Pulp treatment for immature permanent teeth (미성숙 영구치의 치수치료)

  • For dental pulp treatment of immature permanent teeth, direct pulp capping or partial/cervical pulpotomy (apexogenesis) procedures can be used if the dental pulp is vital. MTA (Mineral Trioxide Aggregate) is regarded as the first choice dressing material for these procedures because its higher success rate. It can be also used successfully for devitalized dental pulp which has been treated by calcium hydroxide. This apexification procedure with MTA has a few advantage such as short treatment period and increase of resistance against root fracture. Recently, regenerative endodontic treatment was introduced for devitalized immature pulp. It can maintain pulp vitality and lead to continuing root development although the dental pulp was devitalized.

ONE VISIT CANAL FILLING BY USING MINERAL TRIOXIDE IN A MENTALLY RETARDED CHILD : A CASE REPORT (정신지체장애아의 Mineral trioxide aggregate를 이용한 즉일 근관 충전)

  • A-12year-old boy visited the clinic with chief complaint of anterior maxillary trauma. He was diagnosed with first degree mental retardation and cerebral disorders. By clinical and radiographic examination, intrusion of maxillary central incisors were found. The intrusion was not severe, teeth were luxated with a slight force and the prognosis was followed. After the first year, external root resorption was seen radiographically. Due to difficult behavior management, one visit root canal filling with OrthoMTA(BioMTA, Korea) which is known to generate of cementum and periodontal ligament was planned along with general anesthesia. OrthoMTA was filled from the apex to 1-2mm below cervical area and composite resin used for crown restoration. 6 months after, further resorption, discoloration and mobility was not found. This case is currently checked yearly and further research is needed for inflammatory root resorption and ankylosis.

Effect of Blood Contamination on Vickers Microhardness and Surface Morphology of Mineral Trioxide Aggregate

  • This study aimed to investigate the effects of blood contamination on the Vickers hardness and the surface morphology of premixed MTA and compare them with the effects on conventional MTA. The Vickers microhardness of Endocem MTA Premixed Regular (EP) and ProRoot MTA (PM) was assessed after immersion in fetal bovine serum (FBS) and saline. Stem cells from human exfoliated deciduous teeth (SHED) were seeded on MTA after immersion in FBS, saline, and deionized water (DW). Cell adhesion patterns and surface morphology were visualized via scanning electron microscopy (SEM). The surface microhardness of EP and PM in FBS was lower than in saline. However, short-term exposure of PM to FBS did not reduce the microhardness compared to saline. Angular crystals formed in water, while rounded crystals with more air voids appeared in FBS. Favorable SHED attachment occurred in all groups. Overall, the surface hardness of EP and PM decreased after FBS exposure, although PM was less influenced. We suggest minimizing the amount of bleeding when using MTA clinically; nevertheless, PM remains an option with more expected blood contamination than EP. In summary, exposure to FBS decreased mechanical performance but allowed cell adhesion for both MTAs, with PM being more resistant to these changes.

Retention of BioAggregate and MTA as coronal plugs after intracanal medication for regenerative endodontic procedures: an ex vivo study

  • Objectives: This study compared the retention of BioAggregate (BA; Innovative BioCeramix) and mineral trioxide aggregate (MTA; Angelus) as coronal plugs after applying different intracanal medications (ICMs) used in regenerative endodontic. Materials and Methods: One-hundred human maxillary central incisors were used. The canals were enlarged to a diameter of 1.7 mm. Specimens were divided into 5 groups (n = 20) according to the ICM used: calcium hydroxide (CH), 2% chlorhexidine (CHX), triple-antibiotic paste (TAP), double-antibiotic paste (DAP), and no ICM (control; CON). After 3 weeks of application, ICMs were removed and BA or MTA were placed as the plug material (n = 10). The push-out bond strength and the mode of failure were assessed. The data were analyzed using 2-way analysis of variance, the Tukey's test, and the ${\chi}^2$ test; p values < 0.05 indicated statistical significance. Results: The type of ICM and the type of plug material significantly affected bond strength (p < 0.01). Regardless of the type of ICM, BA showed a lower bond strength than MTA (p < 0.05). For MTA, CH showed a higher bond strength than CON, TAP and DAP; CHX showed a higher bond strength than DAP (p < 0.01). For BA, CH showed a higher bond strength than DAP (p < 0.05). The mode of failure was predominantly cohesive for BA (p < 0.05). Conclusions: MTA may show better retention than BA. The mode of bond failure with BA can be predominantly cohesive. BA retention may be less affected by ICM type than MTA retention.

