부산지역 의원급 외래 노인 골관절염환자의 비스테로이드소염제 사용양상평가 (Nonsteroidal Anti-inflammatory Drugs Utilization Patterns among the Elderly with Osteoarthritis at Primary Ambulatory Care Units in Busan Metropolitan City, Korea)

  • Objectives : To investigate the utilization patterns of non-steroidal anti-inflammatory drugs (NSAIDs) among the elderly with osteoarthritis (OA) undergoing primary ambulatory care in Busan metropolitan city, Korea. Methods : OA patients, aged 65 years and over, were identified from the Korean National Health Insurance Review Agency drug prescription database. The subjects had at least one episode of claim for OA (ICD-10-CM: M15-M19) between August 1, 2000 and February 28, 2002. Trends in the determinations of NSAIDs utilization were identified using chi-squared tests for trend. Results : There were 47,711 osteoarthritic patients. The total number of visits by these patients was 177,443, with a total frequency for NSAID prescriptions of 214,952. Seventy-nine percent of the OA patients were female. NSAIDs were prescribed on 133,284 visits (75.1%) and the proportion of prescriptions was significantly increased with age. Only the proportion of visit when NSAIDs were prescribed decreased, from 65.1 to 43.5%, during the study period (p<0.001). However, the proportion of combined treatments with anti-ulcer drugs was increased. The use of NSAIDs injections was decreased. Of the individual NSAIDs, diclofenac (28.7% of total frequency of NSAID prescriptions), piroxicam (15.0%) and talniflumate (8.7%), were the most frequently prescribed. Among the NSAIDs prescribed OA visits, 45.7% used two or more NSAIDs. Conclusion : The total proportion of NSAIDs prescribed to the osteoarthritic patients was higher than in other studies. The decline in the use of NSAIDs during the study period, and the frequent selection of safer medications, such as combination therapy with anti-ulcer drug, may reflect the risk awareness of the use of NSAIDs.

아스피린과 NSAIDs 과민성 풍열형(風熱型) 두드러기에 대한 방풍통성산가미방(防風通聖散加味方)의 치험례 (A Case Report on a Patient with Wind-Heat Type Urticaria Sensitive to Aspirin and NSAIDs Using Bangpoongtongsung-san Gamibang)

  • Objectives : The purpose of this case study is to report the effectiveness of Bangpoongtongsung-san gamibang on a patient with wind-heat type urticaria sensitive to aspirin and NSAIDs. Methods : We treated a 48-year-old man who had aspirin and NSAIDs sensitive wind-heat type urticaria with Bangpoongtongsung-san gamibang, pharmacopuncture(Hwangnyunhaedok-tang), acupuncture(needle remaining time : 20 min) and photodynamic therapy. Results : The Urticaria Activity Score(UAS) started at 5 and ended with 0, and the Visual Analog Scale(VAS) of heating sensation on whole body started at 10 and ended with 1-3. Conclusions : This study has demonstrated that Bangpoongtongsung-san gamibang could be effective to treat aspirin and NSAIDs sensitive wind-heat type urticaria.

  • Objective : This study describes a case of urticarial drug eruption assumed to be caused by non-steroidal anti- inflammatory drugs (NSAIDs) treated with oriental medicine and acupuncture. Methods : We studied the case of 20-years-old drug eruption patient. The patient complained of urticaria, angioedema, itching and burning sensation after taking NSAIDs. The indications for NSAIDs prescribed were common cold symptom and sore throat. For diagnosis, blood test and urinalysis were done. For treatment, acupuncture and herbal-medicine treatment were applied. Results : The patient of this report were examined for 13 days. The radiological finding was within normal limits. The WBC, neutrophil, lymphocyte and eosinophil level normalized during admission. After 3 days, itching and wheal reaction of whole body disappeared except facial symptom. The angioedema, itching and wheal reaction in the face was almost improved in the 4th day after discharge. Conclusions : We may expect that the oriental medical therapy improves the urticarial drug eruption caused by NSAIDs.

