A Case of Metastatic Brain Cancer from Squamous Cell Carcinoma of the Tonsil (편도 편평세포암종의 뇌전이 1례)

  • Distant metastases of head and neck cancer have become an increasingly common cause of death as local and regional control has improved. The most frequent metastatic sites of head and neck cancer are the lung, liver, bone and kidney; but metastases to the gastrointestinal tract, brain and heart have also been reported. We report a recent case of a 37-year-old male patient with squamous cell carcinoma of the tonsil who had undergone composite operation with left radical neck dissection and postoperative radiotherapy. The patient presented three years later, cachexic and complaining of severe deep seated headache. Radiologic evaluation revealed a cystic mass with peripheral enhancement in left temporal lobe that was proven to be metastatic cancer by burrhole exploration. However, in spite of various modalities, the patient expired.

A Case of Columnar Cell Variant of Papillary Thyroid Carcinoma (갑상선 유두암 원주세포변형 1예)

  • Columnar cell variant of papillary thyroid carcinoma (CCV-PTC) is a rare variant representing 0.15-0.2% of all PTCs. The CCV is aggressive, due to its rapid growth, high local recurrence rate, and frequent lung, brain and bone metastasis. Aggressive surgical and medical management are recommended for these neoplasias. The authors experienced a case of CCV-PTC in a 45-year-old man. We performed total thyroidectomy with neck dissection. The patient received radiation and radioactive iodine therapy. There were no recurrences or complications in the following 24 months after the operation. The patient will closely undergo continuous follow up. We present the clinical characteristics, pathology, treatment, and prognosis of the tumor with a review of the literature.

Effects of Soy Isoflavone Supplementation and Exercise on Bone Mineral Density and Urinary Deoxypyridinoline in Postmenopausal Women (폐경 후 여성에서 대두 이소플라본 공급 및 운동처방이 골밀도 및 소변 디옥시피리디놀린에 미치는 영향)

  • We studied the effects of soy isoflavones supplementation with exercise on bone mineral density and the urinary excretion of deoxypyridinoline as an index of bone resorption rates in postmenopausal women. A total of 67 postmen-opausal women were assigned to Isoflavone (90 mg/day) or placebo groups. These group were further divided into groups that undergone a regular exercise or a rather sedentary state performing daily activity only. Four groups were Placebo-control group (n = 16), Placebo-exercise group (n = 16), Isoflavone-sedentary group (n = 19) and Isofla-vone-exercise group (n = 16). After the intervention, we compared anthropometric mesurement, dietary recall, bone mineral density (femoral neck, lumbar spine), urinary deoxypyridinoline between the groups and between the pre and post studies. There were no significant differences between the four groups in terms of average age, height, weight, period after menopause at the baseline. The average age of the subjects were 55.2 yrs, average height, weight, period after menopause were 154.7 cm, 59.3 kg, 5.58 yrs, respectively. After eight week intervention period, there were no significant differences between the four groups in bone mineral density, but urinary deoxypyridinoline excretion was significantly decreased both in Isoflavone-sedentary and Isoflavone-Exercise groups. These results suggest that Isofla-vone supplementation alone or with exercise may be preventive measures through the decrease of bone reabsorption rate in post-menopausal subjects. Whereas exercise alone did not appear to be an effective measure in bone loss with these subjects.

Discrepancy of Bone Metastases between F-18 FDG PET/CT and Bone Scan in a Patient with Prostate Cancer (전립선암에서 골전이 진단에 대한 F-18 FDG PET/CT와 골스캔의 불일치)

  • We report the case of a 73-year-old man who had prostate cancer with bone metastases. Tc-99m HDP Whole body bone scan revealed multiple areas of increased bony uptake consistent with widespread bone metastases. F-18 fluorodeoxyglucose (FDG) positron emission tomography/computed tomography (PET/CT) demonstrated mild F-18 FDG uptake in the lymph nodes of neck, abdomen, and pelvis. However, abnormal F-18 FDG uptake was not seen in the skeletal system. Biopsy and immunohistochemical stains of left supraclavicular mass showed metastatic prostate adenocarcinoma. Currently, there are a few reported cases of F-18 FDG PET/CT evaluation of bone metastases in prostate cancer. We discuss the discrepancy between F-18 FDG PET/CT and bone scan in the detection of osseous metastases of prostate cancer.

