Clonorchiasis and metagonimiasis in the inhabitants along Talchongang (River), Chungwon-gun (중원군 달천강 유역 주민의 간흡충 및 메타고니무스 감염상)

  • To evaluate the status of clonorchiasis and metagonimiasis of the inhabitants near Talchongang (River) in Chungwon-gun, Chungchongbuk-do, the stools of 67 inhabitants were examined by formalin-ether sedimentation method from August to September, 1993. Also freshwater knish caught in Talchongang were examined by slide compression method. The e99 positive cases of Clonorchis sinenis and Metasonimus sp. were 22 (32.8%), and 14 (20.9%), respectively. Of 17 species of examined fish, 14 species were infected with C. sinensis and 13 species with Metagonimus sp. The adult worm collected from 2 patients after treatment with praziquantel was Metosoninur Miyata type. Also the adult worm obtained from the experimental mice infected with metacercariae from Zacco plntwpus was Metafonimus Miyata type. We found the highly endemic area of clonorchiasis and metagonimiasis along Talchongang.

Epidemiological study of clonorchiasis and metagonimiasis along the Geum-gang (River) in Okcheon-gun (County), Korea

  • The endemic status of clonorchiasis and metagonimiasis along the Geum gang (River) in Okcheon-gun (County) in Korea was examined. From February to December 7000, stools of total 1,081 inhabitants living in 5 villages were examined. Each stool specimen was examined by both the cellophane thick smear method and the formalin-ether sedimentation technique. Egg-positive cases were further analyzed by Stoll's egg-counting technique. and praziquantel was administered to positive cases. The egg-positive rates for Clonorchis sinensis and Metafonimus species were 9.3% and 5.5%, respectively, and the double infection rate was 3.5%. The numbers of eggs per gram (EPG) of feces of C. sinensis and Metafoninus sp. were $918{\pm}1,463$ and $711{\pm}947$, respectively. The egg-positive rates for C. sinensis and Metagoninus sp. in the riverside area were 14.2% and 8.4%, respectively, which were significantly higher than those of the inland area (3.2% and 1.7%. respectively). The egg-positive rates of C. sinensis and Metagoninus sp. in males (16.7% and 10.0%) were significantly higher than those of females (3.5% and 1.8%) However, there were no significant differences of EPG values between localities and sexes. The prevalence of clonorchiasis and metagonimiasis in this survey was significantly lower than that in the previous reports. However. there is still a high prevalence of infection with C. sinensis and Metagoninus sp. in this region. especially in the riverside area.

An Evaluation on the Prevalence and Reinfection after Medication of Patients with Clonorchis sinensis in an Endemic Locality (간흡충 만연 일 지역에서 투약 후 유병률 및 재감염율 조사)

  • A survey of an endemic locality at Songjung Ri, Ogok Myun, Goksung County, South Cholla Province for the prevalence of Clonorchis sinensis and Metagonimus yokogawai was primarily done on 53 residence before medication using formalin-ether concentration method of stool examination. After praziquantel medication of all the clonorchiasis patients, 64 humans consisting 82.1% of total inhibitants and including 84.9% of primarily surveyed residents were secondly surveyed after 2-year post-treatment to evaluate the prevalence and reinfection. The prevalence of clonorchiasis was decreased from 35.8% before medication to 10.9% after medication(P<0.01), indicating this disease was not satisfactorily controlled although the rate was significantly lowered. The prevalence of metagonimiasis was dropped from 17.0% to 6.3% on post-treatment. In clonorchiasis cases, sex ratio showed no difference in both pre- and post-treatment, and mean age was the sixties after medication from the forties and fifties before medication, suggesting more aged people of both sexes were infected with this fluke. The reinfection of clonorchiasis and metagonimiasis following 2-year post-treatment was 15.8% and 25.0%, respectively. The proportion of reinfection among the egg positive cases was 50.0% in clonorchiasis and 100% in metagonimiasis. These findings mean that a high fraction of clonorchiasis and metagonimiasis was reinfected with these trematodes. In conclusion, to eradicate the C. sinensis and M. yokogawai in an endemic area, it should be preceded to control the reinfection of these parasites.