THE INFLUENCE OF OBTURATION TIMING AND THICKNESS OF MINERAL TRIOXIDE AGGREGATE ON SEALING ABILITY OF CANAL WITH OPEN APEX (근관충전 시기와 MINERAL TRIOXIDE AGGREGATE의 APICAL MATRIX두께에 따른 치근단 밀폐도의 평가)

  • 임상에서 점차 Mineral Trioxide Aggregate (MTA)에 대한 우수성이 소개되면서, apical matrix로의 사용은 주목할 만하다 할 수 있다. 본 연구의 목적은 개방형근관에서 MTA가 apical matrix로 사용될 때의 치근단 밀폐효과를 알아 보고, 근관충전의 시기와 matrix의 두께가 치근단 밀폐에 미치는 영향을 치근단 미세누출의 측면과 matrix탈락빈도의 측면에서 관찰하고자 하는 것이다. 개방형 근관을 재현하고자 45개의 발거된 단근치에 #90크기로 근단공을 형성하였고, 투명레진등을 이용하여 치근단 병소를 갖는 치조골을 재현한 후, 4개의 실험 군과 1개의 대조 군으로 분류하였다. A군: 2mm두께의 MTA matrix 형성후, 열연화된 Gutta-percha와 AH26 sealer를 이용하여 즉시 근관충전. B군: 2mm 두께의 MTA matrix 형성후, A군과 같은 방법으로 24시간 후 근관충전. C군: 4mm 두께의 MTA matrix 형성후, A군과 같은 방법으로 즉시 근관충전. D군: 4mm 두께의 MTA matrix 형성후, A군과 같은 방법으로 24시간 후 근관충전. 대조군: matrix를 사용하지 않고 열연화된 Gutta-percha와 AH26 sealer를 이용하여 근관충전하였다. Matrix의 탈락이 있는 경우 기록하고 완전 수세 후 다시 시행하였다. 색소침투정도의 평가를 위해 methylene blue에 치아를 48시간 침수시킨 후 협설로 양분하였다. 각 시편들을 고배율 현미경 관찰 후 비디오촬영 하였고, digitalized image analysing program을 이용하여 두명의 관찰자가 누출의 정도를 평가한 후, One-way ANOVA로 통계적 유의성을 검증하여 다음과 같은 결과를 얻었다. 1. Matrix를 사용한 실험 군이 matrix를 사용하지 않은 대조 군에 비해 유의성 있게 낮은 누출을 보였다(p<0.05). 그러나 근관충전시기와 matrix의 두께를 달리한 실험군 내에서는 누출에 유의성 있는 차이가 나타나지 않았다. 2. 모든 실험군에서 gutta-percha를 이용한 근관충전의 시기에 matrix탈락 및 gutta-percha의 압출은 발생하지 않았다.

Bone repair in defects filled with AH Plus sealer and different concentrations of MTA: a study in rat tibiae

  • Objectives: This study aimed to evaluate the effects on bone repair of different concentrations of mineral trioxide aggregate (MTA) added to AH Plus. Materials and Methods: Bone tissue reactions were evaluated in 30 rats (Rattus norvegicus) after 7 and 30 days. In the AH + MTA10, AH + MTA20, and AH + MTA30 groups, defects in the tibiae were filled with AH Plus with MTA in proportions of 10%, 20% and 30%, respectively; in the MTA-FILL group, MTA Fillapex was used; and in the control group, no sealer was used. The samples were histologically analyzed to assess bone union and maturation. The Kruskal-Wallis and Mann-Whitney tests were performed for multiple pairwise comparisons (p ≤ 0.05). Results: At the 7-day time point, AH + MTA10 was superior to MTA-FILL with respect to bone union, and AH + MTA20 was superior to MTA-FILL with respect to bone maturity (p < 0.05). At the 30-day time point, both the AH + MTA10 and AH + MTA20 experimental sealers were superior not only to MTA-FILL, but also to AH + MTA30 with respect to both parameters (p < 0.05). The results of the AH + MTA10 and AH + MTA20 groups were superior to those of the control group for both parameters and experimental time points (p < 0.05). Conclusions: The results suggest the potential benefit of using a combination of these materials in situations requiring bone repair.

