Effects of Meridian Massage on Weight in Infants and on Mother-Infant Interaction (경락마사지가 신생아의 체중과 모아상호작용에 미치는 효과)

  • Purpose: This study was done to develop a new nursing intervention, Meridian Massage, and to investigate its effect on weight in infants and mother-infant interaction. Method: This study was conducted using a quasi experimental non equivalent control group pretest-posttest design. Thirty-one newborn infants from a postpartum Management Center were selected and assigned to two groups, experimental and control. Data were collected from February 1 to September 30, 2004. Infants in the experimental group (16) were given Meridian massage for 15 minutes daily for 6 days and weighed every day at 10 am. Using the Nursing Child Assessment Feeding Scale(1978), mother-infant interaction was determined before the treatment, after 1 week and 1 month after the massage. Results: Infants in the experimental group had a higher average weight than those in the control group and the difference was significant (Z=-2.29, p=.022). For mother-infant interaction, the experimental group had higher scores tHan the control group, and the difference was significant between both the two groups, and the 3 measurement times. Conclusion: The Meridian Massage in this study showed positive weight gain and positive mother-infant interaction. This study shows that meridian massage is an effective nursing intervention in improving infant development.

  • The Low Birth Weight infant birth rate in this country is a little more than 15 percent and is being increased. The survival rate of Low Birth Weight infant is over 90 percent and recently the rate runs is getting. However, because of the high risk of Low Birth Weight infant for handicap in growth, a preventive nursing intervention program for Low Birth Weight infant and their mother is considered to be necessary. Touch and massage, thus sensory stimulation has been considered to be important ensuring a normal growth of Low Birth Weight infant During the past decades sensory stimulation program has been used for premature and Low Birth Weight infants. Recently a study on the sensory stimulation for Low Birth Weight infants has bee n done in this country. Mother and infant relationship has a great influence on child's development. Especially, mother and infant interaction during one year after birth plays important role in child's social. affective and cognitive developments. But in the study of Low Birth Weight infants, the mother and infant interaction has been rare yet. However, there was no study effectiveness of the sensory stimulation on mother and infant interaction. In this respect, this study based on the importance of the nursing intervention, is intended to measure the effectiveness of the massage therapy in the aspects of weight, daily feeding amount, cortisolurine stress hormone and mother and infant interactions. This study has been conducted on the nonequivalent control group pretestposttest design in quasi experimental basis and Low Birth Weight infants from NICU of two Medical University Hospitals located in Taegu Metropolitan were selected in experimental group of 21 infants and control group of 20 infants. Data has been collected from May 1, 1999 to September 5, 2000. For the experimental group Field's sensory stimulation(tactile and kinesthetic stimulation) was applied 2 times a day for 10 days(10:00 - 11:00 hours in the morning and 19:00 - 20:00 in the afternoon) by nurse and mother. The electronic indicator scale (Cas Co. Korea) was used to measure infant's body weight. To determine urine cortisol concentration level under stress, rad immuno assay method was used. And to determine mother and infant interactions during feeding, tools developed by Kim Mi-Ye (1999) were used. Collected data were analyzed with SAS program using x-test, t-test, paired t-test and repeated measures ANOVA. Findings were as follows : 1. For the daily mean weight gain, the experimental group showed little higher than the control group, even though, there was no Statistically significant differences between two groups. 2. For the amount of daily mean feeding, the experimental group showed little higher than the control group, while there was no Statistically significant differences between two groups. 3. The level of wine cortisol concentration was increased in both groups, while no Statistical significance was shown between the two groups. 4. Mothers in experimental group were more likely to have higher mean scores in mother and infant interaction during feeding than mothers in the control group. Statistical significance was shown between the two groups(t= 5.78, P=.001). In conclusion, the massage therapy in this study showed with regard to even though through there was no statistically significance in the weight gain and urine stress hormone concentration. there was Statistical significantly higher in the mother and infant interaction during feeding. Based on the result of this study, it is considered that the massage therapy should be applied clinical practice and home to help a developmental growth and interaction of Low Birth Weight infants and mothers during the period of recovery.

