Effects of Low Temperature on Mechanical Properties of Steel and Ultimate Hull Girder Strength of Commercial Ship (저온환경이 선박 및 해양플랜트용 탄소강재의 재료강도특성 및 상선의 최종 종강도 거동에 미치는 영향)

  • This paper presents the material properties of carbon steels for ships, and offshore structures (ASTM A131) are tested under a series of arctic and cryogenic temperature conditions. For material tension tests, among the ASTM 131 steels, Grades A and B of mild steel and Grade AH of high tensile steel have been used. The obtained mechanical properties of the materials from the material tension tests were applied in a 13,000TEU class container ship to define the effect of low temperature on the ultimate longitudinal strength of the target structure by using the ALPS/HULL intelligent supersize finite element method. The tensile coupon test results showed increased strength and nonuniform fracture strain behaviors within different grades and temperatures. Increasing the material strength resulted in increasing the ultimate longitudinal strength of the ship.

Guillain-Barré Syndrome after Lung Transplantation in the Immediate Postoperative Period: Case Report

  • A 58-year-old man, incapable of maintaining oxygen saturation with mechanical ventilation, was admitted to our hospital for veno-venous extracorporeal membrane oxygenation (ECMO) treatment. He was diagnosed with acute respiratory distress syndrome (ARDS) due to influenza A pneumonia. His condition stabilized with antibiotics and steroid administration, but weaning from ECMO failed due to post-infectious pulmonary sequelae. On day 84 after admission, he underwent bilateral lung transplantation. In the postoperative phase, he did not regain consciousness even after discontinuation of sedatives for 3 days. However, spontaneous pupillary reflex and eye movements were preserved, while communication and upper and lower limb movements were affected. The nerve conduction study was diagnostic of Guillain-Barré syndrome. He was managed with intravenous immunoglobulins and plasmapheresis. Mild recovery of the facial muscles was seen, but he died 24 days post-surgery due to progressive ARDS and sepsis.

Viruses Associated with Fig Mosaic Disease in Different Fig Varieties in Montenegro

  • Symptoms of fig mosaic disease have been noticed on leaves of fig (Ficus carica) for several decades, in Montenegro. In 2014, leaf samples were collected from trees of six fig cultivars in a plantation located in the main fig-producing area of Montenegro, to study the disease. After RNA isolation, samples were tested by RT-PCR for detection of nine fig viruses and three viroids. Four viruses were detected: fig leaf mottle-associated virus 1 (FLMaV-1), fig mosaic virus (FMV), fig mild mottle-associated-virus (FMMaV) and fig badnavirus 1 (FBV-1). Most of the viruses were present in mixed infections. The amplicons of the viruses were directly sequenced from both directions. A BLAST search of these sequences revealed sequence identities with their closest counterparts at GenBank of 92, 97, 92 and 100%, for FLMaV-1, FMV, FMMaV and FBV-1, respectively. Different responses in symptom expression due to the various virus combinations detected have been demonstrated. Variety $Su{\check{s}}ilica$ had the least symptom expression, with only one virus (FBV-1) found. Considering that the production of figs in Montenegro is increasing and has a substantial relevance in this geographic location, the results indicate that more attention should be given to improving the phytosanitary condition of fig trees in the country.

The Ways of Taking Pills and Trituration in Naegyeong, Dongeuibogam (『동의보감(東醫寶鑑)』 내경편(內景篇)의 환산제(丸散劑) 복용법 연구)

  • Objective : We searched and collected the various ways of taking medicine in Naegyeong, Dongeuibogam and studied the possibilities to apply the traditional ways of taking medicine to the current way of taking medicine. Method : We collected all the ways of taking medicine in Naegyeong and classified the ways according to the relative importance, urgency, pathosis, time of disease, and, the conditions of patients. Result : Medicinal forms are decoction, pills, trituration, and thin porridge. Various kinds of water and the prepared rice forms were used. A single herb was boiled and its water was used to take the medicine. Also, liquor and honey were used to take medicine. More than two herbs or special prescriptions were boiled and the extract water was taken. The same medicine was taken by different boiled water according to the condition and age of a patient, time, acute or chronic illness, and, severe or mild disease. Conclusion : There are a lot of pills and trituration prescriptions in Naegyeong, Dongeuibogam. Water, various rice preparations, and several herbs are used to take these prescriptions. The reason is that these ways of taking medicine promote the medicinal effect and fast treatment to maximize the medicinal effects. From now on, the in-depth and mutilple studies are needed based on this research.

Synthesis and Characterization of Quartz Nanocrystals (석영 나노 결정의 합성과 특성)

  • We report the synthesis and characterization of quartz nanocrystals (NCs). Quartz NCs were synthesized from the dissolution of amorphous silica nanoparticle precursors under the mild hydrothermal condition of ~250 ℃ and autogenic pressure. It was confirmed that the average size of the nanostructure with a highly crystalline phase of α-quartz can be tuned in a relatively narrow range from 407.5 to 826.2 nm with respect to the reaction time. α-Quartz NCs have potential uses for technological applications in optoelectronics, sensing, and rechargeable battery devices.

  • Hypermagnesemia is a rare condition that is usually iatrogenic in patients with elderly or renal failure. Severe hypermagnesemia is uncommon in patients with a normal renal function. Symptoms due to hypermagnesemia can range from mild symptoms, such as nausea, to severe symptoms, such as cardiac and respiratory arrest. This paper describes a case of a 49-year-old woman who ingested a magnesium-containing fertilizer with normal renal function. Cardiac arrest occurred eight hours after poisoning. Electrocardiography changed from a narrow QRS to a wide QRS and then to a complete atrioventricular block. Her hemodynamic state was unstable. Continuous renal replacement therapy was performed to remove magnesium from the blood, with the subsequent resolution of arrhythmia and hemodynamic stabilization. This paper reviews the pathophysiologic effects of magnesium on the cardiovascular system, clinical manifestation, and treatment of hypermagnesemia.

