A Measurement Scale Development of Nursing Competence for Nurses Working in General Wards (일반병동 간호사의 간호역량평가 도구 개발)

  • Purpose: The purpose of this study was to develop a measurement scale of nursing competence for nurses working in general wards, and to test the validity and reliability of the scale. Method: A methodological study design was used, consisting of 3 steps as conceptual framework decision, measurement items development, and testing of validity and reliability. In order to verify content validity, 16 experts reviewed the concept of each competences and measurement items. The developed measurement items were tested from 301 nurses in one hospital. Factor analysis and item analysis were applied to establish the scale's construct validity and reliability. Result: The final scale consists of nine dimensions with 70 items. The overall scale reliability had a Cronbach's alpha of 0.98, and the demensions Cronbach's alpha ranged from 0.88 to 0.95. The factor loading of construct validity was from 0.40 to 0.83. The explained variance from the 9 extracted factors was 70.93% of the total variance, and final factors were labeled as critical thinking, resources management and professional development, ability of ward management, application of nursing process, ethical accountability, respectful consideration, patient centered approach, crisis management, and leadership. Conclusion: This measurement scale can be utilized to evaluate nursing competence for nurses working in general wards.

Characteristic Analysis of Urban Landscape Formed by Container Construction (컨테이너 건축이 만드는 도시경관 특성 분석)

  • Purpose: Various landscapes have been created in the process of changing containers, which were a tool of trade activities, to architecture, and today, containers have formed relationships with people as complex facilities and constantly create new cultures. In this paper, considering these container constructions from the viewpoint of the landscape, consider the meaning as one of landscape factors for creating cultural landscape. Method: In this study, a methodological framework was constructed based on various statistical data and literature materials, and analyzed texts called containers from the viewpoints of physical elements and active elements and analyzed them at the level of function. Based on literature materials, specifically, this study has handled with the cultural landscape as text, the functional dimension of urban design, and the characteristics of container construction. Result: This research analyzed the physical and active characteristics of the target area by utilizing the content of the field analysis and the contents of the interview, previous research, etc., and then content was analyzed in functional dimension. Through this process, the conclusion proposed by this research is to study the construction of a container and present the direction to analyze in the functional dimension showing the landscape characteristics that it brings forward.

  • We examine the response patterns to web survey with a series of experiments embedded in a survey of students at the Sookmyung Women's University. A sample of 960 students was sent e-mail invitation to participate in a internet survey, The response rate was 53.9% except partial and overall non-response. Methodological experiments included the use of a pre-notification, reminder notices as well as the type of questionnaire. These factors that manipulate the perceived burden of the task had an effect on the likelihood of accepting the survey invitation. This paper discusses the overall implementation and outcome of the survey, and describes the results of the imbedded design experiments. Also we compared the representative of self-selected and probability sample.

Development of a Computerized Telephone Triage and Consultation System for Patients Discharged with Ophthalmic Disease (눈질환자의 퇴원 후 증상관리를 위한 전화상담 전산 Triage 시스템 개발)

  • Purpose: This methodological study was done to develop a computerized telephone triage and consultation system for patients discharged with ophthalmic disease in order to provide more efficient practice guidelines for nurses, and evaluate the usability of the system. Methods: Development of the system consisted of six phases: strategic planning, analysis, design, implementation, evaluation, modification, and maintenance. Results: In the strategic planning phase, ophthalmic problems and nursing interventions of triage algorithms and practice guidelines were cross-mapped with the Omaha system. In the analysis phase, users requirements were identified. Then infrastructure including database, nursing knowledge base, and user interface were designed in the implementation phase. Usability and satisfaction of the system presented as very positive. Telephone consultation took about 2 minutes less than time in the previous system. The system was modified based on users' comments during the evaluation phase. Conclusion: This study was the first attempt in Korea to develop computerized triage system to prompt the quality of telephone consultation. It is suggestive that the computerized triage system may improve the quality of nursing.

Development of Needs Assessment Instrument for Hospice Care in Families of the Patients with Cancer (암 환자 가족의 호스피스 요구 조사 도구개발)

  • Purpose : The purpose of this study was to develop a tool to assess the need for hospice care in families of patients with cancer. Method : Research design was a methodological study. The tool was developed in 3 stages : first, preliminary items were developed based on a questionnaire about the needs for hospice care that was given to 8 families of patients with cancer; second, a panel of specialists reduced the number of preliminary items using 2 validity tests on the contents. Finally, reliability and validity were tested by a sample of 98 families who have a patient with cancer from April 2003 to July 2004. Result: Cronbach's alpha coefficient for internal consistency was .94 for the final total 22 items. Using the factor analysis, 4 factors with eigenvalue of more than 1.0 were extracted and these factors explained 65% of the total variance. The four factors were labeled as 'control of terminal physical problems', 'emotional care', 'spiritual care for preparing for death', and 'family support'. The final items of the tool developed on the need of hospice care consisted of 22 items. Conclusion : The instrument, for accessing the need for hospice care in families of patients with cancer, developed in this study was identified as a tool with a high degree of reliability and validity. In this sense, this tool can be effectively utilized for implementing and improving hospice care for patients with terminal cancer.