An Analysis of Nursing Research related to the Mother-Child Interaction in Korea (모아상호작용 관련 간호연구 분석)

  • Purpose: The purpose of this study is to identified patterns and trend of studies and to analyzed the nursing research related to the mother-child interaction in Korea and to improve direction of nursing research in mother-child interaction in Korea. Method: The research studies related to the mother-child interaction were selected from the Korean Nurses Academic Society Journal, and from dissertation, which were conducted between 1961 and 2002 with the consideration of the condition and the cultural specialty of Korea. The total numbers of the studies were 34. These studies were classified according to 1)time of publication or presentation, 2)research design, 3)type of interaction objects, 4)measurement tool, and 5)chief content of studies. Ressult: 1) Until 1984, there was no research studies related to mother-child interaction. The number of studies on the mother-child interaction on the mother-child interaction has been increased rapidly after 1985. The mother-child interaction studies were 13(38.2%) from the doctoral dissertation, 4(11.8%) from the Master's theses and 17(50%) other papers were published in academic journals. 2) Experimental studies of research design was used in these research studies was 13(38.2%). Descriptive studies was 8(23.6%), correlational studies was 4(11.8%) and comparative studies was 4(11.8%). Qualitative study was just one(2.9%) and literature review was 4(11.8%). Especially experimental studies has been increased. 3) Participants of these studies were mother and infant 29(85.5%), father and infant 2(5.9%), mother and fetus 2(5.9%) and nurse and infant 1(2.9%) 4) Utilization of instruments as follows: 13 studies used Walker et al(1986)'s MIPIS (Mother-Infant Play Interaction Scale), 6 studies used Barnard(1978)'s NCAFS (Nursing Child Assessment Feeding Scale), 4 studies used NCATS(Nursing child Assessment Teaching Scale) for assessing mother-infant interaction. 3 studies used Stainton(1981)'s Parent-Infant Interaction Scale. Choi(1987), Kim(1999) and Kim and Son(1997) used measurement tool researcher made which was modified from NCATS (Nursing child Assessment Teaching Scale) and AMIS(Assessment of Mother-Infant Sensitivity Scale). Other tools used to survey influencing factors of interaction. 5) Content of the research studies were classified 3 different types, such as ① studies about the mother-child interactions' pattern, ② studies about influencing factors of the mother-child interaction, ③ studies about effect of nursing intervention program to improve the mother-child interaction. Conclusion: Therefore we make following suggestions which are made based on the above research analysis : 1) In the future research studies need to compare with other area of discipline in mother-child interaction. 2) More attention and in depth research is needed to validate in terms of research design and statistical data analysis. 3) It is important to develop the instruments which is culturally acceptable in Korea society. 4) The results of correlational studies and experimental studies needs to the integrated by meta analysis.

Effects of Maternal Role Education Program on the Mother-Infant Interaction and Infant Development (영아기 어머니역할 교육 프로그램이 모아상호작용과 영아발달에 미치는 효과)

  • The impact of childhood experience has lifelong significance on subsequent health and development. Especially, the experience of infant is mostly affected by the quality of parental care and rearing environment. But the new mothers usually do not know what to do because of the lack of experience in these days. Therefore, an educational program regarding maternal role would be necessary. This study was conducted to evaluate the effectiveness of the maternal role education program for mother-infant interaction, child-rearing environment, and infant development. Non-equivalent control group time-series design was used, and Barnard's mother-infant interaction model was used as a conceptual framework of this study. The subjects were the healthy infants weighing over 2,500gm at birth, whose gestational age was more than 37 weeks, and their mothers. The final sample consisted of 19 mother-infant dyads for intervention group and 18 dyads for control group. Data were collected from March 15th to September 3rd in 1999. For the intervention group, programmed education which focused on mother-infant interaction, breast feeding, and infant care was provided before discharge. Telephone counselling was provided within one week after discharge. Home visiting for maternal role education was provided twice, one month and three months postpartum. For the control group, home visiting was also conducted but only for data collection. The data were analyzed using chi-square test and t-test to test the equivalence of two groups, and the effectiveness of intervention program was determined with repeated measure ANCOVA and t-test. The results were as follows: 1. Significant differences were found in mother-infant interaction between two groups(p=.000). It indicates that intervention program was effective in improving mother- infant interaction. In subscale analysis, four out of six subscale showed significant differences between the groups: sensitivity to cues (p=.000), social-emotional growth fostering (p=.000), cognitive growth fostering(p=.000) in mothers, and responsiveness to caregiver (p=.019) in infants. 2. The difference in the mean score of childrearing environment (HOME) between the intervention group and control group was significant(p=.003). When each subscale of HOME was examined individually, intervention group showed significantly higher scores in the diversity of stimulation(p=.000), and mother's involvement(p=.001). 3. Three-month-Infants of the intervention group showed higher GQ in the Griffiths mental development scale(p=.026). In subscale analysis, significant differences were found in the personal-social(p=.005), and the hearing and speech(p=.003). In conclusion, the maternal role education program proved to be effective in promoting the mother-infant interaction, organizing the childrearing environment, and fostering the infant development. These results are very meaningful that we found maternal role education necessary for normal infants' mothers, and that nurses can make a great contribution in promoting health of infants and mothers.

