The effect of co-contraction exercises of abdominal bracing combined with ankle dorsiflexion on abdominal muscle thickness and strength in patients with chronic low back pain

  • Objective: The purpose of this study was to determine the effect of the abdominal bracing (AB) and abdominal bracing combined with ankle dorsiflexion (ABDF) on abdominal muscle thickness and strength in patients with chronic low back pain (LBP). Design: Two group pretest posttest design. Methods: Sixteen subjects were divided randomly into two group: ABDF group (n=8), and alone AB group (n=8). The ABDF group practiced AB exercise with additional ankle dorsiflexion. AB group practiced only AB exercises. Subjects in both groups received ABDF exercise and AB exercise for 40 min per day, three days per week during a period of three weeks, respectively. All the subjects were evaluated for abdominal muscle thickness and strength before and after intervention using ultrasonography and MedX machine. Results: The external oblique (EO), internal oblique, transverse abdominis (TrA) muscle thickness and the strength produced at $48^{\circ}$, $60^{\circ}$, $72^{\circ}$ showed a significant increase in the ABDF group after intervention, with a more significant improvement in EO and TrA muscle thickness in the ABDF group compared with the AB group (p<0.05). Also, the strength at $48^{\circ}$ strength showed a significant improvement in the ABDF group than the AB group (p<0.05). Conclusions: The study results showed that abdominal muscle contraction exercises with AD in patients with LBP had an influence on abdominal muscle thickness and strength. Therefore, these findings suggest that ABDF may be useful approach for enhancement of abdominal muscle thickness and strength in patients with chronic low back pain.

The Comparison of Muscle Contraction Time and Modulation Ratio during Reaching Tasks in Hemiparalysis, Elderly and Young Adults

  • Objective: The purpose of this study was to evaluate the Electromyography (EMG) of the upper limb during reaching tasks according to two heights in the sitting position. Design: Cross sectional design Methods: Fifteen hemiplegia, fifteen elderly, and fifteen healthy subjects have participated in this study. The targets (90% length of the subject's arm) were located at the two heights (the eye and xiphoid process). We have recorded EMG signals of seven upper limb muscles (anterior deltoid (AD), posterior deltoid (PD), pectoralis major (Pec), infraspinatus (Inf), supraspinatus (Sup), biceps brachii (Bi), triceps brachii (Tri)). The dependent variables were movement time(s), modulation ratio, working ratio, and the co-contraction ratio of the hemiplegia, elderly, and healthy at the reaching task. Two-way repeated-measures ANOVA (2-heights) was analyzed with the LSD post hoc test. Results: The study results were as follows: (1) The movement time to the target during reaching movement was significantly longer for the hemiplegia and elderly groups compared to the healthy group. (2) The modulation rate was significantly higher at eye height than the xiphoid height in AD, PD, Pec, Inf, Bi muscles, and the hemiplegia group and elderly group were significantly lower than the healthy group. Additionally, the modulation ratio showed a significant interaction between heights and groups. Conclusions: It is expected that the variables using the muscle contraction characteristics, the evaluation method of this study, can be used as an electromyography-based feedback method that can be objectively evaluated and quantified in clinical practice.

전동식 운동기기의 부하방식에 따른 근력증진 효과 (Effect on Improvement of Muscle Strength for Loading Pattern using Electric Exercise Instrument)

  • This study is to compare muscle strength between isotonic exercise and isotonic & isokinetic exercise. Participants are 12-man and 10-woman whom they are healthy without medical history in shoulder, elbow and lumbar joint. We performed experiment total 4-weeks that exercise 3-days a week each exercise pattern. We measured shoulder, elbow and lumbar joint torque with BIODEX and circumference of muscle in upper arms once a week. The result showed that isotonic & isokinetic exercise pattern significantly more improved joint torque in shoulder, elbow, lumbar than isotonic exercise pattern. Because that isotonic & isokinetic exercise pattern supplied muscle strengthen and caused muscle contraction. This exercise pattern can be used new exercise training method for major athlete and normal people. Also this pattern can be used rehabilitation treatment.

The Flowers of Carthamus tinctorius : Potential Agent for Postmenopausal Disorder

  • In this study, 75% ethanol extract from the flowers of Carthamus tinctorius was prepared and biological activities were examined. The extract showed the inhibitory activity of vascular smooth muscle contraction and antithrombotic activity judged by bleeding time measurement. It also showed anti-inflammatory and potent analgesic activities in vivo. By oral administration of the extract, no acute toxicity was observed up to 5 g/kg in mice and rats. All these results strongly suggest that this extract may be beneficial for postmenopausal disorder by enhancing blood circulation.

