Comparison of Helical TomoTherapy with Linear Accelerator Base Intensity-modulated Radiotherapy for Head & Neck Cases (두경부암 환자에 대한 선량체적 히스토그램에 따른 토모치료외 선형가속기기반 세기변조방사선치료의 정량적 비교)

  • TomoTherapy has a merit to treat cancer with Intensity modulated radiation and combines precise 3-D imaging from computerized tomography (CT scanning) with highly targeted radiation beams and rotating beamlets. In this paper, we comparing the dose distribution between TomoTherapy and linear accelerator based intensity modulated radiotherapy (IMRT) for 10 Head & Neck patients using TomoTherapy which is newly installed and operated at National Cancer Center since Sept. 2006. Furthermore, we estimate how the homogeneity and Normal Tissue Complication Probability (NTCP) are changed by motion of target. Inverse planning was carried out using CadPlan planning system (CadPlan R.6.4.7, Varian Medical System Inc. 3100 Hansen Way, Palo Alto, CA 94304-1129, USA). For each patient, an inverse IMRT plan was also made using TomoTherapy Hi-Art System (Hi-Art2_2_4 2.2.4.15, TomoTherapy Incorporated, 1240 Deming Way, Madson, WI 53717-1954, USA) and using the same targets and optimization goals. All TomoTherapy plans compared favorably with the IMRT plans regarding sparing of the organs at risk and keeping an equivalent target dose homogeneity. Our results suggest that TomoTherapy is able to reduce the normal tissue complication probability (NTCP) further, keeping a similar target dose homogeneity.

An Experimental Study of Sediment Transport Patterns behind Offshore Structure (외해 구조물 배후의 표사이동에 관한 실험적 연구)

  • Recently, securing a vast land in the land region becomes more difficult and efforts to seek its alternation in the sea area have been increased. As a consequence, the coastal region has been faced to extensive beach erosion problems. In planning offshore structures such as artificial islands, it is necessary to forecast the influence of the structure construction exerting on the beach erosion of the adjacent coast. In the present study, the sediment movement pattern behind offshore structure was examined through a series of three dimensional movable bed experiments, so as to develop the numerical model which forecasts morphological change including beach erosions. The experimental results reveal that the sediment movement patterns of the beach line side and the depth region are separated at a certain boundary line. In details, at the beach side including swash zone the sediment movement becomes dominant, which is governed by a relation between depth contours and incident wave directions, while at the depth region the bed load and suspended load due to the orbit motion of waves are carried by nearshore currents, and both movements are clearly separated at a specified boundary that is related to partial standing wave from the beach. It is expected that these results can be effectively used for verification of a numerical model on morphological change of the coast.

Current Treatment of Tibial Pilon Fractures (경골 천정(pilon) 골절의 최신 치료)

  • Pilon fractures involving distal tibia remain one of the most difficult therapeutic challenges that confront the orthopedic surgeons because of associated soft tissue injury is common. To introduce and describe the diagnosis, current treatment, results and complications of the pilon fractures. In initial assessment, the correct evaluation of the fracture type through radiographic checkup and examination of the soft tissue envelope is needed to decide appropriate treatment planning of pilon fractures. Even though Ruedi and Allgower reported 74% good and excellent results with primary open reduction and internal fixation, recently the second staged treatment of pilon fractures is preferred to orthopedic traumatologist because of the soft tissue problem is common after primary open reduction and internal fixation. The components of the first stage are focused primarily on stabilization of the soft tissue envelope. If fibula is fractured, fibular open reduction and internal fixation is integral part of initial management for reducing the majority of tibial deformities. Ankle-spanning temporary external fixator is used to restore limb alignment and displaced intraarticular fragments through ligamentotaxis and distraction. And the second stage, definitive open reduction and internal fixation of the tibial component, is undertaken when the soft tissue injury has resolved and no infection sign is seen on pin site of external fixator. The goals of definitive internal fixation should include absolute stability and interfragmentary compression of reduced articular segments, stable fixation of the articular segment to the tibial diaphysis, and restoration of coronal, transverse, and sagittal plane alignments. The location, rigidity, and kinds of the implants are based on each individual fractures. The conventional plate fixation has more advantages in anatomical reduction of intraarticular fractures than locking compression plate. But it has more complications as infection, delayed union and nonunion. The locking compression plate fixation provides greater stability and lesser wound problem than conventional implants. But the locking compression plate remains poorly defined for intraarticular fractures of the distal tibia. Active, active assisted, passive range of motion of the ankle is recommended when postoperative rehabilitation is started. Splinting with the foot in neutral is continued until suture is removed at the 2~3 weeks and weight bearing is delayed for approximately 12 weeks. The recognition of the soft tissue injury has evolved as a critical component of the management of pilon fractures. At this point, the second staged treatment of pilon fractures is good treatment option because of it is designed to promote recovery of the soft tissue envelope in first stage operation and get a good result in definitive reduction and stabilization of the articular surface and axial alignment in second stage operation.

