Pulmonary Embolism In Childhood Minimal Change Nephrotic Syndrome (소아 미세변화 신증후군에서 폐색전증에 대한 연구)

  • Purpose : We investigated the incidence and predisposing factors of pulmonary embolism in minimal change nephrotic syndrome(MCNS). Methods : Lung perfusion scan using 99mTC-MAA were done on 14 patients who were diagnosed to minimal change nephrotic syndrome. Group h: Five patients who had perfusion defects on scan, Group B; Nine patients who had no perfusion defect on scan. Between the two groups, the differences of platelet number, hematocrits, albumin, cholesterol, triglyceride, proteinuria were evaluated. Results : Five patients were found to have perfusion defect consistent with pulmonary embolism($35.7\%$). However, there were minimal or no respiratory symptoms and signs. In our laboratory studies, the mean proteinuria on admissions was $676{\pm}31\;mg/m2/hr$ in the group with pulmonary embolism, and $313{\pm}28\;mg/m2/hr$ in the group without pulmonary embolism. There were more severe proteinuria in group with pulmonary embolism(P<0.05). The mean platelet count at early stage of remission after steroid treatment was $746,600{\pm}280,000/mm3$ in the group with pulmonary embolism, $511,890{\pm}90,000/mm3$ in the group without pulmonary embolism. There were significant difference of platelet count between the two groups(P<0.01). In patients with pulmonary embolism, there were more higher and sustained increasement of platelet count. All cases of pulmonary embolism were treated with dipyridamole(5 mg/kg). In 4 cases the perfusion defects were improved in two weeks, however, one case showed persistent perfusion defect after 1 month. Conclusion : Our study suggested that pulmonry embolism might be one of tile major complications in childhood MCNS The occurrence rate was correlated with severity of proteinuria before treatment and sustained increasement of platelet counts in early remission state after steroid treatment. Therefore, the scintigraphic pulmonary perfusion study is mandatory in childhood MCNS, especially in the high risk patients, such as the patients with severe proteinuria and sustained increasement of platelet count. Water Transport during the Single-pass Perfusion Experiments : Effects of Some Perfusates and Their Osmolality (일회통과 관류실험시 물의 수송 : 관류액의 종류와 삼투압의 영향)

  • The single-pass perfusion experiments were performed in anesthetized rats to investigate the effects of perfusates and their osmolality on the water transport and to determine the correlation between the extent of water transport and the volume change of perfusate. Phenol red was used as a nonabsorbable marker. In normal rats, when perfused at a flow rate of 0.5 ml/min, 2-(N-rnorpholino) ethanesulfonic acid (MES) and S$\phi$rensen's phosphate buffers showed minimal net water transport as 0.125 and 0.173 %/cm of intestinal length, respectively. Hypotonic perfusate of 200 mOsm/kg of water and hypertonic perfusate of 400 mOsm/kg of water generated significant water transport compared with isotonic perfusate of 300 mOsm/kg of water. There was a linear correlation between the extent of water transport and the volume change of perfusate, suggesting that the volume change can be used as a measure of water transport.

  • The dwelling transformations of Xiao-He-Long town have been begun with China Economy Policy in 1904. The characteristics of change progress are as follows ; 1. As with pine tree living for a thousand years and increasing profits nearby Yian-Ji-Shi, Xiao-He-Long town as rural district has changed in appearance. 2. In accordance with improvement of rural life after China Reformation and Open Economy policy, residents have a tendency to secure a minimal house site area $400{\sim}500m^2$, and a spacious house which is able to consist barns for farming. 3. Dwelling plan type If Korean-Chinese consists of 'whetongjib, 6 module of Korean traditional unit. 'Whetongjib' has undergone with a corridor attached in porch, main entrance from south to north. 4. Construction materials as structural members bring about a change as result of influencing economic profits nearby Yian-Ji-Shi.

Increased Frequency of Apolipoprotein E4 Genotype in Childhood Minimal Change Nephrotic Syndrome (MCNS) (소아 미세변화 신증후군 환자에서 Apolipoprotein E4 유전자형에 관한 연구)

  • Purpose: We studied to find out apo-E genotype polymorphism in minimal change nephrotic syndrome(MCNS) and IgA nephropathy(IgAN) and to determine the relationship between apo-E genotype and clinical course of MCNS. Materials and Method: 43 MCNS patients and 15 IgAN patients were examined for apo-E polymorphism. 50 healthy blood donors were examined for apo-E genotype as control. Genomic DNA was prepared front peripheral blood leukocytes according to standard procedures. Results: As compared with control group, e4 allele frequency was significantly increased in MCNS (p<0.01). However, in IgAN e2 allele frequency, however, was 2.6 times higher than normal control (P<0.01). The frequency of e4 allele of frequent relapser group was 4.6 times higher than normal control and was 2 times higher than infrequent relapser group. Conclusion: We think that apo-E typing might be one of the parameters, which should be considered to predict the course of MCNS in children. MCNS with risky HLA profile and E4/4 genotype could indicate the need for a longer steroid administration. And apo-E genotype needs to be considered for the evaluation of therapeutic responses to other drugs. (J, Korean Soc Pediatr Nephrol 2001 ; 5 : 87-99)

  • A scene change detection is an important step for video indexing and retrieval. This paper proposes an algorithm by a phased algorithm for fast and accurate detection of abrupt scene changes in an MPEG compressed domain with minimal decoding requirements and computational effort. The proposed method compares two successive I-frames for locating a scene change occurring within the GOP and uses macroblock-coded type information contained in B-frames to detect the exact frame where the scene change occurred. The algorithm has the advantage of speed, simplicity and accuracy. In addition, it requires less amount of storage. The experiment results demonstrate that the proposed algorithm has better detection performance, such as precision and recall rate, than the existing method using all DC images.