Concept Analysis of Effective Breastfeeding (효과적인 모유수유 개념 분석)

  • Purpose: This study was conducted to analyze and clarify the concept of the effective breastfeeding. Methods: This study used Walker and Avant's process of concept analysis. Results: Effective breastfeeding is how to give infant adequate milk, which fulfills needs of mother and infant, from her breast so that they can be satisfied with the process and results. It included the effective breastfeeding properties as follows: feeding behaviors (positioning, latch on, and suckling), transferring a breast milk to an infant, mother-infant interaction, and satisfying their desires (satisfaction, comfort, mother's self-confidence, infant's adequate weight gain and defecation, adequate breastfeeding interval). The antecedent of effective breastfeeding were anatomical and functional normal breast, breastfeeding knowledge and steady-state of mother, and feeding desire, rooting reflex and normal oral cavity of infant. The consequences of effective breastfeeding were exclusive breastfeeding, infant and maternal health and wellbeing, and achievement of mother and infant attachment. Conclusion: The meaning of effective breastfeeding defined in this study will contribute to develop the effectiveness breastfeeding assessment tool and the nursing intervention for ineffective breastfeeding.

Effects of a Mother-fetus Interaction Promotion Program on Sleep and Activity in Mothers and Infants (모-태아 상호작용 증진 프로그램이 임부 및 영아의 수면-활동 양상에 미치는 효과)

  • Purpose: This study was done to evaluate effects of a mother-fetus interaction promotion program on maternal and infant sleep/activity regulation, and examine the correlation between maternal and infant sleep/activity. Methods: Thirty-four mother-infant dyads were recruited from an OB-GY clinic with postpartum care center, and assigned to the intervention group (17) or control group (17). Data were collected from January 18 to August 5 2005. A programmed education focused on mother-fetus interaction was provided to the intervention group in the 3rd trimester. Maternal sleep/activity record was completed at 32-36 weeks gestation and infant sleep/activity record was completed for one month after birth. Results: During the 3rd trimester, there were no significant differences in maternal sleep/activity between the groups, except for frequency of mothers' night awakening and urination at night. There were no significant differences in infant sleep, but significant difference was found in feeding times between the groups. Correlation of maternal sleep in the 3rd trimester and infant waking times was significant. Conclusion: The results of this study indicate that maternal sleep/activity during pregnancy influences infant sleep/activity at 3-4 weeks, and that nursing interventions to regulate maternal sleep/activity provided to primiparas can be beneficial to the regularity of infant sleep/activity patterns.

Comparison of Maternal Attachment and Maternal Role Confidence between Breast Milk in Sanitary Pack Feeding Infant's Mothers and Bottle Feeding Infant's Mothers of Low Birth Weight Infants in NICU (모유팩 모유수유아 어머니와 인공수유아 어머니간의 모아애착과 모성역할자신감 비교 - NICU의 저출생체중아를 중심으로 -)

  • Purpose: This study was performed to compare the difference of maternal attachment and the maternal role confidence between mother who feeds the child with mother's milk in sanitary pack by a nurse instead of her and mother who feeds the child with artificial milk. Methods: The subjects of this study consisted of mothers of premature baby who was in NICU and can not be fed with mother's milk directly. In the sample, 21 mothers were the breast feeding group and 20 were the bottle feeding group. Data were collected from April 3, 2004 to November 2, 2005, and were analyzed using SPSS WIN 10.0. Results: The degree of maternal attachment and maternal role confidence of the breast milk feeding group was higher than that of the bottle feeding group. In accordance with general characteristics, the difference was found in maternal attachment and maternal role confidence both breast feeding group and bottle feeding group. Conclusion: More systematic nursing mediation is required for the lactation of mother's milk in sanitary pack is planned to do positive interaction between mother and the child, which has an influence on the formation of maternal attachment and the of maternal role confidence after hospitalization.

