양측 거골에 발생한 골연골종 (Osteochondromas of the Bilateral Tali)

  • Benign and malignant tumors are found in the foot, although the incidence is low. The most common bone tumor in the foot is osteochondroma, which is thought to develop in bones that form through the process of enchondral ossification. In particularly, osteochondromas in the foot mostly occur in metatarsal bones and phalanges. It is seldom found in talus. It is usually confused with osteophyte or enthesis. We report an osteochondroma case confirmed by roentgenographical and pathological investigation, after removing the masses form bilateral tali of a 19-year-old male patient presented with bilateral ankle joint pain for three years.

종골에서 발생한 골연골종 - 증례 보고 - (Osteochondroma of the Os Calcaneum - A Case Report -)

  • 골연골종은 가장 흔한 양성 골 종양으로, 성장판에서 기원한 명확하게 구별되는 연골모를 가지고 있다. 호발 연령은 20세 이하의 환자에서 주로 발생하며, 성장에 비례하여 종양의 크기가 커지다가 골격계의 성숙과 함께 종양의 성장이 멈춘다. 골연골종의 호발 부위는 주로 장 관골의 골간단부와 편평골에서 발생하나, 족부에서는 낮은 발생 빈도를 보인다. 족부에서는 대개 중족골과 족지골에 발생하며 종골에서의 발생은 매우 드문 것으로 알려져 있다. 본 교실에서는 15세 남아에서 발생한 종골 내측 족저부 조면에 발생한 단발성 골연골종을 경험하였기에 문헌 고찰과 함께 보고하는 바이다.

정상 한국 성인의 전족부에서 관찰되는 종자골과 부골의 종류 및 빈도 (Sesamoids and Accessory Bones of the Forefoot in Normal Korean Adults)

  • Purpose: To determine the locations and incidences of sesamoids and accessory bones of the forefoot in normal Korean adults. Materials and Methods: The plain radiographs of the forefoot of 400 volunteers, 200 males and 200 females, were taken. The three orthopedic surgeons participated in the radiographic evaluation independently. Persons who had history of foot problem or injury were excluded. Results: Sesamoids of first metatarsophalangeal joint were nearly always present; medial 99% and lateral 100%. The bipartite sesamoid was more common in medial (9.8%) and in female (17.0%) than in lateral (0.3%) and in male (2.5%). Sesamoids of the interphalangeal joint of the first toe were seen in 35.8%. Sesamoids of metatarsophalangeal joints in other four toes were rarely observed. Their incidences were 5% in second toe, 0.8% in third, 0% in fourth, and 2.8% (lateral) and 7.5% (medial) in fifth. Sesamoids of interphalangeal joint in four lesser toes were not observed except two cases (0.5%) in the fifth toe. Accessory bone was very rarely observed. Os vesalianum was observed in five feet (1.3%), Os intermetatarseum in 14 feet (3.5%), and Os cuneo-metatarsal I tibiale in none. Conclusion: We determined the location and incidences of sesamoids and accessory bones of the forefoot, and we expected this to help to diagnose the forefoot problem.

정상인에서 보행 속도가 발바닥의 최대압력분포에 미치는 영향 (Effects of walking speed on peak plantar pressure in healthy subjects)

  • Background : Many factors affect foot and ankle biomechanics during walking, including gait speed and anthropometric characteristics. This study examined the effect of walking speed on peak plantar pressure during the walking. Method : Thirty two normal healthy subjects (16 men, 16 women) were recruited. Peak plantar pressure was investigated using pressure distribution platforms (Pedoscan system) under the hallux heads of the first, second, and third metatarsal bones, and heel. Result : The results also suggest that slow walking speeds may decrease forefoot peak plantar pressure in patients with peripheral neuropathy who have a high risk of skin breakdown under the forefoot(p<0.05). Conclusion : The results also suggest that slow walking speeds may decrease forefoot peak plantar pressure in patients with restricted low extremity range of motion who have a high risk of skin breakdown under the forefoot.

한우 송아지에서 발생한 맥각중독 의증 2예 (Two cases of ergotism-suspected Korean native calves)

  • Two 7 months old Korean native beef calves, which were suspected as ergotisms were referred to the Veterinary Teaching Hospitals Chonnam National University in February 1997. Those showed severe lameness as a result of a dry gangrene of the lower part of both hindlimbs which unilaterally were detached, tail necrosis and retarded growth. On the skin of the extremities of both hindlimbs were clearly appeared indented line, hypertropic granuloma and hemorrhage, from which were isolated gicrosp$\theta$rum gysiNㅁt. On the X-ray views of hindlimbs and the longitudinal sectioned metatarsal bones were not observed any changes of osteomyelitis except slightly increased osteophytes and soft tissue swelling. Clnical and anatomical observations were suggested that these calves were affected by the ergotism.

