Purpose: The study was done to investigate the effect of ethanol inhalation on postoperative nausea in patients using Patient Controlled Analgesia (PCA). Methods: The data were collected from June 1st 2006 to September 30th, 2007. The subjects were 70 patients who had had orthopedic surgery under general anesthesia. The levels of the Visual Analogue Scale (VAS) was used to measure postoperative nausea. The experimental group was given ethanol inhalation using ethanol pads and the control group received normal saline pads. All participants were instructed to take two deep sniffs with the pad one inch from the nose. This was repeated every five minutes for three doses. The collected data were analyzed using SPSS/WIN 12.0 program. Results: The study supported all hypotheses. "The experimental group given first dose of ethanol inhalation would have a lower level postoperative nausea compared to the control group"(t = -5.900, p = .000). "The experimental group given second doses of ethanol inhalation would have a lower level postoperative nausea compared to the control group"(t = -7.507, p = .000). "The experimental group given third doses of ethanol inhalation would have a lower level postoperative nausea compared to the control group"(t = -6.685, p = .000). Conclusion: According to these results, the ethanol inhalation can be considered an effective nursing intervention for relieving the postoperative nausea in patients using PCA.

Purpose: The purpose of this study was to examine the effect of three kinds of modes using bolus button of PCA on level of pain and side effects of analgesic and amount of drug consumption in post-operative patients according to whether the medication is controlled by the patient, the caregiver or the nurse. Method: The participants were 684 patients using PCA after an operation. The data collection period was from March 19 to April 6, 2007. Results: It was found that there were statistical differences in gender, age, type of surgery, pain on first post-operative day, amount of drug consumption, nausea, and vomiting. The ratio for patient controlled medication was 55.7% for women, and 70.5% for men, and for care-giver controlled medication, 35.1% for women, and 20.0% for men. Average pain scores for the first post-operative day were $3.9{\pm}2.2$ for patient controlled medication and $4.5{\pm}2.3$ for care-giver controlled medication. There were statistical differences according to mode used for PCA for amount of drug consumptions, nausea and vomiting but not for pain, operation day or pruritus. Conclusion: This study was carried out to examine risks according to who controls the PCA for post-operative patients. The results can help to develop education program for everyone who is involved in PCA, patients, caregivers, nurses and doctors.

Background: The purpose of this study was to compare the availability of propofol and fentanyl (P + F) with diazepam and morphine (D + M) for intravenous conscious sedation during third molar surgery. Methods: Forty patients without systemic disease were operated under IV conscious sedation administered by either of the two techniques. Monitoring consisted of continuous observation of pulse rate, blood pressure, oxygen saturation, and the respiratory rate and were recorded every 15 minutes. Cooperation score was measured 5 and 15 minutes after induction of IV sedation. Following the operation, the surgeon and patients completed questionnaires including pain visual analog scale, amnesia, and side effects. Results: The P + F group was significantly more cooperative than the D + M group. The side effects of D + M group included pain on injection, nausea/vomiting and abdominal pain. The side effects of P + F group included talkativeness, nausea/vomiting, temporary apnea, pain on injection and vertigo. Conclusions: In this study, there were following benefits in the P + F group; more cooperative status and less pain perception. But respiratory depression developed in some patients.
Medial medullary infarction is caused by occlusion of vertebral artery or lower basilar artery. In this report, one case had impaired pain and thermal sense over half the body, and complained of dizziness, nausea and vomiting. The other symptoms were slippery pulse(脈滑), pale tongue with whitish coating(舌淡苔白), white face(面白), obesity(體肥) and unchanged skin color(肌色如故). We diagnosed this patient as the Gastrointestinal Phlegm(食痰) and prescribed Jengjengamiyijin-tang (Zhengchuanjiaweierchen-tang). The symptoms of impaired pain and thermal sense, dizziness, nausea, and vomiting were improved. So, we suggest that Jengjengamiyijin-tang (Zhengchuanjiaweierchen-tang) could be effective to the patient with the symptom of the Gastrointestinal Phlegm(食痰)
Background: Opioid analgesics are widely used to reduce postoperative pain and to enhance post-operative recovery. However, orthostatic intolerance (OI) induced by opioid containing intravenous patient controlled analgesia (IPCA) may hinder postoperative recovery. This study investigated factors that affect OI in patients receiving IPCA for postoperative pain control. Methods: OI was instantly evaluated at the time of first ambulation in 175 patients taking opioid containing IPCA after open and laparoscopic subtotal gastrectomies. Patients were classified as having OI if they experienced dizziness, nausea/vomiting, blurred vision, headache, somnolence and syncope. Factors contributing to OI were assessed with logistic regression analysis. Results: Out of 175 patients, 61 (52.6%) male and 44 (74.6%) female patients experienced OI at the time of first ambulation. The frequency of OI related symptoms were dizziness (97, 55.4%), nausea (46, 26.3%), headache (9, 5.1%), blurred vision (3, 1.7%) and vomiting (2, 1.1%). Significant risk factors for OI were gender (P=0.002) and total amount of opioids administered (P=0.033). Conclusions: The incidence of OI is significantly higher in male than in female patients and is influenced by the opioid dose.
