Benign Metastasizing Leiomyoma of Lung -A case report- (폐에 발생한 양성 전이성 근종-1례 보고-)

  • We performed thoracoscopic resection for diagnosis in 41 year-old-female presenting with multiple pulmonary nodules in both lung fields, which was detected incidentally on routine chest x-ray and followed by additional exmaminations including chest CT scan and percutaneous needle aspiration biopsy under the presumptive diagnosis of metastatic cancer. During thoracoscopy, the result of the frozen section analysis of multiple masses revealed strong evidence of leiomyoma. In her past medical history, she had undergone myomectomy, and hysterectomy, 7 year ago and 10 year ago, respectively. Based on permanent, special staining of specimen, estrogen receptor assay and review of past specimen of uterine myoma the final diagnosis was benign metastasizing leiomyomata from uterine myoma, the report was very uncommon in Korean and English literatures. The patient has been followed up for 2 year without special therapy, such as hormonal therapy.

Treatment of Uterine Myoma as a Cause of infertility, with a Delayed-Release Formulation of a Gonadotropin-Releasing Hormone Agonist(D-$Trp^6$-LHRH) (불임의 원인으로서의 자궁근종 치료에 있어 GnRH agonist (D-$Trp^6$-LHRH)의 효용에 관한 연구)

  • To evaluate the effectiveness of GnRH agonist for the treatment of uterine myoma as a cause of infertility, fourteen women were recruited to the study. The patients were treated with a delayed-release formulation of D-$Trp^6$-LHRH in biodegradable microcapsules(Decapeptyl-CR), administered intramuscularly at four week intervals for a period of six monthes. The first injection was given on day 21 of the cycle. Serum estradiol levels fell significantly to the mean value of 257.7pgjml 4 weeks after the first injection. Eleven patients in fourteen treated patients had a reduction in the size of uterine myoma as assessed by ultrasonography, two patients had no change of size and one patient had a increase of size. After the first or second injection, all patients became amenorrheic, then resumption of menstruation ocurred at 12 to 14 weeks after the last injection. Common side effects were hot flush, sweating and dyspareunia, whitch were acceptale. In Eleven patients who had a reduction in the size of uterine myoma by treatment with a delayed- release formulation of D-$Trp^6$-LHRH(Decapeptyl-CR), after above treatment with GnRH agonist, then four patients were treated with myomectomy, three patients had pregnancy and full term delivered by Cesarean section. These data suggest that administration of a delayed-release formulation of a GnRH agonist can be a worthwhile and convenient approach to the medical treatment of uterine myoma as a cause of infertility.

Clinical Study of Korean Medicine with Boheo-tang-gagambang for Recovery after Gynecological Surgery for Myoma of the Uterus: 5 Cases Report (자궁평활근종의 수술적 치료 후유증에 대한 보허탕가감방 한방 치험 5례)

  • Objectives: The purpose of this case study is to report clinical effectiveness of the Boheo-tang-gagambang with acupuncture, moxa and physiotherapy treatments on the after-effects of myomectomy or hysterectomy. Methods: The subjects of this study were five women who were diagnosed with uterine myoma and underwent myomectomy or hysterectomy. They mainly complained of lower abdominal pain, back pain, and pelvic pain. They received combination treatment of Korean medicine and physiotherapy. The Korean medicine included Boheo-tang-gagambang, acupuncture and moxa. The effects were evaluated through Numeric Rating Scale (NRS). Results: In this study, patient's clinical symptoms, including lower abdominal pain, low back pain, and pelvic pain, improved after hospitalization treatment. Conclusions: This case report shows that the combination treatment of Korean medicine with Boheo-tang-gagambang, acupuncture and moxa may be effective for treating after-effects of myomectomy or hysterectomy.

