Middle Ear Disease Automatic Decision Scheme using HoG Descriptor (HoG 기술자를 이용한 중이염 자동 판별 방법)

  • This paper presents a decision method of middle ear disease which is developed in children and adults. In the proposed method, features are extracted from the middle ear disease images and normal images using HoG (histogram of oriented gradient) descriptor and the extracted features are learned by SVM (support vector machine) classifier. To obtain an input vector into SVM, an input image is resized to a predefined size and then the resized image is partitioned into 16 blocks each of which is partitioned into 4 sub-blocks (namely cell). Finally, the feature vector with 576 components is given by using HoG with 9 bins and it is used as SVM learning and classification. Input images are classified by SVM classifier based on the model of learning features. Experimental results show that the proposed method yields the precision of over 90% in decision.

A Study on Design Parameters for Ready-made Ear Shell of Hearing Aids (보청기용 범용 이어쉘을 위한 설계 파라미터에 관한 연구)

  • In this study, main parameters: aperture, first bend and second bend which express a structure of ear canal are extracted in order to modeling and manufacture the ready-made ear shells of hearing aids. The proposed parameter extraction method consists of 2 important algorithms, aperture detection and feature detection. In the aperture detection algorithm, aperture of 3-D scanned virtual ear impression and parameters relating to ear shell of hearing aid are determined. The feature detection algorithm detects first bend, second bend, and related parameters. Through these two algorithms, parameters for aperture, first bend, and second bend are extracted to model the ready-made ear shell of hearing aid. The values of these extracted parameters from 36 people's right ear impression are analyzed and measured statistically. As a result of the analysis, it has been found that it is possible to classify ready-made ear shell parameters by age and size. The ready-made ear shell parameters are classified 3-size for 20 years old and 2-size for 60 years olde. Using 3D rhino program, virtual ready-made ear shell is reconstructed by parameters of every type, and simulated to model it. A final product was produced by transferring simulation result with rapid prototyping system. The modeled ready-made ear shell is evaluated with the objective and subjective method. Objective method is the comparison volume ratio and overlapped volume ratio of ear impression from randomly chosen 18 people and ready-made ear shell. And subjective method is that the final product of ready-made ear shell is used by users and the satisfaction number drawn from well fitting and comfortable testing was evaluated. In the result of the evaluation, it has been found that volume ration is 70%, big and middle size ready-made ear shell products are possible, and the satisfaction number is high.

  • Objectives : It has been known that immune reaction plays an important role in the pathogenesis of otitis media (OM). We investigated the change of middle ear mucosal inflammation induced by lipopolysaccharide (LPS) following administration of Taglisodog-eum (TSE) in experimental animals. Materials and Methods : Otitis media was induced by injecting 1 mg/kg of Pseudomonas aeruginosa LPS transtympanically in 60 rats. These rats were divided into two groups, the LPS group (n=30, remained naive after OM elicitation) and the TSE group (n=30, treated with TSE after OM elicitation) and 6 additional rats were kept as a normal control group (n=6, remained naive until tissue collection). The rats were killed at the 1st, 3rd, and 7th days after challenge. The temporal bones in each group were harvested and examined histopathologically by hematoxyline-eosin stain. Middle ear mucosa were taken at the 1st, 3rd, and 7th days after challenge. The levels of spicing variants of TNF-${\alpha}$ transcription were evaluated by Realtime-PCR. Results : TSE suppressed LPS-induced TNF-${\alpha}$ mRNA expression and thickness of the submucosal layer and infiltration of inflammatory cells in rat middle ear epithelium. Conclusion : The results suggest that TSE may be effective in decreasing inflammation with particular application to mucosal metaplasia in OM.

Effects of the Nei-Guan Acupressure by Wrist Band on Postoperative Nausea and Vomiting after Middle Ear Surgery (손목밴드를 이용한 내관지압이 중이수술 후 오심과 구토에 미치는 효과)

  • Purpose: This study was to identify the effects of Nei-Guan accupressure using a wrist band on postoperative nausea and vomiting after middle ear surgery. Methods: The study design was a nonequivalent control group non-synchronized posttest design. Forty patients were assigned into two groups of 20 patients, the wrist band group and the control group. In the wristband group, acupressure was applied with a wrist band on the P6 point at both wrists from 30 min before to 24 hr after anesthesia. Results: The 1st hypothesis 'Applied wrist band groups will have a greater reduction in the nausea & vomiting grades by INVR than the control group' was accepted (t=2.303, p=.028). The 2nd hypothesis 'The Applied wrist band groups will have a greater reduction in the occurrence of nausea & vomiting than the control group' was partly accepted. The 3rd hypothesis 'The applied wrist band group will have a greater reduction in injection of antiemetics for 24 hr. post anesthesia than the control group' was rejected. Conclusion: Acupressure on the P6 point using a wrist band is effective to alleviate nausea for middle ear surgery patients.

  • There are varying opinions about theories that the middle ear of Neonates is filled with mucous or other Material that interference with normal mobility of the middle ear system. DeDMon reported on the acoustic reflex as a tool for Neonatal screening, and Keith investigated the Middle ear function of Neonates during first few hours of life with an Electroacoustic impedance bridge. There are no reports on Neonstes in Impedance Audiometry in Korea. We tested 100 Neonates less than 24 hours old and this paper is dealt with its study related with tympanogram, static compliance and acoustic reflex.

