원발성, 다발성 뇌종양 - 증례보고 - (Multiple Primary Brain Tumors - Case Report -)

  • Multiple intracranial tumors of different cell types are rare. We report a case of double primary intracranial tumors of different histologic types : oligodendroglioma, and pituitary adenoma. Von Recklinghausen's disease or other etiologic factors supposedly associated with multiple brain tumors were not recognized in this 36-year-old male patient.

신경섬유종증 II형 소아에서 발생한 거대 삼차신경초종 (A Huge Trigeminal Schwannoma in a Child with Neurofibromatosis Type II)

  • We report a case of huge trigeminal schwannoma in a 10-year-old boy with neurofibromatosis type II, extending into the three spaces of the left middle, posterior, and infratemporal fossa. Initially we thought the child had a solitary trigeminal schwannoma, and most of the mass was resected successfully through one-stage operation, cranio-orbito-zygomatic intradural approach. However during the follow-up of the patient we found radiographically other multiple intracranial tumors of bilateral acoustic schwannomas, right trigeminal schwannoma, and foramen magnum tumor. Eventually the patient was diagnosed as neurofibromatosis type II presenting multiple intracranial tumors. We think childhood trigeminal schwannoma, even though in the case of solitary tumor, should be considered as possible initial manifestation of neurofibromatosis type II and that careful follow-up for the possibility of occurrence of other brain tumors such as schwannomas or meningiomas is necessary.

  • Intracranial involvement in multiple myeloma patients takes up around 1%, and is usually known to be present in the parietal bone or skull base in cases of skull vault involvement, while it presents in the dura and parenchyma in cases of intracranial involvement. Primary pachymeningeal invasion is even rarer with extremely rapid progression and very poor prognosis. It is our intent to report a case in which we had to differentiate multiple myeloma with other metastatic tumors, lymphoma, and leukemia with intracranial involvement. Our patient showed an osteolytic lesion of the skull with dural involvement and subdural mass formations.

운동중추 주변에 위치한 전이성 뇌종양의 수술 후 운동 기능에 대한 평가 (Evaluation of the Postoperative Motor Function for Metastatic Brain Tumors Around the Motor Cortex)

  • Objective : Intracranial metastasis is responsible for nearly 50% of mortalities of systemic cancers. Although its frequency is increasing and there is little doubt that improving the quality and expectancy of life is the final goal, the mode of treatment is still disputed. The authors report the postoperative motor function after surgery in patients with metastatic tumors around the motor cortex. Materials and Methods : We studied 24 patients with metastatic tumors around the central sulcus during the last 22 months. Motor function was assessed pre- and post-operatively as well as its response to corticosteroids. MRI, neuronavigation system and intraoperative ultrasonography were used for tumor localization and functional MRI and cortical stimulator were used to define the motor cortex. Results : Single metastasis was found in 13 cases(54%) and 11 cases(44%) had multiple foci. Thirteen cases were located in precentral, 7 in postcentral, and 4 in superior or middle-frontal lobe. The most common primary focus was the lung(16 cases). There was no difference in postoperative motor function improvement between the steroid responsive group and non-responsive group(92% versus 90%). Ninty-two percent of the patients showed significant improvement of motor function and lived independently but there was worsening in the upper extremity in one and in another no improvement. Whole brain radiation of 3000cGy was given in all cases and 4 patients died of recurrence in primary or intracranial focus during mean follow-up periods of 14 monthes. Conclusion : Surgery may provide substantial improvement of the motor function and quality of life of the patients with metastatic tumors around the motor cortex.

  • Ameloblastoma of the maxilla is an unusual epithelial tumor of odontogenic origin. According to many authors and reports, ameloblastoma account for approximately 1% of all tumors of the jaws, but when pseudotumors and cysts are excluded, the ratio rises to 11%. Of these tumors,80% originate in the mandible, while 20% originate in the maxilla. Although it is considered benign histopathologically, it can behave in a slowly growing infiltrative fashion, with multiple recurrences and eventual intracranial, or even distant, spread. We clinically analyzed common site in maxilla, radiographic findings, recurrence rate, duration between treatment and recurrence, the presence and site of distant metastasis in 15 patients who were diagnosed as ameloblastoma of the maxilla and took treatments from 1985 to 1999 in Dept. of Oral and Maxillofacial Surgery, Dental Hospital, Seoul National University. In this paper, treatment outcomes and our clinical experiences of maxillary ameloblastoma are reported with review of literatures.

