Suture-anchor를 이용한 관절경적 Bankart 술식에서의 Rowe 점수를 통한 예후 인자 분석 (The Prognostic Factor Analysis Through Rowe Scoring System in Arthroscopic Bankart Operation Used Suture-anchor Method)

  • 목적 : 재발성 전방 견관절 탈구 환자에서 비흡수성 봉합사를 사용하여 suture-anchor 방식을 이용한 관절경적 Bankart 술식의 임상적 결과와 병변의 예후에 영향을 미칠것으로 사료되는 여러 인자들을 술 전, 술 후 Rowe 점수를 통해 평가, 비교하였다. 대상 및 방법 : 1997년 6월부터 2001년 5월까지 외상으로 인한 재발성 견관절 탈구로 인해 관절경적 Bankart 병변 봉합술을 시행 후 최소 1년 6개월 이상 추시가 가능했던 29예를 대상으로 하였다. 28명 모두 남자였으며 수상 당시의 평균나이는 21.3세다. 술 전, 술 후 견관절의 기능평가는 Rowe점수를 사용하였으며 예후에 영향을 미칠 것으로 사료되는 9개의 인자를 설정한 후 술 전, 술 후 Rowe 점수를 이용하여 공변량 분석을 이용한 통계학적 검증을 실시하였다. 결과 : 술전 Rowe 점수는 평균 28.4점(보통 11예, 불량 17예)이었는데 반해 술 후 Rowe점수는 87.1점(우수 12예, 양호 15예, 보통 1예)이었고 통계학적으로 유의한 점수의 증가를 나타냈다(p=0.000). 추시상 나타난 합병증으로는 재탈구 1례, 경미한 견관절 운동범위 장애를 나타내는 2례가 있었다. 술 전, 술 후 Rowe점수를 통한 예후 인자 분석에 있어서는 수상 당시의 나이가 20세 미만인 군에서 20세 이상인 군보다 통계학적으로 유의하게 (p=0.023) 예후가 좋은 것으로 나타났다. 결론 : 관절경을 이용한 Bankart 술식은 재발성 견관절 탈구환자에 있어 유용한 치료방법중의 한가지로 사료된다. 또한 수상당시의 나이는 술 전, 술 후 Rowe점수를 비교한 결과 예후에 영향을 미치는 인자로 사료되었다.

만성 외상성 견관절 전방 불안정성의 치료에서 병행한 관절낭 열 수축술 (Additional Thermal Shrinkage in Treatment of Recurrent Traumatic Anterior Shoulder Instability)

  • Purpose: In the traumatic anterior shoulder instability, the laxity of joint capsule and ligament is frequently demonstrated. Although a arthroscopic procedure to address anterior instability with joint capsular redundancy have generally provided good results, its recurrence rate is higher than open procedure. By reducing the capsular redundancy, thermal shrinkage is likely to improve the outcome of arthroscopic anterior stabilization. The objective of this study was to evaluate additional thermal capsular shrinkage as a treatment of joint capsular redundancy in anterior shoulder instability. Materials and Methods: From March 1999 to June 2000, 25 shoulders of 23 patients of recurrent anterior shoulder dislocation underwent arthroscopic Bankart repair with shrinkage procedure. The mean follow up was 29 months and average age at the time of operation was 26 years. Of these patients, 20 were male and 3 were female who had been experienced the average 8 times of dislocation before operation. Thermal shrinkage alone without Bankart repair was performed in two cases who did not have Bankart lesion. The clinical result was evaluated in according to Modified Rowe Score. Results: The Modified Rowe Score was improved from preoperative 35 points to postoperative 88 points. None of cases showed recurrence of dislocation. But, in two cases, temporary sensory hypesthesia of the axillary nerve was developed and in two cases of postoperative stiffness, arthroscopic capsular release and brisement were performed. Conclusion: Additional capsular shrinkage in arthroscopic technique to address recurrent anterior shoulder instability could treat effectively the capsular redundancy.

