병원 및 환자특성에 따른 수지절단 산업재해환자의 요양기간과 진료비 (The Patient Days and Medical Care Benefits of Finger-Amputated Patients due to Industrial Accident by Hospital and Patient Characteristics)

  • This study was conducted to analyze patient days and medical care benefits of finger-amputated patients due to industrial accident. The 161 personal data on medical care for finger-amputated patients due to industrial accident(88 in 1994, 73 in 1995) of Regional Labor Office and hospital characteristics were analyzed. The major results of this study were as follows : According to stepwise multiple regression analysis of patient days, number of amputated finger, location of hospital, bed capacity of hospital, presence of plastic surgery in hospital, number of orthopedic specialist per 100 beds, sick leave benefits per day were the major significant variables in order. In stepwise multiple regression analysis with medical care benefits as a dependent variable, presence of plastic surgery in hospital, number of orthopedic specialist per 100 beds, number of amputated finger, sick leave benefits per day, age, bed capacity of hospital were the major significant variables in order. The minimum optimal size with the lowest medical care benefits was a hospital with 300 beds. This shows that the economy of scale is also applicable for hospital industry. In summary, presence of plastic surgery in hospital, number of orthopedic specialist per 100 beds, number of amputated finger, sick leave benefits per day, bed capacity of hospital were the major significant variables in both patient days and medical care benefits.

산업재해 환자의 수면의 질, 재활동기, 우울, 불안이 삶의 질에 미치는 영향 (Effects of Sleep Quality, Rehabilitation Motivation, Depression, and Anxiety on Quality of Life in Occupational Accident Patients)

  • Purpose: The purpose of this study was to identify the effect of sleep quality, rehabilitation motivation, depression, and anxiety on quality of life in occupational accident patients. Methods: The participants were 138 patients who in and outpatient treatment at a hospital affiliated with the Korea Workers' Compensation and Welfare-service Hospital located in Daegu and Daejeon. Data were analyzed by independent t-test, ANOVA, Scheffé test, Pearson's correlation coefficients, and hierarchical multiple regression analysis using SPSS/WIN 25.0 program. Results: Quality of life had a significant negative correlation with sleep quality, depression, and anxiety, respectively, but had a significant positive correlation with rehabilitation motivation. Hierarchical multiple regression analysis showed that anxiety was a major factor affecting the quality of life of occupational accident patients. Anxiety showed 42.7% of explanatory power for the quality of life of occupational accident patients. Conclusion: Based on the results of this study, in order to improve the quality of life of patients with occupational accidents, psychological interventions for anxiety should be provided first in the early stage of treatment.

  • This paper describes the structural analysis result and load test result of accident EMU(Electric Multiple Units). Structural analysis and load test of EMU were performed for the criteria of safety assessment. Structural analysis using commercial I-DEAS software provided important information on the stress distribution and load transfer mechanisms as well as the amount of damages during rolling stock crash. The purpose of the load test is to evaluate a safety which carbody structure shall be considered fully sufficient rigidity so as to satisfy proper system function under maximum load and operating condition. The results have been used to provide the critical information for the criteria of safety assessment.

교통 사고로 발생한 다발 늑골 골절 환자의 한방복합입원치료로 호전된 증례 보고 5례 (5 Cases of Patients with Multiple Fractures of Ribs after a Traffic Accident who Improved with the Combination of Korean Medical Admission Treatment: Case Series)

  • Objectives: This study aimed to report five patients who had multiple rib fractures after a traffic accident who improved with the combination of Korean medical admission treatment. Methods: We collected the data of traffic accident patients with multiple rib fractures who were admitted to the Daejeon Jaseng Hospital of Korean Medicine from April 2018 to May 2018 to receive the combinational Korean medical treatment. We observed these cases of patients treated by Acupuncture, Pharmacopuncture, Herbal medicine, Oriental physical therapy, Chuna treatment. We measured the validity of the treatment with a numerical rating scale (NRS) and the European Quality of Life-5 Dimension (EQ5D) at admission, at two weeks, and at the discharge date of hospitalization. Results: At the end of the treatment, all patients showed a decrease in NRS scores and in increase in EQ5D. The median NRS score was 6 (5-7) at the date of admission and 4 (2-7) at two weeks and then decreased to 3 (2-6). The median EQ5D score was 0.513 (0.350-0.752) at the date of admission and 0.692 (0.418-0.913) at two weeks, and then increased to 0.783 (0.671-0.913). Conclusions: After the combination of Korean medicine admission treatment, five patients with multiple rib fractures after a traffic accident showed that the treatments were effective. However, the number of subjects was insufficient and individual efficacy was not measured in this study. Therefore, further studies are needed on this topic.

사고유형에 따른 교통사고 심각도 모형 개발 (Developing the Traffic Accident Severity Models by Accident Type)

  • This study deals with the traffic accidents of the arterial link sections. The purpose is to comparatively analyze the characteristics and models by accident type using the data of 24 arterial links in Cheongju. In pursuing the above, this study gives particular emphasis to modeling such the accidents as the side-right-angle collision, rear-end collision and side-swipe collision. The main results are the followings. First, six accident models are developed, which are all analyzed to be statistically significant. Second, the models are comparatively evaluated using the common and specific variables by accident type.

