Analyses on Nutritional and Behavioral Risk Factors of Atopic Dermatitis (AD) in Korean Preschoolers (학령 전 아동에서 아토피피부염의 영양.행동 위험 요인 분석)

  • In order to examine nutritional and behavioral risk factors of atopic dermatitis (AD) in Korean preschoolers, we analyzed data on 144 children aged 3-6 years with AD and their 434 healthy counterparts. The data included breast-feeding history, current weight, food behaviors assessed by the Mini Dietary Assessment (MDA), food intake by a semi-quantitative food frequency questionnaire, and behavior problems by the Preschool and Kindergarten Behavior Scale (PKBS). Multivariate logistic regression analyses were performed after controlling for preschool location and child's age, gender and total energy intake, as appropriate. There was no group difference of child and household characteristics. Breastfeeding history was related to lower AD risk (OR = 0.63, 95% Cl = 0.40-0.99), yet no statistically significant association was found with overweight status. Regarding food behaviors, AD risk was lower in children who drank milk at least one cup per day (OR = 0.52, 95% Cl = 0.35-0.78) and had regular meals (OR = 0.62, 95% Cl =0.42-0.92). Moreover, there were lower risks of AD in the second (OR =0.48, 95% Cl = 0.28-0.82) and the highest (OR = 0.55, 95% Cl = 0.32-0.94) intake quartiles as compared with the lowest quartile of kimchi intake. Similarly, AD risk was lower in the highest quartile of rice (OR = 0.51 Cl = 0.28-0.93) and the second quartile of fruit (OR =0.45, 95% Cl = 0.25-0.82) intakes. AD children had more problems in social interaction (OR = 1.97,95% Cl = 1.26-3.07) and independence (OR = 1.60, 95% Cl = 1.01 -2.54) measures than the healthy controls. Likewise, AD children tended to show more problem behaviors such as anxiety (OR = 1.63, 95% Cl = 0.99-2.69). Our results suggest that nutritional and behavioral dimensions are related to AD risk, yet the case control study design may preclude generalization of these results.

Development of an Electronic Identification Unit for Automatic Dairy Farm Management (가축 사양 관리 자동화를 위한 전자 개체 인식 장치 개발)

  • In Korea, a need of automatic dairy farm management system has been increased to lower production cost and to strengthen international competition. However, the present management system was mostly relied on foreign technologies and caused some problems in post management and after-sales services. Therefore, though there is a problem of price and quality at present, domestic technologies of the management system should be developed for the long run. This study was conducted to develop an electronic identification unit for an automatic dairy farm management system. The developed system was consisted of a tag, a reader, a switching circuit, and a personal computer. The tag attachable to each individual cow was developed to transmit individual radio frequency(RF) code into the air with modulation of ASK(amplitude shift keying). And the switching circuit was added to avoid confusion on reception and transmittance. The reader attached to a feeding device was developed to transmit activating signal periodically and to identify code of the individual tag when the tag was approached to the device. The reader was consisted of an active filter, a detecter, a comparator and a microcontroller. The test result was feasible enough to apply it for the automatic farm management system and the identified maximum distance was about 37cm.

A Comparison of Growth Development and Nutrient Intakes between Double Income Families' and Full-Time Housewives' Preschool Children (맞벌이 가정과 전업주부 가정 미취학 자녀의 신체발달과 영양섭취상태 비교연구)

  • The purpose of this study was to examine the comparison of growth development, bone mineral density and nutrient intakes between double income families' preschool children(DIFPC) and full-time housewives' preschool children(FHPC). Subjects were 111 preschool children. Anthropometric characteristics and bone mineral density in right forearm were measured. The questionnaire was composed of health status, life style, dietary behaviors, and dietary intakes and was completed by the children's mothers. The average age of the DIFPC(n=60) was 53.02 months and that of the FHPC(n=51) was 54.80 months. The birth height and weight of the subjects were 50.47㎝ and 3.27㎏ for DIFPC and 50.85㎝ and 3.36㎏ for FHPC, respectively. The average height, weight, % body fat, and obesity index were 108.50㎝, 18.35㎏, 15.35%, 96.71% in DIFPC and 111.46㎝, 19.64㎏, 16.80%, 97.31% in FHPC, respectively. The bone mineral density in forearm of two groups were 0.24g/㎠ in all. The infant feeding method was significantly different between DIFPC and FHPC; 58.9% of DIFPC was fed formula, while 44.4% of FHPC was fed breast milk(p<0.05). Proportions of children for their regular meal were 59.4%, 89.6%, and 61.0% for breakfast, lunch, and dinner, respectively. The major reasons for irregular meal were lack of time and poor appetite for breakfast and snacks for lunch and dinner. Most of the children answered they have snack over once a day, and 60.0% have unbalanced diet. The intakes of energy, calcium, iron, zinc, vitamin A, vitamin B1, niacin, and vitamin C did not meet the Korean RDAs. The intakes of K and vitamin A for DIFPC were significantly higher than those of FHPC(p<0.05, p<0.05). In conclusion, double income families' preschool children more have a low frequency of breast feeding and low intakes of micro nutrients, such as K and vitamin A than full-time housewives' ones.

