Pulmonary Epithelioid Hemangioendothelioma Association with Subcutaneous Metastasis -Surgical experience of one case- (피하전이를 일으킨 폐 유상피 혈관내피종)

  • Epithelioid hemangioendothelioma (HE) is a very rare malignant tumor that is pathologically benign tumor originating from endothelial cell but clinically presents metastasis and recurrence. A 29-year-old asymptomatic man, preoperatively diagnosed as lung cancer in the left lower lung, underwent a lobectomy, a wedge lung resection of left upper lung, and partial resection of diaphragm. Left lower lobar lesion was confirmed as pulmonary epithelioid hemangioendothelioma, but the lesions of the left upper lung and diaphragm were remained calcified by spontaneous regression of HE. We report a case of subcutaneous metastasis that occurred two times at 10 months and 19 months after previous surgical treatment of pulmonary EH.

Metastatic Malignant Fibrous Histiocytoma in Lung -One Case Report- (폐로의 전이성 악성 섬유성 조직구종 -1예 보고-)

  • At OPD follow-up in December 1998, chest x-ray of a 42-year-old female showed a solit ary pulmonary nodule at a superior basal segment in the right lower lobe. After percut aneous transthoracic needle aspiration failure, wedge resection of the superior basal segment of lower lobe in right lung was performed for diagnosis and therapy. Three years ago, she had received surgery to remove a mass in the left buttock. The mass was pathologically diagnosed as malignant fibrous histiocytoma. She subsequently received 4500 rad radiotherapy for 35 days. Pathology confirmed metastatic malignant fibrous histiocytoma of the lung.

THE HYPERMETHYLATION OF E-CADHERIN GENE IN ORAL SQUAMOUS CELL CARCINOMA (구강 편평세포암에서 E-cadherin 유전자의 과메틸화)

  • Loss of E-cadherin (E-cad) expression has been found in multiple cancers and is postulated to facilitate tumor cell dissociation and metastais. Promotor methylation may provides an alternative pathway for loss of gene function. This study evaluated the role of hypermethylation in the down-regulation of E-cad in oral squamous cell carcinoma (OSCC). We examined the E-cad expression by immunohistochemical staining and detected methylation status by methylation-specific polymerase chain reaction (MSP) in 20 OSCC tissues. Overally, 12 (60%) cases of hypermethylation of E-cad were detected and we found there were no correlation between methylation and age, histologic grade, lympn node metastasis, tumor size and clinical stage. However, Eleven (73.3%) of 15 samples which was negative for E-cad staining showed hypermethylation of E-cad promotor region. On the other hand, only one (20%) of 5 E-cad positive sample was observed with methylated status. The underexpression of E-cad was found to be related to promotor hypermethylation (p=0.035). In conclusion, we suggest that hypermethylation play a role in inactivation of E-cad gene and may be a appreciable biomarker for diagnosis and treatment of OSCC.

Primary Malignant Melanoma of Lung -A case report- (원발성 폐 악성 흑색종 -1예 보고-)

  • Primary malignant melanoma of the lung is extremely rare. A 46-year-old lady was admitted with two month history of dry cough and blood-tinged sputum. Chest CT showed 4.5 $\times$ 5.0 cm sized mass at the right lower lobe. Repeated bronchoscopic and percutaneous biopsies showed no definite diagnosis. Preoperative evaluations revealed no systemic metastais. So, we tried the surgical approach. Right lower lobectomy and lymph node dissection was done. The mass and lymph node were confirmed as primary malignant melanoma. The patient presented with right hemiparesis 40 days after operation. Brain MRI showed 1.5$\times$2.0 cm sized mass lesion on the left parietal lobe. Mass excision was done. However, she expired 8 months later.

The Effect of Transarterial Chemoembolization(TAE) on Lung Metastasis of Hepatocellular Carcinoma (간동맥 색전술이 간세포함의 폐 전이에 미치는 영향)

  • Background: During the follow up period after transarterial embolization(TAE), cases of pulmonary metastasis were more prevalent in TAE-treated patients than in TAE-untreated patients. Therefore, a study was conducted to evaluate whether TAE increases the incidence of pulmonary metastasis of hepatocellular carcinoma and to clarify the risk factors for pulmonary metastasis. Methods: From March 1991 to March 1995, 156 patients who had been diagnosed with hepatocellular carcinoma by serology, and radiographic and histologic methods at Yeungnam University hospital were involved in this study. We excluded 12 patients with lung metastasis on initial diagnosis and the others. The remaining 144 patients had been followed up for at least 5 months and, divided into four groups according to lung metastasis and trans-arterial embolization treatment, and evaluated for age, sex, child-pugh score, liver cirrhosis, and level of AFP. Results: Pulmonary metastasis was found in 18.0% (26/144), of which 92.3%(24/26) and 7.7%(2/26) of the patients with and without transarterial chemoembolization, respectively. Of the patients, 23.5% (24/102) with TAE had lung metastais during follow-up periods and 4.7% (2/42) without TAE had lung metastasis. There was more likelihood for lung metastasis after TAE. but there was no difference between two groups in age, sex, child-pugh class, the presence of liver cirrhosis, and AFP. Conclusions: The incidence of pulmonary metastasis of hepatocellular carcinoma after TAE was higher. Child-pugh class was the only related clinical preciptating factors for pulmonary metastasis in TAE in our study. Our results suggest that regular chest X-ray check-ups may be more frequently needed by patients who had TAE treatment for hepatocellular carcinoma.