Computed tomographic features of Scottish Fold osteochondrodysplasia

  • An 18-month-old, Scottish Fold with severe lameness and pain in the fore- and hind-limbs was diagnosed as Scottish Fold osteochondrodysplasia based on radiography showing mis-shapened phalanges, bone formation around the tarsal and metatarsal joints, joint space narrowing, and ankylosis. Although computed tomography (CT) is not considered a primary method for examining Scottish Fold osteochondrodysplasia, CT was undertaken in this case to investigate the cause of pain and reluctant ambulation in the forelimbs. CT showed immaturely ossified bone proliferation in the carpal bones. CT-based multiplanar reconstruction can reveal the anatomic relationship between the bone lesion and the joint in various directions.

  • Malignant peripheral nerve sheath tumors (MPNSTs) usually arise in soft tissues; they are rarely found in the bone. This paper reports a case of MPNST in the foot and ankle joint involving the distal tibia, talus, calcaneus, navicular, medial intermediate, and lateral cuneiform, cuboid, and 2nd to 4th metatarsal bone. Palliative treatment was performed. The authors encountered a patient with intraosseous MPNST of the midfoot who presented with nonspecific clinical and radiologic findings. This case shows that a high index of suspicion and a histopathology examination, including immunohistochemistry, will be necessary for an accurate diagnosis.

Separated muscle belly of the flexor digitorum brevis for the fifth toe: a case report

  • This case report describes a variation of the flexor digitorum brevis (FDB) with a separated muscle belly and tendon at the fifth toe. The narrow tendon and muscle belly for the fifth toe arose from the intermuscular septum between the FDB and abductor digiti minimi adjacent to the arising fibers of the FDB, separating from its other fibers. The tendon and muscle belly for the fifth toe became wider at the base of the metatarsal bones and narrower as it coursed toward the toes in a fusiform shape. The tendon and muscle belly for the fifth toe became thin at the midfoot and coursed just beneath the flexor digitorum longus tendon and entered the digital tendinous sheath. FDB variations including that described herein should be considered when performing various surgical procedures and evaluating the biomechanics of the foot.

유아 발 전체의 뼈에 대한 합성 전후축방향 엑스선촬영법 연구 (A Composite DorsoPlantar Axial X-ray Projection Study on the Bones of the Whole Infant's Foot)

  • 본 연구는 유아 발의 합성 전후축방향 엑스선 촬영에서 발전체가 다리와 겹침 없이 균일한 농도와 선예한 영상을 평가하기 위하여 Receiver Operating Characteristic 와 Signal to Noise Ratio 분석 결과 Cronbach Alpha 값이 0.813로 유의하게 높았으며 신호대잡음비(SNR)의 연구에서는 발허리뼈에서 발꿈치뼈를 향하여 엑스선관을 10° 기울여 검사 시 5.948 높게 나타났으며, 발꿈치뼈에서 발허리뼈를 향하여 엑스선관을 20° 기울여 검사 시 6.957으로 가장 높았다. 또한, 주관적평가에서는 발허리뼈에서 발꿈치뼈로 10° 기울려 검사시 18점으로 높은 점수를 얻었으며, 발꿈치뼈에서 발허리쪽뼈을 향하여 엑스선관을 20° 기울여 검사 시 18 점을 얻었다. 본 연구의 결과 유아 발 합성 전후축방향 검사 시 엑스선관을 발허리뼈 뒤쪽으로 10° 기울였을 경우와 발꿈치뼈에서 전방으로 20° 기울였을 때 균일한 농도와 선예한 영상을 얻을 수 있다.

Pathological fracture induced by Halicephalobus gingivalis (Nematoda: Rhabditida) in a horse limb

  • Halicephalobus gingivalis is a free-living nematode that occasionally causes infections in horses. We report a rare case of limb fracture of horse caused by infection with H. gingivalis. An 8-year-old mare was referred to the Veterinary Hospital of the Federal University of Lavras with claudication grade 5 of the right hind limb, that had been started 3 months ago. The patient had aseptic arthritis in the tarsal joint and edema that extended to the quartile. The radiographic examination showed punctate osteolysis with exacerbation of bone trabeculation along the calcaneus, talus, proximal epiphysis of the third metatarsal and distal epiphysis of the tibia. Treatment for arthritis was initiated, and the animal showed a slight improvement in limb function. However, 21 days after hospitalization, due to a comminuted fracture of the tibia, it was euthanized. At necropsy, yellowish masses were found from the metatarsal to the tibia, and around the tarsal bones and joint. Similar masses were also found in the left kidney. Numerous nematodes compatible with H. gingivalis were identified. This is the first description of a pathological fracture caused by H. gingivalis infection in an equine limb.
