EFFECT OF INDUCTION CHEMOTHERAPY ON FLAP SURVIVAL RATE IN MICROSURGERY (종양수술전 화학요법이 미세수술시 피판생존율에 미치는 영향)

  • Purpose : Neoadjuvant chemotherapy is commonly used to treat cancer patients as adjunct treatment, but if the microvascular tissue transfer is performed simulataneously with cancer resection surgery, the induction chemotherapy might affect the survival rate of vascularized free flap. Our study will focus on the effect of induction chemotherapy on the free flaps which were made on white rat abdomen after injection of 5-FU. Materials and Methods: The experimental rat groups were divided into three groups (total 24 rats) as a normal control group, 24 hrs group after 5-FU injection, 3 days group after 5-FU injection. Inferior abdominal island flaps of 8 Sprague Dawley rats on each group were made and immediately were induced into an ischemic state by clamping the supplying inferior epigastric artery and vein with microvascular clamp for a hour to induce a similiar free flap circumstance, then the inferior abdominal skin flaps were reperfused by releasing the clamps. The flaps on abdomen were repositioned and sutured. The experimental data for flap survival rate was collected by digital photo taking, analysed by computer image program to compare with the flap luminosity. The rats were sacrificed at 3 days, 5 days, 7 days after flap preparation and specimens of the flap were taken and stained with H-E staining. The microscopic finding was made under magnification of 200 and 400. Results: 1. Gross findings on each groups showed the healing condition was good as following sequences; normal, 24 hrs group after chemotherapy, 3 days group after chemotherpy. 2. The values of flap luminosity for evaluation of flap survival rate also showed the same sequences as gross findings of healing state. 3. The microscopic findings of epidermis necrosis, inflammation state, dermis fibrosis, vessel change, fatty tissue layer thinning were compared with each group. The 3 days group after chemotherapy showed remarkably poor healing condition compared to other groups. Conclusion: Chemotherapy agents affected the healing process of free flap, but healing condition was recovered spontaneously as post-injection periods passed out. In opposite to our expectation, 3 days group showed the bad flap condition in comparing with 24 hours group which was considered as immatured body circulation state of chemotherapy agent. It showed that 3 weeks in human being after chemotherapy was not proper as timing of microvascular tissue transfer if 3 days group in rat was considered as same healing period of 3 weeks in human being. More delayed healing timing than 3 weeks might be required in clinical application of free tissue transfer.

The Ultrasonographic Images of Normal Uterus in Korean Native Cow (한우 정상 자궁의 초음파적 관찰)

  • Ultrasonographic examination was performed to observe the ultrasonographic image of Korean native cows' normal uterus in condition of in vitro and in vivo. The experiment was done 28 slaughtered cows' uterus using immersed in water in vitro, and 41 healthy breeding cows taken rectal ultrasonography in vivo. Ultrasonographic examination of uterine was taken on the reference of cross section of intercornual ligaments' cranial. Each uterus on the experiments was compared by estrous cycle and ultrasonographic frequency. The uterine structure using ultrasonography was 5 layers of uterine horn in vivo as well as in vitro. Uterine horn was observed to be distinguished from inside to outside as endometrium to inner echogenic layer, circular muscle layer to slightly echogenic elliptical layer, stratum vasculare to central echogenic layer, longitudinal muscle layer to slightly echogenic arched layer, and perimetrium to outer echogenic layer, respectively. According to the observation of uterus related to estrous cycle and ultrasonographic examination, uterine endometrium in vitro was constantly founded irrespective of estrous cycle and ultrasonographic frequency. On the low frequency, endometrium and circular muscle layer in estrus were prone to distinguished than in diestrus. On the high frequency, endometrium and circular muscle layer were always distinguished regardless of estrous cycle. In vivo, uterine endometrium and circular muscle layer were observed regardless of estrus and ultrasonographic frequency. On the low frequency, stratum vasculare and longitudinal muscle layer were not likely to be distinguished in diestrus, but estrus. On the high frequency, stratum vasculare and longitudinal muscle layer were observed regardless of estrous cycle. Also, every uterine structure was easily distinguished on high frequency than low frequency owing to precision of distinction in layers. The difference of results followed by the experiments conditions between in vitro and in vivo was that uterine endometrium and circular muscle layer in diestrus in vitro were difficult to be distinguished and uterine lumen was observed during whole estrous cycle. In vivo, It was founded that the distinction of stratum vasculare and logitudinal muscle layer in diestrus was complicated and uterine lumen was observed during only estrus. In view of the result so far achieved, normal uterine structure divided in 5 layers on ultrasonography was accorded with microscopic organization, uterine structure was likely to be observed during estrus than diestrus, high frequency checkup than low frequency, and uterine endometrium, circular muscle, stratum vasculare was easily observed regardless of estrous cycle and ultrasonographic frequency.