복부 동시수축 형태에 따른 하지 근활성도 비교 (Comparison of Lower-Limbs Muscle Activity according to the Abdominal Co-contractive Activation)

  • 본 연구는 복부 동시수축 형태에 따라 하지의 근활성도에 어떤 영향을 미치는지에 대해 알아보고자 하였다. 연구대상자는 허리와 하지에 통증이 없고, 근골격계에 이상이 없는 건강한 20대 여성 30명을 대상으로 진행하였다. 복부 이완(Rest), 복부 당기기 기법(ADIM), 복부 브레이싱 기법(ABM)을 유지하는 각 자세에서 무릎의 굽힘없이 발을 능동적으로 바닥으로부터 20cm 올린 상태(ASLR)로 약 10초간 유지하도록 하여 하지의 5개 근육(중간볼기근, 엉덩긴모음근, 넙다리곧은근, 넙다리두갈래근, 앞정강근)의 근활성도를 측정하였다. 연구결과, Rest에서 ASLR을 유지하였을 때보다 ADIM 수축형태로 ASLR 유지할 때 하지의 모든 근육에서 근활성도가 높게 나타났다. 또한 Rest에서 ASLR을 유지하였을 때보다 ABM 수축형태로 ASLR을 유지하였을 때 중간볼기근을 제외한 나머지 근육에서 모두 근활성도가 높게 나타났다. ADIM과 ABM을 비교한 결과 중간볼기근과 앞정강근은 ADIM 수축형태에서, 엉덩긴모음근, 넙다리곧은근, 넙다리두갈래근은 ABM 수축형태에서 통계적으로 유의하게 높은 근활성도가 나타났다. 본 연구의 결과로 체간중심 안정화 운동이 필요한 요통 환자의 선제적 복부 수축형태로서 뿐만 아니라 하지의 선택적 근력강화가 필요한 환자들에게 개별화된 방법으로써 근거를 제시할 수 있을 것이다.

흰쥐의 적출배뇨근에서 baclofen의 콜린성신경 억제작용 (Effect of Baclofen on the Cholinergic Nerve Stimulation in Isolated Rat Detrusor)

  • 배뇨근의 운동성에 대한 baclofen의 억제작용의 기전을 탐구하기 위하여 다음과 같은 실험을 하였다. 흰쥐 (Sprague-Dawley)의 방광에서 적출한 배뇨근절편을 적출근편실험조에 현수하고, 등척성 장력측정기를 사용하여 그 수축력을 묘기하였다. 실험조내의 영양액의 온도는 $37^{\circ}C$로 유지시키고, 95% 산소와 5% 이산화탄소의 혼합기체를 공급하여 pH를 7.4로 유지하였다. 배뇨근절편은 전기장자극에 의해 수축하였는데, 8분간의 전기장자극 유발수축 중 최초의 급격한 수축반응은 mATP와 baclofen에 의해 약간 억제되는 경향을 보였으며, 후기 4분간의 수축반응은 콜린성 무스카린성 수용체 봉쇄제인 atropine과 $GABA_B$ 수용체 효현제인 baclofen에 의해 유의하게 억제되었다. Atropine은 배뇨근 절편의 acetylcholine 유발 수축을 길항하였고, mATP는 ATP 유발 수축을 완전히 봉쇄하였으나, baclofen 존재하에서는 acetylcholine이나 ATP 첨가에 의한 배뇨근의 수축이 영향을 받지 않았다. 이상의 결과를 종합하면, 흰쥐 방광에는 $GABA_B$ 수용체가 존재하며 baclofen은 이 수용체를 통하여 콜린성신경 말단에서의 신경전달체의 유리를 억제하여 배뇨근의 수축성을 감소시킨다고 사료된다.

실내 승마운동이 고령자의 기초체력 및 ADL 기능에 미치는 효과 (Effect of Basal Physical Fitness and ADL Function in the Aged According to Horse Riding Exercise)

  • The purpose of this study was to verify exercise effect of horse riding exercise according to estimate basal physical fitness and activities of daily living(ADL) function in the aged. Participants were nineteen peoples who have no impediment of activity. They performed horse riding exercise using SRider(Neipplus, Co., Korea) at sixty minutes a day. Exercise has progressed three days a week for eight weeks. We measured trunk flexion, sit up, whole body reaction, leg strength and maximal oxygen uptake as basal physical fitness. Also three meter gait, single stance with eyes opened and single stance with eyes closed as ADL function were estimated once a month. The result of legs strength and whole body reaction showed the higher significantly than before the exercise. Moreover, the result of three meter walking ability only increased significantly among the ADL function. This means that horse riding exercise might be activated continuous muscular contraction with maintained tonus of muscle. We thought that continuous movement of horse riding could be lead to isometric muscle contraction in lower limbs. Our study found that horse riding exercise could improve lower strengths and muscle reaction for exercise effect. Also we suggested that horse riding exercise could be adapted to exercise methods that could provide rehabilitation and treatment enough for the aged or disabled person.

