Case of making maxillary palatal augmentation complete denture for patient with dysphagia after partial glossectomy (부분 혀 절제술로 인해 연하 장애가 있는 환자에서 상악 구개 증대 총의치 제작 증례)

  • If complication arises after glossectomy which leads to trouble in forming food bolus or transfer of the food, it is possible that either food bolus may block the airway or dysphagia may occur as the food bolus goes down into the airway. To solve the issue, palatal augmentation prosthesis could be used. In this case, the patient with an oral cancer is having difficulties swallowing food after glossectomy. Through taking impressions of polishing surface of his denture referring his tongue movement, the complete denture for the upper jaw was created using the concept of palatal augmentation prosthesis. This new upper denture increases the palatal-tongue contact pressure, allowing the patient to perform better swallowing and better pronunciation.

The Validity and Reliability of Reflux Symptom(RSI) Index and Reflux Finding Score(RFS) (역류증상지수와 역류소견점수의 타당성과 신뢰도)

  • Laryngopharyngeal reflux (LPR) is the retrograde movement of gastric contents into the larynx, pharynx, and upper aero-digestive tract. LPR differs from gastroesophageal reflux in that it is often not associated with heartburn and regurgitation symptoms. Otolaryngological manifestations of acid reflux include a wide range of pharyngeal and laryngeal symptoms. Belafsky et al. developed a useful self-administered tool, the reflux symptom index (RSI), for assessing the degree of LPR symptoms. Patients are asked to use a 0 to 5 point scale to grade the following symptoms: 1) hoarseness or voice problems; 2) throat clearing; 3) excess throat mucus or postnasal drip ; 4) difficulty swallowing; 5) coughing after eating or lying down; 6) breathing difficulties ; 7) troublesome or annoying cough; 8) sensation of something sticking or a lump in the throat; 9) heartburn, chest pain, indigestion or stomach acid coming up. A RSI score greater than 13 is considered abnormal. As there is no validated instrument to document the physical findings and severity of LPR, Belafsky et al. developed an eight-item clinical severity scale for judging laryngoscopic finding, the reflux finding score (RFS). They rated eight LPR-associated findings on a scale from 0 to 4 : subglottic edema, ventricular obliteration, erythema/hyperemia, vocal-fold edema, diffuse laryngeal edema, posterior commissure hypertrophy, granuloma/granulation tissue, and thick endolaryngeal mucus. A RFS score of greater than 7 was found to suggest LPR-associated laryngitis. Although both indices (RSI and RFS) are widely used, there is some controversy about their validity (sensitivity and specificity) and reliability (intra-rater and inter-rater) in LPR diagnosis and treatment. We discuss the validity and reliability of RSI and RFS with literature review.

  • Initial stage of emotion has a great influence on building up product image and impression. Because of its influencing effects, measuring initial stage of emotion has potential to be a key factor for designers and marketers to achieve a distinct product concept. While many researchers have studied this topic with the emotion measurement method in product use stage, there are very few cases specialized in the initial stage of emotion. Even though present emotion measurement methods have difficulties to derive accurate user's initial stage of emotion, most case of initial emotion study applies these defective methods. The purpose of this study is to develop initial stage of emotion measurement method and apply this method to real product context. In the design of the initial stage of emotion measurement method, noticeable characteristics of initial stage of emotion were explored and initial emotion measurement framework was presented. Based on this framework, Initial Emotion Measurement System(IEMS) was suggested. This method collects user's eye movement, behavior and verbal data accurately and objectively.

A Study on the Spatial Determination of Red Tide Occurrence Area Using GSIS (GSIS 이용한 적조발생지의 공간결정 연구)

  • Few researches related to the spatial determination of areas where red tide occurs have been performed, so accurately determining the area where red tide occurs and disappears poses difficulties. Therefore, a more objective and scientific method is necessary to analyze the occurrence and movement of red tide based on a geo-spatial information system. In this study, the coastline was extracted using a digital topographic map in order to examine areas where red tide occurs each year. An analysis of red tide occurrence areas, which were determined based on red tide data for the last several years, showed that only Yeoja bay had almost a zero case of red tide of the areas studied, whereas Dolsando in the South Sea and NamHaedo Aenggang bay areas exhibited the highest frequency of red tide occurrences. Based on these results, by using a system that determines the geo-spatial distribution status of areas repeatedly hit by red tide every year, it would be possible to predict the course of red tide and prevent consequent damages.

