Generating Rank-Comparison Decision Rules with Variable Number of Genes for Cancer Classification (순위 비교를 기반으로 하는 다양한 유전자 개수로 이루어진 암 분류 결정 규칙의 생성)

  • Microarray technology is extensively being used in experimental molecular biology field. Microarray experiments generate quantitative expression measurements for thousands of genes simultaneously, which is useful for the phenotype classification of many diseases. One of the two major problems in microarray data classification is that the number of genes exceeds the number of tissue samples. The other problem is that current methods generate classifiers that are accurate but difficult to interpret. Our paper addresses these two problems. We performed a direct integration of individual microarrays with same biological objectives by transforming an expression value into a rank value within a sample and generated rank-comparison decision rules with variable number of genes for cancer classification. Our classifier is an ensemble method which has k top scoring decision rules. Each rule contains a number of genes, a relationship among involved genes, and a class label. Current classifiers which are also ensemble methods consist of k top scoring decision rules. However these classifiers fix the number of genes in each rule as a pair or a triple. In this paper we generalized the number of genes involved in each rule. The number of genes in each rule is in the range of 2 to N respectively. Generalizing the number of genes increases the robustness and the reliability of the classifier for the class prediction of an independent sample. Also our classifier is readily interpretable, accurate with small number of genes, and shed a possibility of the use in a clinical setting.

Spark based Scalable RDFS Ontology Reasoning over Big Triples with Confidence Values (신뢰값 기반 대용량 트리플 처리를 위한 스파크 환경에서의 RDFS 온톨로지 추론)

  • Recently, due to the development of the Internet and electronic devices, there has been an enormous increase in the amount of available knowledge and information. As this growth has proceeded, studies on large-scale ontological reasoning have been actively carried out. In general, a machine learning program or knowledge engineer measures and provides a degree of confidence for each triple in a large ontology. Yet, the collected ontology data contains specific uncertainty and reasoning such data can cause vagueness in reasoning results. In order to solve the uncertainty issue, we propose an RDFS reasoning approach that utilizes confidence values indicating degrees of uncertainty in the collected data. Unlike conventional reasoning approaches that have not taken into account data uncertainty, by using the in-memory based cluster computing framework Spark, our approach computes confidence values in the data inferred through RDFS-based reasoning by applying methods for uncertainty estimating. As a result, the computed confidence values represent the uncertainty in the inferred data. To evaluate our approach, ontology reasoning was carried out over the LUBM standard benchmark data set with addition arbitrary confidence values to ontology triples. Experimental results indicated that the proposed system is capable of running over the largest data set LUBM3000 in 1179 seconds inferring 350K triples.

Scalable RDFS Reasoning Using the Graph Structure of In-Memory based Parallel Computing (인메모리 기반 병렬 컴퓨팅 그래프 구조를 이용한 대용량 RDFS 추론)

  • In recent years, there has been a growing interest in RDFS Inference to build a rich knowledge base. However, it is difficult to improve the inference performance with large data by using a single machine. Therefore, researchers are investigating the development of a RDFS inference engine for a distributed computing environment. However, the existing inference engines cannot process data in real-time, are difficult to implement, and are vulnerable to repetitive tasks. In order to overcome these problems, we propose a method to construct an in-memory distributed inference engine that uses a parallel graph structure. In general, the ontology based on a triple structure possesses a graph structure. Thus, it is intuitive to design a graph structure-based inference engine. Moreover, the RDFS inference rule can be implemented by utilizing the operator of the graph structure, and we can thus design the inference engine according to the graph structure, and not the structure of the data table. In this study, we evaluate the proposed inference engine by using the LUBM1000 and LUBM3000 data to test the speed of the inference. The results of our experiment indicate that the proposed in-memory distributed inference engine achieved a performance of about 10 times faster than an in-storage inference engine.

Polarimetry of solar system small bodies using the Seoul National University 61cm telescope and TRIPOL