노인 보행 시 하지 근 활동 양상과 관절의 안정성이 낙상에 미치는 영향 -전향적 연구(Prospective Study)- (Effects of Muscle Activation Pattern and Stability of the Lower Extremity's Joint on Falls in the Elderly Walking -Half a Year Prospective Study-)

  • Objective: The aim of this study was to determine the peak torques of the knee and ankle joint and local stability of the lower extremity's joints, and muscle activation patterns of the lower extremity's muscles between fallers and non-fallers in the elderly women during walking. Method: Four elderly women (age: $74.5{\pm}5.2yrs.$; height: $152.1{\pm}5.6cm$; mass: $55.3{\pm}5.4kg$; preference walking speed: $1.19{\pm}0.06m/s$) who experienced falls within six months since experiment had been conducted (falls group) and thirty-six subjects ($74.2{\pm}3.09yrs.$; height: $153.6{\pm}4.9cm$; mass: $56.7{\pm}6.4kg$; preference walking speed: $1.24{\pm}0.10m/s$) who had no experience in falls (non-falls group) within this periods participated in this study. They were measured torque peaks of the knee and ankle joint using a Human Norm and while they were walking on a treadmill at their natural pace, kinematic variables and EMG signals were collected with using a 3-D motion capture system and a wireless EMG system, respectively. Lyapunov Exponent (LyE) was determined to observe the dynamic local stability of the lower extremity's joints, and muscles activation and their co-contraction index were also analysed from EMG signals. Hypotheses between falls and non-falls group were tested using paired t-test and Mann-Whitey. Level of significance was set at p<.05. Results: Local dynamic stability in the adduction-abduction movement of the knee joint was significantly lower in falling group than non-falling group (p<.05). Conclusion: In conclusion, muscles which act on the abduction-adduction movement of the knee joint need to be strengthened to prevent from potential falls during walking. However, a small number of samples for fallers make it difficult to generalize the results of this study.

안지오텐신 II의 적출심근 및 대동맥 평활근에 대한 작용기전 (Effects of Angiotensin II on Isolated Cardiac Muscle and Aortic Strips in Rabbit)

  • Contractile responses of myocardium and vascular smooth muscle to angiotensin II were studied in isolated rabbit papillary muscles and aortic helical strips, with respect to the sensitivity and the mechanism of action. All experiments were performed in $HCO-_3\;-buffered Tyrode solution which was aerated with $3%\;CO_2-97%\;O_2$ and kept pH 7.35 at $35^{\circ}C$. Action potentials were measured by conventional microelectrode technique in the papillary muscles. Helical strips of vascular smooth muscle were prepared from the descending thoracic aorta of the rabbit. Angiotensin II elicited a positive inotropic effect in doses from $10^{-8}$ to $10^{-6}\;M$, and this effect was dose-dependent and characterized by a symmetrical increase of maximum dP/dt during contraction and relaxation phase. Slow responses (or slow action potentials) were induced by A. II $(10^{-6}\;M)$ in the papillary muscle hypopolarized by 27 mM $K^+$. These A. II-induced slow action potentials were eliminated by verapamil (2 mg/l), but not affected by propranolol $(10^{-5}\;M)$. In aortic helical strips, contractile force was increased dose-dependently in the range of $10^{-10}{\sim}10^{-7}\;M$ A. II. $ED_{50}$ in aorta was $3{\times}10^{-9}\;M$ A. II, whereas that in paillary muscle was $2.5{\times}10^{-7}\;M$ A. II. A. II contracted vascular smooth muscle in depolarizing concentration of $K^+$ (100 mM $K^+$), and also produced a sustained contraction even in the presence of verapamil and regitine. The results of this experiment suggest that the primarily important physiological role of A. II is the action on the blood vessel, and the positive inotropic effect of A. II in papillary muscle results from the increase of slow inward $Ca^{++}$ current, and that A. II-induced contraction of aorta is independent of transmembrane potential and associated with promoting bet transmembrane $Ca^{++}\;-influx$ and the mobilization of cellular $Ca^{++}$.

저주파 플라이백 스위칭회로를 이용한 고성능 자기자극기 (A Magnetic Stimulator Adopting a Low-Frequency Fly-Back Switching Circuit)

  • Medical magnetic stimulator generates strong magnetic field pulses. Clinical applications of the magnetic pulse are the stimulation of nervous system and the contraction of muscle. The unique source of the strong magnetic pulse is a capacitor-inductor resonator and this inductor generates a strong sinusoidal magnetic pulse by discharging the capacitor with high initial voltage. Continuous muscle contraction needs sequential generation of the magnetic pulses. However, to keep the magnitude of sequential pulses identical, an expensive high-voltage power supply have to support voltage drop of the capacitor between the pulses. A protection circuit between the supply and the resonator is necessary to protect the supply from reverse current caused by capacitor voltage reversal. In this paper, a new circuit structure of the magnetic stimulator adopting a low-frequency fly-back switching is proposed. The new circuit supports sequential pulse generation and allows the reverse current without damage. Performance of the new circuit is examined and a low-cost magnetic stimulator for urinary incontinence therapy is being developed using the presented method.
