercise on Muscle Recovery of Sciatic Nerve Injured Rats (궁둥신경 손상을 유발한 흰쥐에서 운동이 근육재생에 미치는 영향)

  • 이 연구의 목적은 흰쥐의 궁둥신경의 손상을 준 뒤 달리기운동을 시켜 운동이 근육재생에 효과가 있는지를 밝힐 목적으로 시행하였다. 수컷 Sprague-Dawley계 흰쥐의 궁둥신경을 압궤손상을 준후, 하루 60분 씩 주 5회 운동을 시켰다. 궁둥신경 압궤손상 후 12일(5일간 운동), 19일(10일간 운동), 26일(15일간 운동), 33일(20일간 운동 및 61일(40일간 운동)에 흰쥐를 희생시켜 장딴지근육을 절취하여 냉동절편을 만들었으며 중간세사인 desmin과 vimentin에 대한 면역조직화학적 염색방법을 시행하였고 NADH-TR을 이용한 효소조직화학적 방법으로 뼈대근육의 변성 등을 관찰하였다. 중간세사단백 중 desmin은 근육섬유의 변성 및 재생과정 모두에서 발현되며 vimentin은 재생과정에서만 발현되었다. 대조군보다 실험군에서 desmin과 vimentin의 면역 반응성이 높았고, 실험군의 근육섬유들은 손상 후 61일째에 횡단면에서 각을 갖고 있어 정상 근육섬유임을 입증해 주었으나 대조군의 근육섬유는 정상으로 회복되지 않은 둥근 근육섬유가 관찰되었다. 19일째의 NADH-TR반응에서 대조군에서는 근육 섬유형 군집(fiber type grouping)으로 인해 diformazan 과립이 뭉쳐져 있음을 볼 수 있으나 실험군에서는 점점 diformazan 과립이 고르게 분포됨을 볼 수 있었다. 재신경지배가 일어 날 때 나타나는 표적근육섬유(target fiber)는 NADH-TR반응에서 26일 실험군의 일부 근육섬유에서 처음으로 관찰되었다. 61일째 NADH-TR반응에서 대조군은 아직도 diformazan 과립이 근육섬유성 군집을 보였으나 실험군에서는 정상군과 다름없는 염색성이 관찰되었다. 이상의 결과로 흰쥐 궁둥신경 압궤손상 후 트레드밀 달리기 운동이 흰쥐 다리 뼈대근육의 중간세사발현에 효과가 있다고 생각된다.

  • Transverse rectus abdominis muscle (TRAM) free flap is widely used for breast reconstruction, however donor-site morbidities such as abdominal wall weakness, hernia, bulging are troublesome. For the purpose of minimizing donor-site morbidity, there has been a surge in interests in muscle sparing free TRAM flap preserving the anatomy of rectus abdominis muscle, fascia, and motor nerve. The purpose of this study is to investigate complication and morbidity after muscle sparing free TRAM flap. Between August, 1995 and May, 2003, there were 108 cases of muscle sparing free TRAM flap of breast reconstruction. There was no abdominal hernia. There were 4 cases of dog ear, 3 cases of marginal necrosis of apron flap, 2 cases of asymmetry of umbilicus. At 1 year after operation, most patients feel comfortness in physical exercise. Muscle sparing free TRAM flap provides ample amount of well vascularized soft tissue with small inclusion of rectus abdominis muscle and fascia. Also it minimizes donor-site morbidity with rapid recovery of abdominal strength.

A Clinical Experience of Facial Nerve Palsy Treated by Magnetic Resonance Analyser and Drug Therapy -A case of facial palsy- (자기공명분석기와 약물요법을 이용한 안면신경마비의 치험 -1예 보고-)

  • A 43 year old man who suffered from right facial palsy was treated successfully with the application of both magnetic resonance diagnostic analyser(MRA) and drug therapy. Treatment of facial palsy is generally composed of stellate ganglion block(SGB), drug therapy and operative intervention. Short periods of exposure to appropriate magnetic resonance can beneficially modulate the balance of autonomic nervous system that are responsible for sympathetic overflow. It was concluded that recovery of facial palsy by application of both MRA and drug therapy was effective in patient who refused SGB.

  • Recurrent Bell's palsy is a very rare case and have been reported that shows the incidence to be approximately 10 per cent in the Bell's palsy patents. It is generally accepted that facial paralysis caused by compression of the facial nerve by tumor develops slowly and has an unremitting course, however, reported cases have described the rare association of recurrent facial paralysis and intracranial tumor, and the same recovery rate. Usual symptoms of Bell's palsy include subacute facial palsy, hyperacusis on the affected side, postauricular pain on the affected side, altered sensation of taste, and partial trigeminal distribution hypesthesias. Complete resolution of symptoms is usually seen in 2-3months in $75-85\%$ of cases, with $25-35\%$ showing varying degrees of residual effects. We report a case of 4th ipsilateral recurrent Bell's palsy in a 14-year-old women, which was occurred in every winters. We treated her with acupuncture, moxibustion, herbal medication, carbon and silver spike point, and used House-Brackmnn grading system(HBGS) and the Fisch Detailed Evaluation of Facial Symmetry(DEFS) to assess the degree of paralysis in each part of face.

