편부전마비 환자에서의 근육약화에 대한 생리학적 고찰 (Physiological Review of Weakness in Patients with Hemiparesis)

  • This paper reviews physiological changes in the nervous system of patients with hemiparesis that may contribute to muscle weakness. The discussion includes the important role that alterations in the physiology of motor units, notably changes in firing rates and muscle fiber atrophy, play in the manifestation of muscle weakeness. This role is compared with the lesser role that spasticity of the antagonist muscle group appears to play in determining the weakness of agonist muscles. The contribution of other factors that result in mechanical restraint of the agonist by the antagonist is discussed relative to muscle weakness in patients with hemiparesis. More studies on patients with hemiparesis are required to assess what role muscle strength training should play in rehabiliting patients after a stroke.

Comprehensive Relevance of AMPK in Adaptive Responses of Physical Exercise, Skeletal Muscle and Neuromuscular Disorders

  • PURPOSE: This study was conducted to understand the adaptive responses of different modes of physical exercises utilizing skeletal muscle and the comprehensive relevance of AMPK signaling that can be activated by physical exercise as a potential molecular target in human health problems such as neuromuscular disorders (NMDs). METHODS: Most of the contents in this review article are based on recent publications concerning the main topics of interest. The reference literatures cited were obtained by basic searches of overseas academic databases such as PubMed and ScienceDirect using EndNote X7.8. RESULTS: The phenotypic adaptive responses of skeletal muscle during endurance- and resistance-based exercise training (ET and RT respectively) appear to be distinct. To explain the adaptive responses in each single mode of exercises (ET, RT) along with combined exercise training (CT), AMPK signaling is proposed as an important molecular link among those differential modes of exercise and a promising molecular target of NMDs. CONCLUSION: Based on the available evidence, intracellular AMPK signaling activated by diverse stimuli including physical exercise can be a potential and promising therapeutic target for the prevention, amelioration or cure of various human health problems including NMDs and may also be beneficial for physical rehabilitation and emergency situations that may elicit acute metabolic stresses.

  • Purpose: This study was aimed to evaluate the effect of progressive muscle relaxation training using biofeedback on perceived stress, stress response, immune response and climacteric symptoms, Method: This was a crossover, pre-post test design, The study subjects are 36 middle-aged women who were selected at 2 public health centers, The independent variable was Biofeedback training for 4 weeks, twice a week and home training for 4 weeks, Dependent variables were perceived stress, stress response, immune response, and climacteric symptoms measured with Hildtch's scale (1996), Result: Progressive muscle relaxation training using biofeedback was not effective in reducing perceived stress, but it was shown to be effective in reducing physiological stress responses such as pulse rate and EMG, Though blood pressure and skin conductance were repeatedly down, and skin temperature slowly increased, there were no statistically significant differences. Progressive muscle relaxation training using biofeedback was not effective in reducing serum cortisol, enhancing immune responses, or decreasing climacteric symptoms. Conclusion: The findings point to a pressing need for further, well-controlled and designed research with consideration in selection of subjects and instruments, frequency of measurements, the sampling method, and intervention modalities.

요통환자의 체간근 활성도 및 통증과 Bodyblade 훈련의 융합관계에 관한 연구 (A Study on Bodyblade Training of Convergency Relation of Trunk Muscles Activity and Pain in Patients with Low Back Pain)

  • 본 연구의 목적은 만성 요통 환자의 근육 활성도 및 몸통 통증 에 대한 bodyblade 훈련의 영향을 알아보았다. 연구를 위해 30 명의 피험자 샘플을 모집하여 실시하였다. Bodyblade 훈련 그룹 (n = 10)은 6주 동안 주 3 회 bodyblade 훈련을 실시하였고, 요추 안정화 훈련 그룹 (n = 10)은 6 주 동안 주 3 회 요추 안정화 훈련을 실시하였다. 대조군 그룹 (n = 10)은 다른 어떤 훈련도 실시하지 않았다. 몸통 근육 활성도의 평가를 위해 표면 근전도를 사용하였고 통증 평가를 위해 VAS를 측정 하였다. 결과는 다음과 같았다 : 몸통 근육의 활성도와 통증변화에 있어 측정 시점과 상호작용에서 통계학적으로 유의한 차이를 나타내었다. 그 결과, bodyblade 훈련은 만성요통환자의 효과적인 중재 방법이 될 수 있을 거라 기대된다.