Effect of the Nutrition Education for Infant Feeding on Mother's Knowledge, Attitude, Practice of Weaning (영유아 섭식에 관한 영양교육이 어머니의 영양지식.태도 및 실천에 미치는 효과)

  • This study evaluated the effect of the nutrition education about infant feeding on the change of mother's knowledge, attitude and practice of weaning. The subjects consisted of treatment group (n=54) and control group (n=34). The treatment group participated in 6 times nutrition education and 5 times surveys. Nutrition education for weaning of the treatment group improved nutrition knowledge, attitude and practice in infant feeding. Before education, mean scores of control group and treatment group were 7.3 and 7.5 respectively but there was no significant difference. Both group got low score in a statement about weaning period, but they got good score in a statement about weaning method. After education, treatment group got better score(9.1) than control group(8.3) significantly (p<0.05). Treatment group was more desirable than control group specially in a use of Sun-sik, sugar, salt and weaning food mixed with formula after education (p<0.05). Practice of spoon feeding was more frequent in treatment group after education, however, control group used sugar and commercial weaning food more frequently than treatment group(p<0.05). Intake frequency of cow's milk and cookies of infants at age 9-12 month in control group was higher than treatment groups(p<0.01). Thus the nutrition education to mother seems to be effective in achieving desirable dietary behavior of infants.

The Influence of Food Habits on Body Stature of Children (어린이의 식습관(食習慣)이 체위(體位)에 미치는 영향(影響)에 관한 연구(硏究))

  • The Purpose of this study was to determine every possible correlation between food habits and body statures of primary school children, aged ten years old. This study was conducted from July to October of 1975. In July, prepared questionaires concerning life style, anthropometry, food preference, and food behavior were distributed through school teachers to 425 children; 219 boys and 206 girls, in the 5th grade of three elementary schools in the city of Seoul. Then, in October, when subjects had been classified into underweight/obese by statistical analysis, mothers of obese or underweight children were interviewed by the authors to determine weaning history, daily food consumption of their children, and opinions of various snacks for children. Analysis of results in terms of correlation coefficient, chisquare test and percentage calculations, are as follows: 1. Food habits 1) Food $preference{\cdots}{\cdots}A$ varying number of foods were selected from 60 items were accepted. It was found that the food which children liked best was fruit and snacks were popular one. Lowest ranking among LIKED foods were from strongly flavored vegetables and organ meat. In general, girls had more food dislikes than did boys. Selected as liked foods were fruits, rice noodle soup, biscuits, and peanuts. Disliked foods were liver, green onions, onions, green pepper, mushrooms, oysters, shellfish, and pork. Items which children never ate before were liver, mushrooms, fish cake, boiled rice mixed with sorghum, mayonnaise, and fresh water firsh. Reasons which children gave for dislike were undesirable flavor and odor. 2) Food $behavior{\cdots}{\cdots}It$ was found that boys liked sweet and salty flavors more than did the girls who more often liked sour flavor. The majority of children enjoyed evening meals more than lunch and breakfast. A number of children skipped breakfast because of lack of appetite or lack of time before going to school. Snacks were the most popular food, especially among girls. Their snacking habits were formed by the encouragement of the mother, and the relieve boredom. Educational backgrounds of mothers and family economical levels of children were remarkable correlated with mothers' attitudes toward feeding of children. There were several interesting findings relating body stature to some other responses; such as that the obese child has a small number of brothers, higher birth order, higher educated mothers and higher family cultural background. It was also discovered that food perference, except for fat and oil group foods was not related to body stature. Sweet taste was liked best and pepperly taste was mostly disliked. Sour taste was popular in the group of underweight. Underweight children were more finicky, disliked snacking, and didn't get much attention from their mothers. 3) Correlation between body stature and nutrition during their infancy. The majority of children, both from obese and underweight, were breast fed as infant. Twenty five per cent of obese children and 17.4 per cent of underwight children started weaning at $1{\sim}6$ months old. The most popular supplemental food of weaning was cereal gruel for the obese group, while boiled white rice and cereal gruel were most common for the underweight group. Highly significant relationships were found between stature of parents and their children. In the obese group 47.8 per cent of fathers and 45.9 per cent of mothers were overweight; however, none of the fathers and only one mother was found to be underweight. In daily food consumption patterns, meals consisting of bread or noodle were popular in the obese group but disliked by the underweight group. The study found clear contrast in that the obese group liked meat and fish while the underweight group liked fruits and vegetables, especially kimchee. The obese children desired to eat cereal foods, milk and milk products, and fat foods while the underweight group desired to eat fruits and vegetables. Frequency of snacks per day was much greater in the obese group. Foods which mothers believed to be good for the health were carrots, cucumbers, fruits, milk, potatoes, sweet potatoes, and breads, while sweet foods such as candies, drinks. chocolate were considered not good for the teeth. Watching television was not significantly related to statures of children. Most significant relationships were found beween frequencies of family guest meals/and eating-out, and children's statures. Nutritional problems which have been considered for the malnourished children in addition to those of children who have a tendency toward obesity, must be taken into consideration in the development of proper nutrition education through the channels of regular school teaching and teaching by parents in the homes. Korean standards of anthropometric measurements for children should be revised, current measurements show much higher figures than present Korean standards.

