주파수에 따른 신경근전기자극치료가 중증 만성폐쇄성폐질환 환자의 호흡근력, 넙다리네갈래근 활성도 및 삶의 질에 미치는 영향 (Effects of Frequency-Dependent Neuromuscular Electrical Stimulation on The Respiratory Strength, Quadriceps Muscle Activity and Quality of Life in Patients with Severe Chronic Obstructive Pulmonary Disease)

  • PURPOSE: To solve the problems of chronic obstructive pulmonary disease (COPD) patients complaining of muscle fatigue and having limited motor abilities, this study provided the clinical basic data for pulmonary rehabilitation by examining the effects of High Low-Frequency Neuromuscular Electrical Stimulation (NMES) on the respiratory muscle strength, quadriceps muscle activity, and life quality. METHODS: Samples were collected from 20 COPD patients and placed randomly in a low-frequency group (n=10) and high-frequency group (n=10). For a pretest, the respiratory muscle strength (MIP, MEP), quadriceps muscle activity (LF, VM, VL), and life quality (SGRQ) were measured. After applying NMES to each group for 30 minutes at a time, five times a week, for four weeks, a posttest was conducted in the same way as the pretest. RESULTS: Both groups showed significant differences in the respiratory muscle strength, quadriceps muscle activity, and quality of life within the groups and there were significant differences in the quadriceps muscle activity between the groups. CONCLUSION: The four-weeks NMES positively influenced the respiratory muscle strength and life quality by enhancing the function of the quadriceps and exercise tolerance. In particular, high-frequency(75 Hz) NMES was more effective than low-frequency (15 Hz) NMES. This result can be an alternative means to improve the physical functions of COPD patients clinically in the future.

뇌성마비 아동의 일상생활활동 수행능력과 사물조작기능, 대동작기능의 상관관계 연구 (The Correlation Among the Activity of Daily Living, the Manual Ability and the Gross Motor Function)

  • 목적 : 본 연구는 뇌성마비아동을 대상으로 일상생활활동수행 능력을 평가하기 위한 측정 도구인 수정바델지수(Modified Barthel Index: MBI)와 사물조작 기능 분류체계(Manual Ability Classification System: MACS)와 대동작 기능 분류체계(Gross Motor Function Classification System: GMFCS)와의 상관관계를 통하여 장애판정 지표인 MBI를 유용하게 활용할 수 있는 정보를 제공하고자 한다. 연구 방법 : 연구대상자는 전문재활병원에서 작업치료와 물리치료를 받고 있는 뇌성마비 아동 82명을 대상으로 MBI, MACS, GMFCS를 사용하여 평가하였으며, 일상생활활동 수행능력과 사물조작기능, 그리고 대동작기능과의 관계를 알아보기 위하여 상관분석을 사용하여 분석하였다. 결과 : 본 연구 결과는 MBI와 MACS간의 상관계수가 -.765(p<0.001)로 높은 음의 상관을 나타냈고, MBI와 GMFCS 는 상관계수가 -.851(p<0.001)로 높은 음의 상관관계가 있었다. MACS와 GMFCS의 상관계수 또한 .615(p<0.001)로 높은 상관관계를 보였다. 결론 : 뇌성마비 유형에 따른 MBI의 각 항목과 MACS, GMFCS와의 상관관계에 대한 결과는 모든 뇌성마비 유형과 관련하여 MBI의 항목과 MACS와 GMFCS은 유의미한 상관관계를 나타냈다. 뇌병변 장애등급 판정 기준으로 사용하는 MBI와 MACS, GMFCS와의 높은 상관관계는 장애판정 지표인 MBI를 뇌성마비 장애 아동들을 대상으로 유용하게 활용할 수 있을 것이다.

