주상골 골절의 수술적 치험례 (Treatment of Carpal Scaphoid Fracture)

  • 1985년 3월부터 1986년 6월까지 영남대학교 의과대학 정형외과학교실에서 10예의 주상골 골절환자에 나사를 사용하여 치료하였으며 그 결과를 요약하면 다음과 같다. 10예중 신선골절이 2예였고 불유합 골절이 8예이었다. 불유합 골절의 경우 수상후 수술시까지 기간이 최단 5개월에서 5년이었다. 수술후 고정기간은 평균 4주였으며 4주후 능동적 운동을 시행하여 고정기간의 단축으로 관절운동 회복은 매우 양호하였다. 골유합을 보인 사기는 8예에서 3개월에서 9개월 사이였으며 2예에서는 9개월 후에도 방사선상 골절선은 볼 수 있었다. 골절선이 남아있는 경우도 임상적으로는 증세의 호전(동통, 관절 운동)을 나타내었다.

  • The auther had studied dental caries and periodontal disease and other important oral disease in the 806nout patient who came to dental Department in N.M.C. during 1974. The obtained results were as follows ; 1) The dental caries experience rate in the out patient of Dental Departanent in N.M.C. was 8202% and the D.M.F. teeth index was 3.3. 2) The decayed teeth index was 2.18 and the missed teeth index was 0.69 and the filled teeth index was 1065. 3) The periodontal disease rate was 82.8% 4) The oral cancer rate was 2%, mandible and maxillary fracture rate was 4%, and trigeminal neuralgia rate was 0.6%.

Do Radiology Residents Perform Well in Preliminary Reporting of Emergency MRIs of Spine?

  • Purpose: To evaluate interpretation errors involving spine MRIs by residents in their second to fourth year of training, classified as minor, intermediate and major discrepancies, as well as the types of commonly discordant lesions with or without clinical significance. Materials and Methods: A staff radiologist evaluated both preliminary and final reports of 582 spine MRIs performed in the emergency room from March 2011 to February 2013, involving (1) the incidence of report discrepancy, classified as minor if there was sufficient description of the main MR findings without ancillary or incidental lesions not influencing the main diagnosis, treatment, or patients' clinical course; intermediate if the correct diagnosis was made with insufficient or inadequate explanation, potentially influencing treatment or clinical course; and major if the discrepancy affected the main diagnosis; and (2) the common causes of discrepancy. We analyzed the differences in the incidence of discrepancy with respect to the training years of residents, age and sex of patients. Results: Interpretation discrepancy occurred in 229 of the 582 cases (229/582, 39.3%), including 146 minor (146/582, 25.1%), 40 intermediate (40/582, 6.9%), and 43 major cases (43/582, 7.4%). The common causes of major discrepancy were: over-diagnosis of fracture (n = 10), missed cord lesion (n = 9), missed signal abnormalities associated with diffuse marrow (n = 5), and failure to provide differential diagnosis of focal abnormal marrow signal intensity (n = 5). No significant difference was found in the incidence of minor, intermediate, and major discrepancies according to the levels of residency, patients' age or sex. Conclusion: A 7.4% rate of major discrepancies was found in preliminary reporting of emergency MRIs of spine interpreted by radiology residents, probably related to a relative lack of clinical experience, indicating the need for additional training, especially involving spine trauma, spinal cord and bone marrow lesions.

경추 손상 환자에서 전산화 단층 촬영 시행을 위한 임상적 기준 : NEXUS 기준과 Canadian cervical spine rule (NEXUS and the Canadian Cervical Spine Rule as a Screening Tool for Computed Tomography Evaluation in Patients with Cervical Spine Injury)

  • Purpose: National Emergency X-Radiography Utilization Study (NEXUS) criteria and the Canadian Cervical Spine rule (CCR) are commonly used in cervical trauma patients to determine whether a plain cervical X-ray should be performed. However, plain cervical X-rays are so inaccurate that cervical spine computed tomography (CT) is often considered as a screening test. We studied the usefulness of the NEXUS criteria and the CCR for determining the need for a CT evaluation in the emergency department (ED). Methods: This prospective observational study was conducted from January 2007 to March 2008. Plain X-ray and CT scans of the cervical spine were performed on blunt trauma patients with neck pain. The relevancy of CT was examined using the NEXUS criteria and the CCR. Sensitivity, specificity, positive predicted value, and negative predicted value analyses were performed to diagnose the cervical spine injury. Results: During the study period, 284 patients were enrolled in this study. The sensitivity, specificity, positive predicted value, and negative predicted value of the NEXUS criteria were 87.5%, 1.1%, 5.0%, and 60.0% respectively, while those of the CCR were 87.5%, 8.2%, 5.3%, and 91.6%. There were two missed fracture cases when the NEXUS criteria and the CCR were applied independently, however, no cases were missed when both were applied. Conclusion: This study suggests the NEXUS and the CCR in combination can be used as a guide to CT evaluation for cervical spine injury in the ED.