  • It is known that lights scattered by comets and asteroids are partially polarized. From polarimetric observations of those objects, we can investigate physical properties, such as albedos, sizes of cometary dust particles and regolith of asteroids. Since the polarization degrees of those objects highly depend on their phase angles (Sun-object-observer's angles), long-term monitoring observations are required. Moreover, comets show unforeseeable activations (i.e., outbursts) which need follow-up observations to understand the mechanism. In order to realize such monitoring and transient observations, we installed the Triple-Range Imager and POLarimeter (TRIPOL) on the 61cm telescope of Seoul National University (Hereafter, SNU) Gwanak campus. With this combination, we can obtain g', r', i' bands photopolarimetric images simultaneously with $8.0^{\prime}{\times}8.0^{\prime}$ field of view and pixel resolution of 0.94" pixel-1. Here, we make a presentation regarding the photometric and polarimetric performances of TRIPOL on the SNU 61cm telescope. In addition, we introduce initial polarimetric results of asteroid and comets with the instruments. First, we determine the limiting magnitudes (defined as magnitudes for S/N=5) of $15.17{\pm}0.06$ (g'-band), $15.68{\pm}0.01$ (r'-band), $16.24{\pm}0.03$ (I'-band), respectively, with total 240-seconds exposure (four 60-seconds exposure images, each was taken at different rotation angle for the half-wave plate). Second, we found that the instrumental polarization is negligibly small, ($-0.32{\pm}0.04%$ in the g', $-0.36{\pm}0.05%$ in the r' and $-0.21{\pm}0.04%$ in the i'-bands), while the polarization efficiencies are large enough to maximize the performance (i.e., $97.52{\pm}0.03%$ in the g', $98.83{\pm}0.02%$ in the r' and $99.15{\pm}0.02%$ in the i'-bands). With the instruments, we made observations of three Jupiter-family comets, 21P/Giacobini-Zinner, 38P/Stephan-Oterma, and 46P/Wirtanen and plan to observe one near-Earth asteroid, (433) Eros, on a trial basis. Especially for comets, we discriminate signals from dust and gas to eliminate gas contamination, which are known to change observed degree of linear polarization, using multi-band images. We confirm that the phase angle dependency of these comets are consistent with previous observations, probably because polarimetric property of Jupiter-family comets are broadly homogeneous unlike asteroids. We will also describe future observation plans using TRIPOL and SNU 61cm telescope.

Does dexmedetomidine combined with levobupivacaine in inferior alveolar nerve blocks among patients undergoing impacted third molar surgery control postoperative morbidity?

  • Background: Postoperative analgesia (POA) is an important determinant of successful treatment. Dexmedetomidine (DEX) has recently gained attention as a promising adjuvant to local anesthetics (LA). The present study aimed to evaluate the efficacy and safety of levobupivacaine (LB) as an adjuvant during inferior alveolar nerve block (IANB) in the extraction of lower impacted third molars (LITM). Methods: A prospective, randomized, placebo-controlled, triple-blind, parallel-arm, and clinical study was performed on 50 systemically healthy participants who required removal of an asymptomatic LITM. Using a 1:1 distribution, the participants were randomized into two groups (n = 25). Group L (control group) received 1.8 mL of 0.5% LB and 0.2 mL normal saline (placebo) and Group D (study group) received a blend of 1.8 mL of 0.5% LB and 0.2 mL (20 ㎍) DEX. The primary outcome variable was the duration of POA and hemodynamic stability, and the secondary variable was the total number of analgesics required postoperatively for up to 72 h. The participants were requested to record the time of rescue analgesic use and the total number of rescue analgesics taken. The area under the curve was plotted for the total number of analgesics administered. The pain was evaluated using the visual analog scale. Data analysis was performed using paired students and unpaired t-test, Mann-Whitney U test, Chi-square test, and receiver operating characteristic analysis. Statistical significance was set at P < 0.05. Results: The latency, profoundness of anesthesia, and duration of POA were statistically significant (P < 0.05). The differences between mean pain scores at 6, 12, 24, 48, and 72 h were found to be significant (each P = 0.0001). Fewer analgesics were required by participants in group D (2.12 ± 0.33) than in L (4.04 ± 0.67), with a significant difference (P = 0.0001). Conclusion: Perineurally administered LA with DEX is a safe, effective, and therapeutic approach for improving latency, providing profound POA, and reducing the need for postoperative analgesia.

Changes in Automated Mammographic Breast Density Can Predict Pathological Response After Neoadjuvant Chemotherapy in Breast Cancer

