  • 소절개 봉합술은 관혈적 봉합술에 비해 동반된 관절와 상완 관절(glenohumeral joint) 내의 병변을 확인하고 이에 대한 처치를 시행할 수 있으며, 견봉하 감압술과 함께 동반된 견봉 쇄골 관절의 병변을 치료할 수 있다. 또한 삼각근 기시부에 대한 손상을 최소화하기 때문에 빠른 재활과 조기 퇴원을 시행할 수 있는 등의 장점이 있다 관절경적 봉합술에 비해서는 수술 술기가 어렵지 않기 때문에 항상 좋은 결과를 기대할 수 있다 회전근 개 파열에 대한 수술을 시행할 때 관절경적 봉합술이 모든 경우에서 관혈적 또는 소절개 봉합술에 비해 좋은 결과를 나타내는 것은 아니다. 환자가 심한 골다공증이 있는 경우, 기술적 문제가 있는 경우, 봉합 후 회전근 개에 지나친 긴장이 염려될 때는 항상 소절개 봉합술로의 전환을 시도하여야 한다. 소절개 봉합술을 시행한 경우에도 관절경적 봉합술시와 비슷한 좋은 결과를 얻을 수 있고, 치료의 결과는 수술의 방법보다는 정확한 술기 및 수술 전 환자의 상태에 좌우된다고 하겠다.

Direct transparotid approach via a modified mini-preauricular incision for open reduction and internal fixation of subcondylar fractures

  • A transparotid approach, with a retromandibular or preauricular incision, is an alternative surgical approach for treating a subcondylar fracture and reducing the potential for complications such as injury to the facial nerves. However, retromandibular and preauricular incisions are both created far away from the parotid gland-dissection area. Thus, it is necessary to undermine the skin and retract it anteriorly to access the surgical field. Here, we introduce a modified approach wherein the incision allows for direct access to the fracture site. This approach may be adopted to shorten the incision length, reduce the retraction trauma at the surgical site, and help prevent injury to the facial nerve.

OMICS approaches in cardiovascular diseases: a mini review

  • Ranked in the topmost position among the deadliest diseases in the world, cardiovascular diseases (CVDs) are a global burden with alterations in heart and blood vessels. Early diagnostics and prognostics could be the best possible solution in CVD management. OMICS (genomics, proteomics, transcriptomics, and metabolomics) approaches could be able to tackle the challenges against CVDs. Genome-wide association studies along with next-generation sequencing with various computational biology tools could lead a new sight in early detection and possible therapeutics of CVDs. Human cardiac proteins are also characterized by mass spectrophotometry which could open the scope of proteomics approaches in CVD. Besides this, regulation of gene expression by transcriptomics approaches exhibits a new insight while metabolomics is the endpoint on the downstream of multi-omics approaches to confront CVDs from the early onset. Although a lot of challenges needed to overcome in CVD management, OMICS approaches are certainly a new prospect.

  • Lithium-ion battery development is one of the most active contemporary research areas, gaining more attention in recent times, following the increasing importance of energy storage technology. The galvanostatic intermittent titration technique (GITT) has become a crucial method among various electrochemical analyses for battery research. During one titration step in GITT, which consists of a constant current pulse followed by a relaxation period, transient and steady-state voltage changes were measured. It draws both thermodynamic and kinetic parameters. The diffusion coefficients of the lithium ion, open-circuit voltages, and overpotentials at various states of charge can be deduced by a series of titration steps. This mini-review details the theoretical and practical aspects of GITT analysis, from the measurement method to the derivation of the diffusivity equation for research cases according to the specific experimental purpose. This will shed light on a better understanding of electrochemical reactions and provide insight into the methods for improving lithium-ion battery performance.

  • There have been many solar tracking systems developed for the high accuracy in solar tracking. One of the key components of any motion control system is software. LabVIEW offers an ideal combination of flexibility, ease-of-use and well-integration with other I/O pieces for developing solar tracking system. Real-time solar positions which vary with GPS's data are used simultaneously to control the solar tracker by a chain of operating modes between the open and closed loops. This paper introduces a step by step procedure for the fabrication and performance assessment of a precision solar tracking system. The system developed in this study consists of motion controllers, motor drives, step-motors, feedback devices and application. CRD sensors are applied for the solar tracking system which play a primary role in poor conditions for tracking due to a gear backlash and a strong wind. Mini-dish was used as a concentrator for collecting sun light. The solar position data, in terms of azimuth and elevation, sunrise and sunset times was compared with those of KASI(Korea Astronomy & Space Science Institute). The results presented in this paper demonstrate the accuracy of the present system in solar tracking and utilization.