Behavioral Problems in Patients with Prader-Willi Syndrome

  • Prader-Willi Syndrome (PWS) is a neurodevelopmental genomic imprinting disorder involving a lack of gene expression from the paternal chromosome 15q11-q13 region. This is typically due to paternal 15q11-q13 deletions (in approximately 60% of cases), maternal uniparental disomy 15, or when both 15s are from the mother (about 35% of cases). An imprinting center controls the expression of imprinted genes in the chromosome 15q11-q13 region. PWS is a neurodevelopmental disorder characterized by mental retardation and distinct physical, behavioral, and psychiatric features. Characteristic behavioral disturbances in PWS include excessive interest in food, skin picking, difficulty with a change in routine, temper tantrums, obsessive and compulsive behaviors, and mood fluctuations. Individuals with PWS typically have intellectual disabilities (borderline to mild/moderate mental retardation) and exhibit a higher overall level of behavior disturbances compared to individuals with similar intellectual disabilities. This condition severely limits social adaptations and quality of life. Different factors have been linked to the intensity and form of these behavioral disturbances, but there is no consensus regarding the cause. Consequently, there is still controversy surrounding management strategies and there is a need for new data. PWS is a multisystem disorder. Family members, caregivers, physicians, dieticians, and speech-language pathologists all play an important role in the management and treatment of symptoms in an individual with PWS. Here we analyze behavioral problems in children and adults with PWS by age and review appropriate management and treatment strategies for these symptoms.

Modified Design Formula for Predicting the Ultimate Strength of High-tensile Steel Thin Plates

  • Methods for predicting the ultimate/buckling strength of ship structures have been extensively improved in terms of design formulas and analytical solutions. In recent years, the design strategy of ships and offshore structures has tended to emphasize lighter builds and improve operational safety. Therefore, the corresponding geometrical changes in design necessitate the use of high-tensile steel and thin plates. However, the existing design formulas were mainly developed for thick plates and mild steels. Therefore, the calculation methods require appropriate modification for new designs beased on high-tensile steel and thin plates. In this study, a modified formula was developed to predict the ultimate strength of thin steel plates subjected to compressive and shear loads. Based on the numerical results, the effects of the yield stress, slenderness ratio, and loading condition on the buckling/ultimate strength of steel plates were examined, and a newly modified double-beta parameter formula was developed. The results were used to derive and modify existing closed-form expressions and empirical formulas to predict the ultimate strength of thin-walled steel structures.

KM-based Treatment of Viral Hepatitis A accompanied with Pancreatitis: A case report

  • Objectives: Hepatitis A is a typical acute hepatitis caused by hepatovirus, and then most patients recover easily without progression to chronic condition. However, certain cases have the risk of severe symptoms or even death. This case report presented a hepatitis A accompanied with pancreatitis, which had been completely recovered in a Korean medicine hospital. Case presentation: A 38-year woman had felt the malaise, mild chilling, muscle pain and abdominal discomfort for 10 days, which led her visit doctors and took anti-pyretic analgesics and digestants. The symptoms, especially epigastric pain and fatigue, became worse, and then she hospitalized in a Korean medicine hospital. Based on the drastic elevations of hepatic enzymes (aspartate transaminase 1,604 IU/L and alanine transaminase 2,825IU/L) with an anti-HAV IgM positive, she was diagnosed with hepatitis A. After bed rest and herbal drug treatment (CGX and Innae-Tang) for 5 days, the laboratory abnormalities and subjective symptoms had been improved gradually, except the upper gastric discomfort and pain. Those symptoms had anticipated the comorbidity with HAV-induced pancreatitis, supported by the high level of serum lipase release. Another 5-day hospitalized treatment improved all subjective symptoms and then the laboratory results were completely normalized including detection of anti-HAV IgG within 15 days after discharge. Conclusion: This study presented a typical hepatitis A accompanied with pancreatitis, which should be considered in diagnosis and management of hepatitis A.

An overview of Acne Vulgaris (Busoor Labaniya)

  • Acne vulgaris is a common dermatological condition affecting the pilosebaceous units and having a multifactorial etiology. In Unani terminology, acne vulgaris is referred as Busoor Labaniya, characterized by white lesions on the face, nose, and cheeks; on squeezing, release cheesy material. In conventional medicine, mild cases are best addressed with topical regimens, but more severe cases require systemic medications. Retinoids (retinoic acid, adapalene, isotretinoin, tazarotene), benzoyl peroxide, clindamycin, erythromycin, and azelaic acid are a few examples of topical medications, whereas systemic drugs include antibiotics (Doxycycline, minocycline, erythromycin, azithromycin). In the Unani System of Medicine, numerous single and compound drugs have been used to treat Busoor Labaniya. These drugs are considered harmless and do not have any major side effects. In Unani system of medicine, acne vulgaris is effectively managed with natural medicines as well as therapeutic regimens with minimal side effects even after long-term usage. Acne vulgaris is usually treated with systemic blood purifiers along with topical Unani drugs. Hence, clinical studies with proper scientific parameters are needed to be conducted to establish and validate their efficacy in the prevention and control of acne vulgaris.
