유방재건술에서 조직확장기 감염의 위험인자 (Risk Factors of Tissue-Expander Infection in Breast Reconstruction)

  • Purpose: Implant-based breast reconstruction has multiple advantages such as decreased morbidity, shorter operative time and faster recovery. However, postoperative infection with tissue expander increases medical cost and causes a delay in concurrent antineoplastic treatment. To reduce tissue expander infection, it is important to identify related risk factors and minimize them when possible. Methods: A retrospective review of patient records in a single breast cancer center was performed. Eighty-six tissue expanders were placed in 80 women for postmastectomy breast reconstruction. Variables including patients'age, body mass index (BMI), preoperative breast volume, operation time, drain indwelling time, postoperative seroma/hematoma formation, chemotherapy, and radiation therapy were evaluated. Infection was defined as the status that shows any symptom of local inflammation and identification of pathogens. Representative values were compared through Student's t-test and univariate and multivariate analyses. Results: We examined 86 postmastectomy tissueexpanders which were placed between June 2004 and April 2010. Seven cases of tissue expander infection (8.1%) were identified. The infected tissue expander was removed in three of the cases. The relationship between BMI, and preoperative breast volume and that between infection and non-infection groups were significant ($p$ <0.05). Univariate analysis showed significant association between BMI ($p$=0.023) and preoperative breast volume ($p$=0.037). Multivariate analysis revealed that BMI and preoperative breast volume were independent variables regarding tissue expander infection. Conclusion: Certain characteristics of implant-based breast reconstruction patients increase infection rate of tissue expander. These risk factors should be monitored and evaluated before surgeries for more successful outcome.

  • Multiple myeloma is a lymphohematopoietic disorder leading to abnormal hemostasis and significant pathologic changes of skeletal system. It induces multiple circular or oval-shaped radiolucent lesions which are characterized by 'punched-out appearance'. The surrounding trabecular bone normally shows no significant sclerotic reaction. Multiple myeloma patients may visit dental clinics, without perception of the disease themselves, due to discomfort from edema of orofacial region, oral ulcers, tooth mobility, pain or gingival bleeding. Multiple myeloma is susceptible to various complications, including delayed hemostasis and infection, which could occur during routine dental treatment such as periodontal and surgical operation. For radiographic diagnosis of multiple myeloma, common radiologic features of this tumor could be visualized by panoramic radiographs in the dental clinics, and further medical examinations and treatment can be recommended as a result.

제주특별자치도 종합병원의 감염관리 현황 (Status of Infection Control in Jeju-area General Hospitals)

  • 제주지역 종합병원 감염관리 현황을 조사하여, 병원감염관리 실태와 문제점을 파악하고자 하였으며 감염관리의 중요성을 인식시키고 감염관리 실무자로서 임상병리사의 역할 방안을 모색하기 위하여 제주특별자치도 제주시와 서귀포시에 위치한 종합병원 7개 기관을 대상으로 감염관리 현황을 설문지를 통하여 조사하였다. 제주지역 의료기관의 감염관리는 모든 기관이 감염관리실을 운영하고 전담인력을 구성하고 병원감염관리지침을 보유하고 수행하는 우수한 점을 보였지만, 대부분의 기관이 간호사 1인 구성의 감염관리실을 운영하여 전담인력 확보 및 미생물 관련 배양 판독, 다제내성균의 출현빈도, 감염통계 등 미생물 검사에 대한 내용을 보고 받는데 많은 부분 어려움을 호소하였다. 이에 감염관리의 실질적인 업무를 담당할 수 있는 임상병리사가 의료법 시행규칙에 있는 감염관리에 경험과 지식이 있는 사람으로 충분한 자질과 경험이 있다고 생각되며 감염관리실을 운영함에 있어 감염관리간호사와 미생물에 전문적인 지식을 갖춘 임상병리사(감염관리임상병리사)가 전담자로서 업무를 수행한다면 지금보다 더욱 발전된 감염관리실 운영이 될 것으로 보이며, 향후 감염관리를 제대로 하기 위해서는 전국적으로 종합병원을 비롯한 취약한 중소병원의 감염관리에 대한 전폭적 지원과 감염관리 전문 의료시스템 도입이 중요한 과제라 사료된다.

