What Effects Does Necrotic Area of Contrast-Enhanced MRI in Osteoporotic Vertebral Fracture Have on Further Compression and Clinical Outcome?

  • Objective : The objective of this study was to analyze the correlation between further compression and necrotic area in osteoporotic vertebral fracture (OVF) patients with contrast-enhanced magnetic resonance imaging (CEMRI). In addition, we investigated the radiological and clinical outcome according to the range of the necrotic area. Methods : Between 2012 and 2014, the study subjects were 82 OVF patients who did not undergo vertebroplasty or surgical treatment. The fracture areas examined on CEMRI at admission were defined as edematous if enhancement was seen and as necrotic if no enhancement was seen. The correlation between further compression and the necrotic and edematous areas of CEMRI, age, and bone mineral density was examined. Also, necrotic areas were classified into those with less than 25% (non-necrosis group) and those with more than 25% (necrosis group) according to the percentages of the entire vertebral body. For both groups, further compression and the changes in wedge and kyphotic angles were examined at admission and at 1 week, 3 months, and 6 months after admission, while the clinical outcomes were compared using the visual analog scale (VAS) and Eastern Cooperative Oncology Group (ECOG) performance status grade. Results : Further compression was $14.78{\pm}11.11%$ at 1 month and $21.75{\pm}14.43%$ at 6 months. There was a very strong correlation between the necrotic lesion of CEMRI and further compression (r=0.690, p<0.001). The compression of the necrosis group was $33.52{\pm}12.96%$, which was higher than that of the non-necrosis group, $14.96{\pm}10.34%$ (p<0.005). Also, there was a statistically significantly higher number of intervertebral cleft development and surgical treatments being performed in the necrosis group than in the non-necrosis group (p<0.005). Moreover, there was a statistical difference in the decrease in the height of the vertebral body, and an increase was observed in the kyphotic change of wedge angle progression. There was also a difference in the VAS and ECOG performance scales. Conclusion : The necrotic area of CEMRI in OVF had a strong correlation with further compression over time. In addition, with increasing necrosis, intervertebral clefts occurred more frequently, which induced kyphotic changes and resulted in poor clinical outcomes. Therefore, identifying necrotic areas by performing CEMRI on OVF patients would be helpful in determining their prognosis and treatment course.

  • Using FEM, we analyzed the stress changes on early osteonecrosis of femoral head as the necrotic area increases. A 3D FEM model of proximal femur was made from the CT scam data of fresh frozen adult proximal femur with the material properties from literature. The model consisted of 5994 elements and 5275 nodes. FE analyses were peformed using MSC/NASTRAN. At normal states, stress transmission is mainly along the primary compression trabeculae(PCT). Until 60% involvement of PCT with necrotic lesion, stress transmission is still along the remained PCT. When the PCT involvement is from 60% to 85%, stress transmission is along the PCT and margin of the necrotic lesion. When involvement is more than 85%, stress is mostly transferred to posterior part of femoral head.

Unusual Necrotizing Uterine Adenocarcinoma in a Dog

  • A 2-year-old female Maltese dog was admitted with a history of pyometra and resulting peritonitis and septicemia. Uterine specimen sampled by ovariohysterectomy was processed routinely for histopathological observation. Grossly, the uterine mucosa was covered with necrotic debris and on the cut surface, lesion extended into the uterine wall. Microscopically, severe necrosis was observed throughout thickened mucosa, submucosa, and wall of uterus. Tumorous lesions composed of anaplastic cells with bizarre nuclei or tubular structures of cuboidal to short columnar cells were infrequently observed around the necrotic lesions and muscular layer far from necrotic areas. Immunohistochemically, central necrotic area with ambiguous cell and tissue structures, peri-necrotic tumor lesions, and muscular layer were strongly positive for cytokeratin. Since huge necrosis of adenocarcinoma lesions in this case made it difficult to diagnose, immunohistochemical results enable to diagnose as a severe necrotizing adenocarcinoma. Thus, histopathological and immunohistochemical findings in this case may serve as an important knowledge to diagnose uterine adenocarcinoma with huge necrosis in the veterinary field.

