Effects of Inspiratory Muscle Training on Diaphragm Thickness, Pulmonary Function, and Chest Expansion in Chronic Stroke Patients (흡기근 저항훈련이 만성 뇌졸중 환자의 횡격막 두께와 폐기능에 미치는 효과)

  • 연구목적: 본 연구는 만성 뇌졸중 환자를 대상으로 흡기근 저항훈련이 횡격막 두께와 폐기능 및 흉곽 확장에 미치는 효과를 알아보고자 수행하였다. 연구방법: 연구 대상자는 총 29명(남자 17명, 여자 12명)으로 흡기근 저항훈련군(15명)과 대조군(14명)으로 분류하였다. 모든 대상자는 6개월 이상된 만성 뇌졸중 환자로 일반적인 신경발달치료를 받고 있으며, 같은 기간 동안 흡기근 저항훈련군에 역치부하 흡기근육 단련기(threshold IMT device)를 제공하고 주 3회${\times}$1회 20분씩 6주간 시행하였다. 마비측과 비마비측 횡격막 두께측정을 위해 초음파의 7.5MHz linear probe를 사용하여 최대 흡기시(Tdi.con)와 휴식시(Tdi.rel)의 두께를 측정하고 수축률(TR)을 계산하였다. 또한 폐 활량계를 사용하여 노력성 폐활량을 측정하였으며, 줄자를 사용하여 흉곽 확장을 측정하였다. 연구결과: 6주간 중재 후 흡기근 저항훈련군에서 최대흡기시 횡격막 두께(Tdi.con)와 수축률(TR)은 유의한 증가를 보였다(p<.05). 1초간 노력성 호기량 ($FEV_1$)과 최대 호기 속도(PEF)도 유의한 증가를 보였으나(p<.05), 노력성 폐활량(FVC)과 1초간 노력성 호기량의 노력성 폐활량에 대한 비($FEV_1$/FVC), 흉곽 확장은 유의한 증가는 보이지 않았다(p>.05). 결론: 본 연구는 만성뇌졸중 환자를 대상으로 흡기근 저항훈련의 적용이 횡격막의 수축력과 폐기능 및 흉곽 확장력을 향상시켜 호흡근의 협응력을 증가시키고, 비활동성으로 인해 감소된 운동내성을 증가하게 함으로써 향후 재활에서 만성 뇌졸중 환자에게 2차적인 기능향상에 도움을 줄수 있을 것으로 보여진다.

The effects on rehabilitative program on isokinetic muscle power of patients with osteoarthritis of Knee joint (재활프로그램이 슬관절 골관절염 환자의 등속성 근력에 미치는 영향)

  • The purpose of this study was to investigerate the changes of isokinetic muscular function in elderly people who have been to take from osteoarthritis in both knee joints after 36 weeks' rehabilitative therapy programs. In this study the subjects were 20 women residing in S-Tower(n=10) and H-welfare Town(n=10) respectively. The rehabilitative group(equal to experimental group) had taken part in exercise program 5 days per week. And then was performed by warm up, workout(involving aerobic exercise and weight training), cool down, physical therapy(cryotherapy, TENS, ultrasound). Also its programs were classified in conditioning phase($0{\sim}12$ weeks), improvement phase($13{\sim}24$ weeks), and maintenance phase($25{\sim}36$ weeks) respectively. The results of inspections were as followed: In the isokinetic muscular function, there were significant differences in right leg's flexor and extensor in $60^{\circ}$/sec. And there were significant differences in right leg's flexor and left leg's extensor in $180^{\circ}$/sec. At last, there were significant differences in right leg's flexor and left leg's extensor in $240^{\circ}$/sec. In other words, the rehabilitative programs for 36 weeks could increase the muscular function in elderly with OA. In conclusion, the rehabilitative programs of this paper has shown the positive results, which involved in the muscular function variables in elderly people with OA in both knee.

Analysis of Seasonal Injuries in Professional Football Player (프로 축구 팀의 월별 부상실태에 관한 조사)

  • Purpose: To investigate the risk exposure, risk of injury, and injury pattern of football players in seasonal variation. We purposed to expect the injury and its' prevention. Material and Methods: The professional football club(45 players) in Seoul were followed prospectively throughout 2005, January to 2005, December. Injury pattern and incidence were recorded. Results: Total 157 injuries were recorded. Major injuries which could not attend to game and training were 87 cases. Most severe month was June about 16 injuries(18.4%). And, the competitive season injuries were 46(52.9%) in March, April, August and September. Especially, ankle sprain and contusion injuries were common cause of absence in game. And, thigh muscle group injuries had a high risk of reinjury, because of treat insufficiently. Conclusions: We enhance the ability of the soccer team by prevention and education for injuries, especially in season.