  • 현재 성병은 성매개 질환(sexually transmitted disease, STD)은 무증상 감염도 포함하는 의미에서 "성매개 감염"(sexually transmitted infections, STI)이라고 표현하고 있다. STI는 다양한 상호 관계를 가지고 있다. 본 연구는 보건복지부 장관이 고시하는 STI로 매독, 임질, 클라미디아 감염증, 연성하감, 성기단순포진, 첨규콘딜로마, 사람유두종바이러스 감염증과 비임균성요도염(non-gonococcal urethritis, NGU)을 대상으로 하였다. 따라서 다발성 및 동시성 STI를 확인하여 요인을 특성화하는 것을 목적으로 하였다. 본 연구는 2014년부터 2019년까지 서울에 위치한 일개 수탁검사기관의 검사실정보시스템(laboratory information system)의 자료를 활용하였다. 본 연구에서 다발성 STI로 두 가지 감염균의 중복 STI는 10 종류, 세 가지 감염균의 다중 STI는 6 종류로 나타났다. 이러한 16 종류의 다발성 STI 중 가장 많은 비중을 차지하는 균은 U. urealyticum (9가지), HSV-2 (8가지), C. trachomatis (7가지), HPV 6, 11 (7가지), N. gonorrhoeae (6가지), T. pallidum (1가지)에 포함되었다. 따라서 본 연구 결과에서 다발성 STI에 가장 비중이 높은 STI 등 상호 관계연구의 추가연구가 필요할 것으로 사료된다.

치과위생사의 근무환경이 감염관리활동에 미치는 영향 (Influence of working environment on infection control activities in dental hygienists)

  • Objectives: The objective of the study was to infection control by analyzing the influence of working environment on infection control activities in the dental hygienists. Methods: The cross-sectional study was based on a survey on a total of 377 dental hygienists working in dental settings. Multiple linear regression analysis was performed to examine the relationship of general characteristics and infection control activities. All statistical analyses were performed using the SPSS for Windows version 20.0, and p<0.05 was considered to be significant. Results: Predictive powers(=Adjusted $R^2$ of the investigated factors such as operation room, sterilization disinfection laundry, dental unit waterline, staff individual, infection control system, personal protective equipment, medical waste, hand hygiene, oral surgical procedures, clinical contact surfaces were adjusted $R^2=0.394$, 0.306, 0.277, 0.244, 0.241, 0.177, 0.165,, 0.154, 0.134, 0.124 respectively. Conclusions: In order to enhance infection control activities, the program development and implementation for the aseptic procedure is very important. The program should include periodic reinforcement of infection control education and regular monitoring of infection control activities.

경남도민의 COVID-19 감염예방행위에 미치는 영향요인: 불안, 건강신념을 중심으로 (Gyeongnam Inhabitants' Effects of COVID-19 Infection Prevention Behavior: Focus on Anxiety and Health Belief)

  • Purpose: This study was aimed to identify the influencing factors on the COVID-19 infection prevention behavior of adults residing in Gyeongsangnam-do by examining relationships between anxiety, health belief and COVID-19 infection prevention behavior. Methods: The participants of this study were recruited from a website, and the total number of participants was 186. The self-report questionnaires were distributed during July 2020. Results: Multiple regression analysis revealed the significant factors affecting the COVID-19 infection prevention behavior of adult residing in Gyeongsangnam-do were perceived barriers(��=-3.68, p<.001), perceived benefits(��=3.39, p=.001), perceived susceptibility(��=2.83, p=.005), sex(��=2.59, p=.010). The total explanatory power of the study variables for COVID-19 infection prevention behavior was 35.0%(F=17.61, p<.001). Conclusions: Based on these findings, in order to promote COVID-19 infection prevention behavior, there is needed to reduce obstacles for carrying out infection prevention behaviors, and develop multi-faceted promotional strategies for infection prevention behaviors.

코로나19 대유행에 따른 간호사의 감염관리 피로도, 사회적 지지가 소진에 미치는 영향 (Influence of Infection Control Fatigue and Social Support on Nurses' Burnout During the Coronavirus Disease 2019 Pandemic)

  • Purpose: The purpose of this study was to understand the effects of infection control fatigue and social support on burnout among nurses during the Coronavirus Disease 2019 (COVID-19) pandemic. Methods: This study is a descriptive survey study designed to confirm the effects of nurses' infection control fatigue and social support on burnout among nurses during the COVID-19 pandemic. The collected data were analyzed using frequency, percentage, mean, standard devia analyzed using frequency, percentage, mean, standard deviation, independent t-test, One-way ANOVA, Scheffe test, Pearson's correlation coefficient, and stepwise multiple regression analysis using SPSS Statistics 26.0. Results: An analysis of the correlations between fatigue from infection control, social support, and burnout showed a positive correlation between burnout and fatigue from infection control (r=.39, p<.001), and a negative correlation between burnout and social support (r=-.29, p<.001). Conflict and lack of support due to uncertain circumstances (β=.51, p<.001), support from supervisor's (β=-.22, p<.001), and experience of infection management education during the previous 1year (β=-.15, p=.007) were identified as the factors that influenced burnout among nurses, and explained 39.0% of the variance in burnout. Conclusion: The results of this study demonstrate that fatigue from infection control and social support influence burnout levels among nurses, which suggests the need to establish a new kind of work culture. Additionally, the findings call for the development and implementation of interventional programs that can reduce fatigue from infection control and increase social support for nurses.