New Vehicle Classification Algorithm with Wandering Sensor (원더링 센서를 이용한 차종분류기법 개발)

  • The objective of this study is to develop the new vehicle classification algorithm and minimize classification errors. The existing vehicle classification algorithm collects data from loop and piezo sensors according to the specification("Vehicle classification guide for traffic volume survey" 2006) given by the Ministry of Land, Transport and Maritime Affairs. The new vehicle classification system collects the vehicle length, distance between axles, axle type, wheel-base and tire type to minimize classification error. The main difference of new system is the "Wandering" sensor which is capable of measuring the wheel-base and tire type(single or dual). The wandering sensor obtains the wheel-base and tire type by detecting both left and right tire imprint. Verification tests were completed with the total traffic volume of 762,420 vehicles in a month for the new vehicle classification algorithm. Among them, 47 vehicles(0.006%) were not classified within 12 vehicle types. This results proves very high level of classification accuracy for the new system. Using the new vehicle classification algorithm will improve the accuracy and it can be broadly applicable to the road planning, design, and management. It can also upgrade the level of traffic research for the road and transportation infrastructure.

  • With the development of ICT technology, the indoor utilization of robots is increasing. Research on transportation, cleaning, guidance robots, etc., that can be used now or increase the scope of future use will be advanced. To facilitate the use of mobile robots in indoor spaces, the problem of self-location recognition is an important research area to be addressed. If an unexpected collision occurs during the motion of a mobile robot, the position of the mobile robot deviates from the initially planned navigation path. In this case, the mobile robot needs a robust controller that enables the mobile robot to accurately navigate toward the goal. This research tries to address the issues related to self-location of the mobile robot. A robust position recognition system was implemented; the system estimates the position of the mobile robot using a combination of encoder information of the mobile robot and the absolute space coordinate transformation information obtained from external video sources such as a large number of CCTVs installed in the room. Furthermore, vector field histogram method of the pass traveling algorithm of the mobile robot system was applied, and the results of the research were confirmed after conducting experiments.

  • This paper describes the design and evaluation of a driving mode decision algorithm for automated driving for merge situations on highways. For the development of a highly automated driving control algorithm for merge situations, the driving mode decision is crucial for merging appropriately. There are two driving modes: lane keeping and lane changing (merging). The merge mode decision is determined based on the state of the surrounding vehicles and the remaining length of the merge lane. In the merge mode decision algorithm, merge possibility and the desired merge position are decided to change the lane safely and quickly. A safety driving envelope is defined based on the desired driving mode using the information on the surrounding vehicles' behaviors. To obtain the desired steering angle and longitudinal acceleration for maintaining the subject vehicle in the safe driving envelope, a motion planning controller is designed using model predictive control (MPC), with constraints that are decided considering the vehicle dynamics, safe driving envelope, and actuator limit. The proposed control algorithm has been evaluated via computer simulation studies.

Analysis of Scientific Models in Science Textbooks for the 7th Grade (중학교 과학 교과서 물질 영역의 과학적 모형 유형 분석)

  • The purpose of the study was to classify scientific models in the seventh grade science textbooks of the 2007 revised science curriculum. The three chapters of 'three states of material', 'motion of molecule', and 'change of state and energy' were investigated. There were two types of the scientific model as 'mode of representation' and 'attribute of representation'. The mode of representation was composed of 'action model', 'analogical model', 'symbolic model', and 'theoretical model' and the attribute of representation was composed of 'static model' and 'dynamic model'. The results showed that the action model and the analogical model were used primarily in mode of representation. The dynamic model were widely used in attribute of representation. Area of matters dealt with conception of molecules and aimed for students to understand the arrangement and movement of molecule microscopically about macroscopic state in a daily life. Tis study could help to recognize the limitations of scientific models on current textbooks and offer more useful information in planning lessons and organizing textbooks for the future.