Temple and Postauricular Dissection in Face and Neck Lift Surgery

  • Periauricular paresthesia may afflict patients for a significant amount of time after facelift surgery. When performing face and neck lift surgery, temple and posterior auricular flap dissection is undertaken directly over the auriculotemporal, great auricular, and lesser occipital nerve territory, leading to potential damage to the nerve. The auriculotemporal nerve remains under the thin outer superficial fascia just below the subfollicular level in the prehelical area. To prevent damage to the auriculotemporal nerve and to protect the temporal hair follicle, the dissection plane should be kept just above the thin fascia covering the auriculotemporal nerve. Around the McKinney point, the adipose tissue covering the deep fascia is apt to be elevated from the deep fascia due to its denser fascial relationship with the skin, which leaves the great auricular nerve open to exposure. In order to prevent damage to the posterior branches of the great auricular nerve, the skin flap at the posterior auricular sulcus should be elevated above the auricularis posterior muscle. Fixating the superficial muscular aponeurotic system flap deeper and higher to the tympano-parotid fascia is recommended in order to avoid compromising the lobular branch of the great auricular nerve. The lesser occipital nerve (C2, C3) travels superficially at a proximal and variable level that makes it vulnerable to compromise in the mastoid dissection. Leaving the adipose tissue at the level of the deep fascia puts the branches of the great auricular nerve and lesser occipital nerve at less risk, and has been confirmed not to compromise either tissue perfusion or hair follicles.

Comparison of Both Legs EMG Symmetry during Over-Ground Walking and Stair Walking in Stroke Patients

  • Purpose: Gait is the most basic element when evaluating the quality of life with activities of daily living under ordinary life circumstances. Symmetrical use of the lower extremities requires complicated coordination of all limbs. Thus, this study examined asymmetry of muscle activity quadriceps femoris and tibialis anterior as a baseline for training during over-ground walking and stair walking of stroke patients. Methods: Subjects were 14 stroke patients included as one experimental group. Gait speed used in this study was determined by the subject. Low extremity paretic and non-paretic EMG was compared using the surface EMG system. Results: The low extremity EMG difference was statistically significant during over-ground walking and stair walking (p<0.05). The result of low extremity EMG substituted symmetry ratio formula was compared to EMG symmetry ratio in both legs during over-ground walking and stair walking. The average symmetry ratio of quadriceps femoris during over-ground walking was 0.65, and average symmetry ratio of quadriceps femoris during stair walking was 0.47, with significant difference (p<0.05). Conclusion: EMG data was higher in stair walking than over-ground walking. However, in the comparison of symmetry ratio, asymmetric EMG of quadriceps femoris was significantly increased during stair walking. These findings suggested that application of stair walking for strengthening of both legs can be positive, but the key factor is maintaining asymmetrical posture of both legs. Therefore, physical therapists should make an effort to reduce asymmetry of quadriceps femoris power during stair walking by stroke patients.

The Effects of the Upright Body Type Exercise Program on Foot Plantar Pressure of Archers

  • Objective: This study collected data on muscle fatigue and ground reaction force during walking to provide a basis for development of custom-fitted outdoor walking shoes. The study analyzed an upright body exercise program using spine stabilization technique to determine the effect on foot plantar pressure in archers, demonstrate the effectiveness of upright body exercise, and develop a new, effective, and efficient training program. Method: A 12-week upright body exercise program was evaluated for the effect on plantar pressure in archers. Ten prize-winning archers (3 men, 7 women) in B metropolitan city, each with ${\geq}10years$ of experience, were given an explanation of the content and purpose of the program, and provided informed consent. Upright body exercise was performed 3 times a week for 12 weeks. A resistive pressure sensor was used to measure foot plantar pressure distribution and analyze quantitative information on variation in postural stability and weight shifting in dynamic balance during shooting, as well as plantar pressure in static balance with the eyes open and closed. Results: There were no significant differences in foot plantar pressure before and after participation in the exercise program. There was no statistically significant difference in foot plantar pressure in static balance with the eyes open or closed, or in foot plantar pressure in dynamic balance during shooting. Conclusion: An upright body exercise program had positive effects on foot plantar pressure in static and dynamic balance in archers by reducing body sway and physical imbalance during shooting and with eyes closed. This program is expected to help archers improve their posture and psychological state, and thereby improve performance.

Effect of Physical Therapy Based Tailored Exercise Program on Pain, Accident incidence Rates, and Lost Days of Work in Manufacturing Worker: Single Subject Design (제조업 근로자의 근골격계 질환 예방을 위한 물리치료 기반 맞춤형 운동프로그램이 통증, 재해율, 및 근로손실에 미치는 영향: 단일사례 연구)

  • PURPOSE: The purpose of this study was to investigate the effect of a physical therapy-based tailored exercise program on pain, accident incidence rates, the number of work days lost, and economical loss cost for workers in an automobile parts manufacturing company. METHODS: A total of 530 workers with musculoskeletal symptoms were given a physical therapy-based tailored exercise program twice a week, for one hour a day. This exercise program consisted of movement pattern correction, muscle stretching and strengthening, and postural correction exercises, according to principles of movement impairment syndromes and medical training therapy. From 2011 to 2016, the lost days of work, accident incidence rates, and loss cost were examined. The pain measured by VAS (visual analogue scale) and the number of workers participating in the exercise program from 2014 to 2016 were also measured. The single subjects A-B design was applied and analyzed. RESULTS: After applying the exercise program, pain decreased and the number of workers participating in the program increased. Accident incidence rates, number of work days lost, and economical loss cost decreased. There was a significant correlation between the number of workers who received exercise therapy by year and accident incidence rates, lost days of work, and economical loss cost (p<.05). CONCLUSION: It is necessary to expand the physical therapy-based tailored exercise program to prevent musculoskeletal disorders because it has a positive effect on both workers and employers.