Antenatal Depression and Mother-Fetal Interaction (임신 중 우울과 모-태아상호작용)

  • The purposes of this descriptive survey study were to describe antenatal depression and level of mother-fetal interaction, and to assess mother's behavior and feeling during mother-fetal interaction, in order to develop a base for nursing intervention programs for mothers who have antenatal depression. Method: Data were collected from 174 pregnant women who visited one public health center and OBGY clinic in Gangneung city. The instrument used for this study was a self-report questionnaire to identify depression (BDI), and mother-fetal interaction. Results: Of the mothers 63.2% were in the normal range for antenatal depression, 21.3% in the mild group and 15.5% were in the moderate to severe group. For antenatal depression, there were significant differences among the income, planned pregnant, health status, marriage satisfaction, family support, husband's love. The mean for mother-fetal interaction was $29.88{\pm}4.91$. For mother-infant interaction, there were significant differences in education, income, pregnant number, delivery number, feeding plan, marriage satisfaction, family support, husband's love. There was a weak correlations between antenatal depression and mother-fetal interaction but it did not reach statistical significance. Conclusion: Antenatal depression and mother-fetal interaction influence fetal development. It is essential to assess and provide immediate care to mothers who have antenatal depression and lower level mother-fetal Interaction.

Primiparas만 Perceptions of Their Delivery Experience and Their Maternal-Infant Interaction : Compared According to Delivery Method (초산모의 분만유형별 분만경험에 대한 지각과 모아상호작용 과정에 관한 연구)