  • The purpose of this study was to test the hypothesis that even very small change of the cue direction in the treatment of the early osteonecrosis could affect the outcomes of operation. For this, the changes in stress transfer within the necrotic area of the femoral head were investigated under various directions and placements of the core utilizing finite element method. The loading of 3188N, which represents after-heel-strike, was imposed in cubic cosine pattern. All nodes on the most distal surface of the model were constrained in all directions. All materials included were assumed to have linear-elastic behavior. The result says that the critical stress, which causes collapse of the femoral head, was reduced when the core was oriented toward the posterior side of the femoral head regardless of location of the necrotic area. The same result was obtained either fibular bone grafting or cementation was adopted. As a consequence, the biomechanical study suggests that the core should be directed toward the loading point where the resultant force is applied to get more desirable treatment of the osteonecrosis of the femoral head in the early stage.

A Simple Strategy in Avulsion Flap Injury: Prediction of Flap Viability Using Wood's Lamp Illumination and Resurfacing with a Full-thickness Skin Graft

  • Background Extensive degloving injuries of the extremities usually result in necrosis of the flap, necessitating comprehensive skin grafting. Provided there is a sufficient tool to evaluate flap viability, full-thickness skin can be used from a nonviable avulsed flap. We used a Wood's lamp to determine the viability of avulsed flaps in the operation field after intravenous injection of fluorescein dye. Methods We experienced 13 cases during 16 months. Fifteen minutes after the intravenous injection of fluorescein dye, the avulsed skin flaps were examined and non-fluorescent areas were marked under Wood's lamp illumination. The marked area was defatted for full-thickness skin grafting. The fluorescent areas were sutured directly without tension. The non-fluorescent areas were covered by defatted skin. Several days later, there was soft tissue necrosis within the flap area. We measured necrotic area and revised the flap. Results Among all the cases, necrotic area was 21.3% of the total avulsed area. However, if we exclude three cases, one of a carelessly managed patient and two cases of the flaps were inappropriately applied, good results were obtained, with a necrotic area of only 8.4%. Eight patients needed split-thickness skin grafts, and heel pad reconstruction was performed with free flap. Conclusions A full-thickness skin graft from an avulsed flap is a good method for addressing aesthetic concerns without producing donor site morbidity. Fluorescein dye is a useful, simple, and cost-effective tool for evaluating flap viability. Avulsed flap injuries can be managed well with Wood's lamp illumination and a full-thickness skin graft.

  • Root resorption is conventionally divided into internal and external varieties. Internal resorption occurs where there is loss of the internal wall of the root canal. and is usually associated with a localized area of necrotic pulp(Trope '||'&'||' Chivian 1984). External resorption occurs where there is loss of the external surface of the root and the resorption may be either transient or progressive(Tronstad 1988). External inflammatory resorption usually occurs following damage to the periodontal ligament or where there is communication between the periodontal ligament and a necrotic pulp. via open dentinal tubules or accessory canals. This type of resorption is usually progressive until root canal treatment is instituted (Barclay 1993).

Ultrastructural Changes During Programmed Cell Death of Tobacco Leaf Tissues Infected with Tobacco mosaic virus

  • Tobacco (Nicotiana tabacum cvs.Xanthi-nc and NC 82) plants infected with Tobacco mosaic virus (TMV) were examined ultrastructurally. Local lesions produced by TMV were sunken and withered. The plants were subjected to temperature shift (TS), a method to produce programmed cell death (PCD), by placing the infected plants initially at high temperature (35$^{\circ}C$) for 2 days and then shifting them to greenhouse temperature (22-27$^{\circ}C$). As a result, expanded lesions around the original necrotic lesions were produced. The expanded area initially had no symptoms, but it withered and became necrotic 15 h after TS. No ultrastructural changes related to PCD were noted at 0 h after TS in Xanthi-nc tobacco tissues as well as in healthy and susceptible tobacco tissues infected with TMV, At 6 h after TS, chloroplasts were convoluted and cytoplasm began to be depleted; however no necrotic cells were found. At 17 h after TS, ground cytoplasm of affected cells was completely depleted and chloroplasts were stacked together with bent cell wall or dispersed in the intracellular space. Necrotic cells were also observed, containing virus particles in the necrotic cytoplasm. There were initially two types of symptoms in the expanded lesions: chlorosis and non-chlorosis (green). Abundant TMV particles and X-bodies were only found in the chlorotic tissue areas. These results suggest that PCD by TMV infection may start with the wilting of cells and tissues before necrotic lesion formation.