Clinical Evaluation of Surgical Treatment of Benign Mediastinal Tumors (양성 종격동종양의 외과적치료에 대한 임상적 고찰)

  • This is a report on a total of 8 cases of benign mediastinal tumors and cysts in Department of Thoracic Surgery, Chonnam University Hospital during the period from 1961 to 1969. The patients age was distributed between 18 and 38 year old with the highest incidence in the age group of second decade. Sex ratio of male to female was 3: 5. The tumors were classificed as follow; 3 case:, of neurogenic tumors, 2 cases of teratomas, one case of pericardial cyst, one case of cystic hygroma, and one case of brochogenic cyst. The symptomatic patients were 5 cases(62. 6%) and asymptomatic patients were 3 cases(37.5%). The symptomatic patients had the symptoms not referable to their lesion and the mediastinal tumors of asymptomatic patients were incidently found by routine chest X-ray. The Symptoms occurred by compression to adjacent nerve system in 3 cases. by perforation into the lung with infection in one case and by infection of bronchial cyst in one case. The complications were Pancoast's syndrome including Horner's syndrome(2 cases), middle lobe syndrome (one case), bro:1chial infection(one case) and intercostal neuralgia(one case). All tbe tumors were surgicai[y resectable with good recovery postoperatively. In 5 cases of the symptomatic patents, their symptoms were disappeared dramatically after operation.

Reconstruction of Forefoot Degloving Injury with Innervated Anterolateral Thigh Free Flap (전외측 대퇴부 감각 유리피판을 이용한 전족부 탈장갑 손상 재건)

  • The forefoot reconstruction is a challenging field for plastic surgeons. Weight bearing tolerability and stability are important factor of choosing reconstruction methods, but cosmetic aspect has to be considered. 51 year old man visited our clinic with extensive degloving injury on right forefoot by roller. The soft tissue defect started from metatarsal area to the toe tip including nails. We harvested the anterolateral thigh flap and transferred it to the forefoot defect area with nerve coaptation. The flap was successful without skin necrosis or other complications. Secondary flap debulking surgery was performed after ten months from initial operation. Patient was satisfied with functional and cosmetic outcomes. The patient was able to wear shoes and walk with adequate sensory recovery. As there is few report about reconstruction of forefoot soft tissue defects, we report a unique case of the anterolateral thigh innervated free flap reconstruction in degloving injury.

Guillain-Barré Syndrome after Lung Transplantation in the Immediate Postoperative Period: Case Report

  • A 58-year-old man, incapable of maintaining oxygen saturation with mechanical ventilation, was admitted to our hospital for veno-venous extracorporeal membrane oxygenation (ECMO) treatment. He was diagnosed with acute respiratory distress syndrome (ARDS) due to influenza A pneumonia. His condition stabilized with antibiotics and steroid administration, but weaning from ECMO failed due to post-infectious pulmonary sequelae. On day 84 after admission, he underwent bilateral lung transplantation. In the postoperative phase, he did not regain consciousness even after discontinuation of sedatives for 3 days. However, spontaneous pupillary reflex and eye movements were preserved, while communication and upper and lower limb movements were affected. The nerve conduction study was diagnostic of Guillain-Barré syndrome. He was managed with intravenous immunoglobulins and plasmapheresis. Mild recovery of the facial muscles was seen, but he died 24 days post-surgery due to progressive ARDS and sepsis.

  • Evaluating the patient's voice before thyroidectomy is useful for the purpose of identifying patients with vocal cord paralysis without symptoms, identifying other patient's voice abnormalities, and whether it is related to voice disorders that may occur after surgery. Also voice evaluation after thyroid surgery is helpful in diagnosis, treatment, and rehabilitation and follow-up of voice disorders that occur without clear nerve damage after thyroidectomy. And it is helpful for rapid recovery through active early rehabilitation treatment for patients who complain of speech impairment without paralysis. In particular, neck exercise can improve the adhesion of the surgical site and increase the range of motion of the neck as well as improve subjective neck discomfort. In addition, hearing, voice and breathing functions should be improved, and voice hygiene education and counseling should be provided. Vocal cord injection is the first treatment option for unilateral vocal cord palsy. By establishing a protocol for voice disorders before and after thyroid surgery and providing appropriate treatment, the quality of life of patients can be improved.