Rapid Weight Loss and Physical and Psychological Difficulties in National Combat Sports Athletes (국가대표 투기종목 선수들의 급속 체중 감량과 신체적, 심리적 고충)

  • PURPOSE: The purpose of this study was to investigate the current state of rapid weight loss and physical and mental complaints among elite combat athletes who were selected and trained as national representatives. METHODS: A total of 127 combat athletes who had trained at the national training center in 2019 were enrolled. The questionnaire used as the survey tool was interpreted as words that Korean athletes could easily understand based on the Portuguese version of the 'Rapid Weight Loss Questionnaire' (RWLQ). χ2 tests was performed to determine the differences in physical and mental changes according to sports and weight loss method. RESULTS: Combat athletes in Korea regularly undergo rapidly weight loss within a short period of time prior to competition. There were statistically similar results in terms of weight loss method, physical and mental changes and information about weight loss according to the sport. Weight loss methods often limit food and water intake and dehydrate. The physical complaints associated with them are symptoms such as "no energy", "dizziness", and "muscle spasms". The mental complaints include "irritability", "concentration loss" and "anxiety". These symptoms can be caused by side effects such as food intake limitation, water restriction and dehydration. Information sources for weight loss were in the order of "senior or colleagues", "supervisor or coach", and "Internet". No information was obtained from a doctor or dietitian. CONCLUSION: These results provide a baseline for predicting proper weight loss in athletes and provide useful information for developing performance enhancement as well as weight loss.

The Effect of Active Neck Muscle Training in the Treatment of Chronic Neck Pain (만성 경통 환자의 치료에 있어서 능동적 경부 근육 훈련의 효과)

  • The purposes of this paper were to investigate the effect of active treatment compared with a conservative treatment and to provide the information for physical therapy in patients suffering from chronic neck pain. Forty female subjects who were diagnosed with cervical radiculopathy participated in this study and were divided into the conservative and active treatment groups. The active treatment group consisted of 20 patients who were treated with therapeutic modalities and active neck exercise program during the admission (15 days) and one month after discharge. The conservative treatment group consisted of 20 patients who were not received with active neck exercise program. The assessment tools were made using visual analogue scale(VAS), neck disability index(NDI) and modified Zung depression scale(MZDS). All subjects were measured three times: before the admission, at discharge, and at one month after discharge. Data were compared by groups using independent t-test. VAS, NDI and MZDS scores measured at admission and discharge were not significantly different between the groups. On the assessment performed one month after discharge, VAS and NDI scores were significantly lower for the active treatment group compared with those of the conservative treatment group(p<0.05), but MZDS score was not significantly different between the groups. In the comparison of two treatment methods for chronic neck pain, active treatment was more effective than conservative treatment. The findings of the study indicate that active treatment in chronic neck pain has a positive effect in relieving pain and restoring neck function.

Clinical Features and Long-Term Outcome in Adult Stroke Patient due to Moyamoya Disease : A Single Subject Study

  • This case report described a single case of adult stoke patient due to Moyamoya disease through long-term follow-up observation, which included his demographics, brain images, and change of motor function and functional activities. The subject was the 54-year-old male diagnosed with left hemiparesis from a stroke due to multifocal encephalomalacia in both hemispheres. At the time of the stroke attack, he took brain surgery intervention including external ventricular drain. Physical and occupational therapy for stroke rehabilitation were admitted including muscle strengthening exercises, functional activity/ADL training, neurofacilitative techniques with bobath or proprioceptive neuromuscular facilitation concepts, and compensatory strategy. Patient's MRI showed that right frontal lobe, right peri-ventricular area, left parietal, and left occipital lobes were damaged, and MRA showed that abnormal collateral vessel was richly developed in both hemispheres by occlusion of proximal internal carotid arteries in both sides. His motor strength was improved from poor to good grade in all of upper and lower limb motions, that MBC was improved from stage 1 to stage 5. In FAC and barthel index, at the initial evaluation, he could not perform any functional movement, but his FAC and barthel index were on 3 and 14 points at present, respectively. During long-term follow-up for approximately 4 years, the subject's functional motor ability was improved, as similar with recovery progression of usual stroke patient. We believe that this single case report will provide clinical information and concern regarding Moyamoya disease with physical therapist, in terms of such as epidemiology, pathogenesis, diagnostic procedures, clinical features, recovery process, and prognosis.