Effects of Weight-Bearing Exercise on Bone Metabolism in College Women (체중부하운동이 여대생의 골대사에 미치는 효과)

  • Purpose: This study was done to identify the effects of weight-bearing exercise(WBE) on bone metabolism. Method: WBE was performed for 12 weeks by healthy college women. Bone-related parameters were measured four times during this period by evaluating the immunoradiometric assay and enzyme immunoassay. Bone mineral densities(BMDs) were measured by dual energy x-ray absorptiometry before and after the WBE program. Data was analyzed using t-test, paired t-test, $x^2$-test, and repeated measures ANOVA. Result: Osteocalcin, a bone formation marker, increased more in the experimental group than in the control group based on the interaction between time and group(F=3.29 p=.024). Little difference between the two groups was found for the other parameters: urinary deoxypyridinoline, insulin-like growth factorI, parathormone, serum calcium, and serum phosphorus without showing any time interaction between the groups. The femoral trochanter BMD rose in the experimental group while that of the control group fell, showing a significant difference for BMD(t=3.06 p=.005). However, there was no significant difference between the two groups for changes in BMD of the forearm, lumbar spine, femoral neck, and femoral ward's triangle. Conclusion: These findings supported the WBE is beneficial for increasing bone formation in college women and long-term application is needed to substantiate the effects of WBE as a intervention in promotion of bone-health.

A case of intra-abdominal abscess caused by unconsciously ingestion of fish bone in elderly patient

  • Cases showing complications such as esophageal injury, deep neck infection, and mediastinitis caused by accidental ingestion of fish bone are common. But ingestion of fish bone rarely causes perforation of the gastrointestinal tract or an intra-abdominal abscess. We report herein a case of a 78-year-old man with a periumbilical mesenteric abscess caused by fish bone which was ingested unconsciously. The fish bone was found in the terminal ileum and it was removed by colonoscopy. The patient improved and he was discharged after systemic antibiotic therapy. Occasionally, when patients swallow fish bone without a foreign body sensation, clinicians should suspect perforation caused by fish bone in case of an intra-abdominal abscess of unknown cause.

Research on Relation of Nutrients Intake, Health Status, and Bone Mineral Density in Middle-aged Women (장년기 여성의 영양섭취상태, 건강상태와 골밀도와의 관련성에 관한 연구)

  • This study was to investigate the relation of nutrients intake, health status, and bone mineral density in middle-aged women. Daily nutrients intake were analyzed by convenient method. The BMD of subjects were measured by Dual Energy X-ray Absorptiometry(DEXA). BMD was measured at the spine(vertebrae L2-L4) and femur(neck, Ward's triangle and trochanter). The nutrients intake of subjects were higher than recommended dietary allowances(RDA) except for calorie, iron, calcium, vitamin B1. Nutrients intake of vitamin A(p<0.05), iron(p<0.01), vitamin $B_2$(p<0.05), niacin(p<0.05), and vitamin C(p<0.05) between $\leq$49 yr group and $\geq$50 yr group. Mean daily intake of calcium was much less than the Korean RDA, 78.5% in $\leq$49 yr group and 77.3% in $\geq$50 yr group. 35.8% of the subjects under 50 years of age($\leq$49 yr group) and 77.4% of the subjects from 50 years up($\geq$50 yr group) were classified as osteopenia or osteoporosis. BMD of L2-L4 were positively correlated with height(p<0.05), weight(p<0.05) and BMI(p<0.05). BMD of femoral neck was positively correlated with BMI(p<0.05), BMD of Ward's triangle was positively correlated with weight(p<0.001). But BMD of L2-L4 and femoral neck were negatively correlated with age(p<0.05), menarche(p<0.05). BMD of Ward's triangle was negatively correlated with age(p<0.001). The BMD of L2-L4 were positively correlated with animal protein(p<0.05), calcium(p<0.05) and iron(p<0.05). The BMD of femoral neck was positively correlated with animal protein(p<0.05). The BMD of Ward's triangle was positively correlated with animal protein(p<0.001) and iron(p<0.001). The above results suggest that it should be difficult to prevent middle-aged women's bone destruction through nutrients intake. Further investigation is necessary to prove the mutual relations between BMD, exercise, and calcium intake. Therefore, middle-aged women will need proper exercise as well as Ca supplementation in order to prevent osteoporosis with aging.