과제지향적 동작관찰훈련이 뇌졸중 환자의 환측 상지기민성과 일상생활활동에 미치는 영향 (The Effect of Action Observation Training on Dexterity of Affected Upper Extremity Motor Function and ADL in Stroke Patients)

  • 목적 : 본 연구는 과제지향적 동작관찰훈련이 뇌졸중 환자의 환측 상지기민성과 일상생활활동에 미치는 영향을 알아보고자 하였다. 연구방법 : 15명의 뇌졸중 편마비 환자를 선정하여 단일집단 사전사후 설계를 사용하였다. 대상자는 동영상을 통하여 과제지향적 동작수행을 관찰한 후 동작을 반복적으로 모방 연습하였다. 훈련은 주 3회, 회당 20~30분, 3주간 실시하였다. 환측 상지기민성에 대해 MFT, 10초 검사, 박스와 나무상자 검사를 시행하였고, 일상생활활동은 MBI, MAL을 수행하였다. 전, 후 비교는 윌콕슨 부호순위검정를 사용하였다. 결과 : 훈련 후, 대상자의 환측 상지기민성은 훈련 전에 비해 유의한 차이를 보였고, 일상생활활동 또한 훈련 전과 훈련 후에 유의미한 증진을 보였다. 결론 : 과제지향적 과제를 적용한 동작관찰훈련은 뇌졸중 환자의 환측 상지기민성과 일상생활활동에 긍정적인 효과를 보였다. 따라서 동작관찰훈련을 작업치료 중재 유형의 한 전략으로 제안하고자 한다.

수유기동안 납 투여가 성숙 쥐의 불안감 관련 행동양상에 미치는 영향 (Effect of Lead Exposure During Lactational Period on Anxiety in Rat Using Elevated Plus Maze Test)

  • 수유기 동안 납의 투여는 급격한 체중의 변화를 유발하여 3주령 및 11주령 쥐의 경우 납을 투여한 군이 대조군에 비해 각각 50% 및 26% 체중 감소를 보였으나 뇌의 무게에는 유의적 변화를 관찰 할 수 없었다. 납 투여는 뇌 지방산 조성 변화에 큰 영향을 주지 않았지만 특히 AA와 DPAn-6와 같은 고도의 불포화 지방산 함량의 증가를 유발하였다. Motor ac-tivity 실험에서 납 투여군의 경우 대조군에 비하여 주어진 시간 내에서 움직이는 시간과 움직인 거리가 긴 것으로 보아 운동 활동성이 큰 것으로 여겨지나 두 군 간에 유의적 차이는 없었다. Elevated plus maze 실험에서 실험 1일째의 경우 open arm에 머무른 시간에는 유의적 차이가 없었으나 2일째에는 두 군에서 현저히 open arm에 머무른 시간이 감소하였으며 납 투여군의 경우 대조군에 비하여 머무른 시간이 감소하는 경향이었으나 통계적 유의성은 없었다. Open arm에 들어가는 횟수를 관찰한 결과, 실험 1일째의 경우 open arm에 들어가는 횟수가 납 투여군에서 유의적으로 적었고 2일째에는 두 군에서 모두 open arm에 들어가는 횟수가 감소하였으며 대조군에 비해 납을 투여한 쥐의 경우가 유의적으로 낮은 횟수로 open arm에 들어갔음을 관찰 할 수 있었다. 이상의 결과로부터 수유기 동안의 납 투여는 성숙 동물의 불안감을 증가시키는 효과를 나타내었다.

학령전기 아동 어머니의 아동 신체활동 지지 (Factors Influencing Maternal Support for Physical Activity of Preschool Children)

  • 목적 본 연구의 목적은 학령전기 아동 어머니의 아동 신체활동 지지의 수준을 파악하고, 아동 신체활동 지지에 대한 영향요인을 확인하는 것이다. 방법 자가보고식 설문조사 방법을 적용하였으며, 대전 소재 3개 유치원에 다니는 만 3-6세 학령전기 아동의 어머니의 156명의 자료가 분석되었다. 결과 본 연구 결과, 전반적인 신체활동 지지 수준은 4점 만점 $2.69{\pm}0.35$점이었고, 세부적으로 볼 때, 장비 또는 경제적 지원 $2.61{\pm}0.61$점, 모델링 $2.42{\pm}0.54$점, 지역사회자원 이용 $2.55{\pm}0.47$점, 좌식활동제한은 $3.18{\pm}0.51$점으로 나타나 신체활동 지지 수준은 중등도인 것으로 나타났으며, 이는 미국 부모의 지지와 비교하여 낮은 수준이다. 어머니의 아동 신체활동 지지에 대한 영향요인은 가정의 경제적 수준, 특히 월수입 500만 원 이상인 경우(${\beta}$=.417, p =.002)이며, 아동 신체활동 지지에 대한 어머니의 자기효능감(${\beta}$=.231, p =.003), 아동 신체활동 지지를 위한 충분한 정보(${\beta}$=.173, p =.022), 아동 신체활동 지지를 위한 교육경험(${\beta}$=.237, p =.002)이었다. 결론 본 연구의 결과에 의하면, 부모의 아동 신체활동 지지를 증진시키기 위해서는 부모의 신체활동 지지 자기효능감 증진을 위한 전략 개발과 충분한 전문적 정보제공이 필요하다. 추후연구로서 아동 신체활동 지지를 위한 부모교육 프로그램과 가족 중심의 아동 신체활동 증진 프로그램의 개발이 필요하며, 부모의 아동 신체활동 지지에 영향을 미치는 요인이 추가 탐색되어야 할 것이다.