흡수성 suture-anchor를 이용한 견관절 전방 불안정성의 재건술 (Arthroscopic Capsulolabral Repair Using Absorbable Suture-Anchor for the Traumatic Anterior Instability of Shoulder)

  • Purpose: The purpose of this study is to evaluate the short-term clinical outcome of the capsulolabral repair using absorbable suture anchor in traumatic anterior instability of shoulder. Materials and Methods: From June 2000 to September 2001, 15 shoulders with recurrent anterior instability were operated with arthroscopic Bankart repair using absorbable suture-anchor 'PANALOK' (Mitek, westwood, MA), and were followed up over 1 year (average; 13 months). The mean age was 23-years. There were fourteen males and one female. The mean duration from the initial symptoms to the operation was 24 months. Associated pathologies were Hill-Sachs defect in 12 cases, SLAP in 6 cases, and partial rotator cuff tear in 2 cases. The results were evaluated by patien's satisfaction, Modified Rowe Score in regard to joint stability, mobility, pain and function in comparison with the preoperative ones, and other complications. Results: At the last follow-up, the total Rowe Score increased from 38 points to 92 points. There were no recurrence and 14 patients among 15 patients gained pre-operative level of sports activity and no other complications. Conclusion: Effective capsulolabral repair could be obtained by the absorbable anchoring without any untoward complications. This procedure is simple and safe one and this system can be a good substitute for the metallic anchor.

설상형 종골 골절의 부정 유합 후에 발생한 Haglund씨 증후군 - 3례 보고 - (Late Sequelae of Secondary Haglund's Syndrome after Malunion of Tongue Type Calcaneus Fracture - Report of Three Cases -)

  • Haglund's syndrome produces retrocalcaneal bursitis or achilles tendinitis due to impingement of posterior superior bursal projection of calcaneus on insertional fibers of achilles tendon. Haglund's syndrome has been mainly associated with wearing rigid counter shoes and with athletes. We experienced three case of late sequelae of secondary Haglund's syndrome after malunion of tongue type calcaneus fracture. It is to be the first description of secondary Haglund's syndrome after calcaneus malunion and also the first report as the late complication of calcaneus fractures. Three cases were all tongue type intraarticular fractures and were treated with $45^{\circ}$ superior angle resection of superior calcaneal tuberosity. Clinical results by modified Rowe score were excellent with complete pain relief for all three cases.

종골 골절후 외상성 관절염에 대한 삼중 관절 고정술의 임상적 결과 (Clinical Outcome of Triple Arthrodesis for Posttraumatic Arthritis after Calcaneal Fractures)

  • Purpose: To analyze the clinical outcome of triple arthrodesis for the posttraumatic arthritis after calcaneal fractures. Materials and Methods: We retrospectively reviewed 22 posttraumatic arthritis patients who underwent a triple arthrodesis from March 1991 to May 1998. The mean duration of follow up was 74 months(range, 36-123 months). The pain, function and alignment were evaluated by the modified ankle-hindfoot scale of the American Orthopaedic Foot and Ankle Society(AOFAS) clinically and the union rate, the duration of union and the degenerative change of adjacent joints radiographically were evaluated. Results: The mean duration from injury to arthrodesis was 33 months (range, 12-132 months). The AOFAS score improved from 36.4 points(range, 14-64) preoperatively to 67.6(range, 25-90) postoperatively. The union rate was 90.1% radiographically, the mean duration of union was 14.3 weeks(range, 12-21 weeks) and the degenerative change in the adjacent joint was showed in 12 patients(54.5%). There were 2 cases of talonavicular nonunion, one superficial wound infection and one partial skin necrosis. Conclusion: Triple arthrodesis for posttraumatic arthritis after calcaneal fractures is a useful method for relief of pain and correction of posttraumatic hindfoot deformity, as an evidenced by the satisfactory clinical outcome. Although a high prevalence of subsequent arthritis of the ankle and midtarsal joint was noted radiographically, we found that it was not clinically relevant.
