Objective: To determine the impact of dedicated subspecialized radiologists in multidisciplinary team (MDT) discussions on the management of lower gastrointestinal (GI) tract malignancies. Materials and Methods: We retrospectively analyzed the data of 244 patients (mean age ± standard deviation, 61.7 ± 11.9 years) referred to MDT discussions 249 times (i.e., 249 cases, as five patients were discussed twice for different issues) for lower GI tract malignancy including colorectal cancer, small bowel cancer, GI stromal tumor, and GI neuroendocrine tumor between April 2018 and June 2021 in a prospective database. Before the MDT discussions, dedicated GI radiologists reviewed all imaging studies again besides routine clinical reading. The referring clinician's initial diagnosis, initial treatment plan, change in radiologic interpretation compared with the initial radiology report, and the MDT's consensus recommendations for treatment were collected and compared. Factors associated with changes in treatment plans and the implementation of MDT decisions were analyzed. Results: Of the 249 cases, radiologic interpretation was changed in 73 cases (29.3%) after a review by dedicated GI radiologists, with 78.1% (57/73) resulting in changes in the treatment plan. The treatment plan was changed in 92 cases (36.9%), and the rate of change in the treatment plan was significantly higher in cases with changes in radiologic interpretation than in those without (78.1% [57/73] vs. 19.9% [35/176], p < 0.001). Follow-up records of patients showed that 91.2% (227/249) of MDT recommendations for treatment were implemented. Multiple logistic regression analysis revealed that the nonsurgical approach (vs. surgical approach) decided through MDT discussion was a significant factor for patients being managed differently than the MDT recommendations (odds ratio, 4.48; p = 0.017). Conclusion: MDT discussion involving additional review of radiology examinations by dedicated GI radiologists resulted in a change in the treatment plan in 36.9% of cases. Changes in treatment plans were significantly associated with changes in radiologic interpretation.
Aggression can be defined as 'behavior intended to harm another' which can be seen both from humans and animals. However, trying to understand aggression in a simplistic view may make it difficult to develop an integrated approach. So, we tried to explain aggression in a multidisciplinary approach, affected by various factors such as neuroanatomical structures, neurotransmitter, genes, and sex hormone. Parallel with animal models, human aggression can be understood with two phenomena, offensive aggression and defensive aggression. Neurobiological model of aggression give a chance to explain aggression with an imbalance between prefrontal regulatory influences and hyper-reactivity of the subcortical areas involved in affective evaluation, finally in an aspect of brain organization. Serotonin and GABA usually inhibit aggression and norepinephrine while glutamate and dopamine precipitate aggressive behavior. As there is no one gene which has been identified as a cause of aggression, functions between gene to gene interaction and gene to environment interaction are being magnified. Contributions of sex hormone to aggression, especially molecular biologic interaction of testosterone and regulation of estrogen receptor have been emphasized during the research on aggression. This multidisciplinary approach on aggression with types, neurochemical bases, and animal models can bring integrated interpretation on aggression.
With the rapid evolution of diagnostic tools, particularly next-generation sequencing, the identification of genetic diseases, predominantly those with pediatric-onset, has significantly advanced. However, this progress presents challenges that span from selecting appropriate tests to the final interpretation of results. This review examines various genetic testing methodologies, each with specific indications and characteristics, emphasizing the importance of selecting the appropriate genetic test in clinical practice, taking into account factors like detection range, cost, turnaround time, and specificity of the clinical diagnosis. Interpretation of variants has become more challenging, often requiring further validation and significant resource allocation. Laboratories primarily classify variants based on the American College of Medical Genetics and Genomics and the Association for Clinical Genomic Science guidelines, however, this process has limitations. This review underscores the critical role of clinicians in matching patient phenotypes with reported genes/variants and considering additional factors such as variable expressivity, disease pleiotropy, and incomplete penetrance. These considerations should be aligned with specific gene-disease characteristics and segregation results based on an extended pedigree. In conclusion, this review aims to enhance understanding of the complexities of clinical genetic testing, advocating for a multidisciplinary approach to ensure accurate diagnosis and effective management of rare genetic diseases.
As a factor which can help to understand the genesis of (mata) sedimentary and/or hydrothermal uranium depisits, an interpretation of the role of organic matter associated with uranium mineralization, was attempted with the literature published up to the end of 1986. Laboratory studies, in which diageneis and metamorphism are artificially simulated, can help to elucidate how uranium with particular organic materials are formed and destroyed. Similarly, research involving a variety of techniques is needed to characterize both the soluble organic extracts (bitumen) and the insoluble organic matter (kerogen), separated from uranium ores and associated rocks. In the presence of clay minerals and amorphous oxy-hydroxde minerals, an understanding of the role of organic matter must be prudent and may require the incorporation of multidisciplinary approach (mineralogy, inorganic geochemistry ...).