  • Objective: Mammographic density is an independent risk factor for breast cancer that can change after neoadjuvant chemotherapy (NCT). This study aimed to evaluate percent changes in volumetric breast density (ΔVbd%) before and after NCT measured automatically and determine its value as a predictive marker of pathological response to NCT. Materials and Methods: A total of 357 patients with breast cancer treated between January 2014 and December 2016 were included. An automated volumetric breast density (Vbd) measurement method was used to calculate Vbd on mammography before and after NCT. Patients were divided into three groups according to ΔVbd%, calculated as follows: Vbd (post-NCT - pre-NCT)/pre-NCT Vbd × 100 (%). The stable, decreased, and increased groups were defined as -20% ≤ ΔVbd% ≤ 20%, ΔVbd% < -20%, and ΔVbd% > 20%, respectively. Pathological complete response (pCR) was considered to be achieved after NCT if there was no evidence of invasive carcinoma in the breast or metastatic tumors in the axillary and regional lymph nodes on surgical pathology. The association between ΔVbd% grouping and pCR was analyzed using univariable and multivariable logistic regression analyses. Results: The interval between the pre-NCT and post-NCT mammograms ranged from 79 to 250 days (median, 170 days). In the multivariable analysis, ΔVbd% grouping (odds ratio for pCR of 0.420 [95% confidence interval, 0.195-0.905; P = 0.027] for the decreased group compared with the stable group), N stage at diagnosis, histologic grade, and breast cancer subtype were significantly associated with pCR. This tendency was more evident in the luminal B-like and triple-negative subtypes. Conclusion: ΔVbd% was associated with pCR in breast cancer after NCT, with the decreased group showing a lower rate of pCR than the stable group. Automated measurement of ΔVbd% may help predict the NCT response and prognosis in breast cancer.

Clinical Efficacy of Clomiphene Citrate and Letrozole Combined with Gonadotropins for Superovulation in Patients with Clomiphene-Induced Thin Endometrium (클로미펜에 얇은 자궁내막을 보이는 환자에서 성선자극호르몬 병합 과배란유도시 클로미펜과 레트로졸의 임상적 효용성)

  • Objective: The aim of this study was to compare the clinical efficacy of clomiphene citrate (CC) and letrozole combined with gonadotropins for controlled ovarian stimulation (COS) in patients with CC-induced thin endometrium Methods: Fifty-one intrauterine insemination cycles performed in patients who previously had a thin endometrium (<8 mm) to ovulation induction using CC were included in this study. A CC 100 mg/day (CC+gonadotropin group, n=26) or letrozole 2.5 or 5 mg/day (letrozole+gonadotropin group, n=25) was administered on day 3~7 of the menstrual cycle, combined with gonadotropins at dose 75~150 IU every other day starting on day 5~7. We compared total dose of gonadotropin used, endometrial thickness, endometrial pattern, number of follicles ${\geq}14\;mm$ on hCG day, pregnancy rate and multiple pregnancy rate between the two groups, which were statistically analyzed using Mann-Whitney U test or Fisher's exact test, where appropriate. Results: There were no significant differences in clinical characteristics such as age, duration of infertility, number of previous IUI cycles, basal serum hormone levels and cause of infertility between the two groups. In both groups, the endometrium was significantly thicker than that of previous ovulation induction cycles using CC. No significant differences were found in the total dose of gonadotropin used, day of hCG administration, the rate of triple endometrium and pregnancy rate. The number of follicles ${\geq}14\;mm$ was significantly lower ($3.7{\pm}1.7$ vs. $2.8{\pm}1.7$, p=0.03) and the endometrium on hCG day was significantly thicker ($7.7{\pm}1.5$ vs. $9.1{\pm}1.7$, p=0.001) in letrozole+gonadotropin group compared to CC+gonadotropin group. Conclusion: The clomiphene citrate and letrozole combined with gonadotropins appear to avoid the undesirable effects on the endometrium frequently seen with CC for ovulation induction. However, in terms of adequate endometrial development or optimal follicular growth, letrozole may be more beneficial than CC for gonadotropin-combined COS in patients with CC-induced thin endometrium. Further prospective randomized controlled studies in a larger scale will be necessary to confirm our findings.

Dipyridamole Induced Transient Left Ventricular Dysfunction in the Tl-201 Gated Myocardial SPECT (Tl-201 게이트 심근 SPECT에서 디피리다몰 부하로 유도된 일시적 좌심실 기능이상)

  • Purpose: Recently the occurrence of dipyridamole stress-induced short term stunning was proven and it is reported that Bland Altman analysis by repeated acquisition Tl-201 gated myocardial SPECT (gSPECT) revealed the 95% limit of agreement for LVEF was 10.3 %. The purpose of this study was to investigate the clinical value of dipyridamole induced transient LV dysfunction on Tl-201 gSPECT. Materials and Methods: Total 93 patients were included and coronary angiography was peformed in all patients less than 2 month from gSPECT. The patients with myocardial infarction were excluded. All patients underwent both dipyridamole stress and 4-h redistribution Tl-201 gSPECT. Forty nine patients of total 93 showed normal coronary arteries (Group 1) and the remaining 44 patients had coronary artery disease (Group 2). We compared LV EF, EDV and ESV during post-stress and 4-h redistribution period calculated by gSPECT using quantitative gated SPECT software and the incidence of dipyridamole induced transient LV dysfunction between group 1 and 2. The criteria for transient LV dysfunction was defined more decrease ${\geq}11%$ of LVEF during post-stress than 4-h redistribution according to previous reported Bland Altman analysis. Results: During post-stress and 4-h redistribution average of 3.1% increment in LVEF, 6.6% increment in LVEDV and 0.7% decrement in LVESV were shown after stress in Group 1, whereas 4.1% decrement, 9.7% increment and 7.2% increment in Group 2 respectively. Dipyridamole induced transient LV dysfunction was only detected in group 2 (18.2%) and not in group 1. It was more frequently observed in triple vessel disease and left main disease (31.8%, N=22) than one and two vessel disease (4.5%, N=22). Conclusion: As with Tc-99m myocardial agent post-stress LV dysfunction was observed in dipyridamole Tl-201 gSPECT. It was only detected in CAD and more frequently occurred in multivessel disease. Thus this finding seems to provide additional information in the diagnosis of coronary artery disease and prediction of prognosis.

