목적 : 본 연구의 목적은 치매노인에게 다감각과 신체활동을 병합한 그룹 활동 프로그램이 이상행동과 보호자 고통정도의 감소에 영향을 주는지 알아보고자 하였다. 연구방법 : 본 연구는 치매전문요양원에 입소하여 생활하는 5명의 중증 이상의 치매노인을 대상으로 4주간 프로그램을 적용하였다. 프로그램 효과를 검증하기 위해 사용한 평가도구는 신경정신행동검사(Neuropsychiatric Inventory)였고, 12가지 이상행동빈도와 이에 따른 요양보호사의 고통정도를 프로그램 전과 후로 나누어 각각 측정하였다. 결과분석은 비모수 통계방법인 윌콕슨 부호 순위 검정(Wilcoxon signed rank test)을 사용하였다. 결과 : 치매노인의 이상행동증상 빈도의 평균이 70점에서 48점으로 감소하였고, 보호자의 고통정도는 평균이 47점에서 29점으로 감소하였다. 결론 : 본 연구의 결과를 통해 요양원 거주인의 다감각과 신체활동을 적용한 그룹 활동 프로그램이 중증 이상의 치매 노인에게 이상행동과 요양보호사 고통정도가 감소하는데 긍정적인 효과가 있음이 증명되었다.
In order to clarify the neuroprotective effect of Cornu Saigae Tataricae(CST) water extract on cultured mouse spinal motor neuron damaged by hydrogen peroxide (H₂O₂), MTT [3-(4,5-dimethylthiazole-2-yl)- 2,5-diphenyltetrazolium bromide] assay, LDH (Lactate Dehydrogenase) activity assay and SRB (Sulforhodamine B) assay were carried out after the cultured mouse spinal motor neuron were preincubated with various concentrations of CST water extract for 3 hours prior to exposure of hydrogen peroxide Cell viability of cultured mouse spinal motor neurons exposed to various concentrations of hydrogen peroxide for 6 hours was decreased in a dose-dependent manner. MTT50 values were 40 uM hydrogen peroxide. Cultured mouse spinal motor neurons in the medium containing various concentration of hydrogen peroxide for 6 hours showed increasing of LDH activity and decreasing of total protein synthesis. We know that hydrogen peroxide was toxic on cultured spinal motor neurons. Pretreatment of CST water extract for 3 hours following hydrogen peroxide prevented the hydrogen peroxide-induced neurotoxicity such as increasing of LDH activity and decreasing of total protein synthesis. These results suggest that hydrogen peroxide shows toxic effect on cultured spinal motor neurons and CST water extract is highly effective in protecting the neurotoxicity induced by hydrogen peroxide.
The purpose of this study is to clarify the characteristics between the type of daily routine and spatial configuration of activity areas in classroom and provide alternatives for space planning of activity areas in classroom of child-care centers. This study used the content analysis on daily activity plan and floor plans through field survey. Analysis on floor plans was conducted for 35 classrooms in 9 child-care centers which allowed field survey. The results of this study were as follows; There was no significant difference for classroom size according to the type of daily routine in 3 to 5-year-old classrooms. The average size of classrooms was $61.6m^2$ for care oriented type, $41.4m^2$ for indoor activity oriented type and $48.8m^2$ for group activity oriented type. There was no significant difference in composition of activity areas in classrooms according to the type of daily routine. In case of 0 to 2-year-old classrooms, they were composed of 6 activity areas including gross-motor, role play, block building, language, creative expression, exploration/manipulation. Activity area of the most low frequency was gross-motor area. In case of 3 to 5-year-old classrooms, most classrooms were composed of all the 7 activity areas including art, tone and rhythmic, block building, role play, language, math and science. Most accessible and central areas in 0 to 2-year-old classrooms were gross-motor and role play. Also, most separate areas were exploration/manipulation and block building. Most accessible and central areas in 3 to 5-year-old classroom were art and math. Also, most separate areas were tone and rhythmic and science.
This study was performed to examine the influences of the mental practice to the hemiplegic upper limb motor function improvement. 20 minute neurologic treatment based on the neurophysiological theory, 10 minute activities of daily living training, and 10 minute mental practice 5 times a week were given in turn to the experimental group(N=11). On the other hand 20 minute neurologic treatment, and 10 minute activities of daily living training 5 times a week were given in turn to the control group(N=11). Both Fugl-Meyer Assessment Scale and Manual Function Test were used to evaluate upper limb motor recovery, upper limb motor function and movement ability. And the Motor Activity Log; Amount of Use and Motor Activity Log; Quality of Movement before training, 2 weeks after training, and 4 weeks after training were measured to assess the upper limb motor quantitatively and qualitatively each. The results are as follows. 1) Considering the interactions of the rate of change on the upper limb motor recovery, motor function, movement ability improvement, and qualitative motor improvement in ADL of experimental group and control group, the change rates of experimental group were found to be greater than those of the control group. 2) In experimental group, the higher the achievements were, the better upper motor recovery was.
MR acoustic sound or noise due to gradient pulsings has been one of the problems in MRI, both in patient scanning as well as in many areas of psychiatric and neuroscience research, such as brain fMRI. Especially in brain fMRI, sound noise is one of the serious noise sources which obscures the small signals obtainable from the subtle changes occurring in oxygenation status in the cortex and blood capillaries. Therefore, we have studied the effects of acoustic or sound noise arising in fMR imaging of the auditory, motor and visual cortices. The results show that the acoustical noise effects on motor and visual responses are opposite. That is, for the motor activity, it shows an increased total motor activation while for the visual stimulation, corresponding (visual) cortical activity has diminished substantially when the subject is exposed to a loud acoustic sound. Although the current observations are preliminary and require more experimental confirmation, it appears that the observed acoustic-noise effects on brain unctions, such as in the motor and visual cortices, are new observations and could have significant consequences in data observation and interpretation in future fMRI studies.