Evaluation of P57, P53 and Ki67 Expression in Meningiomas

  • Objective : We conducted this study with the aim of predicting the biological behavior of meningiomas, and determining the benefits of associating histological subtype and grade with the expression of proliferative markers and tumor suppressor proteins. Methods : The study included 29 patients with primary intracranial and intraspinal meningioma diagnosed in the pathology laboratory of Konya City Hospital between January 2014 and December 2020. Clinicopathological characteristics of the patients including parameters such as age and gender were obtained from the hospital records. Histopathological findings were obtained by re-evaluating the preparations stained with Hematoxylin-Eosin, which were extracted from the archive, and by evaluating new sections obtained from paraffin blocks of patients stained with Ki67, p53, and p57 immunohistochemical stains. Results : A moderate correlation was found between tumor size and Ki67 proliferation index (PI) (p=0.003, r=0.530). There was no significant difference between grade I and grade II tumors in terms of p53 (p=0.184) and p57 (p=0.487) expressions. There were higher levels of Ki67 PI in grade II tumors. The histological subtypes of the tumor had no significant difference with Ki67 PI (p=0.018), p53 (p=0.662), and p57 (p=0.368) expressions. Conclusion : In order to obtain more definitive results, there is a need for studies, which are conducted with a greater number of patients and in multiple centers, and in which a long prospective follow-up is planned. The combination of histological, surgical, and imaging markers could make a more sensitive tool for predicting recurrence, and this could also be tested in future studies.

미세낭종성 혈관종성 혼합 수막종의 영상 및 병리 소견: 증례 보고 (Imaging Features and Pathological Correlation in Mixed Microcystic and Angiomatous Meningioma: A Case Report)

  • 수막종은 가장 흔한 두개내 종양이지만, 그 아형인 미세낭종성 및 혈관종성 수막종은 매우 드물고 특이한 영상 소견으로 인해 영상의학적 진단이 어려울 수 있다. 이에 저자들은 미세낭종성 혈관종성 혼합 수막종으로 진단된 81세 남자 환자 증례를 보고하고자 한다. 자기공명 T2 강조영상에서 병변은 왼쪽 전두부에 약 1.5 cm 크기의 고신호강도의 축외 종양으로 보이고 있었고, 종양 내에 다수의 아주 작은 낭종들이 관찰되었으며, 종양 주위로 포획된 큰 낭을 형성하고 있었다. 수술적 절제를 통한 병리 검사상 미세낭종성 혈관종성 혼합 수막종으로 최종 진단되었다.

송과선 및 이소성 송과선 생식세포종의 감마 나이프 수술 후 재발 양상 (Failure Pattern of Pineal and Ectopic Pineal Germ Cell Tumor after Gamma Knife Radiosurgery)

