Functional Status and Long-Term Care Services for the Community-Dwelling Low-Income Elderly (저소득층 재가노인의 기능상태와 요구되는 요양서비스 유형 분석)

  • Purpose: This study was conducted to explore the functional status and long-term care services for the community-dwelling low-income elderly. Method: A descriptive research design was used in this study. The functional status of the participants was obtained using Minimum Data Set-Home Care Version 2.0 and the long-term care services were identified via Michigan's choice. Total of 154 persons aged 65 years or older completed Korean Minimum Data Set-Home Care Version 2.0 on the community dwelling low-income elderly. Results: The average of Activities of Daily Living was 4.19, and the range was 0-55, while the average of Instrument of Activities of Daily Living was 4.85 and the range was 0-56. Among the subjects, 46.1% belonged to the Information and Referral group and 1.3% to the Nursing Home group. Severe daily pain was reported by 14.9%, and 76.6% of the participants had impaired vision. The Activities of Daily Living was difference according to living with, education, vision, and depression. The long-term care services differed according to gender, pain, vision, hearing, and depression. Conclusion: The support policy for the elderly needed to focus on impaired visual and depression to enhance the activities of daily living. Moreover, there is a need for the Information and Referral group to arrange and develop nursing intervention resources.

A Pilot Evaluation Study for the Establishment of CPTED Criteria of Flat or Multiple Dwelling Houses (범죄로부터 안전한 다세대·다가구주택 계획기준 마련을 위한 시범평가 연구)

  • Flat or Multiple Dwelling Houses are relatively vulnerable to crime safety. Crime prevention measures are urgently needed because crime is 2.6 times higher than in real apartments. Through the analysis of domestic and foreign crime prevention design standards, field survey, and interviews with experts, 27 items of crime prevention design criteria for flat or multiple dwelling house were derived, and ten design criteria that should be considered first by the expert AHP were derived. As a result of the pilot evaluation of existing flat or multiple dwelling house, the houses completed after 2015 have relatively high level of crime prevention, but the houses constructed before that are very vulnerable. The policy and system improvement plan based on the analysis result is as follows. First, new housing should be promoted to meet minimum criteria by supplying and educating public officials, architects, and building owners in the short term to provide criteria for flat or multiple dwelling house crime prevention plans. Second, existing housing should be supported with basic crime prevention support projects such as security windows for flat or multiple dwelling house where security of residential environment such as urban renewal policy is poor. Third, the Enforcement Decree of the Building Act shall be revised to make the crime prevention environment design of flat or multiple dwelling house obligatory, and the criteria of flat or multiple dwelling house crime prevention plan should be reflected in the notice of crime prevention building standard.

Management Strategy by Evaluation on Comprehensive Health Problem in the Community-dwelling Elderly of Korea (재가노인의 포괄적 건강문제 평가와 관리방안)

  • Purpose: The aim of this study was to define the health problem in the community-dwelling elderly of Korea and to compare differences of CAPs(Client Assessment Protocols) by characteristics. Method: Data was collected by visiting nurse from 556 elderly over 65 years in selected metropolitan areas. To evaluate the functional state of elderly in the community, I used "RAI-MDS HC; Residental Assessment Instrument Minimum Data Set-Home Care(2.0 version)" and established information exchange system among resources, by developing the data into a computer program. Results: The health problem of 'preventive health measures' was the largest(99.6%), and then 'health promotion(85.3%)', 'visual function(75.5%)', 'psychologic drug(68.9%)', 'pain(68.5%)', ‘social function(59.2%)', 'communication disorders(56.2%)', 'environmental assessment(53.2%)', 'depression & anxiety(46.9%)', 'oral health(43.4%)' followed. The number of health problems was average 10.16 in the community-dwelling elderly. Conclusion: The results suggest the need to emphasize the importance of assessment of the health problem of the elderly. We can apply it in the distribution of community resources and the development of service providing programs by figure out the health problem and resource in need for the elderly in the community.