  • Today, great percentage of apparel fabrics comprise knit products. A few apparel items are almost exclusively made from knit products including sweaters, T-shirts, lingerie, and hosiery. In weft knits, intermeshing yarn traverses the fabric crosswise. The production rate of knitting machines is about four times higher than that of conventional looms iin proportion to the amount of fabric produced per unit time. Knit apparels fabrics are noted for freedom of body movement, easy-care, resilience, soft draping and quality. The appearance of fabrics is of prime importance along with tactile features such as smoothness, crispness, stiffness when we deal with the handling evaluation of the textiles. In practice, the quality and performance of fabrics judged by sighted evaluators, and it is perhaps logical, in attempting to establish an effective fabric objective measurement system. It is worthwhile to make objective measurement that correlate well with subjective evaluations made by sight and touch together. However, it often imposes difficulties to effectively measure and analyze the appearance or optical properties of the textile surface features. In this study, an attempt is made to provide a preliminary approach to the analysis of the gloss property, which is one of the optical properties of fabrics. The specimens range from 70/24 nylon 6 filament yarn knit fabric to 70/68 nylon 6 filament yarn knit fabric, the latter being finer. A few three-dimensional models based on Peirce's model of knit stitch have been proposed and material properties were given to calculate the properties. Goniophotometric measurements of the specimens were also carried out.

Disturbance Analysis in an Optical Disk Drive Using Model Based Disturbance Observer and Waterfall Technique (모델 기반 외란 관측기와 Waterfall 해석을 이용한 광 디스크 외란 분석)

  • A novel disturbance measurement method, model based disturbance observer (MBDO) for optical disk drives (ODDs), is proposed and the disturbance analysis using the proposed method is performed under various conditions. In ODDs, the quantitative and qualitative analysis for the generated disturbance during normal operation is very important to successful servo loop design. However, the disturbance measurement is difficult, and high precision measurement is necessary. Furthermore, the conventional disturbance measurement method using a LDV (laser Doppler vibrometer) has many difficulties in eccentricity direction due to the vertical movement of an optical disk. To solve this problem, the MBDO is proposed. First, the relationship between the servo loop for ODDs and the generated disturbance are briefly reviewed. Second, the principle of the MBDO is introduced, and the disturbance measurement results, which are measured by the MBDO and a LDV, are compared. In these experiments, test DVD-ROM disks are used to generate quantitative/qualitative disturbance. Then, the disturbance analysis under various conditions is performed using waterfall technique. This technique clearly shows the disturbance trend from the inner part of an optical disk to the outer part of it. Finally, the various disturbances measurement results are summarized and some remarks for it are commented.

  • Handwriting identifications are often faced with difficulties in evaluating handwriting by persons under the influence of alcohol. Cerebellar dysfunction is associated with deficits in the control of movement extent, as well as changes in the amplitude and relative amounts of acceleration and deceleration and action tremor. Although numerous articles are available on the subject of alcohol influence on handwriting quality, most of them were based on empirical data such as experience method, without any statistical evaluation. In this study, Hangul signature giyeok and nieun of consonant were measured in samples of handwriting. The result of this study can be used as a basic data of comparison on handwriting by identifying consistency of features and relative individualization of.