Usefulness of Bone SPECT/CT for Predicting Avascular Necrosis of the Femoral Head in Children with Slipped Capital Femoral Epiphysis or Femoral Neck Fracture

  • Objective: This study aimed to investigate the usefulness of bone single-positron emission tomography/computed tomography (SPECT/CT) of the hip in predicting the later occurrence of avascular necrosis (AVN) after slipped capital femoral epiphysis (SCFE) or femoral neck fracture in pediatric patients. The quantitative parameters of SPECT/CT useful in predicting AVN were identified. Materials and Methods: Twenty-one (male:female, 10:11) consecutive patients aged < 18 years (mean age ± standard deviation [SD], 11.0 ± 2.7 years) who underwent surgery for SCFE or femoral neck fracture and postoperative bone SPECT/CT were included. The maximum standardized uptake value (SUV), mean SUV, and minimum SUV of the femoral head were measured. The ratios of the maximum SUV, mean SUV, and minimum SUV of the affected femoral head to the contralateral side were determined. Patients were followed up for > 1 year after the surgery. The SPECT/CT parameters were compared between patients who developed AVN and those who did not. The accuracy of SPECT/CT parameters for predicting AVN was assessed. Results: Six patients developed AVN. There was a significant difference in the ratio of the mean SUV among patients who developed AVN (mean ± SD, 0.8 ± 0.3) and those who did not (1.1 ± 0.2, p = 0.018). However, there were no significant differences in the ratios of the maximum and minimum SUV between the groups (all p = 0.205). For the maximum, mean, and minimum SUVs, no significant differences were observed between the groups (p = 0.519, 0.733, and 0.470, respectively). The cutoff mean SUV ratio of 0.87 yielded a 66.7% sensitivity and 93.2% specificity for predicting AVN. Conclusion: Quantitative bone SPECT/CT is useful for evaluating femoral head viability in pediatric patients with SCFE or femoral neck fractures. Clinicians should consider the high possibility of later AVN development in patients with a decreased mean SUV ratio.

Cervical lymph node metastasis form non-head & neck sites: below diaphragm (횡격막 아래 장기에서 발생한 악성 종양의 경부림프절 전이 사례 분석)

  • Background/Objectives: Cancers of the abdominal or pelvic organ rarely metastasize to the cervical lymph nodes. Although it indicates distant metastasis, perceivable prolongation of survival or cure may be possible in selected cases. We sought to identify patients with cervical metastasis from cancers below the diaphragm and identify patients who may benefit from aggressive treatment. Materials & Methods: From 2009 to 2017, patients with pathologically confirmed metastatic cervical lymph nodes originating from below the diaphragm were included for analysis. Patient demographics, cancer characteristics, treatment course, and clinical outcomes were analyzed. Results: 208 patients were identified. Left supraclavicular node (Virchow's node) was the most frequently involved. Irrespective of treatment, survival for uterine cervical and ovarian cancers was significantly longer than that of other primaries. Patients with isolated cervical metastasis (oligometastasis) had significantly longer median survival compared to patients with concomitant bone, lung, brain, and mediastinal metastases. Conclusion: Although cervical metastasis from cancers of the abdominal and pelvic organ represent distant metastasis, patients with uterine cervix and ovary primary and oligometastatic lesions may benefit from aggressive treatment. Prudent patient selection and further investigation is warranted.

  • Legg-Calve-Perthes diseases were diagnosed in 2 dogs showing signs of lameness. In radiographs, the abnormal size and shape was due to the collapse of the left capital bone and enlarged femoral neck was seen at case I. The completely deformed left femoral head resulted from the earlier collapse of the osteolytic bone of the caput femoris at case II. In treatment, femoral head ezcision was performed in one dog.