Occurrence of Mycotoxins in Korean Grains and Their Simultaneous Analysis (한국산 곡류에서의 곰팡이독소 오염현황 및 동시분석)

  • Eleven mycotoxins, including aflatoxins, ochratoxin A, fumonisins, zearalenone, T-2 toxin, deoxynivalenol, and HT-2 toxin, were analyzed simultaneously in rice, barley, and maize produced in 2011 by liquid chromatography coupled with triple quadrupole mass spectrometry (LC/MS/MS). Limits of detection (LOD) are 0.2 ${\mu}g/kg$ for aflatoxin $B_1$, and $G_1$, 0.3 ${\mu}g/kg$ for aflatoxins $B_2$, and $G_2$, 0.1 ${\mu}g/kg$ for ochratoxin, fumonisins, zearalenone, and T-2 toxin and 3.0 ${\mu}g/kg$ for deoxynivalenol and HT-2 toxin. Limits of quantification (LOQ) were 0.6 ${\mu}g/kg$ for aflatoxins $B_1$, and $G_1$, 0.9 ${\mu}g/kg$ for aflatoxins $B_2$, and $G_2$, 0.3 ${\mu}g/kg$ for ochratoxin, fumonisins, zearalenone, and T-2 toxin and 10.0 ${\mu}g/kg$ for deoxynivalenol and HT-2 toxin. Recoveries for 11 mycotoxins ranged from 70.45 to 111.11%. Fumonisins, deoxynivalenol, and zaeralenone were detected from 0.9 to 334.0 ${\mu}g/kg$ in the polished rice, barley and raw corn cultivated in Korea. Other mycotoxins were not detected. Deoxynivalenol contamination was mainly found in barley (24 out of 43 samples) and the average value in positive samples was 113.30 ${\mu}g/kg$.

Complete Myocardial Revascularization Utilizing Parallel Secluential Anastomoses (Parallel sequential anastomosis를 이용한 관상동맥 우회수술의 성적)

  • We evaluated the feasibility and safety of this method by reviewing the early outcome of the patients who underwent coronary artery bypass grafting(CABG) utili ing parallel sequential anastomoses with saphenous vein grafts, comparing with the outcome of the patients revascularized with grafts having only single distal anastomosis. During the one-year period of 1995, a total of 79 patients underwent isolated CABG, among whom 39 patients with sequential vein grafts(sequential group) and 40 patients without sequential grafts(non-sequential group). There was no difference between the two groups in terms of preoperative status, except in the extent of the coronary disease; 87.2% of the sequential group and 45.0% of the non-sequential group had left main andlor triple vessel involvement. 318 distal coronary anastomoses were done; 198 for the sequential group(5.1/patient) and 120 for the non-sequential group(3.0/patient). In the sequential group, the mean durations of cardiopulmonary bypass and aortic clamp per one distal anastomosis were 33.5 and 21.1 minutes, respectively. In the non-sequential group, these were 41.8 and 22.7 minutes. There were two operative deaths, both in the non-sequential group. There was no difference in the incidence of postoperative complications including myocardial infarction. During the follow-up period(2 to 15 months), 8 patients(3 in the sequential and 5 in the nonsequential group) complained of residual or recurrent angina. Comparison of preoperative and postoperative ."'Thallium myocardial perfusion scans in 30 patients showed improved or normal perfusion reserve in 83.3% of segments bypassed with sequential grafts and 82.5% of segments bypassed with non-sequential graft(5), These results show that, utilizing parallel sequential anastomoses with saphenous vein grafts, we could achieve satisfactory short-term clinical results in patients with extensive coronary stenoses. So, we conclude that this technique is a safe, technically feasible strategy for CABG, which can achieve the aim of complete myocardial revascularization with a limited length of graft.raft.