운전 유형에 따른 가로구간 사고모형 개발 (Developing the Traffic Accident Models of Arterial Link Sections by Driving Type)

  • This study deals with the accident models of arterial link sections by driving type. The objectives is to develop models by driving type using the accident data of 24 arterial links in Cheong-ju. In pursuing the above, this study gives particular emphasis to modeling such the accidents as the straight, lane change and others. The main results analyzed are as follows. First, the number of accidents is analyzed to account for about 59% in straight, 31% in lane change and 10% in others. Second, the number of left-turn lane as common variables, and the ADT, number of pedestrian crossings, connecting roads and link length as specific variables are selected in developing models(number of accident and EPDO). Third, 8 models which are all statistically significant are developed. Finally, RMSE of the driving type models was analyzed to be better than that of dummy variable.

  • Site or multi-unit (MU) risk assessment has been a major issue in the field of nuclear safety study since the Fukushima accident in 2011. There have been few methods or experiences for MU risk assessment because the Fukushima accident was the first real MU accident and before the accident, there was little expectation of the possibility that an MU accident will occur. In addition to the lack of experience of MU risk assessment, since an MU nuclear power plant site is usually very complex to analyze as a whole, it was considered that a systematic method such as probabilistic safety assessment (PSA) is difficult to apply to MU risk assessment. This paper proposes a new MU risk assessment methodology by using the conventional PSA methodology which is widely used in nuclear power plant risk assessment. The logical failure structure of a site with multiple units is suggested from the definition of site risk, and a decomposition method is applied to identify specific MU failure scenarios.

  • The Korean nuclear industry had developed the SPACE (Safety and Performance Analysis Code for nuclear power plants) code, which adopts a two-fluid, three-field model that is comprised of gas, continuous liquid and droplet fields and has the capability to simulate three-dimensional models. According to the revised law by the Nuclear Safety and Security Commission (NSSC) in Korea, the multiple failure accidents that must be considered for the accident management plan of a nuclear power plant was determined based on the lessons learned from the Fukushima accident. Generally, to improve the reliability of the calculation results of a safety analysis code, verification is required for the separate and integral effect experiments. Therefore, the goal of this work is to verify the calculation capability of the SPACE code for multiple failure accidents. For this purpose, an experiment was conducted to simulate a Control Element Drive Mechanism (CEDM) break with a safety injection failure using the ATLAS test facility, which is operated by Korea Atomic Energy Research Institute (KAERI). This experiment focused on the comparison between the experiment results and code calculation results to verify the performance of the SPACE code. The results of the overall system transient response using the SPACE code showed similar trends with the experimental results for parameters such as the system pressure, mass flow rate, and collapsed water level in component. In conclusion, it can be concluded that the SPACE code has sufficient capability to simulate a CEDM break with a safety injection failure accident.

A machine learning informed prediction of severe accident progressions in nuclear power plants

  • A machine learning platform is proposed for the diagnosis of a severe accident progression in a nuclear power plant. To predict the key parameters for accident management including lost signals, a long short term memory (LSTM) network is proposed, where multiple accident scenarios are used for training. Training and test data were produced by MELCOR simulation of the Fukushima Daiichi Nuclear Power Plant (FDNPP) accident at unit 3. Feature variables were selected among plant parameters, where the importance ranking was determined by a recursive feature elimination technique using RandomForestRegressor. To answer the question of whether a reduced order ML model could predict the complex transient response, we performed a systematic sensitivity study for the choices of target variables, the combination of training and test data, the number of feature variables, and the number of neurons to evaluate the performance of the proposed ML platform. The number of sensitivity cases was chosen to guarantee a 95 % tolerance limit with a 95 % confidence level based on Wilks' formula to quantify the uncertainty of predictions. The results of investigations indicate that the proposed ML platform consistently predicts the target variable. The median and mean predictions were close to the true value.

고령운전자와 비고령운전자의 인적교통사고 심각도 비교분석 (Comparative Analysis of Elderly's and Non-elderly's Human Traffic Accident Severity)

  • 본 연구는 교통사고자료를 이용하여 고령자 집단과 비고령자 집단 간의 교통사고요인을 파악하여 각 요인들이 교통사고 심각도에 미치는 영향의 크기를 분석하였다. 나이에 의해 구분된 고령자와 비고령자 집단을 교통사고자료를 이용한 통계학적 방법에 의해 재분류하였으며, 교통사고 심각도에 미치는 영향요인을 분석하기 위해 심각도를 종속변수로 두고, 교통시설, 도로환경, 개인별특성에서 17개 독립변수로 정하여 순서형 로짓모형으로 분석하였다. 고령자 집단과 비고령자 집단의 교통사고 영향요인을 분석하여 비교한 결과, 사고 당사자의 연령뿐만 아니라, 사고유형이나 운전자 개인의 특성, 도로환경도 교통사고에 영향을 미치고 있음이 나타났다. 또한 고령자 집단과 비고령자 집단은 교통사고 심각도에 미치는 영향변수의 종류와 크기가 각각 다른 것으로 나타났다. 이는 각 집단의 개인적 특성에 적합한 교통시설이 개선된다면 교통사고의 심각도를 줄일 수 있음을 의미하는 것이라 할 수 있다.
