A Case of Mucosa Associated Lymphoid Tissue Lymphoma in Accessory Parotid Gland (부이하선에 발생한 점막관련 림프조직 림프종 1예)

  • The tumor in accessory parotid gland (APG) is rarely occurred and its incidence is about 7.7% of all parotid gland neoplasms, but has a higher frequency of malignancy than major salivary glands. The mucoepidermoid carcinoma is the most common malignancy in APG, while B-cell lymphoma is less than 2%. It is often appeared as superficial mass in mid-cheek area. This lesion requires differential diagnosis with epidermoid cyst, lipoma, neurogenic tumors, Stensen's duct stone, lymphadenopathy and hemangioma etc. The mucosa associated lymphoid tissue (MALT) lymphoma, which is also termed extra-nodal marginal zone B-cell lymphoma tends to be localized disease for long time and has a relatively indolent course. We recently encountered a 69-year-old man with superficial solitary mass on the right cheek area that finally diagnosed as MALT lymphoma in APG. We report the rare and unique case with brief literature review.

Duplication of the Esophagus -Report of A Case- (식도중복증의 1례)

  • Duplication of the alimentary tract, especially of the esophagus, have been regarded as rare cong- enital anomalies. However, they are being reported with increasing frequency in the literature. In the . ,- past they have been described by a variety of names, such as "enteric cysts", "intestinal cysts", "giant diverticula", "`mediastinal cysts of foregut origin" "enterogenous cysts" and other descriptive terms. . Most authorities now agree that these anomalies are best described by the term "duplications of the alimentary tract. The duplications [of the alimentary tract] are spherical or tubular structures which poses a well developed smooth muscle layer and are lined with a mucous membrane from any part of the alimentary tract. They may occur at any place in the digestive tube from the tongue to the rectum and usually are intimately attached to some portion of the alimentary tube. We have experienced a duplication of the esophagus in 14 years old middle schoolboy. He complained dysphagia, eructation and substernal pain associated with intermittent high fever and chilliness, increasing in severity for recent three weeks. Routine chest X-ray film revealed nore markable abnormal finding but esophagogram. revealed marked narrowing of the esophagus throughout with a large blind pouch in lower half with fistulous communication at mid portion of the esophagus. On thoracotomy, a large infected blind pouch communicating with the lumen of normal esophagus proximally, Was extended from the level of 5th to 10th thoracic spine. The duplicated segment of the esophagus has a common muscular .wall and proximal communication with the adjacent esophagus. The infected, duplicated esophagus was segmentally resected, and esophagogastrostomy with pyloroplasty was done by displacing the stomach into the right thoracic cavity through midline laparotomy. His Postoperative course was uneventful and discharged without complication.

Intestinal duplication revealed by posterior reversible encephalopathy syndrome

  • We report a unique case of intestinal duplication detected on posterior reversible encephalopathy syndrome (PRES) in a 13-year-old girl. She was admitted to the pediatric Emergency Department because of generalized seizures. Radiological assessment revealed a large, well-defined, thick-walled cystic lesion in the mid abdomen, suggestive of duplication cyst associated to a PRES. Exploration confirmed the diagnosis of ileal duplication cyst, and the mass was resected. The postoperative course was uneventful. Both hypertension and neurological dysfunction resolved after the mass resection. A follow-up brain magnetic resonance imaging was performed 9 months later and showed complete resolution of the cerebellar changes. Although extrinsic compression of the retroperitoneal structures has not been reported in the literature as a complication of duplication cyst, we strongly believe that this is the most logical and plausible hypothesis that would explain the pathogenesis of PRES in our patient.

Prosthetic Correction for Coarctation of the Aorta: Report of A Case (대동맥 Coarctation 수술 1예)

  • Coarctation of the aorta is rare condition among the congenital cardiovascular defects in Korea. We experienced one case of coarctation of the aorta [postductal type], which was successfully corrected with Dacron graft. This 20 year old man was admitted to our hospital for evaluation of hypertension in the upper extremities and frequent occipital headache of 2 month duration. On physical examination, grade II systolic murmur was heard over the apex and mid back, but thrill was not palpable. Blood pressures were measured at both extremities, its values were 190/70mmHg. in the arms and 120/60mmHg in the legs. Routine chest roentgenogram showed the dilated left subclavian artery high on the left mediastinal border and associated with left ventricular prominence, but notchings of lower border of rib was not seen. Electrocardiogram also showed left ventricular hypertrophy. Final Pre-operative diagnosis was made by the aortic catheterization and aortography, which showed the typical configuration of postductal type of coarctation with poststenotic dilatation of aorta. Under the moderate hypothermia by surface cooling, coarcation was resected 3.5cm in length and then Dacron graft was inserted. After prosthetic correction, blood pressures were recorded 120/60mmHg in arms and 160/120mmHg in legs. The post-operative course was uneventful.

  • Up until the mid-90s, the housing market of Korea has been supplier-oriented with the constant shortage of housing supply. But today, with the surplus of supply over demand and ever-increasing competitiveness in the housing market, understanding consumers needs have become one of the keys to successful business. Consumers overall living standard have increased and they are now looking for better living environment. As a matter of course, there have been much studies focusing on the evaluation factors and consumers preferences of the living environment, somewhat overlooking the asset value of a residential property. Consumers see their house in terms of both a living space and an asset. Therefore, it is these two values altogether that influences the buying decision process of consumers. This study seeks to stress on the importance of the fact that it is both the living environment and asset value of a house that are considered in the buyers valuation of residential property. And the study suggests that influencing factors of housing attributes in the decision-making process of buyers be sub-categorized into two broad groups, value as a living space and value as an asset. This is to serve both the researcher in the academic field and marketers for better understanding of consumers needs and influencing factors of buyers valuation of housings vis-a-vis market.