Patient Satisfaction, Vaginal Bleeding, Sexual Function following Laparoscopic Supracervical Hysterectomy

  • Purpose: This study was done to evaluate postoperative patient satisfaction, vaginal bleeding, and sexual function in women after laparoscopic supracervical hysterectomy (LSH). Methods: A retrospective study was conducted using a questionnaire mailed to 131 women who underwent LSH between 2008 and 2011at the Department of Obstetrics &Gynecology, D University Hospital in Chungnam province. Indication for LSH was uterine myoma. The questionnaire contained questions on overall postoperative satisfaction, influence on quality of life of vaginal bleeding, and sexual satisfaction following surgery. Data were collected from March to July 2013 and 109 (83.2%) patients returned the questionnaire. Results: Most women reported being very satisfied (90.8%) or satisfied (7.3%), but 2 women (1.8%) were not satisfied with LSH. Four patients (3.4%) reported experiencing vaginal bleeding but with no negative influence on quality of life. Of sexually active women, 82 patients (90.1%) reported improvements in sexual function, 8 patients (8.8%) reported "no change", and one patient (1.1%) reported a deterioration Conclusion: Results of this study indicate that LSH is associated with a high degree of patient satisfaction, no negative influence on quality of life from vaginal bleeding, and improvement in sexual function to a minimum 2 years after the procedure.

Combination Treatment of Korean Medicine with Ohrim-san for Lower Urinary Tract Symptoms (LUTS) after Laparoscopic Myomectomy Patient : A Case Report (복강경하 근종적출술 후 발생한 하부요로증상에 대한 오림산을 포함하는 복합한의진료의 경과 : 증례보고)

  • Objectives: This case report aims to report the clinical effectiveness of the combination treatment of Korean medicine on a post-laparoscopic myomectomy patient's lower urinary symptoms (LUTS). Methods: The patient who diagnosed with uterine myoma had Laparoscopic myomectomy. After the operation, the patient complained of dysuria, nocturia, frequent urination, urgent urination and urinary retention. The patient received combination treatment of Korean medicine during 14 days of hospitalization. The treatment included Ohrim-san, acupuncture, moxibustion, and pharmacoacupuncture. The effects were evaluated through the Numeric Rating Scale (NRS), Overactive Bladder Symptom Score (OABSS), Frequency-Volume chart, and 5-Level version of EuroQol-Five Dimension (EQ-5D-5L). Results: After the treatment, the clinical symptoms such as dysuria, nocturia, frequent urination, urgent urination, urinary retention were improved. Also, the quality of life was enhanced. Conclusions: This case report shows that the combination treatment of Korean Medicine with Ohrim-san may be effective for treating lower urinary tract symptoms (LUTS) after laparoscopic myomectomy.

Two Cases of Extrapelvic endometriosis following Laparoscopy-assisted vaginal hysterectomy and Cesarean section (복강경하 질식 자궁적출술 및 제왕절개술 후 발생한 골반외 자궁내막증 2 예)

  • Extrapelvic endometriosis is a rare disease. The majority of extrapelvic endometriosis cases involve scar tissue following obstetric and gynecologic procedures. We have treated two cases of extrapelvic incisional endometriosis. A 39 year old female patient with cyclic vaginal spotting after laparoscopic assisted vaginal hysterectomy due to uterine myoma and a 35 year old female patient with a painful palpable abdominal mass after cesarean section. Both underwent complete excision and were proven to have endometriosis by pathology. Here we report on both cases and review the medical literatures.

3 Case Reports of Uterine Fibroid treated by Herbal medicine based on Shanghanlun provisions (『상한론(傷寒論)』 변병진단체계(辨病診斷體系)에 근거하여 한약 투여 후 호전된 자궁근종 3례)

  • Objective : The purpose of this paper is to report the improvement of 3 patients with uterine fibroid treated by herb medication based on Shanghanlun disease pattern identification diagnostic system. Methods : According to 'Disease Pattern Identification Diagnostic System based on Shanghanlun Provisions', they were diagnosed with Taeyang-byung, Yangmyung-byung. They took different herb medications more than 8 weeks according to their diagnosis. The size of uterine fibroid was estimated by ultrasonography. Results : The size of uterine fibroid reduced from 3.94cm to 2.55cm on first patient, 5.20cm to 4.86cm on second patient, 12.63cm to 7.77cm on third patient. Conclusions : Herb medication not only reduced the size of uterine fibroid but also improved symptoms such as dysmenorrhea, vaginal bleeding, pelvic discomfort, fatigue. In addition, this is the first case report treating the uterine fibroid with herb medication of shanghanlun in Korea.

