A Case of Bronchogenic Cyst Presenting as Lateral Neck Mass (측경부 종괴로 표현된 기관지원성 낭종 1례)

  • A bronchogenic cyst is an uncommon benign congenital anomaly of the primitive ventral forgut which was generally encountered within the mediastinum and detected in pediatric patients. It is rarely detected in adult population as a lateral neck mass. We have recently experienced one case of bronchogenic cyst as a left lateral neck mass in a 49-year-old male. He complaints of a $2{\times}1$ cm sized, soft, non tender, and movable mass on the low lateral neck. The surgical excision of mass was performed and the final histopathologic diagnosis was a bronchogenic cyst. Here, we report this case with the review of literatures.

A Case of Basaloid Squamous Cell Carcinoma of the Palatine Tonsil Presenting as Recurrent Neck Mass (반복적인 경부 종물로 발현된 구개편도의 기저양 편평상피세포암 1예)

  • Basaloid squamous cell carcinoma(BSCC) is a high-grade variant of squamous cell carcinoma, with a prediction for multifocal involvement of the base of tongue, pyriform sinus, supraglottic larynx, hypopharynx and palatine tonsil. It primary affects men in the seventh decade of life with frequent cervical lymph-node metastasis at presentation. Grossly, these tumors are usually firm to hard, with associated central necrosis, occuring as exophytic to nodular masses. Histologically, the this infiltrating tumor offers a variety of growth patterns, including solid, lobular, cribriform, cords, trabeculae, nests and glands or cyst. We present a 55-year-old female who was treated with surgical excision and radiotherapy. She was firstly presented as a recurrent inflammatory neck mass and finally diagnosed with basaloid squamous cell carcinoma in the palatine tonsil.

The Anti-tumor Effect of Polysaccharide from the Leaves of Panax Ginseng C.A. Meyer(MB40) in a Murine Squamous Cell Carcinoma Model (편평세포암 동물 모델에서 고려인삼잎-다당체(MB40)의 항암치료효과)

  • Background and Objectives : Panax ginseng C.A. Meyer is a medical plant that has been widely utilized as a tonic and nutritional agent since ancient times in Korea. Ginseng has anti-metastatic property of cancer and immunomodulating activity. The novel acidic polysaccharide compound(MB40) was isolated from the leaves of Panax ginseng C.A. Meyer. To determine immunomodulating activities of MB40, we evaluate anti-cancer and anti-metastatic effects of MB40 in tumor bearing immune competent mice. Material and Methods : C3H mice were divided into three equal groups(Cisplatin treatment group, MB40 treat-ment group, Cisplatin and MB40 treatment group) and were transplanted SCC(Squamous Cell Carcinoma) cells(2${\times}$106) to the lateral side of abdomen. From day 4 after transplantation, MB40 was administrated at dose of 10mg/kg, respectively, every other day by intratumoral injection. Cisplatin was systemically administrated at doses of 1mg/kg, respectively, every week by intraperitoneal injection. Results : 5 days after administration, tumors can be palpated in every mice group. After 13 days of administration, the mice group to which MB40 were administrated exhibited reduction in tumor size respectively, compared to cisplatin group. Overall status of mice such as body weight and activity were superior in MB40 group than cisplatin group. Conclusion : The result of this study indicates MB40 may have significant therapeutic effect and decreases complications induced by systemic chemotheraphy. MB40 may be developed as a novel and potent immunotropics to improve the cell immune system and anti-cancer drug for the treatment of cancer patients in head and neck squamous cell carcinoma.

Three-dimensional intraoperative computed tomography imaging for zygomatic fracture repair

  • Objectives: Zygomatic complex (ZMC) fractures comprise up to 40% of all facial fractures. Misaligned bone fragments and misplaced fixation hardware traditionally detected postoperatively on plain radiographs of the skull might require re-operation. The intraoperative O-Arm (Medtronic, USA) is a three-dimensional (3D) computed tomographic imaging system. Materials and Methods: This retrospective single-center study evaluated the utility of O-Arm scanning during corrective surgeries for ZMC and zygomatic arch (ZA) fractures from 2018 to 2020. Three females and 16 males (mean age, 31.52 years; range, 22-48 years) were included. Fracture instability (n=6) and facial deformity (n=15) were the most frequent indications for intraoperative 3D O-Arm scan. Results: The images demonstrated that all fracture lines were properly reduced and fixed. Another scan performed at the end of the fixation or reduction stage, however, revealed suboptimal results in five of the 19 cases, and further reduction and fixation of the fracture lines were required. Conclusion: Implementation of an intraoperative O-Arm system in ZMC and ZA fracture surgeries assists in obtaining predictable and accurate results and obviates the need for revision surgeries. The device should be considered for precise operations such as ZMC fracture repairs.

