Effect of robot arm reach training on upper extremity functional movement in chronic stroke survivors: a preliminary study

  • Objective: The purpose of this study was to investigate the effect of robot arm reach training on upper extremity functional movement in chronic stroke survivors. Design: One group pretest-posttest design. Methods: Thirteen chronic stroke survivors participated in this study. Robot arm reach training was performed with a Whole Arm Manipulator (WAM) and a 120-inch projective display to provide visual and auditory feedback. During the robotic arm reach training, WAM provided gravity compensation and assist-as-needed (AAN) force according to the robot control mode. When a participant could not move the arm toward the target for more than 2 seconds, WAM provided AAN force to reach the desired targets. All patients participated in the training for 40 minutes per day, 3 times a week, for 4 weeks. Main outcome measures were the Fugl-Meyer Assessment (FMA), Action Research Arm Test (ARAT) and Box and Block Test (BBT) to assess upper extremity functional movement. Results: After 4 weeks, significant improvement was observed in upper extremity functional movement (FMA: 42.15 to 46.23, BBT: 12.23 to 14.00, p<0.05). In the subscore analysis of the FMA upper extremity motor function domains, significant improvement was observed in upper extremity and coordination/speed units (p<0.05). However, there were no significant differences in the ARAT. Conclusions: This study showed the positive effects of robot arm reach training on upper extremity functional movement in chronic stroke survivors. In particular, we confirmed that robot arm reach training could have a positive influence by leading to improvement of motor recovery of the proximal upper extremity.

정신훈련이 편마비환자의 상지기능 향상에 미치는 영향 (Effect of the Mental Practice on the Upper Limb Motor Function Improvement of Hemiplegic Patients)

  • This study was performed to examine the influences of the mental practice to the hemiplegic upper limb motor function improvement. 20 minute neurologic treatment based on the neurophysiological theory, 10 minute activities of daily living training, and 10 minute mental practice 5 times a week were given in turn to the experimental group(N=11). On the other hand 20 minute neurologic treatment, and 10 minute activities of daily living training 5 times a week were given in turn to the control group(N=11). Both Fugl-Meyer Assessment Scale and Manual Function Test were used to evaluate upper limb motor recovery, upper limb motor function and movement ability. And the Motor Activity Log; Amount of Use and Motor Activity Log; Quality of Movement before training, 2 weeks after training, and 4 weeks after training were measured to assess the upper limb motor quantitatively and qualitatively each. The results are as follows. 1) Considering the interactions of the rate of change on the upper limb motor recovery, motor function, movement ability improvement, and qualitative motor improvement in ADL of experimental group and control group, the change rates of experimental group were found to be greater than those of the control group. 2) In experimental group, the higher the achievements were, the better upper motor recovery was.

  • 억제유도치료는 건측 상지의 운동을 제한하고 환측 상지의 운동을 유도함으로써 손상된 상지의 기능 및 사용을 향상시키는 치료이다. 본 연구의 목적은 동물모델을 이용하여 억제유도치료의 효과를 운동행동검사와 면역조직 화학법으로 검증하기 위함이다. 본 연구에서는 Sprague-Dawley계 흰쥐 수컷 실험군 40마리 대조군 40마리를 사용하였다. 실험동물은 마취 후 체성운동영역을 제거하여 억제유도치료를 적용한 실험군과 적용하지 않은 대조군으로 나누었다. 실험동물은 수술 후 1일, 3일, 7일, 14일째 10마리씩 나누어 자세 반사 검사, 막대 걷기 검사, 앞다리 배치 검사, 면역조직 화학반응을 실시하였다. 결과는 실험군의 운동행동 검사에서 모두 유의하게 높았고(p<.05), BDNF 발현은 시간이 지날수록 실험군의 발현량이 높게 나타났다. 이와 같은 결과는 중추신경계의 가소성을 검증 하는데 기여하리라 생각된다.