  • Many epidemiological studies have revealed that the use of aspirin or other non-steroidal anti-inflammatory drugs (NSAIDs) can reduce the risk of colon cancer. Since the well-documented pharmacological action of aspirin and other NSAIDs is the inhibition of cyclooxygenase [COX, the rate-limiting enzyme in prostaglandin (PG) biosynthesis], it has been inferred that the beneficial effect of NSAIDs may be mediated through the inhibition of PG biosynthesis.(omitted)

  • Many of the chiral NSAIDs are marketed as racemates. There is an increasing interest in developing the enantiomerically pure forms of the NSAIDs because the anti-inflammatory activity of NSAIDs have previously been shown to be largely sterospecific for the (S)-(+)-enantiomer. Therefore, simple and economic preparative method to identify the (S)-(+)-enantiomer of NSAIDs (aryl propionic acids) as diastereomeric solvation complex was developed using several chiral solvating agents by recrystallization of racemate and solvent fractionation.   • Objectives This study is to examine case that has an significant result on treating Polyarticular Type Juvenile Idiopathic Arthritis(JIA) the patient who did not response to DMARDs and NSAIDs with oriental medicine. JIA is the most common form of persistent arthritis in children before age 16. The symptoms of JIA is pain and edema of joints, fever, lethargy, reduced physical activity, and poor appetite. But the symptoms vary. Methods The patient felt pain and had limitation of mobility on his joints, but the DMARDs and NSAIDs were not effective. Therefore, he was treated with oriental medicine and withdrew from DMARDs and NSAIDs. The patient took DMARDs and NSAIDs from May 2007 to November 2008(He did not took DMARDs and NSAIDs from December 2007 to June 2008), and took oriental medicine from November 2008 until now. Results The patient's pain and limited mobility on his joints, edema, morning stiffness, fatigue was gone, and could maintain the condition. He doesn't have DMARDs or any other western medicine anymore. Conclusions This study shows that a Polyarticular Type Juvenile Idiopathic Arthritis patient who had ineffective response to DMARDs and NSAIDs can reach to the clinical remission by oriental medicine treatment. His morning stiffness, fatigue, pain of joints, edema was gone and ESR was in normal range with Oriental medicine treatment.

  • Verapamil is a calcium channel blocker and is classified as a class IV anti-arrhythmic agent. It is used in the control of supra ventricular tachyarrhythmias, and in the management of classical and variant angina pectoris. It is also used in the treatment of hypertension and used as an important therapeutic agent for angina pectoris, ischemic heart disease, hypertension and hypertrophic cardiomyopathy. Verapamil commonly co-administered with NSAIDs (non-steroidal anti-inflammatory drugs) i.e. diclofenac sodium, flurbiprofen, Ibuprofen, mefanamic acid and meloxicam. A simple and rapid RP-HPLC method for simultaneous determination and quantification of verapamil and NSAIDs was developed and validated. The mobile phase constituted of acetonitrile: water (55:45) whose pH was adjusted at 2.7 and pumped at a flow rate of 2.0 mL $min^{-1}$ at 230 nm. The proposed method is simple, precise, accurate, low cost and least time consuming for the simultaneous determination of verapamil and NSAIDs which can be effectively applied for the analysis of human serum.

  • The present study was designed to evaluate effects of the commonly used NSAIDs(acetaminophen, aspirin, and ibuprofen) on human periodontal ligament cells. Human periodontal ligament cells were grown from a cell line provided by Kyungpook National University. Effects of NSAIDs on the proliferation of human periodontal ligament cells were assessed using MTT assays. And then $PGE_2$ concentrations were determined by ELISA and the changes of cellular configuration were found by electron micrograph. The results were as follows; 1. The MTT assay demonstrated that the commonly used NSAIDs(acetaminophen, aspirin, and ibuprofen) had not significant cytotoxic effect on human periodontal ligament cells. 2. NSAIDs inhibited the $PGE_2$ synthesis of human periodontal ligament cells compared with the control group. These inhibitory effects had no significant differences with NSAID type and concentration. 3. Electron micrographs of human periodontal ligament cells treated with NSAIDs showed more narrow and irregular shape.

