고령화로 인해 기대수명이 높아지고 있으며, 다양한 노인성 질병 등의 문제가 증가하고 있다. 본 연구의 목적은 산림치유 활동이 노인에게 어떠한 영향을 미치는지 체계적으로 문헌을 분석하여 노인 대상 산림치유 활동 확대를 위한 기초자료를 제공하는 것이다. 문헌 수집을 위해 국내는 RISS, KISS, Korea Med, Science On, 국외는 PubMed, Cochrane central, MDPI, Google Scholar를 이용하였다. 수집된 문헌의 방법론적인 질적 평가를 위해 코크란 그룹의 RoB2와 RoBANS를 이용하여 비뚤림 평가를 진행하였다. 2000년부터 2022년 1월 사이에 게재된 노인 대상 산림치유 활동의 연구 논문으로 한정하여 총 1,856편에서 최종 21편의 문헌을 선정하였다. 대상자는 60세 이상이며, 선정된 문헌의 참가자는 최소 7명에서 최대88명으로 총 750명이 참가하였다. 분석결과, 각 지표에서 가장 많이 수행된 검사는 심리지표에서 우울 7편, 인지지표에서 MMSE(Mini Mental State Examination) 2편, 생리지표에서 혈압 4편, 생화학지표에서 멜라토닌 2편, 신체지표에서 체지방과 근력 3편으로 나타났다. 총21편의 연구 중 19편의 연구가 2가지 이상의 검사항목을 사용하였으며, 심리적 지표를 가장 많이 측정하였다. 향후 노인대상 산림치유 활동의 적용을 위해서는 인지기능 예방의 산림치유 프로그램이 다양하게 개발 및 보급되어야 하며, 산림치유 활동에 대한 근거를 제공하기 위해 후속연구는 지속적으로 제시되어야 한다.
목적 : 본 연구의 목적은 경도인지장애 고령자에게 인공지능 동작 인식 기술을 활용한 전산화인지훈련 프로그램을 실시하여 인지 기능, 우울감, 삶의 질을 향상시키고자 한다. 연구방법 : 연구 참여자는 총 7명(실험군 = 4명, 대조군 = 3명)이며 코로나-19 발생 기간인 2021년 10월부터 12월까지 시행되었다. 프로그램은 직접 개발한 인공지능 동작 인식 기술을 활용한 전산화 프로그램 "MOOVE Brain"을 활용했으며 실험군은 한 달 동안 주 3회 30분씩 프로그램을 진행하였고 대조군에게는 중재를 제공하지 않았다. 치료의 전후 평가는 Korean version of the Mini-Mental State Examination-2, Korean version of the Consortium to Establish a Registry for Alzheimer's Disease Assessment Packet for Daily Life Evaluation, Korean version of the short form Geriatric Depression Scale (SGDS-K), 그리고 Geriatric Quality of Life Scale (GQOL)을 이용하였다. 결과 : 치료 전후로 실험군의 주의력 지표인 Stroop Color Test에서 평균 점수가 향상되었고(p = .068), 그룹 간 비교를 했을 때는 실험군의 실행 기능 평가 지표인 Stroop Color/Word Test 평균 점수가 향상되었다(p = .057). 그룹 간의 변화량을 비교했을 때는 실험군의 삶의 질 측정 도구인 GQOL (p = .057)과 우울증 지표인 SGDS-K (p = .057)의 평균 점수가 개선되었다. 하지만 각 영역들은 통계적으로 유의미하지 않았다. 결론 : 본 연구의 결과는 코로나-19로 인해 격리되어 있거나 의료 서비스를 받기 힘든 경도인지장애 고령자의 인지 및 심리 사회적 문제를 해결하는 데 활용될 수 있을 것으로 기대된다.
The objective of this study was to investigate the health status and nutritional intake of the old population living in Soonchang, Jeollabuk-do, Korea. 69 subjects aged 65 years and older were recruited in July 2016. The WHR, was significantly higher in elderly male group than elderly female group (p<0.05), but both groups had abdominal obesity (0.85 and over). T-score mean of elderly male and female groups were below $-2.5mg/cm^3$ that they were osteoporotic. In the elderly male group, the higher concentrations of creatinine, homocysteine and uric acid were found to be significantly unfavorable factors (p<0.001, respectively). The blood vitamin $D_3$ levels of elderly male group was significantly higher than that of elderly female group (p<0.05). The physical activity and self-rated health were significantly higher in elderly male group than in elderly female group (p<0.05, p<0.01, respectively). The nutrient intakes of male group were found to be significantly favorable factors than in female group. The score of mini nutrition assessment was significantly lower in elderly female group than in the elderly male group. These results could be useful to plan effective strategies to increase the health-life expectancy and the prevention of disease of Korean elderly people living in rural areas.