  • The establishment and/or registration of an optimal or physiologic relationship between the maxillae and mandible has long been a subject of considerable interest and controversy in dentistry. Centric relation has been generally accepted as a repeatable starting point for restoring the mouth. Recently, it has been claimed that an electronic device (Myo-Monitor) will produce an accurate, reproducible occlusal registration at the vertical and horizontal occlusal position most compatible with the muscular of each patient. The objectives of this study was to compare Myo-Monitor centric to centric relation at the points of reproducibility and anteroposterior, superoinferior position. A Vericheck instrument was employed for examining difference in the position and reproducibility of mandible reproduced by the various check bite records. For this study, 8 dental students and dentists who had no missing teeth and no difficulties of mandibular movement were selected. The following three different positions of the mandible were registered (a) centric relation manipulated by means of chin-point technique with Lucia-jig, (b) centric relation manipulated by means of bilateral technique, (c) Myocentric manipulated by Myo-Monitor. From this experiment, the following results were obtained. 1. Bilateral manipulated centric relation was more reproducible than Myocentric in anteroposterior and superoinferior position , but more reproducible than centric relation manipulated by means of chin point technique with Lucia-jig in anteroposoterior position. Centric relation manipulated by means of chin Point technique with Lucia-jig was more reproducible than Myocentric in right anteroposterior and superoinferior position. 2. Centric relation by means of chin point technique with Lucia-jig was posterior($2.16{\pm}0.78mm$) and superior($0.41{\pm}0.16mm$) to centric occlusion. Bilateral manipulated centric relation was posterior($1.68{\pm}0.10mm$) and superior($1.02{\pm}0.45mm$) to centric occlusion. Myocentric was anterior($0.75{\pm}0.70mm$), inferior($0.59{\pm}0.44mm$) to centric occlusion.

Social investment in Europe: bold plans, slow progress and implications for Korea

  • ${\cdot}$ Recent social policy and labour markets debates in Europe, responding to the difficulties faced by the traditional neo-Keynesian welfare state settlement, stress the value of positive investment alongside de-regulation and greater flexibility as a way of achieving both economic and social goals. ${\cdot}$ Patterns of policy reform are complex and reflect differing national circumstances. A general move towards deregulation, constraints on entitlement to passive benefits, programmes to enhance employment, particularly among high-risk groups such as single parents and young people, targeted subsidies for low earners and casemanagement may be identified. ${\cdot}$ In relation to investment in education, research and development and combined training and benefit programmes to enhance mobility between jobs the picture is less clear. Education standards continue to rise, but research and development spending stagnates and few countries have developed substantial ‘flexi-curity’ programmes to support job mobility. ${\cdot}$ The labour market tradition in much of Europe has been one of conflict between labour and employers. As labour grows weaker, new approaches develop. These tend to stress productivity agreements and greater flexibility in work practices within firms and reforms to passive social security systems more broadly, but movement to support the more challenging investment and flexi-curity policies is slow. ${\cdot}$ In general, social and labour market policies in Europe stress deregulation and negative activation more strongly than social investment and ‘flexi-curity’. The countries with high growth and employment achieve that goal by different routes: Sweden has a closely integrated social democratic corporatism with high spending on benefits and training programmes and the UK a more liberal market-oriented system, with lower spending, highly targeted benefits and less mobility support. ${\cdot}$ Europe has something to learn from Korea in achieving high investment in human capital and R and D, while Korea may have something to learn from Europe in social investment, particularly flexi-curity and equal opportunity policies.

A study on the Acceptable Cap Torque for Processed Food Containers (고령자에 적합한 가공식품 마개의 Torque에 관한 연구)

  • Korea has become an aged society according to the UN specifications. The number of the aged population will increase drastically, and the number of single aged people will also increase. Products used in our daily life are manufactured within the normal capabilities of people. Thus, aged people have encountering problems in using them. Numerous products are used in our daily lives. The purpose of this study is to find the acceptable torque for aged people to open the caps of processed food containers. Depending on the size and shape of the opening hand, hand operation has been classified into three categories. A total of one hundred and nine people participated in this study. Fifty nine people in the 20-30 year old range and fifty people in their 60-70's. Results of the study are as follows: 1) For all categories the acceptable cap torque data for the aged people are established. To satisfy 95% of the aged people, it was found that $74N{\cdot}cm$ for type A caps, and $141N{\cdot}cm$ for B type caps. and $214N{\cdot}cm$ for type C were the acceptable torque levels. 2) The appropriate level of torque for opening the cap was found to be 70% of the maximum torque for the aged people.(71.4% for type A. 71.4% for type B. 72.2% for type C). 3) Capabilities of the 70's drops sharply in function related to opening cap (maximum torque, gripping force, and wrist movement range). 4) Among aged subjects, 65.0% for type A, 42.0% for type B, and 68.8% for type C, had difficulties in opening the container caps.