  • 비스테로이드 소염제(non-steroidal anti-inflammatory drugs, NSAIDs)는 세계적으로 관절염과 같은 만성 통증에 가장 흔히 사용되는 약물이며 성분 및 기전에 따라 다양한 종류가 있다. 장기간 사용 시에는 소화기계 및 심혈관계 합병증 등의 다양한 부작용이 발생할 수 있는 것으로 알려져 있다. 기존의 비선택적 NSAID와 유사한 진통 효과를 지니면서 위장관계 내약성을 보완해줄 수 있는 cyclooxygenase-2 (COX-2) 선택적 NSAID가 많은 기대를 모았으나 2004년 및 2005년도에 심혈관계 안전성에 대한 우려로 rofecoxib과 valdecoxib의 허가가 취하되면서 NSAID 약물의 부작용에 관한 관심은 더욱 증가하고 있다. 따라서 각 약물의 부작용 및 상호작용을 고려하여 필요한 약물을 최소한으로 사용하는 것이 매우 중요하다. 본 논문에서는 NSAID 약물을 복용할 때 발생할 수 있는 부작용 및 각 약물의 특성을 숙지하며 비선택적 NSAID 및 COX-2 선택적 NSAID의 사용과 관련된 최근 연구 및 지침에 대해 알아보고자 한다.

노인환자에서 위장관계 및 심혈관계 부작용 발생 예방을 위한 NSAIDs 사용의 적절성 평가 (Evaluation of Proper Use of NSAIDs to Prevent Gastrointestinal and Cardiovascular Problems in Elderly Patients)

  • Background: Elderly patients with gastrointestinal (GI) and cardiovascular (CV) risk factors may be more easily exposed to NSAID-related side effects (SEs). Based on the ACG guideline of year 2009, the aim of the study is to evaluate proper use of NSAIDs and gastroprotective drugs according to the degree of GI and CV risk strengths in the patients. Methods: Retrospectively surveyed 410 elderly patients with NSAIDs for more than 30 days at a general hospital in Korea. GI risk factor includes age, ulcer history, high-dose NSIADs, concurrent aspirin use, steroids or anticoagulants. CV risk factor includes angina, myocardial infarction, cerebral infarction, atrial fibrillation or coronary intervention requiring low-dose aspirin. These factors were classified as high/low cardiovascular groups and high/moderate/low GI groups. Results: There were 14 patients in high CV risk group and high GI risk group. The group was recommended not to use NSAIDs as it is not adequate. There were 101 patients in high CV risk group and moderate GI risk group. This group was recommended to use naproxen and PPI/misoprostol. But all patients except one were not adequate. There were 9 patients in low CV risk group and high GI risk group. This group was recommended to use selective COX-2 inhibitor and PPI/misoprostol. 5 cases were proper while 4 cases did not. There were 285 patients in low CV risk and moderate GI risk group who were recommended to use non selective NSAIDs and PPI/misoprostol or selective COX-2 inhibitor only. 103 patients were proper while 182 patients not adequate. Overall, the SEs were higher in those cases for inadequate use of drugs comparing to the adequate. CV SEs were statistically significant. However, SEs for each risk groups were different. For the case of low CV risk group and high/moderate GI risk group, the inadequate use of drugs makes the SE high and the other groups are not. Also, it was not statistically significant. Conclusions: In elderly patients, the inappropriate use of NSAIDs can increase the risk of the disease. Therefore, GI and CV risk must be considered simultaneously, and the proper use of NSAIDs and gastroprotective drugs for each risk groups should be reconsidered.