Background: The purpose of this study is to determine how the cognitive state of the elderly influences the activities of daily living(ADL) in order to increase their achievement capacity and their ability to exercise this capacity in daily life. The MMSE-K(Mini-Mental State Examination-Korean) was used to evaluate the daily activities and cognitive state of a group of subjects more than 65 years of age. The goal was to understand (1) How their achievement level in their daily activities changes according to the increase and decrease of their cognition points by analyzing and comparing their achievement points in the mental state examination, and (2) What variables influence that daily achievement level. Methods: The survey was conducted by a group of students from AnSan College who were trained for this method of data collection. The group surveyed 224 senior citizens older than 65 to determine their ADL performance. The subjects were selected from Silver Care Centers in the Gyounggi Province area including centers in Suwon, Anyang, Kwachun, and Sanbon City. The subjects were given enough information to fully understand the purpose of the study and the method by which it would be conducted. The survey method was a direct interview: which involved an interviewer reading the questionnaire to the subject and recording the answer. Result: The following results were calculated using SPSS WIN ver. 13.0: In the lower score of the MMSE-K regarding ADL performance (K-MBI) for activities such as individual hygiene, bathing activity, food activity, toilet management, going up stairs, getting dressed ad undressed, controlling stools and urination, walking, and chair/bed movement, most of the subjects responded that they could not perform these activities by themselves (p<.01). In the higher scores for the same activities, most response that they could perform the tasks by themselves (p<.01). In the results of the multiple regression analysis, there influence of the variables for ADL performance (K-MBI) and MMSE-K score was higher for females than males. Conclusion: This study evaluated the cognitive state of the elderly using the MMSE-K, which is the most convenient method, and examined how the cognitive state influences the achievement capacity of the daily lives with other variables. In conclusion, the higher the score on the MMSE-K, the more independent the elders are in the activities of daily living, especially among women more than men.
Purpose: The aim on this study was to establish the validity, reliability and efficiency of a Pain Self-Report Scale for elderly with dementia and compare these results with an observational pain rating scale. Methods: Study subjects were 136 elderly with dementia who were residents in a nursing home, geriatric hospital, or day care center. The subject's pain was measured by five self-report scales and observational scale. DS-DAT (discomfort scale-dementia of the Alzheimer's type) was used for pain behavior observational measure. Cognitive state was assessed using the MMSE (Mini-Mental State Examination). Results: Observational rating correlated moderately with self-report (r=.225~.585, p<.05) and tended to underestimate pain intensity. Test-retest reliability was high for all five self-report scales, and the correlation between these scales was very strong (r=.735~.856, p<.05). Comprehension rate of VDS (verbal descriptor scale) was 88.3%, and NRS (numeric rating scale) 69.9%, FPS (face pain scale) 66.9%, HVAS (horizontal visual analog scale) and VVAS (vertical visual analog scale) 65.4%. Conclusion: Nurses should not apply observational scales routinely in demented patients as many of these are capable of reporting their own pain. Self-report, the highest standard of pain measurement can be reliably performed in a large proportion of demented elderly.
Objectives: The purpose of this study is to investigate cognitive function, performance of activities of daily living, and recognition on oral health with the cognitive function testto dementia or dementia-suspected patients in the outpatients. Methods: The subjects were 94 dementia or dementia-suspected patients visiting C University hospital for the dementia test. Study instruments included Korea Mini-Mental State Examination KMMS, The Bayer-Activities of Daily Living Scale; B-ADL, Seoul-Instrumental Activities of Daily Living; S-IADL, Global Deterioration Scale; GDS, Korean Dementia Screening Questionnaire; KDSQ, and underlying diseases. Results: Dementia or dementia-suspected patients were 42 by KMMSE test, 25 patients had impaired functioning of daily living by B-ADL test, 27 patients showed the presence of depression by GDS test, and 45 patients showed impaired functioning of daily living. There was a statistically significant difference in the subjective recognition on oral health conditions. There was a statistically significant difference in the subjective recognition on oral health conditions by ADL. There was a positive correlation between the cognitive function and ADL performance. Higher cognitive function is proportional to ADL performance. Conclusions: The cognitive function was closely associated with ADL and subjective oral health conditions.