  • One of the important tasks for new parents. especially mothers, is to establish warm, mutually affirming interpersonal relationships with the new baby in the family, with the purpose of promoting the healthy development of the child and the wellbeing of the whole family. Nurses assess the quality of the behavioral characteristics of the maternal-infant interaction. This study examined the relationships between primiparas pereptions of their delivery experience and their maternal infant interaction. It compared to delivery experience of mothers having a normal vaginal delivery with those having a casearean section. The purpose was to explore the relationships between the mother's perceptions of her delivery experience with her maternal infant interaction. The aim was to contribute to the development of theoretical understanding on which to base care toward promoting the quality of maternal-infant interaction. Data were collected directly by the investigator and a trained associate from Dec. 1, 1987 to March 8, 1988. Subjects were 3 random sample of 62 mothers, 32 who had a normal vaginal delivery and 30 who had a non-elective cesarean section (but without other perinatal complications) at three general hospitals in Seoul. Instruments used were the Stainton Parent -infant Interaction Scale(1981) and the Marut and Mercer Perception of Birth Scale(1979). The first observations were made in the delivery room (for vaginally delivered mothers only), followed by day 1, day 2, day 3, and 2 weeks, 4 weeks, 6 weeks and 8 weeks after birth, for a total of 7-8 contacts(Cesarean section mothers were observed on days 4 and 5 but the data not used for analysis). Observations in the hospital were made during the hour prior to scheduled feedings. The infant was placed beside the mother. Later contacts were made at home. Data analysis was done by computer using as SPSS program and indulded X² test, paired t-test, t-test, and Pearson Correlation coefficient ; the results were as follows. 1. Mothers who had a normal vaginal delivery tended to perceive the delivery experience more positively than cesarean section mothers(p=0.002). The finding supported the hypothesis I that perception of delivery would vary according to the method of delivery. Mothers' perceptions of birth were classified into three dimensions, labor, delivery and the bady. There was a significantly different and positive perception by the vaginally delivered mothers to the delivery experience(p=0.000) but no differences for labor or the bady according to the delivery method(p=0.096, p=0.389), 2. Mothers who had a normal vaginal delivery had higher average maternal-infant interaction scores(p=0.029) than mothers who had a cesarean section. There were similar higher scores for the 1st day(p=0.042), 2nd day (p=0.009), and the 3rd day(p=0.006) after delivery but not for later times. The findings supported the hypothesis Ⅱ that there would be differences in maternal-infant interaction for mothers having vaginal and cesarean section deliveries. However these differences deccreased section deliveries. However these differences decreased over time . by eight weeks the scores for vaginal delivery mothers averaged 8.1 and for cesarean section mothers, 7.9. 3. The more highly positive the pereption of the delivery experience, the higher the maternal-infant interaction score for all subjects(F=.3206, p=.006). The findings supported the hypothesis Ⅲ that there would be correlations between perceptions of delivery and maternal-infant interaction. The maternal infant interaction was highest when the perception of the bady and deliery was positive(r=.4363, p=.000, r=.2881, p=.012). No correlations between perceptions of labor and maternal-infant interaction were found(p=0.062). 4. The daily maternal-infant interaction score for the initial contact after birth to 8 weeks postpartum had the lowest average score 5.20 and the highest 7.98(in a range of 0-10). However, there were significantly different increases in maternal-infant interaction only from the first to second day(p=0.000) and from the fourth to sixth weeks after birth(P=0.000). 5. When the eight items of maternal-infant interaction were evaluated separately, “Expresses feelings about her role as mother” had the highest average score, 1.64(ina range of 0-3)and “Speaks to baby” the lowest, 0.9. All items, with the possible exception of “Expresses feelings about her role as mother”, suggested the subjects' need of nursing intervention to promote maternal-infant interaction. 6. There were positive correlations between certain general charateristis, namely, both a higher economic status(p=0.002) and breast feeding(p=0.202) and maternal - infant interaction. There were positive correlations between a mother's confidence in her role as a mother and the perception of the birth experience(p=0.004). For mothers who had a cesarean section, a positive perception of the birth experience was related to the duration of her marriage(p=0.010), a wanted pregnancy (P=0.030) and her confidence in her role as a mother(p=0.000). Pereptions of birth for mothers who had a normal vaginal delivery were positive than those for mothers who had a cesarean section. The level of maternalinfant interaction for mothers delivered vaginally was higher than for cesarean section mothers. The relationship between perception of birth and materanalinfant interaction was confirmed. Cesarean section has an impact on the mother's perceived experience of birth which, in turn, is positively related to maternal-infant in turn, is positively related to maternal-infant interaction. Nursing intervention to enhance maternal-infant interaction should begin in prenatal classes with an exploration of the potential impact of cesarean section on the perceptions of the birth experience and continue throughout the perinatal and post-natal periods to promote the mother's ability to control with this crisis experience and to mobilize social support. Nursing should help transform a relatively negatively perceived experience into an accepted, positively perceived and self affirming experience which enhances the maternal-infant relationship.

A Study on the Change of Primipara's Mother - Infant Interactional process (모아상호작용과정의 진행양상에 관한 연구)

The purpose of this longitudinal descriptive correlational study is to investigate the changes of the mother - infant interactions from postpartum 1 day to postpartum 8weeks of the transition to parenthood. The aim was to contribute to the development of theoretical under standing on which to base care toward promoting the quality of maternal - infant interaction. Data were collect ed directly by the investigator and at rained from Jul, 1, 1990 to Jun 8, 1991, Subjects were a random sample of 44 mothers, 44 who had a normal delivery (but with out other perinatal complications) at four general hospitals in Seoul. Instruments used were the Stainton Parent-infant Interaction Scale (1981). The first observations were made in the delivery room, followed by day 1, day 2, day 3 and 2 weeks, 4 weeks, 6 weeks and 8 weeks after, birth, for a total of 8 contacts. However, there were significantly different increases in maternal-infant interaction only from the first to second day (p=0.000) and from the fourth to sixth weeks ofter birth.(p=0.000) 3. When the seven items of maternal-infant interaction were evaluated items of maternal-infant inter action were evaluated separately, "Care taking for the baby" had the highest average score, 1.60(in a range of 0-2) and "Speaks to baby" the lowest, 0.8. All items, suggested the subjects' need of nursing intervention to promote maternal-infant interaction 4. There were positive correlations between certain general characteristics, namely, both a higher economic status (p=0.027) and breast feeding (p=0.021) and maternal-infant interaction.