Connective tissue diseases (CTDs) can affect all compartments of the lungs, including airways, alveoli, interstitium, vessels, and pleura. CTD-associated lung diseases (CTD-LDs) may present as diffuse lung disease or as focal lesions, and there is significant heterogeneity between the individual CTDs in their clinical and pathological manifestations. CTD-LDs may presage the clinical diagnosis a primary CTD, or it may develop in the context of an established CTD diagnosis. CTD-LDs reveal acute, chronic or mixed pattern of lung and pleural manifestations. Histopathological findings of diverse morphological changes can be present in CTD-LDs airway lesions (chronic bronchitis/bronchiolitis, follicular bronchiolitis, etc.), interstitial lung diseases (nonspecific interstitial pneumonia/fibrosis, usual interstitial pneumonia, lymphocytic interstitial pneumonia, diffuse alveolar damage, and organizing pneumonia), pleural changes (acute fibrinous or chronic fibrous pleuritis), and vascular changes (vasculitis, capillaritis, pulmonary hemorrhage, etc.). CTD patients can be exposed to various infectious diseases when taking immunosuppressive drugs. Histopathological patterns of CTD-LDs are generally nonspecific, and other diseases that can cause similar lesions in the lungs must be considered before the diagnosis of CTD-LDs. A multidisciplinary team involving pathologists, clinicians, and radiologists can adequately make a proper diagnosis of CTD-LDs.
Molecular imaging is a rapidly growing field due to the advances in molecular biology and imaging technologies. With the introduction of imaging reporter genes into the cell, diverse cellular processes can be monitored, quantified and imaged non-invasively in vivo. These precesses include the gene expression, protein-protein interactions, signal transduction pathways, and monitoring of cells such as cancer cells, immune cells, and stem cells. In the near future, molecular imaging analysis will allow us to observe the incipience and progression of the disease. These will make us easier to give a diagnosis in the early stage of intractable diseases such as canter, neuro-degenerative disease, and immunological disorders. Additionally, molecular imaging method will be a valuable tool for the real-time evaluation of cells in molecular biology and the basic biological studies. As newer and more powerful molecular imaging tools become available, it will be necessary to corporate clinicians, molecular biologists and biochemists for the planning, interpretation, and application of these techniques to their fullest potential. in order for such a multidisciplinary team to be effective, it is essential that a common understanding of basic biochemical and molecular biologic techniques is achieved. Basic molecular techniques for molecular imaging methods are presented in this paper.
본 연구는 서예 활동에서 인식된 주관적 경험을 분석하여 서예치료 프로그램의 효과를 검증할 평가 도구 개발을 위한 기초 자료를 제공하기 위해 수행되었다. 서예활동을 하고 있는 150명을 대상으로 개방형 질문지에 서예의 심리적 및 신체적 장, 단점을 최대 3가지씩 적게 하였다. 수집된 자료를 의미에 따라 범주화하였고, 각 범주별로 빈도 분석을 실시하였다. 심리적 장점 435개 항목은 10개의 범주, 단점 129개의 항목은 6개의 범주, 신체적 장점 302개의 항목은 9개의 범주, 단점 150개의 항목은 7개의 범주로 나뉘었다. 서예에 대한 주관적 경험은 융복합적 관점(정신 건강, 신체 건강, 그리고 인성 교육)에서 해석되었다. 본 연구의 결과를 바탕으로 추후 연구에서 서예의 특성을 제대로 반영하는 구조화된 평가 도구를 개발한다면 서예치료의 활성화에 크게 기여할 것이다.
자연생태계와 인간사회의 공통적인 이익을 위한 생태공학 기술의 필요성이 대두되고 있다. 응용생태공학회가 설립되어 생태공학 분야의 연구와 기술개발에 기여하고 있는데, 생태공학분야 인력 양성에 필수적인 교육의 목표나 내용은 체계가 잡혀있지 못하다. 2015년 응용생태공학 포럼-'생태공학 교육의 터잡기'에서 생태공학 교육의 국제적 동향과 국내 실정에 대해 발제하고 토의한 내용을 중심으로 정리하였다. 생태공학 교육은 인간에 의해 교란된 생태계를 지속가능하도록 복원시킴과 아울러 인간과 생태계의 공동이익을 추구하는 새로운 지속가능한 생태계를 설계 운영할 수 있는 인력을 양성할 수 있어야 한다. 생태공학이 직면하게 될 세 가지의 도전에 대응할 수 있는 교육이 되어야 한다. 인간과 생태계의 공동 이익이라는 목표에서 인간사회와 자연 사이의 균형을 어떻게 정의할 것인가 하는 윤리적 도전, 유관학문 분야는 물론 산업, 정책, 교육, 훈련의 융화를 꾀하는 관계적 도전, 마지막으로 생태학과 공학뿐 아니라 사회과학과 인문과학이 소통하여 융합하는 지적 도전을 극복할 수 있는 교과과정이어야 한다. 이를 위해 생태학과 생태공학을 중심으로 하는 핵심교과목의 하위에 자연과학과 공학 기초과목을 두어 학부과정에 편성하고, 상위에 전공학문 별로 심화된 핵심교과목을 두어 대학원과정으로 편성하는 것이 필요하다.