Objectives Non-major depression with fewer symptoms than required for a Diagnostic and Statistical Manual of Mental Disorders-4th edition diagnosis of major depressive disorder (MDD) has consistently been found to be associated with functional impairment. In this study, we aim to estimate the cognitive impairment and the quality of life in elderly patients with subsyndromal depression (SSD) compared with non-depressive elderly (NDE). Methods The Korean version of Mini International Neuropsychiatric Interview was administered to 194 outpatients with depression and 108 normal controls. SSD is defined as having five or more current depressive symptoms with core depressive symptoms (depressive mood or loss of interest or pleasure) during more than half a day and more than seven days over two weeks. Depression was evaluated by the Korean form of Geriatric Depression Scale of a 15-item short version. Global cognition was assessed by Mini-Mental State Examination in the Korean version of CERAD assessment packet (MMSE-KC). Subjective cognitive impairment was assessed by the Subjective Memory Complaint Questionnaire. Quality of life was evaluated by the Korean Version of Short-Form 36-Item Health Survey. Results The mean score of the MMSE-KC in the SSD group was lower than that in the NDE group with adjustment for age, gender, and education [F = 4.270, p = 0.04, analysis of covariance (ANCOVA)]. If we defined those having Z-score of MMSE-KC < -1.5 as a high risk group of cognitive impairment, the odds ratio for the high risk group of cognitive impairment was 1.86 [95% confidence intervals (CI) 1.04-3.34] in SSD and 7.57 (95% CI 3.50-16.40) in MDD compared to NDE. The scores of physical component summary (F = 9.274, p = 0.003, ANCOVA) and mental component summary (F = 53.166, p < 0.001, ANCOVA) in the SSD group were lower than those in the NDE group with adjustment for age, gender, and education. Conclusions The subjects with SSD, as well as those with MDD, showed impairment of global cognition and also experienced low quality of life in both physical and mental aspects, compared to the NDE group.
연구목적 본 연구는 알츠하이머병(Alzheimer's disease, AD), 경도인지장애(Mild Cognitive Impairment, MCI), 정상군에서 체질량지수(Body Mass Index, BMI)의 차이를 분석하여 낮은 BMI 수치와 인지기능 저하와의 연관을 확인하고자 하였다. 또한, 한국판 간이정신상태검사(Mini Mental State Examination-Korean version, MMSEK), 치매임상평가척도(Clinical Dementia Rating, CDR), 전반적 퇴화척도(Global Deterioration Scale, GDS)과 BMI 사이에 연관성이 있는지도 알아보고자 하였다. 방 법 총 257명의 연구대상자들이 이 연구에 포함되었고, AD 및 MCI 진단을 위해 병력 청취, 정신상태검사, 신경인지기능검사, 신체 검사가 시행되었다. 대상자의 성별 및 연령과 신체 질환도 함께 조사하였다. 전반적인 인지기능과 질병의 심각도 단계 평가는 MMSE-K, GDS, CDR으로 측정하였다. 결 과 MMSE-K 점수는 정상군>MCI>AD 순이었으며 통계적으로 유의한 차이를 나타내었다(p=0.000). CDR과 GDS점수는 정상군이 가장 높았고, AD군이 가장 낮았으며 역시 통계적으로 유의한 차이를 나타내었다 BMI와 MMSE 점수 사이에는 유의한 양의 상관관계가 있었다(r=0.238, p=0.000). BMI와 CDR과는 음의 상관관계를 나타내었으며(r=-0.174, p=0.008), BMI와 GDS 역시, 음의 상관관계를 보였다(r=-0.233, p=0.000). 결 론 BMI와 인지기능과의 연관성이 있으므로, 치매나 경도인지장애 환자를 대상으로 BMI를 측정하는 것은 임상적 표지자로서 의미가 있을 것으로 예상된다. 본 연구는 AD의 진단 및 예방, 그리고 치료적 접근에 도움을 줄 수 있을 것으로 기대하며, 향후 이에 대한 대규모의 장기 추적 연구가 필요할 것으로 생각된다.
Community-based group exercise programs for people with chronic stroke are relatively uncommon in Korea. In addition, it is currently not known whether a community-based group exercise program is effective or not. The purpose of this study was to evaluate an 8 week community-based functional exercise program for its effects on balance performance and occupational performance in persons with chronic stroke. Twenty-five community-dwelling individuals with stroke participated in this program. Outcome of the program was assessed by the Berg Balance Scale and the Canadian Occupational Performance Measure (COPM). The functional exercise program lasted for eight weeks, with a 1-hour program twice per week and it consisted of mobility, stability, balance, functional strength, and gait training. The subjects were trained by one physical therapist but were under one-to-one supervision from students. The data of sixteen individuals who scored more than 24 on the Mini-Mental State Examination (MMSE) were analyzed. There was a significant effect both in terms of the COPM Performance Score & the Satisfaction Score (p=.002) and with the Berg Balance Score (p=.001). It was found that a short-term community-based exercise program could improve both performance of activities and balance. Further, all subjects reported that they were satisfied with this program.
Purpose: To examine the effects of Tai Chi exercise (TCE) on muscle strength, flexibility of low extremities, postural balance and cognition in patients with Parkinson disease (PD). Method: A nonequivalent control-group pre and post-test design was used. Of the 30 participants 15 were assigned to the experimental group and 15 to the control group. The experimental group participated in 8 weeks of TCE which included one day of exercise with instructor on site and 3 days of self-exercise at home guided by a videotape. Results: The 8 weeks of TCE were found to be significantly effective in enhancing strength of low extremities, flexibility of ankles and shoulders, time of tandem stand with eye closed and level on the K-MMSE (Korean mini mental state examination). Conclusion: These results suggest that TCE has positive effects on physical fitness and cognition in patients with PD. Research over an extended period with the TCE intervention is recommended to identify further effects.