A Study on Decrease in Degree of Deacetylation of Low Molecular Weight Chitosan Prepared by H2O2 and Change of Whiteness upon Time (H2O2로 분자량이 조절된 저분자화 키토산의 탈아세틸화도와 백도변화에 관한 연구)

  • Chitosan should be satisfied a certain Mw range for the final application, Low molecular weight chitosan(LMWC) can be acquired by depolymerizing high molecular weight starting chitosan. Using $H_2O_2$ in depolymerizing chitosan is very effective and reproducible in controlling Mw of resulting LMWC. However, $H_2O_2$ can break glycoside linkage of chitosan and oxidize some $-NH_2$ groups in chitosan. It has been reported that decrease in DA(degree of deacetylation) of LMWC prepared by $H_2O_2$. However, any quantitative data of decrease in DA has not been reported yet. In this study, DA of initial chitosan and DA of $H_2O_2$ treated chitosan were measured and the change in DA of chitosan upon $H_2O_2$ treatment were investigated. Change in DA was very different upon $H_2O_2$ treatment condition. LMWC also showed DA change upon time passage. Pre-swelling treatment of initial chitosan and low ratio of $H_2O_2$/chitosan prevented a decrease in DA significantly. Yellowing of LMWC was detected upon time passage, however, decrease in DA was minimal (around 1%).

  • Effects of chitin and chitosan treatments on soil microorganisms and the mesofauna were investigated in a microcosm and a fumigated field experiment. Responses of microorganisms were determined using microbial phospholipid fatty acid (PLFA) analysis, whereas responses of the mesofauna were measured in terms of the abundances of nematodes and microarthropods. Soil nitrate concentration increased on the application of chitin. Overall, chitin promoted bacterial and fungal abundance, leading to an increase in abundance of free-living soil nematodes that feed on decomposers. The ratio of saturated to unsaturated fatty acids was highest in the chitin-treated soil. Chitosan had a minimal effect on the abundance of microorganisms; however, it reduced the abundance of collembolans in the microcosm experiment. These results indicate that the application of chitin has beneficial effects on the supply of nutrients and promotion of the abundance of soil organisms.

Minimal Medial-row Tie with Suture-bridge Technique for Medium to Large Rotator Cuff Tears

  • Background: The purpose of this study was to evaluate the postoperative magnetic resonance imaging (MRI) results of minimal-tying (one medial-row tie among 4 medial-row sutures) on the medial-row in double-row suture-bridge configuration ($2{\times}2$ anchor with $4{\times}4$ suture stands). Methods: From 2011 March to 2012 July, 79 patients underwent arthroscopic rotator cuff repair using $2{\times}2$ anchor double-row configuration. The mean age was 61.3 years (range, 31-81 years). Two double-loaded suture anchors were used for medial-row. Four medial-row stitches were made with only one medial-row knot-tying (the most anterior suture). Lateral-row was secured using the conventional suture-bridge anchor technique; all 4 strands were used for each anchor. Repair integrity was evaluated with MRI at mean 6.2 months postoperatively. Retear and the pattern of retear, change of fatty infiltration, and muscle atrophy of supraspinatus were evaluated using pre- and postoperative MRI. Results: Repaired tendon integrity was 38 for type I, 30 for type II, 6 for type III, 4 for type IV, and 1 for type V, according to Sugaya classification. Considering type IV/V as retear, the rate was 6.3% (5 out of 79 patients). Medial cuff failure was observed in 4 patients. Fatty atrophy of supraspinatus was significantly improved postoperatively according to Goutallier grading (p=0.01). The level of muscle atrophy of supraspinatus was not changed significantly after surgery. Conclusions: Minimal tying technique with suture configuration of four-by-four strand double-row suture-bridge yielded a lower retear rate (6.3%) in medium to large rotator cuff tears.

Progressive Reconstruction of 3D Objects from a Single Freehand Line Drawing (Free-Hand 선화로부터 점진적 3차원 물체 복원)

  • This paper presents a progressive algorithm that not only can narrow down the search domain in the course of face identification but also can fast reconstruct various 3D objects from a sketch drawing. The sketch drawing, edge-vertex graph without hidden line removal, which serves as input for reconstruction process, is obtained from an inaccurate freehand sketch of a 3D wireframe object. The algorithm is executed in two stages. In the face identification stage, we generate and classify potential faces into implausible, basis, and minimal faces by using geometrical and topological constraints to reduce search space. The proposed algorithm searches the space of minimal faces only to identify actual faces of an object fast. In the object reconstruction stage, we progressively calculate a 3D structure by optimizing the coordinates of vertices of an object according to the sketch order of faces. The progressive method reconstructs the most plausible 3D object quickly by applying 3D constraints that are derived from the relationship between the object and the sketch drawing in the optimization process. Furthermore, it allows the designer to change viewpoint during sketching. The progressive reconstruction algorithm is discussed, and examples from a working implementation are given.