Correlation between Concerns about the Infection of Economic Workers due to the COVID-19 Pandemic and the Practice of Tooth Brushing after Lunch

  • Background: Like direct infection from COVID-19, psychological concern about infection could affect health. Concern about COVID-19 infection was associated with individual habits to practice rules for preventing infection. Therefore, this study aimed to check occupational types and whether to practice tooth brushing after lunch depending on the occupation of economic workers and find correlations between concerns about infection due to COVID-19 pandemic and tooth brushing after lunch. Methods: The raw data was from the community health survey conducted in 2020. Among 229,269 adult participants aged 19 years and older, 138,970 economic workers were included in the final analysis. The chi-squared test was used to find differences in psychological concerns due to the COVID-19 pandemic. According to the participants, the rate of practicing tooth brushing after lunch was based on COVID-19-related psychological concerns. Multiple logistic regression analysis was conducted to check the influence of psychological concerns due to the COVID-19 pandemic on the rate of practicing tooth brushing after lunch. Results: According to occupational classifications, professionals and office workers and career soldiers had 1.551- and 1.581-times higher practicing rates than managers, respectively, whereas machine operators, agricultural and fishery sector workers, and daily laborers had lower practicing rates. Regarding COVID-19-related psychological concerns, the group with a lower concern about infection had a 1.076 times higher practicing rate than that with greater concern. The group with greater concern about blame from neighbors had 1.119 times higher practicing rate than that with lower concern. Conclusion: The correlations between higher economic workers' concerns about infection and blame from neighbors and higher recognition of the necessity to prevent COVID-19 and practice tooth brushing after lunch were confirmed. It is necessary to prepare measures for practicing tooth brushing after lunch suitable to the characteristics of occupational types and work environments of economic workers.

Assessment of infection control in oral radiology during the COVID-19 outbreak: An international collaborative study

  • Purpose: In the context of COVID-19, studies evaluating the adherence of dentists and dental students to infection control are relevant due to their high occupational exposure and risk, as well as their responsibility for disseminating information and good practices. This study evaluated the adherence of dentists and dental students to infection control in the oral radiology field in the setting of the COVID-19 pandemic on different continents. Materials and Methods: This cross-sectional observational study involved individuals who performed intraoral radiographic examinations during the COVID-19 pandemic. The Questionnaire on Infection Control in Oral Radiology was administered virtually using a Google Form. Participants from different continents(the Americas, Africa, Europe, Asia, and Oceania) were recruited. Data were subjected to descriptive analysis and simple and multiple binary regression (5%). Results: There were 582 valid answers, and 68.73% of the participants were from the Americas, 18.90% from Europe, and 12.37% from Asia. The median score for infection control protocols was 94 points for dental students and 104 points for dentists, and participants below the median were considered to have low adherence to infection control in oral radiology. Low access to infection control was found for 53.0% of dentists in the Americas, 34.0% from Europe, and 26.9% from Asia. Conclusion: The adherence to infection control protocols in oral radiology was low even in the face of the COVID-19 pandemic. The results may help improve the awareness of students and professionals, since oral radiology routines have the potential for transmitting COVID-19.

체외 Interferon-gamma 검사를 이용한 결핵감염의 진단 (Diagnosis of Mycobacterium tuberculosis Infection using Ex-vivo interferon-gamma Assay)

  • Until recently, the tuberculin skin test (TST) has been the only tool available for diagnosing a latent TB infection. However, the development of new diagnostic tools, using the Mycobacterium tuberculosis (MTB)-specific early secreted antigenic target 6 (ESAT-6) and culture filtrate protein 10 (CFP-10) antigens, should improve the control of tuberculosis (TB) by allowing a more accurate identification of a latent TB infection (LTBI). Antigen-specific interferon-gamma ($IFN-{\gamma}$) assays have greater specificity in BCG-vaccinated individuals, and as less biased by nontuberculous mycobacterial infections. Many comparative studies have suggested that those assays have a higher specificity than the TST, and the sensitivity of these assays are expected to remarkably improved if more MTB-specific antigens can become available. Nevertheless, the major obstacle to the widespread use of these tests is the limited financial resources. Similar to other diagnostic tests, the predictive value of $IFN-{\gamma}$ assays depends on the prevalence of a MTB infection in the population being tested. Therefore, prospective studies will be meeded to establish the applicability of these new assays at multiple geographic locations among patients of different ethnicities, and to determine if the $IFN-{\gamma}$ responses can indicate those with a high risk of progressing to active TB.