종골 골절 후 거골하 관절면의 전위 정도와 운동 범위의 상관 관계 (Correlationship Between Degree of Displacement and Range of Motion of the Subtalar joint after Calcaneal fracture)

  • The characteristics of the patients after the calcaneal fracture that were associated with an unsatisfactory outcome were subtalar incongruity, decreased Bohler angle ratio of the fractured to the normal side, an age of more than fifty years, work involving strenuous labor, and increased time missed from work due to the injury. The purpose of this study was to examine the reliability of measurements of the range of motion of the subtalar joint. To determine reliability, evaluates of the correlatioinship between the degree of the displacement of the subtalar joint and Circle draw test after the calcaneal fracture. Fifty patients who had had fifty five calcaneal fractures were managed with open reduction and internal fixation. The results were reviewed retrospectively, between 4months and three years after the operation, with use of an evaluation system for the subtalar joint and with plain radiographs. At follow up evaluation, the result was assessed on the basis of restoration of anatomy and function of the subtalar joint. We evaluated the subtalar joint with plain films that consist of anteroposterior projection, lateral projection, calcaneal axial view, and Broden's view, and the measurements of the displacement of the subtalar joint surface after the calcaneal fracture. And we evaluated the range of motion of the subtalar joint with Circle draw test for physical evaluation. Circle draw test was evaluated and demonstrated the motion of flexion-supination-adduction and extension-pronation-abduction of the subtalar joint. And there are correlationship between the degree of the displacement and range of motion of the subtalar joint after the calcaneal fracture. The report critically reviews methords used to measure Circle draw test for physical examination of the follow up after the calcaneal fracture.

족근 관절 경비 인대 결합 손상 (Ankle Syndesmotic Injury)

  • Ankle injuries may involve the distal tibiofibular syndesmosis and can be associated with a variable degree of trauma to the soft tissue and osseous structures that play an important role in ankle joint stability. Ankle syndesmotic injury may occur solely as a soft tissue injury or in association with variable ankle fractures. Ankle syndesmotic injury does not necessarily lead to ankle instability; however, the coexistence of deltoid ligament injury critically destabilizes the ankle joint. The prevalence of these injuries may be higher than previously reported. The diagnosis of syndesmotic injury as not always easy because isolated ankle sprains may be missed in the absence of a frank diastasis and syndesmotic instability may be unnoticed in the presence of bimalleolar ankle fractures. Controversies arise at almost every phase of treatment includings : type of fixation(screw size, type of implant), number of cortices required for fixation and of need for hardware removal. Regardless of controversies, the most important goal should be restore and maintain the normal tibiofibular relationship to allow for healing of the ligamentous structures of the syndesmosis.

강원도 일부 초등학생의 자가구강건강인식과 구강건강상태의 관련성 (Association between Perceived Oral Health and Oral Health Status of Elementary School Students in Gangwon Province, South Korea)

  • Objectives: The purpose of this study was to clarify the relationship between perceived oral health and oral health status, oral health symptoms in elementary school students. Methods: Oral examination and oral hygiene survey were performed on 446 students in 5th grade from 4 elementary schools in Wonju, Gangwon province. Oral examination assessed decayed teeth, filled teeth, missing teeth from decay. Then, oral hygiene status was evaluated by O'Leary index and self-reported questionnaires were surveyed. We analysed the collected data using SPSS program ver. 20.0, significant difference level was p<0.05. Results: 38.4% of the subjects rated their oral health as poor, 61.6% as good, female students was worried about their appearances than male students. Their decayed teeth was 1.01, missed teeth was 0.01, filled teeth was 1.75, O'Leary index was 64.49, oral hygiene status of them was poor. Perceived oral health was associated to decayed teeth, DMFT, oral health symptoms except tooth fracture and pain of mucosa (p<0.05). However, there is no correlation between perceived oral health and oral hygiene status (p>0.05) Conclusion: Self-rated oral health status of the elementary school students was related to decayed teeth and oral health symptoms, but it was not related to oral hygiene status. Therefore it was needed to improve the oral hygiene status in elementary students by devise effective way to motivate them.
