Neck metastasis of invasive ductal carcinoma of breast causing voice change: a case report (음성변화를 주증상으로 내원한 유방암의 경부연조직 전이환자 1례)

  • We present a metastatic carcinoma from the breast to the neck soft tissue around common carotid artery, with a rare finding of voice change. A 60 year-old female patient presented with voice change for 7 months. Neck ultrasound revealed a soft tissue mass between internal jugular vein and common carotid artery. Result of fine needle aspiration biopsy was a metastatic carcinoma. Computed tomography and magnetic resonance image revealed $2.5{\times}3.0cm$ sized irregular marginated soft tissue mass in right lower neck encasing common carotid artery and internal jugular vein. Surgical resection was performed and pathologic result with immunohistochemical analysis confirmed the diagnosis of a metastatic invasive ductal carcinoma originated from breast.

A case of a Spindle cell/pleomorphic lipoma in the lateral neck (측경부에 발생한 방추세포 / 다형성 지방종1예)

  • The Spindle cell lipoma is a slow-growing benign tumor seen generally in the shoulders, upper back, and back of the neck of male. The Pleomorphic (giant-cell) lipoma is a benign tumor of adipose tissue with atypical histological features. It is mainly seen in the same lesion as the Spindle cell lipoma. The Pleomorphic lipoma is cytogenetically similar to spindle cell lipoma with a consistent loss of chromosome 16q material. For this reason, these two entities are regarded as a similar spectrum in the adipose tumors. Herein, we present a 53-year old man with Spindle cell/pleomorphic lipoma in the lateral neck. Physical and radiologic examinations of the Spindle cell/pleomorphic lipoma in the lateral neck are not specific and preoperative diagnosis is usually difficult. Therefore, clinicians should consider the possibility that Spindle cell/pleomorphic lipoma may occur in the lateral neck mimicking the other more frequently observed lesions.

  • Lipoblastoma is a rare benign tumor with 80-90% occurring in children less than 3 years of age and 40% occurring in children less than 1 year of age. The most common site of incidence is limb, and then trunk. Neck is the rare site of incidence. The main symptom that the patient complains about is a rapidly growing neck mass without pain. When the size of mass increases, it can cause dyspnea, Horner's syndrome. Lipoblastoma is usually diagnosed as a lipoma in the fine needle aspiration. Since it is not differentiated from lipoma, liposarcoma, and hibernating adenoma in CT and MRI, the definitive diagnosis is histologic diagnosis through surgical resection. The treatment is complete surgical resection. And recurrence rate is 9-25% due to incomplete resection. Authors report this case with a review of literatures since we experienced a case of lipoblastoma diagnosed histopathologically after surgical treatment of neck mass.

  • Mucinous adenocarcinoma is rarely reported in head and neck region. In other organ, it usually occur in breast, gastrointenstinal tract and so on. The specific histologic findings of mucinous adenocarcinoma are the large amount of extracellular mucin and tumor cell nests such as floating in mucin pool. It may develop rarely in major or minor salivary gland, but only one case of mucinous adenocarcinoma originating from parotid gland was presented in south korea. We report a case of mucinous adenocarcinoma in the tongue base considered to develop from minor salivary gland with a review of literatures.

  • Neurilemmomas are uncommon benign tumors originating from the schwann cells of various cranial nerves, spinal nerves, and autonomic nerves. About 25% of the disease is found in the head and neck region and the most common single site of origin is known to be the acoustic nerve. However neurilemmomas of the peripheral segment of the lingual nerve are extremely rare neoplasms. We recendy experienced a case of a benign neurilemmoma of the lingual nerve and this case may be the second to be reported in the literature.

  • Schwannomas are benign tumors arising from the nerve sheaths of peripheral, autonomic and cranial nerves. Between 25% and 45% of all schwannomas occur in the head and neck region but only 4% involve the nasal cavity and paranasal sinus. Moreover, schwannoma of the pterygopalatine fossa is extremely rare. We present a case of schwannoma of the pterygopalatine fossa in a 43-year-old woman. The patient was surgically managed with endoscopic transnasal approach combined with transantral approach. After 16 months, the patient shows no evidence of recurrence.

  • Acute suppurative thyroiditis is rare, infectious thyroid disorder because the thyroid gland is resistant to infection. We report a 26-year-old man with acute suppurative thyroiditis due to a pyriform sinus fistula. He presented with anterior neck swelling and tenderness for 2 weeks. Antibiotic treatment failed to improve his symptoms and signs. Diagnosis was made by bariums studies, computed tomography scan and endoscopic examination. The pyriform siuns fistula was successfully treated by chemical cauterization, partial thyroidectomy and ligation of fistula tract.

A Case of Vascular Leiomyoma Presenting as a Soft Mass of the Hard Palate (경구개 연성종물로 발현된 혈관평활근종 1예)

  • Vascular leiomyomas are benign tumors of smooth muscle origin arising from the muscularis layer of blood vessel walls. They can occur anywhere in the body where smooth muscle is found and usually occur in the lower extremity as a slow-growing, firm, occasionally painful mass. However they are rare in the head and neck and very rare in hard palate. Here we report a case of a vascular leiomyoma presenting as a soft mass of the hard palate and review the literatures.

  • Neurilemmoma is a relatively rare benign tumor which may be derived from nerve sheath of the peripheral, sympathetic and cranial nerve except the optic nerve and olfactory nerve. And it is a characteristically solitary and well-encapsulated mass. About 25% of neurilemmoma occurs in the head and neck region and the acoustic nerve is most frequently involved, but neurilemmoma of the larynx is rare. Recently, we experienced a case of neurilemmoma occurred in the right aryepiglottic fold of the larynx. The tumor was successfully removed by transoral approach under direct laryngoscope. So we report this case with literature review.

  • Squamous cell carcinoma, which is a common primary head and neck malignant neoplasm that is usually restricted to the mucosal surfaces of the upper aerodigestive tract and skin, is very unusual in the major salivary gland. Among them, few cases are regarded as primary carcinomas. In this article, we present two cases of squamous cell carcinoma in the parotid gland, who first presented with painful mass on infraauricular area.