PURPOSE: The objectives of this study were to investigate the effects of physical therapy provided at home on the motor function, activity of daily living (ADL), and level of pain in veterans men with disabilities, and to provide the information. METHODS: The participants were 108 patients men with disabilities living in the cities Seoul, Pusan, Daegu, Gwangju and Daejeon; they performed physical therapy at home-provided by physical therapy services- that included therapeutic exercise for 50 minute, 1~2 times a week for 24 weeks. The motor function was measured by using motor assessment scale (MAS) and ADL, the level of pain were recorded both before and after physical therapy. The data form both before and after physical therapy were analyzed by using the paired t-test. All statistical tests in this study were performed with the level of significance set at .05. RESULTS: After 24 weeks of physical therapy, we observed significant improvement in the MAS and ADL scale, as well as significant decrease in the level fo pain in the participants (p<.01). Significant improvement was also observed in the MAS and ADL scale, as well as a decrease in the level of pain among participants with stroke and spinal disease (p<.05). CONCLUSION: The findings of this study show that physical therapy provided at home can lead to an improvement in the motor function as well as improved ADL and pain control; physical therapy also has positive effects in relieving symptoms of veterans with disabilities.
Objective : The effect Saenggitang (GT), which has been used for amnesia, in Oriental Medicine, on memory and learning ability, was investigated. Methods : Hot water extracts (HWE) of SGT were used for the studies. In passive avoidance performances (step through test), active avoidance performances (lever press test), Motor activity, pentobarbital-induced sleep, 20 and 50 mg/100g of SGT-HWE ameliorated the memory retrieval deficit induced by 40% ethanol. Results : The SGT-HWE did not affect the ambulatory activity of normal mice in normal condition. 20 and 50 mg/100g of SGT-HWE enhanced contextual fear memory, but not cued fear memory in a fear conditioning task, which requires the activation of the NMDA (N-methyl-D-aspartase) receptor. SGT-HWE did not affect the motor activity measured by the titling type ambulometer test performed immediately and 24 hr after the administration. SGT-HWE prolonged the sleeping time induced by 50 mg/kg pentobarbital in mice and decreased SMA (spontaneous motor activity) in active avoidance performances (lever press test). Conclusion : These results indicate that the SGT-HWE have an improving effect on the memory retrieval disability induced by ethanol and may act as a stimulating factor for activating the NMDA receptor. and the SGT-HWE has a tranquilizing and anti-anxiety action.
Purpose: This is designed to study the effect of weight-support walking training through motor learning on motor functions of children with cerebral palsy, in particular their activity of daily living and balance. Methods: Thirteen children with spastic cerebral palsy, at gross motor function classification system (GMFCS) levels III~IV, underwent treadmill walking training. It used principles of weight support, 4 times a week for 7 weeks, 10 minutes at a time, before and after neurodevelopmental physical therapy. Everyday functions were measured using Functional Independence Measure for Children (Wee-FIM). The ability to keep their balance was measured using electronic measuring equipment from good balance system and the assessment was made before and after the experiment. Results: There were significant differences (p<0.05) between pre and post experiment levels of functional independence in everyday life, in self-care activities, mobility, locomotion and social cognition. With regard to changes in standing balance, there were significant differences before and after the experiment (p<0.05) in GMFCS level III. There was a reduction in the agitation velocity in the x- and y-axes which measures the left-to-right shaking; in GMFCS level IV, velocity moment was reduced. Conclusion: Walking training using a treadmill can help improve the everyday activity and balance in children with spastic cerebral palsy. It can also be served as a useful purpose as a method of intervention in pediatric care.
On the basis of morphological and functional studies, it is now established that there exist multiple motor representation areas in the frontal lobe of subhuman primates. Recent development of analysis on cerebral critical organization in human subjects, utilizing novel techniques of PET and MRI, provides evidence of corresponding motor areas. Each area has its unique sources of inputs from the thalamus and from other parts of the cerebral cortex. To understand functional roles of these multiple motor areas, it is necessary to study neural activity while subjects are performing a variety of motor tasks. In view of high accuracy in spatial and temporal resolution, the analysis of single cells in relation to specific aspects of motor behavior remains to be a powerful research technique. It is with this technique that a number of novel concepts on functional roles of multiple motor areas have been proposed.
Parkinson's disease (PD) is a debilitating disorder resulting from loss of dopamine neurons. In dopamine deficient state, the basal ganglia increases inhibitory synaptic outputs to the thalamus. This increased inhibition by the basal ganglia output is known to reduce firing rate of thalamic neurons that relay motor signals to the motor cortex. This 'rate model' suggests that the reduced excitability of thalamic neurons is the key for inducing motor abnormalities in PD patients. We reveal that in response to inhibition, thalamic neurons generate rebound firing at the end of inhibition. This rebound firing increases motor cortical activity and induces muscular responses that triggers Parkinsonian motor dysfunction. Genetic and optogenetic intervention of the rebound firing prevent motor dysfunction in a mouse model of PD. Our results suggest that inhibitory synaptic mechanism mediates motor dysfunction by generating rebound excitability in the thalamocortical pathway.