Development of Film Verification as the QA of IMRT for Advanced Hepatoma Patients (간암 환자의 세기조절 방사선치료에서 임상적응 가능한 QA 기법의 개발)

  • Hepatoma is one of 3 most common malignancies in Korea, the survival rate is not improved since last decades because of delayed diagnosis and limited treatment conditions. Radiation was one of treatment options but the impact on the survival is not remarkable. High dose exposure to target area was suggested for improved effect but low tolerance dose of normal liver tissue is the main limited factor. IMRT is the advanced form of 3DCRT, for focusing high dose on target with minimal dose to surrounding normal tissues. Motion of the tumor by respiration, cardiac pulsation and peristalsis is the main treatment harrier of IMRT for treatment of hepatoma patients. Development of QA technique for acceptable geometrical uncertainties and dose error on target volume is essential for IMRT in clinical treatment but proper QA technique is not yet developed. This study compared the verification film dosimetry with measured dose in phantom and calculated dose in planning computer on exactly same conditions of patient treatments. Within 3% dose differences between 3 groups were confirmed. We suggest that our verification QA technique is easy, economic, iterative and acceptable in clinical application for advanced hepatoma patients.

Dental Treatment of a Patient with Cerebral Palsy under General Anesthesia (뇌성마비 환자의 전신마취 하 치과치료)

  • Background: Cerebral palsy (CP) is non-progressive disorder of motion and posture. In CP patient, there are difficulties in dental treatment because of uncontrolled movement of limb and head, and conjoined disabilities such as cognitive impairment, sensory loss, seizures, communication and behavioral disturbances. It is reported that CP patients have high incidence in caries and a higher prevalence of periodontal disease. But, despite the need for oro-dental care, these patients often are unlikely to receive adequate treatment without sedation or general anesthesia because of uncontrolled movements of the trunk or head. Methods: We reviewed the 58 cases of 56 patients with CP who underwent outpatient general anesthesia for dental treatment at the clinic for the disabled in Seoul National University Dental Hospital. Results: The mean age was 19 (2-54) years. The number of male patient was 40 and that of female was 18. They all had severe spastic cerebral palsy and 22 had sever mental retardation, 15 epilepsy, 8 organic brain disorder, 1 blindness, 2 deafness and cleft palate. For anesthesia induction, 14 cases was needed physical restriction who had sever mental retardation and cooperation difficulty, but 44 cases showed good or moderate cooperation. Drugs used for anesthesia induction were thiopental (37 cases), sevoflurane (14 cases), ketamine (3 cases ) and propofol (4 cases). All patients except one were done nasotracheal intubation for airway management and 4 cases were needed difficult airway management and 1 patient already had tracheostomy tube. Mean total anesthetic time was $174{\pm}56$ min and staying time at PACU was $88{\pm}39$ min. There was no death or long term hospitalization because of severe complications. Conclusion: If general anesthesia is needed, pertinent diagnostic tests and workup about anomaly, and appropriate anesthetic planning are essential for safety.

Research on the Process of British Housing Types' Development, and Case study of Comtemporary Perimeter Block Collective Housing (영국의 주택유형 발달과정과 현대 블록형 집합주택 사례 연구)

  • British housing types began with detached houses using wood based on early Germanic cultures, and gradually divided into multi-story houses such as terraces houses, with the influx of houses using stone from Latin cultures influenced by France. Today, various types of housing types such as detached houses, townhouses, and apartments are mixed, and their origins can be found in housing types that have continued from the past. Housing types existing in the UK continue their context through this historical development process, but housing types in the UK also have been combined with those of other cultures, leading to modern housing types. In this paper, we examine the process of the UK's peculiar development of housing types superimposed with the recurring motion to the traditional European urban structures prior to the industrial revolution during contemporary years, and perimeter block collective housings of today; then, we study the process of applying the UK case as a concept of practical design and planning in order to continuously consider Korea's modern urban structure and corresponding types of housing. Through this process, we explore the identity of the housing type in Korea, which is pursuing fundamental changes in residential culture beyond quantitative housing supply in the process of rapid urbanization.
