Background Xeroderma pigmentosum (XP) is an autosomal recessive disorder characterized by xerosis, ultraviolet light sensitivity, and cutaneous dyspigmentation. Due to defects in their DNA repair mechanism, genetic mutations and carcinogenesis inevitably occurs in almost all patients. In these patients, reconstruction of cutaneous malignancies in the head and neck area is associated with some challenges such as likelihood of recurrence and an aggressive clinical course. The aim of this study is to discuss the therapeutic options and challenges commonly seen during the course of treatment. Methods Between 2005 and 2015, 11 XP patients with head and neck cutaneous malignancies were included in this study. Demographic data and treatment options of the patients were evaluated. Results The mean age of the patients was 32 years (range, 10-43) (4 males, 7 females). The most common tumor type and location were squamous cell carcinoma (6 patients) and the orbital region (4 patients), respectively. Free tissue transfer was the most commonly performed surgical intervention (4 patients). The average number of surgical procedures was 5.5 (range, 1-25). Six patients were siblings with each other, 5 patients had local recurrences, and one patient was lost to follow-up. Conclusions Although genetic components of the disease have been elucidated, there is no definitive treatment algorithm. Early surgical intervention and close follow-up are the gold standard modalities due to the tendency toward rapid tumor growth and possible recurrence. Treatment must be individualized for each patient. In addition, the psychological aspect of the disease is an important issue for both patients and families.
Advances in nanobiotechnology have presented numerous possibilities of more effective diagnostic and therapeutic options. In particular, gold nanoparticles have demonstrated the potential for application in molecular imaging and treatment of cancers, including drug delivery system of certain target molecules, enhancement of radiation therapy, and photothermal treatment. This review discusses the properties, mechanism of action, and clinical application of gold nanoparticles. Although the safety of nanoparticles is yet to be ascertained, there is no doubt that in the future, nanotechnology will play an important role in the development and enhancement of a wide range of diagnostic and treatment modalities.

Spinal subdural hematoma[SSDH] is rare disease. Furthermore, it rarely occurs as a complication of intracranial surgery. There are few case reports which describing SSDH after craniotomy. Although the exact pathogenetic mechanism is obscure, some investigators propose that downward migration of intracranial hematoma by the effect of gravity is one of the cause of SSDH, and which is commonly suggested. But others propose that cerebrospinal fluid[CSF] hypotension is an another possible mechanism In this paper, we report two cases of SSDH after clipping of an aneurysmal neck.
Spark-Plasma Sintering(SPS) is one of the new sintering methods which takes advantages both inconventional pressure sintering and electric current sintering. It is known that SPS is very effective for the densification of hard-to-sinter materials like refractory metals, intermetallic compounds, glass and ceramics without grain growth. However, a clear explanation for sintering mechanism and an experimental evidence for the formation of weak plasma during SPS are not given yet. In this study, fundamental study on sintering behavior and mechanism of SPS was investiged. For this study, various spherical Fe powders were prepared such as as-received, as-reduced, and as-oxidized and then sintered by SPS facility. In order to confirm the surface cleaning effect during SPS neck region and fracture surface of sintered body was observed and analyzed by SEM/EPMA. Densification behavior was analyzed from the data of deflection along the pressure axis. Some specimens were additionally produced by Hot Pressing and the results were compared with those of SPS.