Observation of Inflammatory Marker Levels in Sprague-Dawley Rats with Youngyopaedoc-san-related Anti-atherogenic Effect (연교패독산(連翹敗毒散) 복용 후 항동맥경화 효과가 나타난 백서에서 염증 지표 관찰)

  • Objectives: Sprague-Dawley rats were fed with high fat diet, and atherogenic changes were seen in the aorta. However, when Sprague-Dawley rats were fed with a high fat diet and administered Youngyopaedoc-san together, these atherogenic changes were rarely seen. This study was aimed to find the inflammatory marker level changes in Sprague-Dawley rats with Youngyopaedoc-san-related anti-atherogenic effect. Methods: The extract from Youngyopaedoc-san was made by the pharmacy department of Kyung-hee Oriental Medical Hospital. The animals were divided into five groups: normal diet, high fat diet, high fat diet with Youngyopaedoc-san, high fat diet with Vytorin, and high fat diet with Youngyopaedoc-san and Vytorin. A light microscopic image of a cross section taken from the aorta of the Sprague-Dawley rat was analyzed. We compared inflammatory marker levels among the five groups. Results: The complex of Youngyopaedoc-san and Vytorin has more anti-atherogenic effects in the aorta of Sprague-Dawley rats fed with high fat diet than Vytorin alone. Youngyopaedoc-san has inhibitory effect on the increase of IFN-${\gamma}$ and IL-2 levels. The difference on eosinophil levels of each group was statistically significant, but the eosinophil level of each group was within normal limits, so the difference on eosinophil levels was not clinically significant. Conclusions: Youngyopaedoc-san-related anti-atherogenic effect could be a result of inhibitory mechanism on IFN-${\gamma}$ and IL-2.

A Study on the Improvement of Legal Policies for Activating Public Open Space in Urban Environment Improving Project of Seoul City (서울시 도시환경정비사업 내 공개공지 활성화를 위한 제도개선에 관한 연구)

  • The district units planning suggests not only detailed standards of building as parts of the whole plan in the district but also a guideline of the arrangement, shape and pavement of the public open space, in order to move away from the rigid image of an institution which causes the formal creation of it. This triggered various problems. While the basic plan for adjustment of urban environments(established in 2005) was being reorganized in order to reflect changed conditions of the city center, the sector of public open space was subdivided according to the guidelines of development by district. We conducted a comparative analysis of conditions before and after the adjustment, so as to examine whether or not the reorganized institution is effective for the plan of public open spaces. The analysis showed that there was no remarkable change between the basic plan and the reorganized institution in that items which were not any different from the existing building codes were established due to lack of guidelines related to public open space under the law of urban and residential environmental improvement. Low regulations should set up criteria for establishment from a concrete, effective microscopic aspect appropriate to the significance of public open space by the linking of public open space and the surrounding environment, whereas high regulations such as building codes should specify essential components.