Analysis of Heat Transfer Performance for Mini-Channel Tube Bundles in Cross flow using CFD (전산유체역학을 이용한 직교류 미세관 관군의 전열 성능 해석)

  • Heat transfer performance of tube bundles have long been investigated since they were widely used. Most of previous experimental and numerical works for tube bundles were performed with tube diameter in the range of 25~51mm and Reynolds number of $8.000{\leq}Re{\leq}30.000$. Recently, tube bundles with small diameter tube collects interests since the mini-channel tube provides higher compactness. The present work aims to investigate the applicability of previous correlations available in the open literature to the tube bundles with small diameter of 1.5mm and $3.000{\leq}Re{\leq}7.000$. A commercial CFD package was used to analyze the thermal-hydraulic performance of them. The results show that the Zukauskas correlation developed for larger diameter tube and higher Reynolds number are still in good agreement with them within the discrepancy of 4.7%. The analyses also show that the Nuselt number increases with a decrease in the longitudinal pitch.

Mini-open Treatment Using Plate of Clavicle Mid-shaft Fractures

  • Background: Increased frequency of comminuted clavicle mid-shaft fractures and importance of functional satisfaction through early joint exercise has resulted in higher emphasis on surgical treatments. This study aimed to evaluate the clinical radiological results of treatment of clavicle mid-shaft fractures by open reduction and internal fixation using a plate with a small incision. Methods: The subjects of this study were 80 clavicle mid-shaft fracture cases treated with internal fixation using a plate from October 2010 to July 2014. Clavicle mid-shaft fractures were internally fixated using anatomical plates or locking compression plates. Achievement of bone union, union period, and clavicle length shortening were evaluated radiologically, and clinical assessment was done by using Constant and University of California at Los Angeles (UCLA) scores. Results: All 80 cases were confirmed to have achieved bone union through radiographs with an average union period of 10.9 weeks (range: 7-18 weeks). The average clavicle length of shortening in the affected side was 1.8 mm (range: 0-17 mm). The average UCLA score and Constant score were 33.6 (range: 25-35) and 92.5 (range: 65-100), respectively. Regarding complications, four cases reported skin irritation by metal plates, and one case reported a screw insertion site fracture due to minor trauma history. Conclusions: We were able to induce successful bone union and obtain clinically satisfactory results in displaced mid-shaft fractures of the clavicle without major complications such as nonunion through treatment of internal fixation using a plate.

Genetic variation of sensitivity to photoperiod and accumulated temperature in soybean mini core collection lines

  • The sensitivities to photoperiod and temperature give guidance to choose an adaptable genotype for specific area in soybean production. However, there is insufficient information about the variation of sensitivities to photoperiod and temperature with wide genetic background. We investigated the sensitivities to photoperiod and temperature using 82 soybean mini core collection lines provided by NIAS gene bank of Japan. The seeds were sown on 28 May and 4 August in 2015, 24 May and 5 August in 2016 at field in Saga, Japan ($33^{\circ}$ 14' 32'' N, $130^{\circ}$ 17' 28'' E) for the early (average photoperiod and temperature: 15.2 h and $25.1^{\circ}C$) and late (13.6 h and $27.2^{\circ}C$) sowing respectively. The plants were also grown in the growth chamber under 12 h photoperiod with three temperature regimes (day/night temperature: $25/18^{\circ}C$, $28/22^{\circ}C$ and $33/28^{\circ}C$). Emergence date, days to first flower were recorded with 10 plants in the field and 2 plants in the growth chamber for each line. The data for daily average temperatures and photoperiodic hours were collected from weather station. The days from emergence to first flower open (DEF) were varied from 23-92 (2015 and 2016) in early sowing whereas 18-68 (2015) and 18-59 (2016) in late sowing. The shortened DEF in late sowing could be caused by both short photoperiod and high temperature in late sowing. However, the accumulated temperatures during emergence to first flower open (ATEF) were less variable in comparison with DEF, suggesting the ATEF is dependent mostly on the photoperiod. The ATEF were found same between early and late sowing in some early flowering lines (e.g. $686.7^{\circ}C$ and $687.6^{\circ}C$ in HEUKDAELIPS, $728.8^{\circ}C$ and $706.3^{\circ}C$ in WILLIAMS'82) which indicated that these would be insensitive to day length. In the growth chamber experiment, the variation in both DEF and ATEF was a little greater at low temperature ($25/18^{\circ}C$) but almost same at middle ($28/22^{\circ}C$) and high ($33/28^{\circ}C$) temperatures. Since the less differences in ATEF were found between the three temperatures, it is suggested that the temperature plays only a quantitative effect on the flower initiation, and the large ATEF in some lines may indicate the stronger photosensitivity even at 12 h or longer juvenile phase. Some lines with the lowest ATEF regardless of growth conditions, such as FISKEBY V, KE 32 (ATEF: 559.6-666.5, 587.7-709.5) might lack the sensitivities to both photoperiod and temperature. The results suggested that soybean genotypes has wider variation in sensitivity to photoperiod, whereas less variation to temperature.