Background and objective : Postoperative nausea and vomiting(PONV) are frequent complications of general anesthesia in children. High cost and associated side effects of antiemetic drugs, have increased interest in nonpharmacological methods for treatment of PONV. The aim of this study was to report the effect of acupuncture for prevention of PONV in children. Material and Method : Randomized clinical trials retrieved by a search of articles indexed on the Medline and NDSL database. Key words to search were 'PONV', 'POV', 'acupuncture', 'children', 'pediatric'. Results : Eleven articles were selected and six of them showed significant effectiveness of acupuncture in preventing PONV in children. Acupuncture methods were needle acupuncture acupressure, electro-acupuncture, laser acupuncture, acupuncture point injection. The main acupuncture point was P6(Neiguan). Conclusion : The effectiveness of acupuncture for prevention of PONV in children has been shown in some, but not all trials. Further studies are needed.
Objectives This study is to report a case that has an important meaning as a result of treating functional dyspepsia with oriental treatment. We investigated functional dyspepsia in 6 years child who had to continue abdominal distention for a month after gastroenteritis. The child has recovered from all dyspepsia symptoms after treating with oriental medicine. Methods The patient had dyspepsia symptoms all day, especially repeated abdominal distention. He sometimes had nausea, vomiting, diarrhea, abdominal pain. For those symptoms, we treated him with herbal medicine and acupuncture. The aim of treatment was to improve functional dyspepsia without western treatment. We measured patient's abdomen circumference for change of abdominal distention. Results The symptoms of functional dyspepsia were vanished and the patient maintains his condition with oriental medicine treatment without western treatment. After herb medicine treatment and acupuncture treatment the patient's dyspepsia symptoms (nausea, vomiting, diarrhea, abdominal pain, and abdominal distention) were vanished and maintained his condition for two months after complete the treatment. Conclusions This study showed that oriental medicine can elevate the functional dyspepsia children's quality of life with continuous health care and treatment. For more accurate studies, further studies would be needed with more cases.
This study was conducted to develop and evaluate guidelines for cancer patients' symptoms management such as nausea/vomiting, fatigue, constipation, diarrhea, and oral mucositis. Based on the literature review, assessment path to identify each stage of five symptoms were also developed. Guidelines for symptom management of each stage of the symptoms were developed. Guidelines then were evaluated by a panel of experts. Finally, 95 cancer patients were recruited and asked to use the guidelines for their symptom management. Levels of understanding of and satisfaction with assessment path and management guidelines were surveyed. Prevalence rate of five symptoms varied ranging from 20% (diarrhea) to 47% (nausea/vomiting). Regarding the level of understanding of each symptom most of the cancer patients indicated that they were easy and sufficient. Regarding the easiness of use of the symptom management guidelines, most of cancer patients indicated that they were easy to use. Regarding the nursing intervention on each symptom, most of cancer patients indicated that they were easy and helpful. More information was added with feedback from the patients. The result of this study has implications on development of customized patient education materials based on assessment path and symptom management guidelines.
Purpose: To investigate short-term response rate, quality of life and toxicities of mannan peptide combined with TP regimen in treating patients with non-small cell lung cancer (NSCLC). Patients and Methods: Forty one patients with NSCLC were divided into an experimental group treated with TP regimen combined with mannan peptide (21 patients) and a control group treated with TP alone (20 patients). Results: Response rates were 61.9% (13/21) for the experimental and 60% (12/20) for the control group (p>0.05). Regarding toxicity, white blood cell decreased more frequently in the control group (65%, 13/20) than in the experimental group (33.3%, 7/21) (p<0.05); nausea and vomiting also occurred more frequently in the control group (55%, 11/20 vs 23.8%, 5/21) (p<0.05). In terms of quality of life, this index was improved by 57.1% (12/21) and 25% (5/20) in experimental and control groups, respectively (p<0.05). Conclusions: Response rate of TP after combined with mannan peptide is mildly increased, while this combination alleviates bone marrow suppression as well as nausea and vomiting of TP, and improves quality of life when treating patients with NSCLC. However, this conclusion should be confirmed by randomized clinical trails.
Purpose : This study was conducted to determine the relationships between conditioned variables and the quality of life in the gastric cancer patients receiving chemotherapy after surgery. Method : 56 gastric cancer patients receiving chemotherapy included Cisplatin and 5-Fu were recruited from one university hospital located in Seoul. Questionnaires for this study were Quality of life Index developed by Padilla et al. at 1983, Rhode's nausea and vomiting scale and tool for side effects after chemotherapy at home developed by authors. Result : Quality of life in this study was correlated significantly with age, stage of cancer, nausea and vomiting, and level of side effects of chemotherapy. Quality of life of woman was lower than that of man. Conclusion : It is suggested that nurses be reinforce nurse's discharge education for the patients about increasing oral intake and preventing side effect after discharge from hospital and pay more attention to the change of the quality of life and relevant variables among the gastric cancer patients.