The potential of non-movement behavior observation method for detection of sick broiler chickens

  • The poultry industry, which produces excellent sources of protein, suffers enormous economic damage from diseases. To solve this problem, research is being conducted on the early detection of infection according to the behavioral characteristics of poultry. The purpose of this study was to evaluate the potential of a non-movement behavior observation method to detect sick chickens. Forty 1-day-old Ross 308 males were used in the experiments, and an isolator equipped with an Internet Protocol (IP) camera was fabricated for observation. The chickens were inoculated with Salmonella enterica serovar Gallinarum A18-GCVP-014, the causative agent of fowl typhoid (FT), at 14 days of age, which is a vulnerable period for FT infection. The chickens were continuously observed with an IP camera for 2 weeks after inoculation, chickens that did not move for more than 30 minutes were detected and marked according to the algorithm. FT infection was confirmed based on clinical symptoms, analysis of cardiac, spleen and liver lesion scores, pathogen re-isolation, and serological analysis. As a result, clinical symptoms were first observed four days after inoculation, and dead chickens were observed on day six. Eleven days after inoculation, the number of clinical symptoms gradually decreased, indicating a state of recovery. For lesion scores, dead chickens scored 3.57 and live chickens scored 2.38. Pathogens were re-isolated in 37 out of 40 chickens, and hemagglutination test was positive in seven out of 26 chickens. The IP camera applied with the algorithm detected about 83% of the chickens that died in advance through non-movement behavior observation. Therefore, observation of non-movement behavior is one of the ways to detect infected chickens in advance, and it appears to have potential for the development of remote broiler management system.

  • The design and use of a micro-computer-based system for quantitative study of ocular kinetics are described. An algorithm for microcomputer analysis of electro-oculographically recorded saccadic eye movement is presented. From a brief, 4-min recording session detailed statistical information about saccade amplitude, duration, and velocity can be obtained. Since this data is not significantly altered by practice of motivational factors, it provides a sensitive functional test of the oculomotor system and its brain control system.

뇌압 펄스에 의한 션트밸브의 압력-유량제어 특성의 변화 (Changes in The Pressure-Flow Control Characteristics of Shunt Valves by Intracranial Pressure Pulsation)

  • 수두증 환자를 치료하기 위해 머리에 시술 되는 션트밸브가 받는 뇌압 펄스가 밸브의 유량조절 특성에 미치는 영향을 수치적으로 해석하였다. 션트밸브를 통한 유량을 계산하기 위하여 밸브 다이아프램이 형성하는 유량 오리피스를 모델링 하여 맥동압력과 변동하는 압력에 의한 다이아프램의 움직임을 고려하였다. 연구 결과, 압력 펄스와 이에 따른 다이아프램의 움직임에 의하여 유량이 40% 이상 증가함을 보였다. 이 결과는, 뇌실에 시술 되는 션트밸브의 압력-유량 조절특성이 제조 직후 맥동이 없는 유량을 공급하는 시린지 펌프를 사용하여 구한 실험 결과와 상당히 다르다는 사실을 보여주었다.

  • This paper describes the actual application of a feedback control loop as a means for minimizing turbine impulse chamber pressure variation during the turbine steam valve tests at a 1,000 MW nuclear power plant. The chamber pressure control loop was implemented in the new digital control system which was installed as a replacement for the old analog type control system. There has been about 40MW of the generator output change during the steam valve tests, especially the high pressure governing valve tests, because the old control system had not the impulse chamber pressure control so the operators had to compensate steam flow drop manually. The process of each valve test consists of a closing process and an reopening process and the operators can make sure that the valves are in their sound conditions by checking the valves movement. The control algorithm described in this paper contributed to keep the change in megawatt only to 6MW during the steam valve tests. Thereby, the disturbance to reactor control was reduced, and the overall plant control system's stability was greatly improved as well.

Comparisons of orthodontic root resorption under heavy and jiggling reciprocating forces during experimental tooth movement in a rat model

