The treatment of keloid and hypertrophic scars (HTSs) remains one of the most difficult challenges, with a high recurrence rate regardless of the method of treatment. The latest trend in scar management is a combined approach using multiple modalities that are individualized to the patient and that would provide successful results for keloid and HTSs. There are previous reports that stromal vascular fraction (SVF) is effective for scar remodeling. Based on these reports, we introduced the concept of a combination treatment using SVF injection and fractional ablative CO2 laser. In this report, we present a 21-year-old woman who was involved in a car accident. A defect on her foot was covered with a skin graft, but the scars became elevated, which turned out to be HTSs. She was treated with a fractional ablative CO2 laser for five sessions. A month later, SVF injection and fractional ablative CO2 laser were conducted simultaneously. The result of a year's follow-up showed a flattened scar with resolution of pigment deposition. In conclusion, the combination treatment for HTSs with SVF injection and ablative fractional CO2 laser is one of the modalities to achieve an excellent outcome for treating HTS.

Rhabdomyosarcoma (RMS) is an aggressive and fast-growing malignant tumor. RMS predominantly arises in the head and neck of infancy and children. Metastasis is usually via the blood vessel. We report a case of a recurred RMS of the tongue base with the metastasis to multiple lymph nodes in a 37-year-old female. On the follow-up examination using advanced imaging modalities after surgical treatment of RMS, the lymph nodes should be carefully evaluated like in other malignancies, such as a carcinoma, showing frequent lymph node metastasis. Purpose The proliferation of data on the internet has created a need for innovative methods to analyze user satisfaction data. Traditional survey methods are becoming inadequate in dealing with the increasing volume and diversity of data, and new methods using unstructured internet data are being explored. While numerous comment-based user satisfaction studies have been conducted, only a few have explored user satisfaction through video and audio data. Multimodal sentiment analysis, which integrates multiple modalities, has gained attention due to its high accuracy and broad applicability. Design/methodology/approach This study uses multimodal sentiment analysis to analyze user satisfaction of iPhone and Samsung products through online videos. The research reveals that the combination model integrating multiple data sources showed the most superior performance. Findings The findings also indicate that price is a crucial factor influencing user satisfaction, and users tend to exhibit more positive emotions when content with a product's price. The study highlights the importance of considering multiple factors when evaluating user satisfaction and provides valuable insights into the effectiveness of different data sources for sentiment analysis of product reviews.
The aim of this study is to present a nonoperative treatment for abdominal injuries in patients with multiple traumas and to discuss the role of metropolitan tertiary hospital, non-regional trauma centers. We collected data from patients with multiple traumas including abdominal injuries from 2009 to 2014. Patient characteristics, associated injuries, short-term outcomes and departments that managed the patients overall were analyzed. Based on treatment modalities for abdominal injury, patients were divided into two groups: the operative treatment group and the nonoperative treatment group. We compared differences in patient characteristics, injury mechanisms, initial vital signs, detailed injury types, lengths of hospital and ICU stays. Of the 167 patients with multiple traumas, abdominal injuries were found in 57 patients. The injury mechanism for 44 patients (77.2%) was traffic accidents, and associated extra-abdominal injuries were shown in 45 patients (78.9%). The mean lengths of hospital and ICU stays for the 57 patients were 36.4 days and 8.3 days, respectively. The in-hospital mortality rate was 8.8%. Ten patients (17.5%) were treated operatively, and 47 patients (82.5%) were treated nonoperatively. Among the 47 patients in the nonoperative treatment group, 17 patients received embolization, and 3 patients underwent a percutaneous drainage procedure. Operative treatments were used more in patients with injuries to the pancreas and bowel. No patient required additional surgery or died due to the failure of nonoperative treatment. No differences in the clinical characteristics except for the detailed injury type were observed between the two groups. In appropriately selected patients with multiple traumas including abdominal injuries, nonoperative treatment is a safe and feasible. For rapid and accurate managements of these patients, well-trained trauma surgeons who can manage problems with the various systems in the human body and who can decide whether nonoperative treatment is appropriate or not are required.

Multiloculated hydrocephalus (MLH) is a condition in which patients have multiple, separate abnormal cerebrospinal fluid collections with no communication between them. Despite technical advancements in pediatric neurosurgery, neurological outcomes are poor in these patients and the approach to this pathology remains problematic especially given individual anatomic complexity and cerebrospinal fluid (CSF) hydrodynamics. A uniform surgical strategy has not yet been developed. Current treatment options for MLH are microsurgical fenestration of separate compartments by open craniotomy or endoscopy, shunt surgery in which multiple catheters are placed in the compartments, and combinations of these modalities. Craniotomy for fenestration allows better visualization of the compartments and membranes, and it can offer easy fenestration or excision of membranes and wide communication of cystic compartments. Hemostasis is more easily achieved. However, because of profound loss of CSF during surgery, open craniotomy is associated with an increased chance of subdural hygroma and/or hematoma collection and shunt malfunction. Endoscopy has advantages such as minimal invasiveness, avoidance of brain retraction, less blood loss, faster operation time, and shorter hospital stay. Disadvantages are also similar to those of open craniotomy. Intraoperative bleeding can usually be easily managed by irrigation or coagulation. However, handling of significant intraoperative bleeding is not as easy. Currently, endoscopic fenestration tends to be performed more often as initial treatment and open craniotomy may be useful in patients requiring repeated endoscopic procedures.

