Preliminary Study of Ambulation Training on Electromechanical Gait Trainer in Stroke Patients (전동식 보행 훈련기를 이용한 뇌졸중 환자 보행훈련의 사전연구)

  • Purpose : The purpose of this study was to investigate the effect of electromechanical gait trainer therapy in stroke patients. The gait trainer was designed to provide nonambulatory subjects the repetitive practice of a gait-like movement without overstraining therapist. To simulate normal gait, discrete stance and swing phase, lasting 60% and 40% of the gait cycle respectively, and the control of the movement of the centre of mass were required. Methods : This preliminary study investigated during 8 weeks therapy on the gait trainer could improve gait ability in 5 subacute and chronic hemiparetic stroke patients. Gait ability(time up & go [TUG], comfortable and maximal gait speed and functional ambulation category[FAC]), functional movement of lower extremity(Fugl-Meyer Assessment [FMA] and composite spasticity score [CSS]) and sensory of lower extremity(Fugl-Meyer Assessment sensory [FMA-s])were the measured. Results : TUG, comfortable and maximal gait speed and FMA were improved significantly. Although FAC, FMA-s and CSS were improved, there were not statistically significant. Conclusion : Therefore, the gait trainer enabled affected patients the repetitive practice of a gait-like movement, which is important for the restoration of walking ability.

The Effect of Their Sense of Depression and Suicidal Thinking for Managerial Characteristics in Hypertense and Diabetic Patients (고혈압과 당뇨병 환자의 관리적 특성이 우울과 자살생각에 미치는 영향)

  • This study aims to analyze the effect of hypertense, diabetic patients, managerial characteristics on their depression and suicidal thinking. Thus, this study selected 6,217 adults over the age of 19 from the 5th first-year data of Korean National Health and Nutrition Examination Survey conducted by KCDC(Korea Centers for Disease Control and Prevention) in 2010. As factors having effect on their suicidal thinking, unmarried patients(p=.000), middle school(p=.045), high school(p=.015), college and higher(p=.018), medicare(p=.045), hypertense(p=.042), exercise walking irregularly(p=.021), sense of depression(p=.035) showed a higher rate of suicidal thinking which were found to be statistically significant variables. Hypertense show a higher rate of suicidal thinking than normal people. Accordingly, it is necessary not only to pay attention to establishing plans to provide Hypertense and Diabetic patients with medical treatments, but to have more concern about their mental health so that they may be provided with a political support future.

Development of a Stance & Swing Phase Control Transfemoral Prosthesis (입각기와 유각기 제어 대퇴의지의 개발)

  • In this study, a transfemoral prosthesis system of which stance phase and swing phase are controlled during walking has been developed for the recovery of the biomechanical function of the amputated leg. It consists of a 5 bar link mechanism, a hydraulic-rubber knee damper for stance phase control and a pneumatic cylinder controlled via a microprocessor for stance phase control. The mechanical characteristics and behaviour of the knee damper which absorbs the impact energy generated at the heel contact was investigated. The characteristics of the pneumatic cylinder essential for the speed adaptation of the prosthesis during swing phase was also studied for its mechanical characteristics. The prosthesis was subject to the clinical test ant the gait characteristics obtained were very close to those of normal. The stance and swing controlled prosthesis that were developed in this study showed good stability during the stance phase and showed good controllability during the swing phase.

A Study of the Limits of Stability in Hemiplegic Patients (편마비환자의 안정성한계에 대한 연구)

  • The purpose of this study was to evaluate and compare the Limits of stability(LOS) in hemiplegic patients who can walking independently. The LOS was measured at stable surface, unstable surface with eye open and eye closed. In this study, 18 out-patients were evaluated who were treated at Yonsei University Medical Center Rehabilitation Hospital. In order to determine the statistical significance of results, T-test, paired t-test, and Kruskal-Wallis 1-way ANOVA were applied at 0.05 level of significance. The results were as follows: 1. The mean of lateral limits of stability was 9.89 degree. 2. The mean of anteroposterior limits of stability was 6.43 degree. 3. There was a significant difference of limits of stability between sound side and affected side(p<0.05). 4. The limits of stability was significantly decreased with eye closed(p<0.05) 5. The limits of stability was significantly decreased at unstable surface(p<0.05). 6. The limits of stability was a significant difference as spasticity degree of ankle plantar flexors(p<0.05). These results showed that the limits of stability in hemiplegic patients was more decreased than that of normal adult. In order to improve the balance in hemiplegic patients, we need to increase the limits of stability.