Comparison of gene expression profiles of human dental pulp cells treated with mineral trioxide aggregate and calcium hydroxide (인간치수세포에 Mineral Trioxide Aggregate와 수산화칼슘 제재 적용 시 유전자 발현 양상 비교)

  • Objectives: This study investigated changes in gene expressions concerning of differentiation, proliferation, mineralization and inflammation using Human-8 expression bead arrays when white Mineral Trioxide Aggregate and calcium hydroxide-containing cement were applied in vitro to human dental pulp cells (HDPCs). Materials and Methods: wMTA (white ProRoot MTA, Dentsply) and Dycal (Dentsply Caulk) in a Teflon tube (inner diameter 10 mm, height 1 mm) were applied to HDPCs. Empty tube-applied HDPCs were used as negative control. Total RNA was extracted at 3, 6, 9 and 24 hr after wMTA and Dycal application. The results of microarray were confirmed by reverse transcriptase polymerase chain reaction. Results: Out of the 24,546 genes, 43 genes (e.g., BMP2, FOSB, THBS1, EDN1, IL11, COL10A1, TUFT1, HMOX1) were up-regulated greater than two-fold and 25 genes (e.g., SMAD6, TIMP2, DCN, SOCS2, CEBPD, KIAA1199) were down-regulated below 50% by wMTA. Two hundred thirty nine genes (e.g., BMP2, BMP6, SMAD6, IL11, FOS, VEGFA, PlGF, HMOX1, SOCS2, CEBPD, KIAA1199) were up-regulated greater than two-fold and 358 genes (e.g., EDN1, FGF) were down-regulated below 50% by Dycal. Conclusions: Both wMTA and Dycal induced changes in gene expressions related with differentiation and proliferation of pulp cells. wMTA induced changes in gene expressions related with mineralization, and Dycal induced those related with angiogenesis. The genes related with inflammation were more expressed by Dycal than by wMTA. It was confirmed that both wMTA and Dycal were able to induce gene expression changes concerned with the pulp repair in different ways.

  • For this study copper ions-containing bioactive glass nanoparticles commonly used in mineral trioxide aggregate (MTA) was developed to improve the mechanical properties and promote antibacterial effect of MTA with the original material. The mechanical properties and antibacterial activity of Cu-BGn incorporating varying amounts cooper incorporated bioactive glass nano particles(BGn) 0.5,1.0,2.0 and 4.0 wt% in MTA were characterized composition of the resulting were investigated. The compressive strength was calculated by weighing specimens with a diameter of 4 mm and a thickness of 6 mm according to ISO 6876 (2012). The antimicrobial effect was evaluated using two strains of S. mutans and E. faecalis. The mechanical properties of the test results was Cu-BGn increased no statistically significant difference was observed (p>0.05). Adhesion experiment results S. mutans in contrast to the control group Ortho MTA, 4.0 wt% of Cu-BGn added experimental group showed a significant difference was observed (p<0.05). Also, E. faecalis statistical analysis indicated a significant difference for antibacterial agents between control and Cu-BGn containing(p<0.05). It seems that this Cu-BGn proved that even a antibacterial effect was demonstrated. Therefore, it was suggest that it is necessary for in-depth research into various environments that can reproduce the oral environment.

High-plasticity mineral trioxide aggregate and its effects on M1 and M2 macrophage viability and adherence, phagocyte activity, production of reactive oxygen species, and cytokines

  • Objectives: This study evaluated the effects of high-plasticity mineral trioxide aggregate (MTA-HP) on the activity of M1 and M2 macrophages, compared to white MTA (Angelus). Materials and Methods: Peritoneal inflammatory M1 (from C57BL/6 mice) and M2 (from BALB/c mice) macrophages were cultured in the presence of the tested materials. Cell viability (MTT and trypan blue assays), adhesion, phagocytosis, reactive oxygen species (ROS) production, and tumor necrosis factor (TNF)-α and transforming growth factor (TGF)-β production were evaluated. Parametric analysis of variance and the non-parametric Kruskal-Wallis test were used. Results were considered significant when p < 0.05. Results: The MTT assay revealed a significant decrease in M1 metabolism with MTA-HP at 24 hours, and with MTA and MTA-HP later. The trypan blue assay showed significantly fewer live M1 at 48 hours and live M2 at 48 and 72 hours with MTA-HP, compared to MTA. M1 and M2 adherence and phagocytosis showed no significant differences compared to control for both materials. Zymosan A stimulated ROS production by macrophages. In the absence of interferon-γ, TNF-α production by M1 did not significantly differ between groups. For M2, both materials showed higher TNF-α production in the presence of the stimulus, but without significant between-group differences. Likewise, TGF-β production by M1 and M2 macrophages was not significantly different between the groups. Conclusions: M1 and M2 macrophages presented different viability in response to MTA and MTA-HP at different time points. Introducing a plasticizer into the MTA vehicle did not interfere with the activity of M1 and M2 macrophages.