This study develops a new type of latent class analysis (LCA) in order to explain the associations between one latent variable and several other categorical latent variables. Our model postulates that the prevalence of the latent variable of interest is affected by another latent variable composed of other several latent variables. For the parameter estimation, we propose deterministic annealing EM (DAEM) to deal with local maxima problem in the proposed model. We perform simulation study to demonstrate how DAEM can find the set of parameter estimates at the global maximum of the likelihood over the repeated samples. We apply the proposed LCA model in an investigation of the effect of and joint patterns for drug-using behavior to violent behavior among US high school male students using data from the Youth Risk Behavior Surveillance System 2015. Considering the age of male adolescents as a covariate influencing violent behavior, we identified three classes of violent behavior and three classes of drug-using behavior. We also discovered that the prevalence of violent behavior is affected by the type of drug used for drug-using behavior.
Objective : The aim of this study is to evaluate the therapeutic effects of gamma knife radiosurgery (GKRS) in patients with multiple brain metastases and to investigate prognostic factors related to treatment outcome. Methods : We retrospectively reviewed clinico-radiological and dosimetric data of 36 patients with 4-14 brain metastases who underwent GKRS for 264 lesions between August 2008 and April 2011. The most common primary tumor site was the lung (n=22), followed by breast (n=7). At GKRS, the median Karnofsky performance scale score was 90 and the mean tumor volume was 1.2 cc (0.002-12.6). The mean prescription dose of 17.8 Gy was delivered to the mean 61.1% isodose line. Among 264 metastases, 175 lesions were assessed for treatment response by at least one imaging follow-up. Results : The overall median survival after GKRS was $9.1{\pm}1.7$ months. Among various factors, primary tumor control was a significant prognostic factor ($11.1{\pm}$1.3 months vs. $3.3{\pm}2.4$ months, p=0.031). The calculated local tumor control rate at 6 and 9 months after GKRS were 87.9% and 84.2%, respectively. Paddick's conformity index (>0.75) was significantly related to local tumor control. The actuarial peritumoral edema reduction rate was 22.4% at 6 months. Conclusion : According to our results, GKRS can provide beneficial effect for the patients with multiple (4 or more) brain metastases, when systemic cancer is controlled. And, careful dosimetry is essential for local tumor control. Therefore, GKRS can be considered as one of the treatment modalities for multiple brain metastase.
Objectives : The purpose of this study is to summarize the historical aspects of parabolic reflector moxa and to suggest the applications of light moxibustion in Korean medicine. Results : Getting a fire by parabolic reflector(concave mirror) is well known because of the Olympic torch lighted with a solar reflector in Greece. From anecdotes or myths for Diocles and Archimedes(third century B.C.) a long history has been chronicled in books and films in western culture. However, it has another history in Asian countries since 2,000 years ago. Some reflectors had a tiny size with 6.5 to 10 cm in diameter, 4 mm to 1 cm thickness, and these mobile handheld devices could make us guess that they have been used for multiple purposes. Bronze reflectors were described in Dongeui-bogam, and excavated from the ancient remains of the Shilla and Koryo dynasties. This common firing device was used as a moxibustion device, one of the acupuncture and moxibustion modalities in traditional Korean medicine. Conclusions : Reflector moxa has been used as a light moxibustion to deliver heat energy to acupuncture points, muscles, and skin along meridians. We present a plausible proposal to improve other phototherapy modalities including reflector moxa in Korean medicine practice.

Phantom limb pain is a phenomenon in which patients experience pain in a part of the body that no longer exists. In several treatment modalities, spinal cord stimulation (SCS) has been introduced for the management of intractable post-amputation pain. A 46-year-old male patient complained of severe ankle and foot pain, following above-the-knee amputation surgery on the right side amputation surgery three years earlier. Despite undergoing treatment with multiple modalities for pain management involving numerous oral and intravenous medications, nerve blocks, and pulsed radiofrequency (RF) treatment, the effect duration was temporary and the decreases in the patient's pain score were not acceptable. Even the use of SCS did not provide completely satisfactory pain management. However, the trial lead positioning in the cauda equina was able to stimulate the site of the severe pain, and the patient's pain score was dramatically decreased. We report a case of successful pain management with spinal cauda equina stimulation following the failure of SCS in the treatment of intractable phantom limb pain.