Methodological Problems in Information Retrieval Research (정보검색 연구의 방법론에 관한 고찰)

  • A major problem for information retrieval research in the past three decades has been methodology, even though some progress has been made in obtaining useful results from methodologically sound experiments. Within a methodology, potential problems include artificial data generated by the researcher, small sample size interpretation of findings. Critics have pointed out that some room exists for improving methodology of information retrieval research; using existing data, having big enough sample size, including large numbers of search queries, introducing more control in relation to variables, utilizing more appropriate performance measures, conducting rests carefully and evaluating findings properly. Relevance judgments depend entirely on the perception of the user and on the situation of the moment. In an experiment, the best judge of relevance is a user with a well defined information need. Normally more than two categories for relevance judgments are desirable becase there are degrees of relevance. In experimental design, careful control of variables is meeded for internal validity. When no single database exists for comparison, existing operational databases should be used cautiously, Careful control for the variations of search queries, inter-searcher sonsistency, intra-searcher consistency and search strategies is necessary. Parametric statistics requiring rigid assumptions are not appropriate in information retrieval research and non-parametric statistics requiring few assumptions are necessary. Particularly, the sign test and the Wilcoxon test are good alternatives.

Development of a Nasogastric Tube Feeding Algorithm to Prevent Aspiration Pneumonia (흡인성 폐렴 예방을 위한 비위관 영양 알고리즘 개발)

  • Purpose: The purpose of this study was developing a nasogastric tube feeding algorithm to prevent aspiration pneumonia. Methods: The algorithm was developed through a methodological design. First, a pilot study was performed to determine the incidence of pneumonia. The second step was development of a preliminary algorithm through a literature review and collection of nurse opinions. The third step was to establish content validity using a panel of 12 experts. The fourth step was revision of the algorithm. Next, 20 intensive care unit nurses applied the revised algorithm for six months to their actual treatment, and the practical feasibility was verified after that. Results: In the patients for whom this algorithm was applied, no cases of pneumonia occurred. The algorithm that was developed by the present author was suitable for clinical application. Conclusion: The effect and practical feasibility of the algorithm was tested with a few patients in this study. The effect of this algorithm should be examined by applying it to more patients on an ongoing basis.

Analysis of studies on Maekmundong-tang (맥문동탕(麥門冬湯)에 대한 연구 논문 분석)

  • Objectives: This study analyzed the contents of the research papers of Maekmundong-tang (MMDT). This study was conducted to help development of new clinical application and clinical studies for treating COPD with Oriental medicine. Materials and Methods: We inspected 26 theses and scrutinized their classification, objective diseases, study design, participants, methodological quality of clinical trial. Results: The following results were obtained in this study. 1. The studies of MMDT started in 1989 and have continuously increased, but it decreased recently. 2. The studies were mainly focused on experimental models rather than clinical studies. The topics of studies were mainly relaxation of airway contraction and anti-asthmatic effect. 3. MMDT was showed to have effects on chronic cough and asthma in these papers. Conclusion: MMDT is being used in respiratory disease. However, mechanism study should be conducted at experimental study and more clinical studies on the efficacy of MMDT for chronic respiratory disease are needed.

The Validity and Reliability of a Lifestyle Evaluation Tool for Patients with Metabolic Syndrome (대사증후군 대상자의 생활습관 평가 도구 개발을 위한 타당도와 신뢰도 검증)

  • Purpose: This study examined the validity and reliability to develop a lifestyle evaluation tool for metabolic syndrome patients. Methods: A methodological research design was used. The construct factors and preliminary items were identified by reviewing previous researches and tools related to lifestyle and reviewed by ten experts. It was tested with 195 patients with metabolic syndrome in a university hospital. The data were analyzed with SPSS/WIN 14.0. Results: To test the validity, principal component analyses were used and resulted in the extraction of six components. The convergent validity resulted r= .72 (p<.001) with Health Promotion Lifestyle Profile. The discriminant validity with Center for Epidemiologic Studies Depression Scale resulted r= -.15 (p=.004). The Internal consistency of the tool had an Cronbach's a of .92. The self-report format Lifestyle Evaluation Tool for the patients with metabolic syndrome was developed with 36 items and four-rating scales:'physical activity and weight control' eight items, 'dietary habits' sixteen items, 'drinking and smoking' three items, 'sleep and rest' two items, 'stress' three items, 'drug and health management' four items. Conclusion: This Tool will evaluate health behaviors in patients with metabolic syndrome. Also, it will contribute to the development of nursing intervention to improve the metabolic syndrome patients' lifestyle.

Development of a Tool to Measure the Need for Child Hospice Care in Families of Children with Cancer (암 환아 가족의 아동 호스피스 요구 측정도구 개발)

  • Purpose: The purpose of this study was to develop a tool to assess the need for child hospice care in families of children with cancer. Method: The research design was a methodological study. The tool was developed in 4 stages : first, preliminary items were developed based on a questionnaire about the needs for child hospice care that was given to 20 families of children with cancer; second, a panel of specialists reduced the number of preliminary items using 3 validity tests for the content; third, final items were selected from the results of a pre-test. Finally, from February to July 2004, reliability and validity were tested with a sample of 104 families who had a child with cancer. Results: The final tool on the need for child hospice care consisted of 22 items and Cronbach's alpha coefficient for internal consistency was .93. Using factor analysis, 5 factors were extracted and these factors explained 69% of the total variance. Conclusion: The instrument, for assessing the need for child hospice care in families of children with cancer, developed in this study was identified as a tool with a high degree of reliability and validity. In this sense, this tool can be effectively utilized for implementing and improving hospice care for children with cancer.
