Effects of sleep quality and occupational stress on health-related quality of life among a university's security workers (일개 대학 경비근로자의 수면의 질과 직무스트레스가 건강관련 삶의 질에 미치는 영향)

  • Objectives: The purpose of this study was to identify the effects of sleep quality and occupational stress on health-related quality of life among a university's security workers. Methods: 128 security workers voluntarily participated in the study. The survey asked demographic, health related, job related factors, the Korean Occupational Stress Scale Short Form(KOSS-SF), sleep quality questionnaire, and 12-Short Form Health Survey version 2.0(SF-12v2). The data were analyzed by using descriptive analysis, t-test, ANOVA, multiple linear regression, and multiple logistic regression. Results: The interrupted rest time was significantly negatively related to occupational stress. The occupational stress significantly affected sleep quality. The significant variable affecting Physical Component Score(PCS) and Mental Component Score(MCS) was sleep quality and occupational stress, respectively. Conclusions: HRQOL was founded to be affected by occupational stress and sleep disorder among the security workers. Based on this study, we suggest that the management of both occupational stress and sleep quality is crucial to improve the HRQOL.

The Effect of Reform of New Diagnosis-Related Groups on Coverage of National Health Insurance (신포괄수가 시범사업 모형개선이 건강보험 보장률에 미친 영향)

  • Background: Korea set up a new diagnosis-related group as a demonstration project in 2009. The new diagnosis-related group was reformed in 2016. The main purpose of the study is to identify the effect of reform on coverage of national health insurance. Methods: This study collected inpatient data from a hospital that contains medical information and cost from 2015 July to 2016 June. The dependent variable was the coverage of national health insurance. The dependent variable was divided by total, internal medicine partition, surgical partition, and psychiatric partition. To analyze the effect of the reform, this study conducted an interrupted time series analysis. The final sample included 23,695. Results: The health insurance coverage of internal medicine has the highest, followed by surgery and psychiatry. The health insurance coverage of bundle payment is higher than that of unbundled payment. The proportion of bundled payment and non-benefit decreased and the proportion of unbundled payment increased. The coverage of national health insurance significantly increased after policy reform in internal medicine partition (p-value=0.0356). Conclusion: The results of the study imply that policy reform enhanced the coverage of national health insurance in internal medicine. The government needs to monitor side effects such as an increase of unbundled payment.

Factors Influencing the Smoking Behavior of Adolescents (청소년 흡연행위 영향요인)

  • Objectives: The purpose of this study was to investigate factors influencing the smoking behavior of adolescents, in order to provide basic data to develop a future nursing intervention program for smoking prevention. Methods: The study subjects were 162 adolescents attending high schools, who were living in K city. The instruments included the Self Esteem Scale translated by Jeon (1974), beliefs about the social rule scale developed by the Committee for Adolescence Guidance (1988), differential peer association developed by Krohn et. al. (1982), perceived behavioral control scale developed by Hanson (1997), intention of smoking scale developed by Newman et. al.(1982), and self-efficacy scale developed by Sherer et. al. (1982). The data were analyzed using descriptive statistics, Pearson correlation coefficient, and stepwise multiple regression. Results: 1. The smoking behaviors of the subjects were significantly correlated with beliefs about social rule, perceived behavioral control. differential peer association, intention of smoking, self efficacy, grade, father's level of education, monthly pocket money, time of onset for smoking, degree of alcoholic intake, and drug abuse. 2. The multiple regression analysis revealed the most powerful predictor for smoking behavior was time of onset for smoking. A combination of beliefs about social rule, perceived behavioral control, grade, differential peer association, and intention of smoking accounted for 54.0% of the variance for smoking behavior in adolescents. Conclusion: It is recommended that these influencing factors for smoking behavior be considered when developing future nursing intervention programs for the antismoking behaviors of adolescents.