Objective : Vertebral distraction is routinely performed during anterior cervical discectomy and fusion (ACDF). Overdistraction can injure the facet joints and may cause postoperative neck pain consequently. The purpose of this study was to investigate the clinical relevance of distraction force during ACDF. Methods : This study included 24 consecutive patients with single level cervical disc disease undergoing single level ACDF. We measure the maximum torque just before the the arm of the Caspar retractor was suspended by the rachet mechanism by turning the lever on the movable arm using a torque meter. In order to turn the lever using the torque driver, we made a linear groove on the top of the lever. We compared the neck disability index (NDI) and visual analogue scale (VAS) scores between the high torque group (distraction force>6 $kgf{\cdot}cm$) and the low torque group (distraction force${\leq}6kgf{\cdot}cm$) at routine postoperative intervals of 1, 3, 5 days and 1, 3, 6 months. Results : The VAS scores for posterior neck pain had a linear correlation with torque at postoperative 1st and 3rd days ($y=0.99{\times}-1.1$, $r^2=0.82$; $y=0.77{\times}-0.63$, $r^2=0.73$, respectively). VAS scores for posterior neck pain were lower in the low torque group than in the high torque group on both 1 and 3 days postoperatively ($3.1{\pm}1.3$, $2.6{\pm}1.0$ compared with $6.0{\pm}0.6$, $4.9{\pm}0.8$, p<0.01). However, the difference in NDI scores was not statistically significant in all postoperative periods. Conclusion : Vertebral distraction may cause posterior neck pain in the immediate postoperative days. We recommend not to distract the intervertebral disc space excessively with a force of more than 6.0 $kgf{\cdot}cm$.
This study proposes a new instructional strategy and corresponding materials designed from various alternative frameworks to help students understand evolution as a biologically acceptable theory. Biology teachers have normally taught the evolutionary mechanism by means of comparing Lamarckism with natural selection. In this study, a new instructional strategy in which the Lamarckian explanation is first excluded because Lamarckism is known to be subsumed in a learner's cognitive structure as a strong preconception of evolution is suggested for teaching evolution. After mutation theory is introduced, Darwinism including natural selection is explained separately during the next class hour. Corresponding instructional materials that aid student understanding of the evolutionary mechanism were developed using recently published articles on human genetic traits as scientific evolutionary evidence instead of the traditional evolutionary subject matter, giraffe neck. Evolutionary evidence from human genetic traits allows students to exclude anthropocentric thoughts effectively and raise concern for the phenomenon of evolution positively. The administered instructional strategy and materials in this research improved student conception, concern, and belief of evolution and it is believed that they helped students understand the evolutionary mechanism effectively.

In response to problems such as insufficient extraction information, low detection accuracy, and frequent misdetection in the field of Thangka image defects, this paper proposes a YOLOv5 prediction algorithm fused with the attention mechanism. Firstly, the Backbone network is used for feature extraction, and the attention mechanism is fused to represent different features, so that the network can fully extract the texture and semantic features of the defect area. The extracted features are then weighted and fused, so as to reduce the loss of information. Next, the weighted fused features are transferred to the Neck network, the semantic features and texture features of different layers are fused by FPN, and the defect target is located more accurately by PAN. In the detection network, the CIOU loss function is used to replace the GIOU loss function to locate the image defect area quickly and accurately, generate the bounding box, and predict the defect category. The results show that compared with the original network, YOLOv5-SE and YOLOv5-CBAM achieve an improvement of 8.95% and 12.87% in detection accuracy respectively. The improved networks can identify the location and category of defects more accurately, and greatly improve the accuracy of defect detection of Thangka images.
Xeroderma pigmentosum is a rare autosomal recessive disease, related to defects in DNA repair mechanism. It presents skin lesions on sun-exposed areas, leading to various skin cancer. Skin lesions can be treated with cryotherapy, skin resurfacing, 5-FU, Imiquimod, topical T4 endonuclease V, radiotherapy and genetic therapy, but invasive skin cancer should be treated by a surgery. We report a 12-year-old female xeroderma pigmentosum patient with recurrent basal cell carcinoma successfully treated by skin grafting. In that there is no cure for this disease, prevention and patient education is most important.
Anaplastic thyroid carcinoma, which is one of the most aggressive tumors of the thyroid, has been seldomly reposted to have altered thyroid function. There have been few reports of patients with anaplastic thyroid cancer presenting in a hyperthyroid state. In the literatures, the mechanism of hyperthyroidism in anaplastic thyroid cancer is supposed that the rapid invasive growth of cancer seems to cause destruction of thyroid tissue and develops a hyperthyroid state, which is thought to be anlalogous to that of subacute thyroiditis and several types of metastatic cancer of the thyroid: rapid tissue necrosis with resultant release of thyroid hormone. Recently, we experienced a case of anaplastic thyroid cancer presenting with rapid growing mass and hyperthyroidism in a 67-year-old woman and report it with the review of the literatures.