The Neurotoxicological Alterations Induced by Narcotic Drugs and Industrial Chemicals in the Rat are Associated with Quantitative Changes in Glial Fibrillary Acidic Protein (마약류 및 산업환경화학물질에 의한 GFAP의 신경독성표지물질화에 관한 유용성)

  • Diverse neurotoxic insults result in proliferation and hypertrophy of astrocytes, a subtype of glia in central nervous system. The hallmark of this response, often terms "reactive gliosis", is the enhanced expression of the major intermediate filament protein of castrocytes, glial fibrillary acidic protein (GFAP). These changes in the astrocytes suggest that GFAP may be a useful biochemical indicator of neurotoxicity. To investigate this possibility, we administered intra-peritoneally prototype nerotoxicants, metharnphetamine (MAP, 5 mg/kg), cocaine (30 mg/kg), N-buthyl benzenesulfonamide (NBBS, 300 mg/kg) and trimethytin (TMT, 8 mg/kg) to Wistar Rats and then assessed the effects of these agents on content of GFAP, which were determined by Sandwish ELISA and evaluated with neurotoxic symptoms, and quantitative changes of imrnunoreactivity of GFAP by light microscopic image analysis in specific regions. We found that assay of GFAP revealed time- and region-dependant patterns of neurotoxicity. The GFAP immunoreactivity of rat brain was increased in substantia nigra and hippocampus by MAP, NBBS and TMT; in roedial septal nucleus and nucleus accurnbens, it was also increased by RrBBS. Sandwich ELISA showed that GFAP levels of cerebrum in all groups on days 3 and 7 and that of brainstem(including cerebellum) in MAP, NBBS groups on day 1 and 3 were increased. A review of the background, design and results of these experiments are presented in this paper. Our findings indicate that GFAP is a sensitive and specific biomarker of neurotoxicity.otoxicity.

Variation of Meat Quality Parameters Due to Conformation and Fat Class in Limousin Bulls Slaughtered at 25 to 27 Months of Age

  • The aim of the present study was to determine the influence of age of animal, hot carcass weight, pH, conformation and fat class on basic beef quality attributes (tenderness, sarcomere length, basic chemical composition, marbling and colour) in a group of purebred animals. The object of the study was beef of Limousin bulls (25 to 27 months, hot carcass weight - $432{\pm}31$ kg, "U" conformation class, "2"-"3" fat class). Analysed cuts were Infraspinatus muscle from the blade and Longissimus dorsi muscle from the cube roll and the striploin. Tenderness was analysed with universal testing machine, colour - chromometer analysis, sarcomere length - microscopic method, basic chemical composition - near-infrared spectroscopy and marbling - computer image analysis. No differences in tenderness and sarcomere length were observed within the age groups of Limousin bulls (age of 25, 26, 27 months) (p>0.05). Moisture (p = 0.0123) and fat (p = 0.0250) content were significantly different for meat of animals slaughtered at the age of 25 and 27 months. No influence of pH value on tenderness was observed, but at the same time, influence on sarcomere length (p = 0.039) and $b^*$ component of colour (0.045) was found. For "U" conformation class, in subclasses, as well as for fat classes "2"-"3", there were no differences in tenderness, sarcomere length and colour components. Higher fat content was observed in the higher fat class, rather than in lower, however this feature was not associated with marbling.

A Scanning electron microscopic study of enamel surface by debracketing of ceramic bracket (도재브라켓의 제거방법에 따른 법랑질표면의 주사전자현미경학적 관찰)

  • The purposes of this study were to evaluate and compare the frequency of ceramic bracket fracture, frequency of enamel fracture, bond fracture site, adhesive remnant index after mechanical and electrothermal debracketing, to evaluate effectiveness of high and low speed rotary instrument and ultrasonic instrument during residual adhesive remnants removal, and to measure resin film surface(percentage) using by image analyser(Leco 300). Bond fracture site, bracket fracture, and enamel surface damage were examined by scanning electron microscope. The following results were obained : 1. In the mechanical debracketing group, the bond failed predominantly at enamel-adhesive interface with the bulk of adhesive remaining on bracket base. 2. In the eletrothermal debracketing group, the bond failed predominantly at adhesive-bracket interface with the bulk of adhesive remaining on enamel surface. 3. The most effectiveness of residual resin removal was obtained by means of the resin polishing bur and the order of scratch formation was the procedure using tungsten carbide bur, ultrasonic scaler, sof-lex disc, and polishing bur. 4. The order of the resin film surface percentage was ultrasonic scaler, tungsten carbide bur, sof-lex disc, and resin polishing bur.