Salvage of Unilateral Complete Ear Amputation with Continuous Local Hyperbaric Oxygen, Platelet-Rich Plasma and Polydeoxyribonucleotide without Micro-Revascularization

  • In many cases of complete ear amputation, microvascular surgery is required for tissue perfusion and organ survival. However, microvascular reconstruction is not always feasible in the absence of suitable vessels. Here, we present the case of a 76-year-old man who underwent complete amputation of the left ear after a collapse at home because of cardiogenic syncope. He was treated with primary replantation and underwent a postoperative salvage course including continuous local hyperbaric oxygen therapy (HBOT), platelet-rich plasma (PRP) injections, and polydeoxyribonucleotide (PDRN) injections. The ear was almost completely salvaged, with a tiny eschar at the mid-scapha on both the anterior and posterior aspects. This case demonstrates the efficacy of local HBOT with PRP and PDRN injections.

A Study on Species of Indigo Genus Found in Chosen Dynasty (조선시대 남종에 관한 연구)

  • In several literature in China were recorded various species of the indigo genus, but in Chosen documents, the two have been intensively mentioned, that is, Polygonum tinctorium (PT) and liatis Tinctuna(IT). Allowing for some slight contradictions of the records between the two countries, we insist that J)T is the aborigines historically longer than any other indigo plant in Korea and that IT is the one Implanted from China in late Chosen period. Indigos can be grouped into two major categories : 'Chon' or Indigo forming sediment in the course of making, and 'Nam' or the one without. The dyestuffs of blue, or dark blue tineged with red, which had been ocasionally recorded until the mid Chosen, could be made from the . species without sediment. The period when the color thus obtained was prevalent can be traced back to the era of Yongio, when the import of blue-dyed textiles from China was prohibited to encourage the domestic dyeing industry. However, a more clarification is needed on this matter, since all of the previous researches are arguing, with little validity, that the indigo genus in Korea be PT. Judging from the documents recording that PT did not form any sediment, and that from it was obtained only light color like indigo, it is a matter of re-discussion in terms of botanical taxonomy to define as PT the species being cultivated in some areas in Chollanam-do. In conclusion, a joint research, including specialists in traditional dyestuffs and in botany in relation to the taxonomical problem of the indigo genus, would be expected for further Investigation on this matter.

  • A Scanning Electron Microscopic Study of the root surface changes was carried out in advanced periodontal disease. The results obtained were summerized as follow; 1. The root surface of cementoenamel junction of healthy root showed smooth surface, while the root surface of diseased state showed covering of deposits which regareded as a course of calcification. 2. At the mid-portion of the root, the regular cemental projection were observed on the healthy root surface and on the root surface of diseased state, cemental projection showed relatively irregular pattern. 3. On the root surface which consist of wall of periodontal pocket, there were various deposits which could be considered as subgingival calculus, bacterial plaque and epithelial attachment remnant. 4. The bottom area of the pathologic pocket, tearing altered collagen fibers were seen on the root surface. 5. At the apical portion of root surface, calcified fibers ran parallel to the root surface in healthy tooth and the fiber bundles of periodontal ligament were seen in the diseased state.

  • Purpose : The purpose of this study was conducted to find out observation at gait analysis of the stroke patient with proprioceptive neuromuscular facilitation(PNF) concept. Methods : This is a literature study with books, seminar note and international PNF course book. Results : Stroke patient gait was poor initial contact by weakness of tibialis anterior or weakness of contralateral plantar flexor, poor loading response by loss of deep sensation, poor mid stance by loss of deep sensation, weakness of tibialis anterior and weakness of plantar flexors eccentric control, poor terminal stance, pre-swing, initial swing by loss of deep sensation and stiffness fo deep toe flexors. Conclusion : Stroke patient gait determine on loss of mobility, pain, fear, trunk muscle weakness, loss of coordination, loss of deep sensation, neglect and apraxia. Therefore observational gait analysis of the stroke patient focus on gait cycle and take out hypotheses from the gait cycle. These hypotheses have to define accept or not by parameters. Treatment plan made with the hypotheses.

  • The thery of measure is significant in that we extend from it to the theory of integration. AS specific metric outer measures we can take Hausdorff outer measure and Lebesgue-Stieltjes outer measure connecting measure with monotone functions.([12]) The purpose of this paper is to find some properties of Lebesgue-Stieltjes measure by extending it from $R^1$ to $R^n(n{\geq}1)$ $({\S}3)$ and differentiation of the integral defined by Borel measure $({\S}4)$. If in detail, as follows. We proved that if $_n{\lambda}_{f}^{\ast}$ is Lebesgue-Stieltjes outer measure defined on a finite monotone increasing function $f:R{\rightarrow}R$ with the right continuity, then $$_n{\lambda}_{f}^{\ast}(I)=\prod_{j=1}^{n}(f(b_j)-f(a_j))$$, where $I={(x_1,...,x_n){\mid}a_j$<$x_j{\leq}b_j,\;j=1,...,n}$. (Theorem 3.6). We've reached the conclusion of an extension of Lebesgue Differentiation Theorem in the course of proving that the class of continuous function on $R^n$ with compact support is dense in $L^p(d{\mu})$ ($1{\leq$}p<$\infty$) (Proposition 2.4). That is, if f is locally $\mu$-integrable on $R^n$, then $\lim_{h\to\0}\left(\frac{1}{{\mu}(Q_x(h))}\right)\int_{Qx(h)}f\;d{\mu}=f(x)\;a.e.(\mu)$.