Effect of Game-Based Balance Training with CIMT on Pain, Muscle Strength, Range of Motion and Dynamic Balance in Female Patients with Total Knee Replacement

  • Purpose: The aims of this study were to determine if game-based training with constraint-induced movement therapy (CIMT) is effective in improving the balance ability in female patients with a total knee replacement, and to provide clinical knowledge of CIMT game-based training that allows the application of total knee replacement. Methods: Thirty-six patients who had undergone a total knee replacement were assigned randomly to CIMT game training (n=12), general game training (n=12), and self-exercise (n=12) groups. All interventions were conducted 3 times a week for 4 weeks. All patients used a continuous passive motion machine 5 times a week and 2 times a day for 4 weeks. The visual analog scale (VAS), muscle strength of knee flexion and extension, and range of motion (ROM) of knee flexion and extension were assessed, and the functional reach test (FRT), and timed up and go (TUG) test were performed to evaluate the balance ability. Results: All 3 groups showed significant improvement in the VAS, knee flexion and extension muscle strength, FRT, and TUG test after the intervention (p<0.05). Post hoc analysis revealed significant differences in FRT, and TUG of the CIMT game training group compared to the other group (p<0.05). Conclusion: Although the general game training and CIMT game training improved both the knee extension muscle strength and dynamic balance ability, CIMT game training had a larger effect on dynamic balance control.

전기자극을 이용한 등척성 수축훈련이 뇌허혈 유발 쥐의 환측 Type I, II 근육에 미치는 영향 (Effects of Isometric Contraction Training by Electrostimulation on Type I and II Hindlimb Muscles in Cerebral Ischemia Model Rats)

  • Purpose: The purpose of this study was to examine the effects of cerebral ischemia on Type I(soleus) and Type II(plantaris, gastrocnemius) muscles, and to determine the effects of isometric contraction training by electro- stimulation on Type I and II muscles in cerebral ischemia model rats. Method: Twenty-five male Sprague-Dawley rats were randomly divided into four groups: ST(stroke), STES(stroke+electrostimulation), SH(sham) and SHES (sham+electrostimulation). The ST and STES groups received a transient right middle cerebral artery occlusion operation. The SH and SHES groups received a sham operation. The STES and SHES groups had daily isometric contraction training by electrostimulation(100Hz, 45mA, 7.5V) on hindlimb muscles for 7days. Result: Plantaris and gastrocenmius muscle weight, myofibrillar protein contents of soleus and gastrocnemius, and the muscle fiber cross-sectional area of gastrocnemius in the ST group significantly decreased compared with the SH group. Soleus, plantaris, gastrocnemius muscle weight, myofibrillar protein contents of soleus and gastrocnemius, and the Type I muscle fiber cross-sectional area of soleus and the Type II muscle fiber cross-sectional area of gastrocnemius in the STES group significantly increased compared with the 57 group. Conclusion: Hindlimb muscle atrophy occurs after acute stroke and isometric contraction training by electrostimulation during early stages of a stroke attenuates muscle atrophy of Type I and Type II muscles.

요부 안정화 운동 프로그램이 축구선수의 체간 및 하지 근활성도와 균형에 미치는 영향 (Effects of Lumbar Stability Exercise Program on Trunk, Lower Extremity of Muscle Activity and Balance in Soccer Player)

  • Purpose: The purpose of this study is to provide an efficient and scientific basis for muscle activity (%MVIC) of RA, EO, VL, HS muscles and balance in soccer players through dynamic lumbar stability exercise and static lumbar stability exercise. Methods: This study included 23 soccer players belonging to D University of J province who attended the program for 30 minutes at a time and three times a week for 4 weeks. Of these 13 attended the dynamic lumbar stability exercise (DLSE) program and 10 the static lumbar stability exercise (SLSE) program. The differences between the effects of the dynamic lumbar stability exercise program and static lumbar stability exercise program were analyzed. Results: To increase muscle activity (%MVIC) and balance (WPL), the dynamic lumbar stability exercise program was more effective than was the static lumbar stability exercise program. 1) The %MVIC of trunk muscle (RA &EO) and lower extremitys muscle (VL & HS) increased from before training to after training in the case of the participants who performed the dynamic lumbar stability exercise. 2) The whole path length (WPL) decreased from before the training to after the training. The 2 groups significantly differed in this regard. Conclusion: Dynamic lumbar stability exercise program helps to improve the balancing ability and muscle activity in a soccer players who requires both muscle activity and balance than does any other players.