A STUDY ON THE PHYSICAL PROPERTIES OF GLASS IONOMER CEMENT FOR RESTORATIVE FILLING USING VISIBLE LIGHT POLYMERIZATION (가시광선중합화에 따른 충전용 Glass Ionomer Cement의 물리적 성질에 관한 연구)

  • The aim of this study was to investigate the physical properties of visible light curing Glass Ionomer cement for restorative esthetic filling. The control group was the autopolymerizing GC Fuji II Glass Ionomer cement (2.2: 1 P/L ratio) and the experimental groups were made by following procedure. To induce the polymerization by visible light, the powder of GC Fuji II GI cement and the liquid of Vitrabond for base & liner were mixed in an amalgam capsule with 2.5:1, 3.0:1, 3.5:1 P/L ratio (% wt/wt). After fabrication of specimens, compressive strength, fracture toughness ($K_{IC}$) Scanning Electron Microscope and X-ray Diffraction, water-leachable content, marginal leakage and surface roughness were studied. The results were as follows: 1. Only experimental No. 1 group (visible light curing) showed less compressive strength than control group 1 hour after curing. Strength was increased with aging in all groups, so the compressive strength of light curing groups was no less than that of autopolymerizing group after 3 weeks. 2. Experimental No.3 group (visible light curing) was inferior to No.2 group (visible light curing) in fracture resistance but light curing groups were more resistant to fracture than autopolymerizing group and showed ductile fracture pattern as compared with the brittle fracture pattern of autopolymerizing group. 3. From scanning electron microscopic image, various sized unreacted powder particles, surrounded by silica gel, were embedded in polysalt matrix. Light curing groups showed little crack and more dense unreacted particles than autopolymerizing group. 4. From X-ray diffraction analysis, GC Fuji II Glass Ionomer cement powder and all groups showed glassy appearance but light curing groups seemed to be more intensive in crystaline than autopolymerizing group. S. The most significant dissolution was shown in early setting period in all group. Light curing groups were dissolved less than autopolymerizing group. 6. Marginal leakage was not different significantly in case of cavity margin composed of same tooth structure (ex. only enamel margin, only dentin margin) but much more leakage was shown in dentin/cementum margin than enamel margin. In only case of only enamel margin, light curing groups were superior to autopolymerizing group. 7. All groups showed relatively smooth surface, which irregularity was less than $1{\mu}m$. Light curing groups were smoother than autopolymerizing group.

In vitro study of compressive fracture strength of Empress 2 crowns cemented with various luting agents