단양-영월-정선지역 옥천습곡단층대에서 북동-남서 방향의 충상단층대를 따라 분포하는 중생대 반송층군은 페름기-트라이아스기 송림조산운동 혹은 쥐라기 대보조산운동의 기록을 간직하고 있기 때문에 한반도 중생대 조산윤회와 지구조진화를 설명하는 데 있어 매우 중요하다. 본 논평에서는 반송층군의 층서, 퇴적시기, 그리고 분지발달사에 관한 그간 연구를 종합하고 향후 연구방향을 제시하고자 한다. 한반도 남부의 중생대 지구조진화 과정에서 반송층군이 가지는 의미에 대한 시각은 크게 두 가지로 나뉜다. 전통적인 견해는 송림조산운동 후기 혹은 종료 후에 (횡)인장 분지에 반송층군이 퇴적된 후 대보조산운동에 의해 변형이 발생한 것으로 간주한다. 이러한 해석은 단양탄전지역을 중심으로 획득한 층서 및 고생물자료와 구조지질연구 결과에 근거한다. 반면 근래 연구 결과로서 반송층군을 대보조산운동과 관련된 조산동시성 퇴적작용의 산물로 보는 해석이 존재한다. 이러한 대안적 시각은 영월지역 일대에 분포하는 화산쇄설암에 대한 U-Pb 절대연령과 해당 지역의 구조지질학적 해석 결과를 바탕으로 한다. 하지만 두 모델 모두 반송층군으로 분류된 퇴적층에서 도출된 고생물자료와 절대연령자료를 종합적으로 설명하지 못한다. 이는 퇴적학, 구조지질학, 지질연대학, 암석학, 지화학 연구를 통해 획득한 다학제적 자료를 종합한 새로운 분지 진화 모델이 필요하다는 점을 시사한다.
자세한 고환경복원 및 해수면 변화를 파악하기 위하여 델라웨어만 중부연안에서 조밀한 코어를 채취 분석하였다. 연구지역은 키즈흐머크해변, 세인트존스강, 세인트존 스강 하구의 바월즈해변의 습지지역으로 각자 고유의 지형적인 특성과 해수면 역사를 기록하고 있다. 충서대비에서 가장 중요한 것은 퇴적상과 고환경을 세분하는 것이며 이를 위하여 본 연구에서는 암상분석, 입도분석, 유기물과 무기물의 비, 함수율비, 광 물구성성분, 식물상의 분석, 고생물학적, 분석, 탄소연 대측정 등을 사용하였다. 연구 지역에서 습지환경은 담수습지, 초기담수습지, 약기수습지, 기수습지, 염수습지로 다 섯 등분 하였다. 습지 환경 외에는 조간대 환경과 하천환경이 간혹 발견되었으며, 전 홀로세층의 퇴적물은 사질, 니질, 사니질 퇴적물로 구성되어 있다. 층서의 하부는 담 수습지가 대부분이며 그 위에 기수습지나 조간대 내지 조간대 내의 하천환경으로 나타 났다. 전홀로세의 지형형태가 홀로세 고환경변화에 중요한 영향을 미쳤다. 해수면이 상승하거나 하강하면서 염수나 기수환경을 형성하는 과정에도 해수의 영향이 미치지 않는 고립된 지역에서 담수환경이 형성되었다. 같은 시기에 유사한 고도에서 담수환경 과 기수환경이 형성되는 것은 바로 전홀로 세층의 불규칙한 지형에 기인한 것이다. 따 라서 이러한 복잡한 고환경 형성은 층서대비나 지역적 상대적 해수면 변화에 대한 해 석에 어려움을 가져왔다. 연구지역에서 퇴적층은 퇴적물 공급, 고결작용, 하천활동, 생태계내의 경쟁, 지역적인 지구조적운동, 지각평형, 기후 그리고 지역적인 상대적인 해수면 변화에 의하면 영향을 받았다. 담수환경과 기수환경의 반복이 여러 층에서 발 견 되었는데 이 시점마다 해침이나 해퇴가 일어난 것으로 판단된다. 따라서 연구지역 에서는 여러분의 해침과 해퇴가 일어난 것으로 본다.