A Study on the Evacuation Character and Regulation for High-rise Apartment Buildings in Korea (고층 공동주택의 피난특성 및 관련 법규정에 관한 연구)

  • Apartment buildings have teen increasing it's height since first appearance in the late 1970 in Korea. Likewise, fire risk and the fire damage of this new dwelling type have become a serious problems. However the applying fire safety design method and regulations are minimum level of requirements based on the current fire codes. The purpose of this study is to figure out the evacuation characteristics in high-rise apartment buildings and improve countermeasure by comparison egress codes between Korea, Japan and U. S. A. The result shows that means of egress of this dwelling should be reviewed by the fire code as well as expected evacuation safety performance by a fire protection engineer to minimize fire 1osses including life safety.

A Study on the Relationship between Dwelling Environment and Household Density in Multi-Family Housing Development (공동주택단지 건설에 있어서 주거환경과 가구밀도에 관한 연구 -택지개발사업지구를 중심으로-)

  • To accomodate increasing housing shortage in many cities, Korean Government has implemented various residental land development projects to provide more land for new housing construction. Most of the residential areas developed by these projects were used for multi-family housing units. Under this process, the size and location of the neighborhood service facilities in the development area were regulated by uniform standards. For this reason, the neighborhood service facilities were not able to be provided according to the household density but provided in minimum status required by these standards. Based on these considerations, this study aimed to evaluated residential land development project conducted by pubilc authority, one of the residential land development projects in Korea. Results of the case studies confirmed findings from literature review and proposed that existing regulations applied to these projects need to be amended not only to accomodata unique situation of each development area but also to enhance level of the dwelling environment. As ways to resolve these problems, this study suggests followings: (1) more flexible operation of the regulations, (2) inclusion of the socio-economic characteristics of the perspective resident in planning process, and (3) introduction of design techniques in the project. Finally, this study proposed future study directions in relation to this issue.

  • Korean living culture raised the users' needs for sub-kitchen. In the traditional Korean house, there was large space related to kitchen area for preparing food and stock big and bulky housing stuffs. As apartment housing became dominant as Korean dwelling, sub-kitchen has been planed in the balcony that is not included in the sales area. In this study, the case of the apartment housing in Esiapolis, Daegu was analyzed. To plan the user-oriented sub-kitchen, the consumer research was carried out. Consumers needed a pantry, more storage near the kitchen, and wanted to place washing machine and washing stand in a sub-kitchen. Sub-kitchens were planed to meet those consumers' needs. Through this case study and former studies analysis, sub-kitchen's function unit was derived: wash, storage, auxiliary work. By combining each function unit, sub-kitchen was classified into 3 types, wash & auxiliary work, wash & storage, and wash & storage & auxiliary work. For each sub-kitchen type, components of function units, available layouts, and minimum size were recommended.

A Survey on Community dwelling Mentally Ill Patients Who could not be Accessed by a Community Health Center (보건소에서 접근할 수 없는 재가 정신질환자에 대한 조사연구: B시 일 보건소를 중심으로)

  • Purpose: The purpose of this study was to identify the frequency and status of mentally ill patients who registered in a community health center (CHC) but they could not be accessed. Methods: A total of 233 mentally ill individuals who were registered to a CHC in B Metropolitan City participated in this survey. Data were collected from December 1 to 31, 2010 and analyzed. Results: There were 140 (60.1%) community dwelling mentally ill patients who had been registered to that CHC but were not accessible. The major reasons of this inaccessibility were patients' rejection to be accessed (27.1%) and the wrong or missing address in hospital's discharge note (22.9%). The nursing problems of the subjects were rejection of medication (93.4%); poor personal hygiene (85.8%); violence including harassment of family member(s), assault, property destruction, threat with weapon (38.7%); and so on. Among 159 cases of referrals and requests for help immediate action with patient's compliance were only 8.8%. Conclusion: The findings show that mentally ill patients who had registered to the CHC but were not able to be accessed failed to be managed properly. This would become risk factors of social problems which threat to the community as well as individuals. Thus, we need to prepare programs for this population to make them take medicine at a minimum.