  • All-ceramic restorations have had a more limited life expectancy than metal ceramic restorations because of their low strength. Their relatively lower strength and resistance to fracture have restricted the use of all-ceramic crowns to anterior applications where occlusal loads are lower. But there has been increasing interest in all-ceramic restorations because patients are primarily concerned with improved esthetics. Many efforts have been made to in prove the mechanical properties of dental ceramics. This study was designed to elucidate the influence of the luting agent on the strength of the Empress 2 crown (staining technique) cemented on human teeth. Seventy extracted human permanent molar teeth were chosen. Teeth were prepared for Empress 2 crowns with milling machine on a surveyor. A dental bur was placed in the mandrel that was positioned so that the long axis of the bur was perpendicular to the surveyor base. Dimensions of the Empress 2 crown preparation were $6^{\circ}$ taper on each side, $1.5{\pm}0.1mm$ shoulder margin, and 4mm crown height. The luting cements used in this study were as follow: 1. Uncemented 2. Zinc phosphate cements (Confi-Dental) 3. Conventional glass ionomer cement : Fuji 1 (GC) 4. Resin-modified glass ionomer cements : Fuji plus (GC) 5. Adhesive cements : Panavia F (Kuralay), Variolink II (Vivadent), Choice (Bisco). Fracture test using Instron. The crowns were loaded in compressive force to evaluate the effect of these cements on the breaking strength of these all-ceramic crowns. A steel ball with a diameter of 4mm was placed on the occlusal surface and load was applied to the steel ball by a cylindrical bolt with a crosshead speed of 0.5mm per minute until fracture occurred. The fractured surface was examined using Scanning Electron Microscopic Image (SEM) to discover the correlation between fracture strength and bonding capacity. Within the limitation of this in vitro study design, the results were as follows : 1. fomentations significantly increased the fracture resistance of Empress ceramic crowns compared to control. Uncemented (206.9 N): ZPC (812.9 N): Fuji 1 (879.5 N): Fuji Plus (937.7 N): Choice (1105.4 N): Variolink II (1221.1 N): Panavia F (1445.2 N). 2. Resin luting agent, treated by a silane bond enhancing agents, yielded a significant increase in fracture resistance. In some of the Panavia F group, a fracture extended into dentin. 3. According to SEM images of fractured Empress crowns, the stronger the bond at both interfaces(crown and die), the more fracture strength was acquired.

BONE RESPONSE OF THREE DIFFERENT SURFACE IMPLANTS : HISTOMORPHOMETRIC, PERIO TEST VALUE AND RESONANCE FREQUENCY ANALYSIS IN BEAGLE DOGS

  • Statement of problem. The intial stability for osseointegration of implant has been an interesting factor. Especially, in the case of poor bone quality or immediately loaded implant, various strategies have been developed focusing on the surface of materials to improve implant fixation to bone. The microscopic properties of implant surfaces play a major role in the osseous healing of dental implants. Purpose. The aims of this study are to perform a histologic and histomorphometric comparison of the healing characteristics of three different surfaces and the comparison of resonance frequency analysis (RFA) values measured by $Osstell^{TM}$ and perio-test values (PTV) measured by Periotest. Material and methods. A total of 24 screw titanium implants (Dentium Co., Seoul, Korea) with 6mm in length and 3.4mm in diameter, were placed in the mandible of 4 beagle dogs. Implants were divided into three groups following the surface treatment methods: Group I is machined(control group). Group II is anodically oxidized. Group III is coated 500nm in thickness with hydroxyapatite(HA) by ion beam assisted deposition(IBAD) on the anodized oxidization. Bone blocks from 2 dogs were caught after 3 weeks of covered healing and another blocks from 2 dogs after 6 weeks. RFA values and PTV were measured right after insertion and at 3 and 6weeks. Histomorphometric analysis was made with Kappa Image Base System to calculate bone-to-implant contact (BIC) and bone area inside the threads. Pearson's correlation analyses were performed to evaluate the correlation between RFA and PTV, BIC and bone area ratio of three different surfaces at 3 and 6 weeks. Results. 1) In all surface treatment methods, the RFA values decreased and the PTV values increased until 6 weeks in comparison to initial values. 2) At 3 weeks, no significant difference was found from bone-to-implant contact ratio and bone area ratio of three different surface treatment methods(P>0.05). However, at 6 weeks, different surface treatment methods showed significantly different bone-toimplant contact ratio and bone area ratio(P<0.05). 3) In the implants with the IBAD on the anodic oxidization, significant difference was found between the 3 weeks and the 6 weeks bone area ratio(P<0.05). 4) Correlation was found between the RFA values and the bone area ratio at 3 and 6 weeks with significant difference(P<0.05). Conclusions. These results indicate that the implants with the IBAD on the anodic oxidization may have a high influence on the initial stability of implant.