  • In the present study. the design of multi-axial lower extremity orthosis was presented with adduction and abduction force data which were measured from three children with cerebral palsy and a normal child. The measurements of adduction and abduction forces were conducted in standing and wolking condition. Adduction and abduction forces were measured by strain gages which were attached on the lateral uprights of lower extremity orthoses. In the standing condition. addcution force of childrens was distributed from 0.11 kgf to 0.26 kgf. During the walking condition. adduction force was reached to 1.56 kgf and abduction force was reached to 1.52 kgf.

  • Purpose: Hypertension tends to increase after the age of 65. Proper hypertension management requires physical activities such as habitual activities and exercise. However. it is reported that many elders do not have regular physical activities. This study was to identify physical activity patterns and blood pressure according to physical activity in the elderly with hypertension. Methods: This study assessed physical activity frequency, amount, type during the latest one week, and blood pressure and BMI. The subjects were 53 elders aged over 65 years. Collected data were analyzed through descriptive statistics, $x^2-test$, and logistic regression analysis using SPSS 12.0. Results: It was found that most of the subjects had not been participating in regular physical activities. The most frequent physical activity patterns were walking and home activity at a low or moderate intensity. The elderly who met the normal hypertension and pre-hypertension criteria involved significantly more regular physical activities. Conclusion: This study suggested that regular participation in physical activities should be recommended for elders in order to optimize the state of their blood pressure.

Diagnosis of Anaplasmosis by PCR and Long Term Medication in a Dog (PCR을 이용한 개의 아나플라즈마병 진단과 장기간 치료)

  • A 4-year-old female Shunauzer dog was referred to the Veterinary Teaching Hospital of Chungbuk National University due to anorexia and depression. The dog had a history of regular walking on grass fields, weight loss, and hyperthermia $(40.6^{\circ}C)$. In the physical examination, lymph node enlargement was confirmed. Complete blood count result revealed leukocytosis and thrombocytopenia but there was no decreasing of red blood cells. On blood chemistry, serum ALP, GGT, CPK, and LDH were elevated. Abdominal radiograph showed splenomegaly. Anaplasma platys infection was suspected with inclusion body-like substances in platelets on blood smear. Anaplasma platys was confirmed by PCR. On the basis of laboratory examination, final diagnosis was anaplasmosis. Treatment was followed for 3 months with tetracycline and doxycycline. The patient was monitored every week during the treatment. The patient has recovered to normal condition without any clinical signs. We are going to emphasize the need of PCR technique in diagnosis and to report the possibility of long term treatment more than two months in rickettial disease.

Acupuncture Treatment in a Case with Equine Laminitis (말 제엽염의 침 치료 1례)

  • One male Arabian horse, 13-year-old reared in Galway, Ireland, was diagnosed as laminitis. Hemo-acupuncture at TH01, SI01 and ST45 was used. Injection acupuncture with dexamethasone was also applied at the same acupoints after homo-acupuncture twice a week. Bee-venom was injected into GV01 at session 2. After session 3, the patient showed almost normal walking.

  • An investigation was conducted to evaluate both the time required and the time allowed for persons to cross streets. Currently, the local municipality uses a standardized formula to determine the time allotted for 'WALK' signals to function allowing pedestrian traffic to cross thoroughfares. The formula to determine the 'Theoretical Time(in seconds)' is the width of the street(in meter) divided by 1.2m/s. The basis of the denominator is 'normal' walking speed. Initially, 3 locations were chosen to evaluate the time between the appearance of the 'WALK' signal and the appearance of the 'DON'T WALK'. The interval between the two signals was assumed to allow a person to begin crossing the street at the appearance of the 'WALK' signal and terminate their crossing at the appearance of the 'DON'T WALK' signal. Of the 3 locations, 2 locations(elementary?middle schools and general hospital areas), the duration of the 'WALK' signal were not properly set and therefore need more time for those who use these cross walks. Specific details regarding the crossing locations and validity of the standardized formula were also presented and discussed.

A Case Report on Patient with Guillain-Barre Syndrome Improved by Korean Medical Combined Treatment (한방 복합치료로 호전된 Guillain-Barre 증후군 환자 증례)

  • Objectives The purpose of this case is to report the improvement after Korean medical combined treatment about patient with Guillain-Barre syndrome. Methods We treated patient using electroacupuncture, moxibustion, herbal medication, and rehabilitation therapy for 59 days. Results The weakness of muscles of upper extremities were recovered to normal level, and the muscle power of lower extremities was improved enough to walking with walker. Conclusions We conclude that Korean medical combined treatment is an effective to improve the power of muscle having weakness. But there is a limit on this report due to sufficient number of case. Further studies will be needed.