운동 트레이닝과 카이로프랙틱의 복합처치가 전방 머리 자세와 거북목 대상자의 악력과 경추부 근력에 미치는 영향 (Effects of Exercise Training and Chiropractic on Grip Strength and Cervical Muscle Strength of Subjects with Forward Head Posture and Turtle Neck)

  • PURPOSE: Forward head posture and turtle neck are common musculoskeletal disorders of the cervical vertebrae. The purpose of this study was to investigate the effects of combined exercise training and chiropractic on the grip strength and cervical muscle strength of subjects with forward head posture and turtle neck. METHODS: The subjects were divided into two groups: forward head posture (n=9) and turtle neck (n=9). Both groups performed combined exercise training and chiropractic. The subjects were instructed about the exercise training once a week. This training was performed for 30 minutes every day for 8 weeks, and the chiropractic was performed for 15 minutes once a week. The cervical muscle strength and grip strength were measured. Two-way repeated measured ANOVA was performed for statistical analysis. RESULTS: In changes to the left grip strength, the main effect over time was significant, but the interaction effect and the main effect in the groups were not significant. In changes to the right grip strength, no difference was found to be statistically significant. In changes to the cervical muscle strength, no difference was found to be statistically significant. CONCLUSION: The increases of grips strengths and cervical muscle strength in forward head posture group were greater than turtle neck group but there were not found to be statistically significant.

Effects of intermittent ladder-climbing exercise training on mitochondrial biogenesis and endoplasmic reticulum stress of the cardiac muscle in obese middle-aged rats

  • The aim of this study is to investigate the effects of intermittent ladder-climbing exercise training on mitochondrial biogenesis and ER stress of the cardiac muscle in high fat diet-induced obese middle-aged rats. We induced obesity over 6 weeks of period in 40 male Sprague-Dawley rats around 50 weeks old, and were randomly divided into four experimental groups: chow, HFD, exercise+HFD, and exercise+chow. The exercising groups underwent high-intensity intermittent training using a ladder-climbing and weight exercise 3 days/week for a total of 8 weeks. High-fat diet and concurrent exercise resulted in no significant reduction in body weight but caused a significant reduction in visceral fat weight (p<0.05). Expression of $PPAR{\delta}$ increased in the exercise groups and was significantly increased in the high-fat diet+exercise group (p<0.05). Among the ER stress-related proteins, the expression levels of p-PERK and CHOP, related to cardiac muscle damage, were significantly higher in the cardiac muscle of the high-fat diet group (p<0.05), and were significantly reduced by intermittent ladder-climbing exercise training (p<0.05). Specifically, this reduction was greater when the rats underwent exercise after switching back to the chow diet with a reduced caloric intake. Collectively, these results suggest that the combination of intermittent ladder-climbing exercise training and a reduced caloric intake can decrease the levels of ER stress-related proteins that contribute to cardiac muscle damage in obesity and aging. However, additional validation is required to understand the effects of these changes on mitochondrial biogenesis during exercise.

불안정한 지지면에서의 균형훈련이 발목관절 근활성도에 미치는 영향 (The Effect of Balance Training on Balance Ability and Ankle Joint Muscle Activity)

  • Purpose : This study investigates the effects on balanced ability and ankle joint muscle following balance pad and trampoline training periods in order to report a clinical directing about this matter. Methods : A total of nine members participated in a balance training program that put a balance pad and trampoline to use. Participants trained three times a week for thirty minutes in groups of six. Resting time was set for two minutes. EMG (median frequency and root mean square) was used to measure muscle activity at 3 weeks and 6 weeks, respectively. The tibialis anterior and gastrocnemius muscles were measured. Participants were measured with eyes open and closed. Results : The median frequency showed a significant difference according to the change in time in both muscles. Also, results showed a significant interaction between eyes open and closed in the gastrocnemius muscle. The root mean square only showed a significant difference according to the change in time in the gastrocnemius muscle. Conclusion : We suggest that the balance pad and trampoline training influence a decrease in the recruitment rate of slow twitch muscles through an improvement in balance ability; however, the training did not show a statistically significant difference with regards to eyes open and closed.