The effect of physical training on glutamate transporter expression in an experimental ischemic stroke rat model

  • Objective: The present study was aimed at determining the effect of physical training on glutamate transporter activity in a middle cerebral artery occlusion (MCAO)-induced ischemia injury rat model. Design: Randomized controlled trial. Methods: In this study, we randomly divided them into three groups. Group I included non-occlusion sham controls (n=10), Group II included non-physical training after MCAO (n=10), and Group III included rats that were subjected to physical training after MCAO (n=10). Rats in the physical training group underwent treadmill training, which began at 24 h after MCAO and continued for 14 consecutive days. The training intensity was gradually increased from 5 m/min on the first day to 12 m/min on day 3, and it was maintained until day 14. Focal cerebral ischemia was examined in adult male Sprague-Dawley rats by using the MCAO model. We determined the functional outcomes for each rat on days 1, 7, and 14. Glutamate transporter-1 (GLT-1) activity in the cortex of rats from all three groups was examined at the end of the experiment. Results: Out result show that MCAO rats exhibited severe neurological deficits on the 1 day, and there was no statistically significant in each groups. We observed that the functional outcomes were improved at days 7 and 14 after middle cerebral artery occlusion, and GLT-1 activity was increased in the physical training group (p<0.05). Conclusions: These results indicated that physical training after focal cerebral ischemia exerts neuroprotective effects against ischemic brain injury by improving motor performance and increasing the levels of GLT-1 activity.

심부전 환자의 신체활동과 규칙적 운동에 대한 측정의 일관성 및 운동역량과 삶의 질과의 관련성 (Consistency in Measuring Physical Activities and Regular Exercises and Their Relationship with Exercise Capacity and Quality of Life in Patients with Heart Failure)

  • Purpose: In order to examine whether daily physical activity (DPA) can be considered the same as a regular exercise (RE) in patients with Heart Failure (HF), we examined the relationship between RE and DPA, and we explored the relationship of RE and DPA with exercise capacity and quality of life (QOL) in patients with HF. Methods: This cross-sectional, correlational study utilized pretest data (N=136) of a long-term intervention study. Data of the parent study were collected from May 1st to September 26th, 2013. Measures included questions about regular exercise, International Physical Activity Questionnaire, a 6-Minute Walk Test (6MWT), and the Kansas City Cardiomyopathy Questionnaire (KCCQ). The data were analyzed using descriptive analysis, Chi-square test, and ANOVA using the SPSS 21.0 program. Results: There were inconsistencies in measures between RE and DPA. About 42.0% of the participants engaging in health enhancing physical activity did not consider themselves to be regular exercisers. Depending on the levels of RE and DPA, significant differences in 6MWT and QOL were observed. Conclusion: RE is more likely to be related to the health status of patients with HF. Deliberate assessment and cautious interpretation of DPA in patients with HF is needed.