A Study on the Perceived Stress of Mothers in Neonatal Intensive Care Unit (신생아 중환자실에 입원한 환아 어머니의 스트레스)

  • The parents have much expectation upon the pregnancy and child birth, and in most cases, they expect the healthy parturient child. However, we can be placed on the high-risk conditions which have the physical, social and immature infant, due to the unexpected results, among the new-born. Accordingly, these high-risk newborn and premature infants will be mostly in NICU, which the concentrated medical treatment can be given, upon their conditions. After their birth and during these periods, they will be divided from the parents, and the nurse will accomplish the bringing-up activities which they can take care of the infant, expected by the parents after their birth. The hospitalization of high-risk newborn including these premature infants is the shocking experience to the parents of family, and thus they can feel the fear and uneasiness, and these reactions of parents are troubled in the behavior at the usual days, and cause the disorder and spiritless status, and these results break the supporting ability of parents, and cause the obstruction. Also, the unavoidable division between the parents and the children as like hospitalization of children can make the parents to feel the alienation emotionally, and this causes the results which the pride on the bringing-up ability of baby gets to be lost. These problems can cause the difficulties on the bonding or the parenting in the further days, and can be related to the neglect and abuse of children. Also, it is gradually increased to study and report which the emotional division by the physical division between the mother and the baby obstructs the normal affection course between the parent and the infant. The stress caused by the birth and the hospitalization of high-risk newborn, as like this, is important in the points which it can uncertainly affect the potential energy for the relationship of parent-child who are finally healthy. Accordingly, the significance and purpose of this study are to understand the contents and degree of stress which the parents of high-risk newborn including the immature child can be experienced from the hospitalization of ICU for their new borns, and thus to offer the basic program to the nursing intervention program for these. The subject of this study is the mother of newborn in NICU of 10 General Hospitals located at the 3one of Pusan, Korea from September 1997 to October 1997, and thus makes the subject of 95 person of parents who agreed to take part in the study and it is descriptive study related to the stress of mother having the newborn in NICU. The method is based on the preceding study related to the stress of mother having the experience of child hospitalization and chronic disease child, and then acquires the advice of specialists group as like 5 nursing professors, and then is amended and supplemented. Total number of questions is 43 items and consists of 5 factors as like medical treatment &nursing procedures, disease status & prognosis, role of parents, communication & inter-personal relationships, hospital environment, and is 5 point Likert Scale. The reliability of this study method is very highly shown to be Cronbach α=0.95. The collected data is analysed as Average, Frequency, Standard Deviation, T-test, ANOVA, Pearson Correlation Coefficient, Duncan multifulrange test by use of SPSS /PC (V7.5). The results of this study is summarized as under. 1. Every characteristics of subject is which the party of mother is 28.70age(±7.48) in the average ages, 51% in the high-school graduate, 38.5% in the christianity, total monthly income is 212.55 thousand won(±1.971), 74.5% in the housewife, 72.9% in the parents and children together living and the number of children to be 1.48person(± 0.6) in average, the recognition on the prognosis of baby is 74.0% in 'Don't know', the relationship with the husband after the hospitalization of babyis 37.3% in 'More Intimate', the relationship with the family of husband to be 48% in 'No-change', and the degree which is consulted with the husband about the baby is 55% in 'very frequently' and the visiting number per week is 4.59(±1.63) in average and the accompanying person in the time of visiting is which the number of husband is 56.3% and thus is the highest. The characteristics of baby is which the age is 21.88days(±16.47) after the birth in average, the sex to be 50 person in the female 52.1% and the order of birth to be 54.2% in the first chid, and the weight in the birth to be 2770gm(±610) and the height in the birth to be 46.26cm(±7.62) in aver age. The medical diagnosis is 37.5% in the premature infant, the career of hospitalization is 96.9% in 'None', and the operation plan is 90.6% in 'None' and the execution of operation is 88% in 'None' and the nursing of incubator is 55.2% in 'Yes', and the method of feeding is 50.5% in 'Oral' and the contents of feeding is 46.9% in the 'Milk'. 2. The total stress degree of subject is almost highly shown to be as 3.36(±0.86). If it is compared upon each cause, 'stress on disease status & prognosis' is highest 3.79(±1.28), and it is in the order of 'stress on medical treatment & nursing procedures' 3.70(±0.93), 'stress on hospital environment' 3.14(±0.86), 'stress on role of parents' 3.18(±0.92) and 'stress on communication & inter personal relationship' 2.62(± 0.77) 3. As the results of checking the notworthiness of stress degree upon each variable of subject, the variable showing the noted difference was the birth weight(γ=-0.16, P=0.04), birth height(γ=-0.23, P=0.03), nursing in the incubator(F=8.93, P=0.04), feed method(F=2.94, P=0.04). That is to say, it is shown which the smaller the birth weight is, the higher the stress degree of mother is noteworthily. Also, the smaller the birth height baby is, the higher the stress of mother is. In the incubator, it os shown which the mother whose baby is nursing in the incubator is higher in the stress degree than other mothers. Upon the feeding method of baby, that is to say, TPNis the highest, and it is shown in the order of NPO, Tube feeding, and P.O. feeding. When we review the above-mentioned results, as the status is serious, it is thought which we include the supporting nursing for coping with the stress of parents in the setting-up od nursing plan for the baby in the NICU.