The Operative Treatment using Mini-open Sinus Tarsi Approach for Displaced Intraarticular Calcaneal Fractures (전위성 관절내 종골 골절에서 최소 침습적 족근동 도달법 및 압박나사 내고정술을 이용한 수술적 치료)

  • Purpose: This study was performed to evaluate the clinical outcomes of operative treatment using mini-open sinus tarsi approach for displaced intraarticular calcaneal fractures. Materials and Methods: We studied 18 cases (16 patients) of intraarticular calcaneal fractures who were treated with sinus tarsi approach by same surgeon. The mean age of patients was 44.8 years, and mean follow-up period was 17.2 months. The measurement of B$\ddot{o}$hler angle, Gissane angle, the degree of articular surface depression, and the period to union were performed through preoperative and postoperative radiographs. The clinical evaluation was performed according to hindfoot score of the American Orthopaedic Foot and Ankle Society (AOFAS) and scale of the Creighton-Nebraska health foundation (CNHF). Results: B$\ddot{o}$hler angle and Gissane angle had improved significantly from preoperative average $9.8^{\circ}$, $117.6^{\circ}$to average $22.4^{\circ}$, $113.4^{\circ}$ immediate postoperatively, and had maintained to average $21.8^{\circ}$and $114.2^{\circ}$ at the last follow-up. The degree of articular surface depression had improved significantly from preoperative average 5.2 mm to 1.2 mm at the last follow-up. All cases achieved bone union, and the period to union was average 10.5 weeks. AOFAS score was average 86.2 points at the last follow-up. There were 7 excellent, 9 good, and 2 fair results according to the CNHF scale. Therefore, 16 cases (88.8%) achieved satisfactory results. Conclusion: The minimally invasive sinus tarsi approach using headless compression screw seems to be an effective surgical method for displaced intraarticular calcaneal fractures, because of the possibility of accurate restoration of articular surface and the low risk of postoperative soft tissue complications.

Minimally Invasive Multi-Level Posterior Lumbar Interbody Fusion Using a Percutaneously Inserted Spinal Fixation System : Technical Tips, Surgical Outcomes

  • Objective : There are technical limitations of multi-level posterior pedicle screw fixation performed by the percutaneous technique. The purpose of this study was to describe the surgical technique and outcome of minimally invasive multi-level posterior lumbar interbody fusion (PLIF) and to determine its efficacy. Methods : Forty-two patients who underwent mini-open PLIF using the percutaneous screw fixation system were studied. The mean age of the patients was 59.1 (range, 23 to 78 years). Two levels were involved in 32 cases and three levels in 10 cases. The clinical outcome was assessed using the visual analog scale (VAS) and Low Back Outcome Score (LBOS). Achievement of radiological fusion, intra-operative blood loss, the midline surgical scar and procedure related complications were also analyzed. Results : The mean follow-up period was 25.3 months. The mean LBOS prior to surgery was 34.5, which was improved to 49.1 at the final follow up. The mean pain score (VAS) prior to surgery was 7.5 and it was decreased to 2.9 at the last follow up. The mean estimated blood loss was 238 mL (140-350) for the two level procedures and 387 mL (278-458) for three levels. The midline surgical scar was 6.27 cm for two levels and 8.25 cm for three level procedures. Complications included two cases of asymptomatic medial penetration of the pedicle border. However, there were no signs of neurological deterioration or fusion failure. Conclusion : Multi-level, minimally invasive PLIF can be performed effectively using the percutaneous transpedicular screw fixation system. It can be an alternative to the traditional open procedures.