일부 동물에서 분리한 Clostridium perfringens 독소형의 검색 (Detection for Toxin Types of Clostridium perfringens Isolates from an Animals)

  • Outbreak patterns were observed in 250 chickens of necrotic enteriris(0.3%) among 81,000, 90 enterotoxemic pigs(9.5%) among 950, and 9 enterotoxemic cattle(13.8%) among 65 in a farms of Gyeonggi, Chungnam, Kwangwon, and Chonnam area from 1993 to 1996. Clostridium perfringens was isolated from 36 strains(14.4%) among 250 chickens of necrotic enteritis, 21 strains(23.3%) among 90 enterotoxemic pigs, and 3 strains(33.3%) among 9 enterotoxemic cattle. Toxin types by the neutralization test were categorized as A-type in 36 strains(100.0%) among 36 chickens of necrotic enteritis, and as A-type in 17 strains(81.0%), as B-type in 2 strains(9.5%) and as C-type in 2 strains(9. 5%) among 21 enterotoxemic pigs, and also 3 strains(100.0%) among 3 enterotoxemic cattle.

  • Various concentration of formaldehyde solution (1%, 5%, 10%) were applied to cut pulp surface, for 3 minutes and the remaining pulp were carefully examined histo-pathologically. The result were as follows, 1. One week survival group which were treated by 1% formaldehyde solution showed seperation of odontoblast layer randomly and the weil's zone was undistinguishable. Deeper portion of this area was necrotic or coagulated. 2. Two week survival group which were treated 1% formaldehyde solution showed necrotic appearance spread from odontoblastic layer to weil's zone. 3. One and two week survival group which were treated 5% formaldehyde solution showed the odontoblastic layer was highly necrotic, under this zone empty net-work was seen. 4. One and two week survival group treated 10% formaldehyde solution showed complete destruction of pulp.

포도갈색무늬병 발병수준이 '켐벨얼리' 유기포도의 광합성률에 미치는 영향 (Photosynthetic Rates of 'Campbell Early' Organic Grape as Affected by Degree of Leaf Spot Disease Caused by Pseudocercospora vitis)

  • 포도 캠벨얼리품종은 우리나라 포도재배면적의 70%를 차지하고 있으며 다른 품종에 비해서 갈색무늬병에 대해서 감수성으로 병 발생이 심할 경우 과실당도의 저하를 가져와 품질하락과 생육불량 등의 문제점을 발생시킨다. 이러한 켐벨얼리 포도품종에서의 갈색무늬병에 의한 생육피해 해석을 위해서 7년생의 유기재배 켐벨얼리 품종대상으로 2015년 8월에 갈색무늬병이 발병된 포도나무를 대상으로 광합성 효율 특성, 엽록소함량 및 감염잎의 화학성분 등을 비교분석하였다. 갈색무늬병 감염면적이 증가함에 따라서 $CO_2$ differential value (기준기와 분석기 $CO_2$ 농도차이), WUE value (광합성수분사용효율)와 A value ($CO_2$의 흡수율/호흡률) 등은 크게 감소하는 경향이었으며 stomatal conductance value (기공전도도)은 큰 영향이 없는 것으로 조사되었다. 총 엽록소의 함량은 병징이 심할 경우 50% 이상 감소하여 엽록소 함량의 감소가 포도갈색무늬병 발병잎의 광합성 효율 저하에 큰 영향을 끼치는 것으로 생각되며 병반면적률에 따른 화학성분을 조사한 결과 병반면적률이 증가할수록 잎의 질소, 인산, 칼륨 함량은 감소하였으며 반면에 총탄소, 칼슘 그리고 마그네슘의 함량은 병반면적률에 비례해서 증가하였다.