Clinical Features of Dietary Protein Induced Proctocolitis (식이 단백 유발 직결장염의 임상적 고찰)

  • Purpose: Dietary protein induced proctocolitis (DPIPC) can be considered as a cause of rectal bleeding or blood streaked stool in otherwise healthy-looking infants in the first several months of life. Failure to appreciate this entity may lead to inappropriate diagnostic or therapeutic intervention. This study aimed to ascertain the clinical features, treatment and prognosis of DPIPC. Methods: We reviewed 13 infants retrospectively, presented with bloody stool in early infancy. They were diagnosed as DPIPC clinically in Pusan National University Hospital from May 2002 to June 2004. Results: Seven males and six females were included. The mean age at onset of bleeding was $96.8{\pm}58.8days$. The mean frequency of hematochezia was $2.6{\pm}2.5$ times a day. Duration from onset of symptom to diagnosis was $35.5{\pm}55.0days$ and duration from onset of symptom to resolution of bleeding was $58.7{\pm}67.0days$. Nine (69.2%) were exclusively breast-fed infants and two (15.4%) were formula-fed infants. All but one infant did not have family history of other allergic diseases. A dietary history of ingestion of cow's milk, nut or shellfish was present in three mothers. Peripheral eosinophil count was normal to slightly elevated (total WBC count $10,555{\pm}3,145/mm^3$, relative eosinophil count $6.3{\pm}3.0%$, absolute eosinophil count $659.0{\pm}532.2/mm^3$). Sigmoidoscopy revealed lymphonodular hyperplasia with surrounding hemorrhagic spots in the rectosigmoid colon in 6 infants. Histopathologic finding of colonic biopsies in 5 infants showed chronic inflammation with lymphoid follicular hyperplasia (5 infants), crypt abscess (3 infants), or mild infiltration of eosinophils (less than 20/high power field) in the lamina propria. Spontaneous resolution of rectal bleeding occurred in all infants without dietary change or medicine. Conclusion: Most infants with DPIPC experience a very benign course and have spontaneous resolution of rectal bleeding without changes in the mother's diet. In the case of strong evidence for DPIPC we suggest deferring further invasive investigation and continuing breast feeding.