  • Epiglottic cysts cause stridor mimicking asthma and hinder intubation in surgery. A huge cyst can obstruct the airway, making laryngeal examinations impossible. Before surgery, complete visualization of the larynx is necessary for the successful excision of a cyst. If laryngeal examination fails, computed tomography (CT) scanning can provide detailed information. Virtual endoscopy based on CT data reconstruction can yield a detailed three-dimensional image of the larynx. Here, we report two cases of epiglottic cysts along with virtual endoscopic findings and surgical photographs.

Myxoma in the Laryngeal Ventricle and the True Vocal Cord:A Case Report (후두실과 진성대에 발생한 점액종 1예)

  • Myxoma is an uncertain mesenchymal cell origin, characterized by irregular round, stellate or spindle cells surrounded by the matrix containing abundant mucoid material and scant vascularity. Their occurrence in descending order of frequency is in the heart, subcutaneous tissue, bone and genitourinary tract. In the head and neck region, the most predilection sites are mandible and maxilla(more than 80%). Laryngeal myxoma is extremely rare:only 5 cases have been reported in the English literature. We report a rare case of laryngeal myxoma. A 60-year-old man with hoarseness visited the out-patient department. The mass was located between the vocal fold and the vocal ligament, filling with the left laryngeal ventricle. We planned the laryngo-microsurgery and successfully excised using $CO_2$ laser. The histopathologic finding revealed the myxoma. After 18 months of surgery, there is no evidence of recurrence and mucosal scarring in the vocal fold. This report is the first case of laryngeal myxoma involving the laryngeal ventricle and the true vocal cord together.

  • A myoepithelial carcinoma, a rare malignant salivary gland neoplasm, occurs mostly in the parotid gland. The incidence of myoepithelioma is less than 1% of all salivary gland tumors, and malignant myoepithelioma is even more rare. Biological behavior of myoepithelial carcinoma has not been fully clarified. Malignant myoepithelioma shows clinicopathologic diversity and presents with various stages of myoepithelial differentiation. The definite treatment for malignant myoepithelioma is surgical excision, and the role of radiation therapy and chemotherapy is not yet established. In this study, we have experienced two patients with malignant myoepithelioma of the parotid gland who were treated with surgical excision.

  • The segmental mandibulectomy should be performed, if mandiblie invasion is suspected clinically and radiologically. And if tumor is located to mandible very closely or when microinvasion to mandible is suspected, marginal mandibulectomy is recommended. But in segmental mandibulectomy, reconstruction is difficult and cosmetic problem remains. In this case, we performed modified segmental resection of mandible, preserving the inferior margin of mandible, and maintains the continuity of the bone, in mucoepidermoid carcinoma of parapharynx, invading mandible. We reviewed the diagnosis, pathology, and treatment, and report the case with reviews of literature.

  • 후두낭종은 후두소낭의 비정상적인 팽창으로 발생하는 드문 질환이다. 후두낭종의 크기와 증상에 따라 치료 방법은 단순 경과관찰에서부터 흡인, 경화제 주입, 수술적 치료까지 다양하다. 수술적 치료는 외적 접근법, 내시경적 접근법 또는 두 접근법을 혼용한 접근법을 통해 이루어진다. 최근 저자들은 경부 외적 접근법 및 연골막하 절제를 통한 혼합 후두점액낭종 1예의 치료를 경험하였기에 문헌 고찰과 함께 보고하는 바이다.

  • An unusual case of nodal metastases from thyroid neoplasm known as parapharyngeal space mass is likely to be overlooked. And identification of the primary lesion by excisional biopsy calls for a secondary operation. Therefore, it is important to be aware of the possible lymphatic spread of the thyroid neoplasm to the parapharyngeal space. In this case, completion thyroidectomy should be considered. Here, we present a case of thyroid papillary carcinoma masquerading as a parapharyngeal space tumor. The mass was removed by transcervical approach and pathologically diagnosed as a metastatic thyroid papillary carcinoma. Successful results were obtained after additional completion thyroidectomy.