Inflammatory Polyps in 2 Cats (두 마리 고양이에서의 염증성 용종 예)

  • Inflammatory polyps in feline ear are nonneoplastic, inflammatory growths that arise from the middle ear or the eustachian tube and extended into the pharynx or external ear canal. Two 2-year-old female Russian blue cats showed 2-3 weeks history of aural discharge, crust formation in external ear, and head or ear shaking. Two masses were surgically excised from ear canal, and submitted for diagnosis. Histopathologically, these masses were covered with hyperplastic ciliated epithelium or nonkeratinizing squamous epithelium with partial erosion and ulceration. The core of masses was consisted of proliferated connective tissue and massive infiltration of mononuclear cells. Immunohistochemically, about 90% of infiltrated mononuclear cells demonstrated CD3 positive T cell. According to both polymerase chain reaction (PCR) and reverse transcriptase-PCR, tissues samples were negative for feline viral pathogens. Based on the clinical, gross, histopathologic findings, these two cases were diagnosed as inflammatory polyps originated from the middle ear in cats.

Molecular Biological Identification of Bacteria in Middle Ear Effusion Using 16S rDNA Multiplex PCR (중이 삼출액 미생물의 16S rDNA 복합중합효소연쇄반응을 이용한 분자생물학적인 진단)

  • The rapid and reliable 16S rDNA multiplex polymerase chain reaction (PCR) assay was established to characterize bacterial etiologies of middle ear effusion. These etiologies included Haemophilus influenzae, Moraxella catarrhalis and Streptococcus pneumonia, which were detected in middle-ear effusion (MEE) samples taken from patient with otitis media. A total of 39 MEE samples were aspirated from 26 patients. DNA was extracted from MEE samples, and PCR was done with DNA extracts by using the common primers, which is localized at C4 region in the 16S rDNA gene of all bacterial species, and species-specific primers: (i) Haemophilus-specific primer, (ii) Moraxella- specific primer, and (iii) Streptococcus-specific primer. Among 39 samples tested, 24 (61.5%) were positive for H. influenzae, 10 (25.6%) were positive for M. catarrhalis, 3(7.7%) were positive for S. pneumonia, and 11 (28%) were negative for 165 rDNA multiplex PCR reaction. Nine samples (28.6%) exhibited a mixed infection and were positive for both H. infuenzae and M. catarrhalis. We suggested that 16S rDNA multiplex PCR is a useful method to identify rapidly for rapid identification of the pathogenic bacteria and characterization of bacterial etiologies of middle ear effusion.

The External Auricular Reconstruction with Inferior Based Retroauricular Flap Including the Posterior Auricular Artery

  • Purpose: The external ear is a common area of trauma on the body prone to exposure of ultraviolet light, which can lead to skin cancer. Thus, variable techniques have been developed and used for reconstruction of the external ear. The aim of this study is to review the surgical method, its area of application, as well as advantages and pitfalls of reconstruction of the external ear with inferior based retroauricular flaps. Materials and Methods: Eight patients underwent external ear reconstruction with inferior based retroauricular flap for external ear defects in our institute from September 2012 to June 2015. According to the area of the defect, patients were classified as middle 1/3 (n=4), inferior 1/3 (n=2), superior auroculo-cephalic sulcus (n=1), and external auditory canal (n=1). Results: All of the flaps survived the operation and there was no marginal necrosis. Mean size of the defect was $2.8{\times}1.8cm$ and mean size of the retroauricular flap was $5{\times}2cm$. For insetting of the flap, a subcutaneous tunneling technique was used in 6 cases and rotation without subcutaneous tunneling was used in 2 cases. Transient paresthesia occurred in 3 cases. Two cases recovered within 3 months but one case did not recover until 6 months. Conclusion: The inferior based retroauricular flap is an available technique in external ear reconstruction with one stage operation.

Implementation a Physical Ear Model for Determinating Location of the Microphone of Fully Implantable Middle Ear Hearing Device (완전 이식형 인공중이용 마이크로폰의 위치 결정을 위한 물리적 귀 모델의 구현)

  • Generally, implantable microphone has been implanted in the temporal bone for implantable middle ear hearing devices (IMEHDs). In this case, the microphone's membrane can be damaged and can be generated biological noise. In order to overcome the these problems, the location of implanted microphone should be changed. As an alternative, the microphone can be implanted in the external auditory canal. However, the sound emission can be produced because of vibration transducer toward reverse direction from the tympanic membrane to the external auditory canal. In this paper, an amount of the emitted sound is measured using a probe microphone as the changing the position of microphone in the external auditory canal of a physical ear model, which is similar to acoustical and vibratory properties of the human ear. Through the measured value, the location of the microphone was assumed in the external auditory canal. According to the analysis, the microphone input sound can be decreased when microphone position become more distance from the tympanic membrane in the auditory canal. However, the external auditory canal is not appropriated to implantable microphone position, because sound emission is not completely eliminated.

  • The differential electromagnetic transducer for IME(implantable middle ear) system, which have two small permanent magnets glued with the same pole facing each other in the coil, has high vibration efficiency and does not influenced by external magnetic field. In this paper, using finite element method, highly efficient structure of the transducer was proposed and vibration force of the transducer was calculated by electromagnetic theory. And the necessary vibration force of transducer to transmit the sound signal to inner ear when the transducer is attached at stapes was calculated and the design parameters of the transducer were investigated. Using the parameters, the differential electromagnetic transducer was manufactured in small size to implant in confined human middle ear. And it was examined by unloaded and loaded vibration experiment using temporal bone sampled from cadaver.