  • Because interventional procedure operates looking at premier as real time when perate intervention enemy, by patient is revealed during suitableness time in radiation, side effect such as radiation injury of skin is apt to happen. It established by purpose of study that measure exposure dose that patient receives about these problem, and find solution for radiation injury and repletion method. In this study, we used Rando phantom of identical structure with the human body which becomes accomplished with 4 branch ingredient of the attempt and system equivalent material them and absorbed dose were measured by TLD. According to the laboratory, it shows that operations such as TFCA procedure or uterine myoma embolization are more dangerous than TACE procedure. If both operations are inspected during a short time, it is not affected in being bombed. However, it can lead to palliative agenesis or depilate, definitive agenesis only if operations are repeated more than three times. Dose distibution based on experiment, to reduce radiation exposure to patients result from reduction of scatter ray as we control field size of radiation and protection of side organs except for tumor. also we knew that we can protect patients form radiation exposure, if we increas SOD and decrease SID.

The Pregnancy Rate following Myomectomy in Infertile Women (불임여성에서 자궁근종절제술 후 임신율에 관한 연구)

  • To evaluate the efficacy of transabdominal myomectomy in the management of infertile patients, and to analyze on the results of abdominal myomectomy in 38 infertile patients with no other detectable cause except myomas were undertaken at the Department of Obstetrics and Gynecology in Yonsei University Hospital from 1990 to 1996. The results are as follows; 1. Average age of patients was 31.1 years. The infertility duration ranged 12 months to 144 months, and average infertility period of patients was 29.4 months. 2. Fourteen of the 38 patients (8 of 23 patients with primary infertility, 6 of 15 patients with secondary infertility) conceived following myomectomy, with a pregnancy rate of 36.8%. 3. Patients with less than 4 years of infertility showed a higher pregnancy rate after myomectomy than those with more than 4 years of infertility (42.4% vs 0%, p<0.05). 4. Patients younger than 35 years showed significantly higher pregnancy rate than those older than 35 years (46.4% vs 9.0%, p<0.05). 5. The removal of a solitary myoma produced a significantly higher pregnancy rate than that of multiple myomas (47.8% vs 20.0%, p<0.05), and the size of the myomas did not influenced the pregnancy rate after myomectomy (p>0.1). 6. The average time period from operation to conception was 12.1 months. Eight of the 14 patients (57.1%) conceived in the first year after operation and 12 patients (85.7%) condeived within two years. In conclusion myomas are a possible cause of infertility and myomectomy can be strongly recommended with good success expectation for the infertile women if uterine myoma be considered as the main cause of infertility. And factors affecting the pregnancy rate after myomectomy in these patients are the age of the patient, the duration of infertiluty, and the number of myoma.

Clinical Study on the Effect of Gyebongnyeong capsule in the Treatment of Primary Dysmenorrhea (계지복령환(桂枝茯笭丸) 제제의 원발성 월경통에 대한 임상적 효과)

  • The purpose of this study is to identify the clinical effects of Gyebongnyeong capsule in the treatment of primary dysmenorrhea patients. We studied ten patients visit Se-Myung University Oriental Medical Center from February, 2005 to March, 2005. Women with organic disease such as ut. myoma, ov. cyst and pelvic inflammatory disease were excluded from this study. We treated them with Gyebongnyeong capsule for two menstrual cycles. The severity of dysmenorrhea were measured by VAS (Visual Analog Scale) and Patient's Global Assessment. Gyebongnyeong capsule significantly decreased the severity of dysmenorrhea. Gyebongnyeong capsule did not show hepatic and renal virulence. This study shows that Gyebongnyeong capsule has remarkable effects on dysmenorrhea patients.