  • 목적 :본 연구의 목적은 뇌 생식 세포종 환자들의 방사선치료 시 가장 적절한 조사 영역을 알아보고자 시행하였다. 대상 및 방법 : 1993년부터 1998년까지 뇌 생식세포종으로 진단되거나 또는 추정되어 감마 나이프를 시행 받은 환자 19 명을 대상으로 분석하였다. 송과선 9예, 안상(suprasellar) 1예, 그 외 2군데 이상 다발성 병소가 9예였다. 조직이 확인이 된 예는 7예이었고 배아종(germ ceil tumor)이 5명, 내배엽동종(endodermal sinus tumor)이 2명이었다. 종양의 부피는 2.4 cm$^{3}$부터 74 cm$^{3}$ 까지 이었다. 감마나이프 방사선 치료는 50% 등선량 곡선을 중심으로 10 Gy부터 20 Gy에 걸쳐 조사되었다. 추적 기간은 10개월에서부터 54개월까지였다. 결과 : 총 19명 중 14명(74%)에서 재발을 하였다. 완전 관해와 부분 관해는 각각 2명(11%), 10명(53%)이었다. 무반응은 7명(36%)이었다. 원발 병소가 있었던 자리에서 재발한 경우가 2예, 치료 조사영역을 벗어났으나 원발 병소와 연결되어서 그 주위로 재발한 경우가 6예 이었다. 원발 병소와 떨어져서 뇌실 재발이 된 경우 3예, 척수 전이가 된 경우가 4예 이었다. 종양의 부피가 20 cm$^{3}$ 이하인 경우는 8예이었으며 이중 2예는 치료 조사영역 내에서만 재발한 경우, 4예는 원발 병소와 연이어져서 치료 부위 주위로 재발한 경우, 1예는 척수 전이된 경우이었다. 종양의 부피가 20 cm$^{3}$ 보다 큰 경우는 6예 이었으며 그 중 원발 병소와 연이어져서 치료 부위 주위에 재발한 경우 1예, 원발 병소와 떨어져서 뇌실 전이가 된 경우가 2예, 척수 전이를 일으킨 경우가 3예였다. 재발을 하지 않은 5예는 종양의 부피가 20 cm$^{3}$ 이하인 경우이고 모두 단일 병소이며 종양기표가 모두 정상이었다. 척수 전이는 4예(21%)에서 발생하였으며 모두 뇌실 침범이 있는 경우에 발생하였다. 총 9명의 다발성 병소 중 국소 재발만을 한 경우는 3경우이었고 나머지는 모두 치료 조사영역을 벗어나 원발 병소와 떨어져서 재발하였다. 결론 : 감마 나이프 치료가 뇌 생식세포종에 대한 치료로서는 부적절한 치료이며 이것은 감마 나이프의 특성인 작은 치료 용적과 조사 선량의 부적절함에서 기인하는 것으로 판단된다. 뇌 생식 세포종에서 병소 부위 만을 치료하는 경은 종양의 부피와 다발성 병소의 뇌실 침범 유무가 치료 성공의 열쇠이다. 20 cm$^{3}$ 이하, 단일 병소, 뇌실 침범이 혀는 경우, 정상적인 종양지표, 등이 가장 이상적인 적응증이 될 수 있다. 다발성 병소에서 뇌실 침범이나 뇌실 병소가 있을 경우는 예방적 뇌 척수 조사를 고려해야 할 것으로 생각된다. 병소의 크기가 cm$^{3}$ 보다 클 경우 다발성 병소인 경우, 종양지표의 증가가 있는 경우에는 확정적인 제안을 하기는 어렵지만 전 뇌실 조사 또는 부분방사선 조사가 시도될 수 있을 것으로 생각되며 이 경우가 선행 화학 요법과 함께 치료할 수 있는 대상이며, 앞으로 이 부분에 대한 연구가 계속 이루어질 것으로 생각된다.

포도막 흑색종에 대한 감마나이프 방사선수술 - 증례보고 - (Gamma Knife Radiosurgery on Uveal Melanoma - Cases Report -)

  • Uveal melanoma is uncommon but life-threatening intraocular malignancy and has been treated by irradiation, local excision and enucleation. Gamma-Knife radiosurgery allows a high dose of radiation to be delivered to an intracranial target with a very high spatial accuracy and has been used for the treatment of ocular melanomas. We have treated two cases of uveal melanoma between October 1994 and December 1999. They include one man and one woman(34, 62 years, respectively). They were followed up for 12 momths. Mean maximal dose was 65Gy. In one case, the tumor disappeared 7 months after gamma-knife radiosurgery. In another case, multiple tumors (uveal, suprasellar and cerebellar tumor) had decreased in size. These results show that single and high dose gamma-knife radiosurgery is may be an option in the local control of uveal melanoma which can spare the eyeball and vision.

파종성 악성 핍지신경교종 환자에서 시행한 뇌실내 Methotrexate의 효과 - 증례보고 - (The Effect of Intraventricular Methotrexate in a Patient of Disseminated Anaplastic Oligodendroglioma - Case Report -)

  • Oligodendrogiomas account for about 4 per cent of intracranial gliomas and surgery is known to be an essential first step to establish an accurate diagnosis and when oligodendrogliomas recur with or without anaplastic features after initial resection, radiation and chemotherapy consisting of the administration of procarbazine, lomustine, and vincristine are usually indicated. We report our experience of an excellent result with intraventricular methotrexate chemotherapy for a patient with disseminated anaplastic oligodendroglioma. A 29-year-old male patient presented with diplopia and headache for two months. MRI showed a irregular, faintly enhanced mass in the posterior fossa. The hisotological diagnosis was an anaplaplastic oligodendroglioma and he was treated with chemotherapy of PCV regimen and radiotherapy followed by surgery. CSF dissemination was revealed by a follow-up MRI during the period. Intraventricular methotrexate(0.175mg/kg) was given twice a week for 4 weeks through ommaya reservoir and the size of the multiple tumors was decreased significantly on follow-up MRI. This case report suggests that an aggressive treatment involving intravent-ricular chemotherapy may be helpful even when anaplastic oligodendrogliomas disseminates to leptomeninges.