Analysis of Functional Status and Care Needs among the Community-Dwelling Disabled Persons with Cerebral Impairment (재가 중증 뇌병변 장애인의 기능상태 및 케어요구 목록 평가)

  • Purpose: Using comprehensive and valid instrument, MDS-HC 2.0, this study aimed to analyze the functional status and to evaluate the care needs of the community-dwelling disabled with cerebral impairment. Method: With a convenient sample of 88 disabled with cerebral impairment, the data were collected at a community health center located in rural area in Choongchung providence in August 2005. Subject's functional status and care needs were evaluated using Minimum Data Set-Home Care version 2.0. Result: Significant proportion of subjects were totally dependent for locomotion-outdoor (26.1%), personal hygiene (24.1%), bathing (24.1%). For IADLs, over 40% of subjects were totally dependent for ordinary house work, managing finances, or shopping. Top five ranked care needs were preventive health care measures (100%), communication disorders (71.6%), visual function (55.7%), health promotion (52.3%), and pressure ulcers (48.9%). The proportion of triggered clinical assessment protocols were significantly higher in disability level I group for the risk of institutionalization (p=<.001), communication disorders (p=.004), cognitive problems (p=.001), pressure ulcers (p=<.001), skin and foot conditions (p=.010), and urinary incontinence and indwelling catheters (p=<.001). Conclusions: It is necessary to provide community based rehabilitation services that are individualized for their service needs thus enhance optimal level of functioning.

A Study on the Effects of Stress in the Health Condition of Blue-Collared Workers (스트레스가 일 산업장 근로자의 건강상태에 미치는 영향에 관한 연구)

  • This study was attempted to evaluate and explain the correlation between stress and utilization of Dispensary, Frequency of absence, Frequency of accident as the health condition of bluecollared workers. The subjects for this study were 260 workers selected conveniently from 360 workers working at one chemical manufacturer located in Seoul. The dataas measure the Stress was collected through SRRS (The Social Readjustment Rating Scale) which is developed by Holmes & Rahe (1976) from 27th July to 10th August, 1983. The data of Utilization of Dispensary and Frequency of Accident were brought out from the record of Dispensary during past 8 months. The Frequency of Abscence from January to August were collected through diligence and indolence cards of each department. All collected data were analysed based on the SPSS (Statistical Package for the Social Science) program. The results of the study are as follows: 1. The average of Utilization of Dispensary was 7.4, Frequency of abscence 14.6, frequency of Accident 0.4. 2. The average score of Stress was 134 (minimum : 0, maximum : 593, Standard deviation : 109. 3) It can be considered as a slight stressed group by the requlations of Holmes and Rahe. 3. According to the analysed results of correlation between Stress and the Utilization of Dispensary, Frequency of Absence, frequency of Accident, the Utilization of dispensary and stress were significantly related (r= .1955, p<.001), the Frequency of Absence (r=.0284, p >, 05), and the Frequency of Absence (r=.0284, p>.05), and the Frequency of Accident (r= .0831, p>.05) were not significantly related with Stress. 4. In the relation between general characters of subjects and the Utilization of Dispensary, 20 to 25 aged group, women, single, christian, lent-house dwelling, 1 to 5 years employed group used at a high ratio. 5. In the relation between general character of subjects and the Frequency of Absence, men, 51 to 55 aged group, 21 to 25 years employed group indicated high Frequency, and other char-noters were not significantly related. 6. In the relation between general characters of subjects and the Frequency of Accident, women. single, 20 to 25 aged group, catholic, lent-house dwelling, below 1 year employed group showed high frequency. 7. Stress showed a significant difference only with a religion. Catholic group represented mode-rate stressed scored 209.67, and not significantly related with other general characters.

OSL Age Determination of the Hearths in a Bronze Age Dwelling Site by using Bayesian Statistics (베이지안 통계학을 이용한 청동기시대 주거지내 화덕자리들의 광자극발광(OSL) 연대 결정)

  • OSL dating for three hearths having the sequence of use and discard in No. 29 and 29-1 dwelling sites at Sogol cultural site was carried out. Resulting from the deconvolution of natural CW-OSL decay curve and thermal zeroing test, it was turned out that OSL signal was entirely composed of the heat- and light-sensitive fast component with high photoionization cross-section and all quartz OSL signals were thermally bleached under $300^{\circ}C$ which is the minimum temperature related to heating and cooking in Bronze age. After dose recovery test and plateau test, paleodose of each hearth sample was evaluated by using SAR method, and OSL age was determined from the ratio of paleodose to annual dose rate. For the purpose of the precision improvement of OSL age, Bayesian statistics was applied to each hearth's age and the archaeological sequence information. Finally, it could be concluded to the accurate use period of each hearth from the resultant OSL ages.