Comparison of EMG Activity of the Posterior Oblique Sling Muscles and Pelvic Rotation During Prone Hip Extension With and Without Lower Trapezius Pre-Activation

  • Background: Prone hip extension (PHE) can be performed to measure the lumbopelvic motor patterns and motions. Imbalances in lumbopelvic muscle activity and muscle weakness can result in instability including pain in lumbopelvic region. The posterior oblique sling (POS) muscles contribute to dynamic lumbopelvic stability. In addition, POS are anatomically aligned with the trapezius muscle group according to shoulder positions. Objects: This study compared the electromyography (EMG) activity of POS and pelvic compensations during PHE with and without pre-activation of lower trapezius muscle (lowT). Methods: Sixteen healthy males were recruited. PHE was performed in randomized order: PHE with and without lowT pre-activation. Surface EMG signals were recorded for biceps femoris (BF), gluteus maximus (GM) (ipsilateral), lumbar multifidus (MF) (bilateral), and the lowT (contralateral). An electromagnetic tracking motion analysis was used to measure the angle of pelvic rotation and anterior tilting. Results: The ipsilateral GM and bilateral MF EMG amplitudes were greater during PHE with lowT pre-activation compared to PHE without lowT pre-activation (p<.05). The BF amplitude during PHE without lowT pre-activation was significantly greater than that during PHE with lowT pre-activation (p<.05). The angles of pelvic rotation and anterior tilting during PHE with lowT pre-activation were significantly smaller compared to PHE without lowT pre-activation (p<.05). Conclusion: PHE with lowT pre-activation, which is aligned with the POS, showed more increased MF and GM muscular activity with smaller lumbopelvic compensations in rotation and anterior tilting compared to PHE without lowT pre-activation.

유산소 운동이 소아 및 청소년의 인지기능에 미치는 영향 : 문헌고찰 (Effect of Aerobic Exercise on Cognitive Functions in Childen and Adolescents : A Review)

  • The purpose of this review was to investigate the relationship between aerobic exercise and cognitive function as well as synthesize the effect of aerobic exercise intervention studies centered on psychiatric symptoms associated with general cognitive deficit. Prospective studies on aerobic exercise and improvement of cognitive function were reviewed and synthesized. In addition, this literature review provides significant positive results on the relationship between aerobic exercise and general cognitive deficit associated with psychiatric symptoms. Review of this literature suggests that there is a positive relationship between participation in aerobic exercise and cognitive function. In this text, there are at least three general pathways by which aerobic exercise may facilitate executive function in children : 1) cognitive demands inherent in engaging in physical activity, 2) cognitive demands inherent in cooperation of complex motor tasks, and 3) physiological changes resulting from aerobic exercise. Another main finding of this review is that physical activity has a stronger influence on cognitive deficit, including attention-deficit hyperactivity disorder, depression, and anxiety. Development of cognitive function is the most important factor for children and youth. Therefore, future research should prove relationship between physical activity and cognitive function using a more scientific and quantitative approach design.

Sports injuries: a 5-year review of admissions at a major trauma center in the United Kingdom

  • Purpose: Sports offer several health benefits but are not free of injury risk. Activity dynamics vary across sports, impacting the injury profile and thereby influencing healthcare resource utilization and health outcomes. The purpose of this study was to investigate sports-related major trauma cases and compare differences across sports and activity groups. Methods: A retrospective case notes review of sports-related major traumas over a 5-year period was conducted. Demographic, hospital episode-related, and health outcome-related data were analyzed, and differences were compared across sports and activity groups. The Glasgow Outcome Scale (GOS) at discharge was used as the primary outcome measure and the length of hospital stay as the secondary outcome measure. Results: In total, 76% of cases had good recovery at discharge (GOS, 5), 19% had moderate disability (GOS, 4), and 5% had severe disability (GOS, 3). The mean length of hospital stay was 11.2 days (range, 1-121 days). The most severely injured body region was the limbs (29.1%) and vertebral/spinal injuries were most common (33%) in terms of location. A significant difference (P<0.05) existed in GOS across sports groups, with motor sports having the lowest GOS. However, no significant differences (P>0.05) were found in other health-outcome variables or injury patterns across sports or activity groups, although more competitive sports cases (67%) required admission than recreational sports cases (33%). Conclusions: Spinal injuries are the most frequent sports injuries, bear the worst health outcomes, and warrant better preventive measures. Head injuries previously dominated the worst outcomes; this change is likely due to better preventive and management modalities. Competitive sports had a higher injury frequency than recreational sports, but no difference in health outcomes or injury patterns.