  • Objective: Root mobility due to reciprocating movement of the tooth (jiggling) may exacerbate orthodontic root resorption (ORR). "Jiggling" describes mesiodistal or buccolingual movement of the roots of the teeth during orthodontic treatment. In the present study, buccolingual movement is described as "jiggling." We aimed to investigate the relationship between ORR and jiggling and to test for positive cell expression in odontoclasts in resorbed roots during experimental tooth movement (jiggling) in vivo. Methods: Male Wistar rats were divided into control, heavy force (HF), optimal force (OF), and jiggling force (JF) groups. The expression levels of cathepsin K, matrix metalloproteinase (MMP)-9 protein, interleukin (IL)-6, cytokine-induced neutrophil chemoattractant 1 (CINC-1; an IL-8-related protein in rodents), receptor activator of nuclear factor ${\kappa}B$ ligand (RANKL), and osteoprotegerin protein in the dental root were determined using immunohistochemistry. Results: On day 21, a greater number of root resorption lacunae, which contained multinucleated odontoclasts, were observed in the palatal roots of rats in the JF group than in rats from other groups. Furthermore, there was a significant increase in the numbers of cathepsin K-positive and MMP-9-positive odontoclasts in the JF group on day 21. Immunoreactivities for IL-6, CINC-1, and RANKL were stronger in resorbed roots exposed to jiggling than in the other groups on day 21. Negative reactivity was observed in the controls. Conclusions: These results suggest that jiggling may induce ORR via inflammatory cytokine production during orthodontic tooth movement, and that jiggling may be a risk factor for ORR.

슬관절에 대한 국소 진동 적용이 자세 조절 능력에 미치는 영향 (Effects of Local Vibration on Knee Joint on Postural Control)

  • Purpose : The purpose of this study was to evaluate the effects of local vibration on knee joints on ability of postural control. Methods : The subjects(50) were divided into control group(25) and vibration group(25). Vibration group was given vibration on knee joint for 10 minutes and control group was given resting for 10 minutes. All subjects of each group were tested on MFT balance tester board for 30 seconds and MFT Balance Test English 1.7 was used to measure ability of postural control pre and post test. Results : 1. Laterality didn't have statistically significant difference pre and post test in both groups(p<0.05). 2. In the control group Body stability didn't have significant difference pre and post test(p>0.05), but had significant difference in the vibration group(p<0.05). 3. At assessment Movement of COG, sector2 in the control group and sector 1, 5 in the experimental group had significant difference pre and post test(p<0.05). Conclusion : From this result vibration on knee have an effect on Body stability. Therefore, the vibration will be effective in treatment of patients who have disability of postural control.

가상현실 프로그램이 노인의 정적균형 조절과 낙상효능감에 미치는 효과 (The Effect of a Virtual Reality Program on Static Balance Control and Fall Efficacy of Elderly People)

  • 신체는 연령이 증가하면서 자세조절에 관여하는 고유수용성 감각, 시각, 전정기능이 감소하고 근력이 약해지고 갑작스런 움직임의 변화에 대처하는 반사능력이 감소하여 균형조절에 영향을 주고 낙상을 유발하는 원인이 된다. 균형조절의 어려움은 낙상에 대한 두려움으로 인하여 낙상효능감에 영향을 주고 독립적인 일상생활에 많은 장애를 일으킨다. 본 연구는 요양원에 입소한 65세 이상의 노인을 대상으로 가상현실 프로그램이 노인의 정적균형 조절과 낙상효능감에 미치는 효과에 대한 연구이다. 연구방법은 65세 이상 노인 14명을 대상으로 하여 가상현실군(n=7)과 대조군(n=7)으로 매회 30분씩, 총8회 실시하였다. 가상현실군은 일반적 운동치료법근법과 가상현실 프로그램을 실시하였고, 대조군은 일반적 운동치료접근법을 실시하여 치료 전·후를 평가 비교하였다. 대상자의 정적균형 조절과 낙상효능감은 Bio-Rescue와 낙상효능감척도로 평가를 하였다. 결과에 따른 자료 분석은 Wilcoxon signed test와 Mann-Whitney U test를 실시하였다. 연구의 결과 가상현실군이 정적균형 조절 평가에서 눈을 뜬상태에서의 정적균형 조절이 향상되었으며, 안정성 한계 변화에서 움직임의 범위가 증가하였다. 낙상효능감의 변화에서도 증가하였다. 결론적으로, 가상현실 프로그램은 노인에게 정적균형 조절과 낙상효능감에 효과가 있으며, 노인에게 가상현실 프로그램을 이용한 다양한 치료 방법과 치료에 대한 객관적인 평가가 필요하다고 사료된다.