An Analysis of the Trends of Korean National Health Insurance Administrative Cost (건강보험 관리운영비 추이 분석)

  • Social solidarity, equity in financing, and efficiency in administration have been core issues in the development of Korean health insurance reformation since 1988. This study is to investigate the trend of administrative cost in Korean National Health Insurance from various aspects. For the analysis of administrative cost, the expenditures of each insurance society and the National Health Insurance Corporation are divided into 4 items of (1) insurance benefit, (2) administrative cost, (3) an agency provision accounts, (4) other expenses, and then they are reorganized. The analyses based on 5 types of the health insurance administrative cost showed that efficiency in administration has been improved generally. We, however, should consider qualitative aspects such as customer's satisfaction with health insurance administration, prompt service, control of unjust expenditure (unjust claims), and provision of medical service including health consultation in assessing efficiency of administration. And, in order to connect the administrative costs of health insurance with efficiency, we need to give a fundamentally new definition, which can contain elaborateness of expenditure in details including the structure and evaluation method of administrative costs. It may be necessary to develop new indicators or analyzing methods hereafter.

A Study of Hospital Foodservice Management after Covering Hospital Foodservice in The National Health Insurance (식대 급여화에 따른 입원 환자 병원 급식 실태 조사)

  • The purpose of this study is to investigate the overall operations of National Hospital Food service after it was benefited by National Health Insurance (NHI). The survey was conducted between July and August, 2007. Among questionnaires mailed to 2,558 medical care institutions, 2,090 returned (81%) questionnaires were analyzed by descriptive statistics, $x^2$-test and ANOVA using the SPSS 13.0. The general foodservice characteristic of medical care institutions were as follows. The type of foodservice operations were 'self-operated' (86.9%), 'contracted' (10.5%) and 'Both' (2.6%). Only 6.4% of medical care institutions provided 'hospital food menu not benefited by NHI'. The number of dietitians and cook for medical care institutions were 1.1 and 1.0, respectively. The cost of a general diet meal was 4,205 won and therapeutic diet meal was 4,434 won. The overall operations of hospital foodservice were different depending on the types of medical care institution. After hospital foodservice was benefited by NHI, the overall quality of hospital foodservice including manpower, facilities, and environment was improved. The future direction of hospital foodservice should 1) differentiate the cost of hospital foodservice by the types of medical care institution, 2) increase in co-payment, and 3) provide same service with equal expenses in each party as medical aid or NHS beneficiary.

Korea National Oral Health Survey Systems and recent oral health status in children (한국 공공의료 구강건강조사 체계 및 아동의 구강건강 현황)

  • This study aimed to introduce what Korea national oral health surveys are and examine recent oral health status in Korean children. Korea Government has implemented national oral health-related surveys during the past two decades. The surveys with oral health examination are Korean Children's National Oral Health Survey (KCNOHS) since 2000 in 5-year-old and 12-year-old children and Korea National Hhealth and Nutrition Examination Survey (KNHANES) since 2007 aged 1 year and more. KCNOHS provides the information about the distribution of prevalence of dental caries, gingival health, behavioral factors, and dental clinic demands etc. KNHANES has huge amount of health data for representative Korean population including oral health and general conditions. Based on KCNOHS, the prevalence of dental caries and its experience has decreased steadily, and the status of oral hygiene and preventive treatment called sealents has been also improved after the year of 2000. However, there is still a gap to reach to those of Western developed countries. Therefore, more effective oral health policy plans and strategies for Korean children and adolescents are needed to prevent and manage for dental caries in private and clinical field of dentistry with public sector.

The Recognition of Groups between the Korean National Health Insurance and the Private Health Insurance (국민건강보험과 민영의료보험에 대한 집단 간의 인식도 분석)

  • The purpose of this study is to provide basic information for improving medical security between the Korean National Health Insurance Corporation and the private health insurance in Pusan Metropolitan area by investigating of the recognition with the benefit groups. Questionnaires of 431 were taken between Feb. 11th and Feb. 25th 2008. The survey was carried out to gather information about SES and contents of insurance and recognition between the benefit group of the Korean National Health Insurance and the private health insurance on the financial conditions and the stability. The result of survey is as follows. First, there is a difference between the National Health Insurance and the private health insurance on the financial conditions and the stability of the Korean National Health Insurance. Second, there is a high score at the private health insurance on the financial conditions and the stability of the private health insurance. Third, privatizing of a National Health Insurance is high score at increase of the premium, medical payments of the National Health Insurance group and is high score at enhance of quality of medical service and decrease of loss of medical payment of the private health insurance group. Therefore, to provides more information and improved medical security with the benefit group. it is necessary for concerns to put more efforts in creating Conflicting vs. Complementing of systemic base.