  • Antibody test is sometimes necessary for the diagnosis of acute human metagonimiasis because eggs may not be detected in stool. The antibody test (ELISA) was evaluated for its significance by reacting human sera from clinically diagnosed metagonimiasis, fascioliasis, clonorchiasis and paragonimlasis with 4 crude extracts of Metosonimn vokognwai (metacercariael , adults of Fosciola hepatica Cronorchis sinenis and Paragonimus westermoni. By ELISA, 10 of 11 metagonimlasis sera showed the highest absorbance (abs.) to the homologous antigen. Cross reactions to M. yokogawai antigen occurred most frequently in clonorchiasis sera. The antigenic protein fractions in M. vokogawai metacercarial extract were observed by SDS-PAGElimmunoblot using patients and control sera together with experimental cat sera. Out of 14 protein bands In the extract, 11 bands were reacting. Cross reacting bands to other trematodiasis sera were frequently observed. Of the reacting bands, 66 and 22 kDa proteuls were recognized as specific for metagonimiasis.

  • In order to observe the feasibility of serologic diagnosis of metagonimiasis, saline extracts of metacercariae and 4-week old adults were prepared. Sera from 25 experimentally infected cats were collected from 3 days to 12 weeks after infection. Their levels of specific IgG antibody were measured by ELISA together with 3 sera from non-infected cats. Specific IgG antibody levels began to rise in 7 days after infection, reached their peak in 2-4 weeks and made a plateau thereafter. Cats infected with hundreds of adult worms showed minimal rise of the antibody level. Adult antigen was superior to metacercarial antigen in detecting the specific IgG antibody.

  • Three species of the families Viviparidae and Pleuroceridae, the first intermediate host of paragonimiasis, metagonimiasis and echinostomiasis were studied cytologically. The observed diploid chromosome number was as follows: Semisulcospira libertina 36, S. dolichostoma 34, and Viviparus rivularis 64. The mitotic chromosome complement of S. libertina has nine metacentric pairs and nine submetacentric pairs, and S. dolichostoma has three metacentric pairs and 14 submetacentric pairs of chromosomes. Viviparus rivularis showed two metacentric pairs and 30 submetacentric pairs of chromosomes.

  • Semisulcospira libertina, a species of freshwater snail, is widespread in East Asia. It is important as a food source. Additionally, it is a vector of clonorchiasis, paragonimiasis, metagonimiasis, and other parasites. Although S. libertina has ecological, commercial, and clinical importance, its whole-genome has not been reported yet. Here, we revealed the genome of S. libertina through de novo assembly. We assembled the whole-genome of S. libertina and determined its transcriptome for the first time using Illumina NovaSeq 6000 platform. According to the k-mer analysis, the genome size of S. libertina was estimated to be 3.04 Gb. Using RepeatMasker, a total of 53.68% of repeats were identified in the genome assembly. Genome data of S. libertina reported in this study will be useful for identification and conservation of S. libertina in East Asia.

A Clonorchis sinensis-specific antigen that detects active human clonorchiasis (간흡충 현증감염 특이항원)

  • A Czonorchis sinensis-specific antigen in excretory-secretory product of C. sinensis (CsE) was assessed in human clonorchiasis by immunoblot. Thirty and 7 kDa antigens of CsE2, one of four different batches of CsEs reacted strongly with infection sera from clonorchiasis patients; however, the antigens reacted weakly with 6-month post- treatment sera from praziquantel-cured cases, but were still highly detected by the sera from praziquantel∼failed patients, indicating that the 30 and 7 kDa antigens can detect antibodies during an active infection. The 30 kDa antigen showed some cross reactions with sera from patients with Pcragonimus westemani and Metcfonimw vokogcujci, while the 7 kDa antigen did not, suggesting that the 7 kDa antigen has high specificity. The 30 kDa antigen reacted with some past clonorchiasis sera, whereas the 7 kDa antigen did not, supporting that antibodies to the 7 kDa antigen are not present in sera from past clonorchiasis patients. In an endemic area, 92% (23/25) of active clonorchiasis patients and 91% (10/11) of mixed infection patients with C. sinensis and M. Wokosawai had IgG antibodies to the 7 kDa antigen, while 40% (6/15) of past clonorchiasis individuals and 43% (3/7) of metagonimiasis patients cross-reacted to the antigen. These data suggest that the 7 kDa antigen in an excretory-secretory antigen may serve as a marker of an active clonorchiasis with reliable specificities in past clonorchiasis, paragonimiasis and metagonimiasis.

Infection status of sweetfish from Kwangjune-stream and Namdae-stream in Yangyang-gun, Kangwon-do with the metaeereariae of Metagonimus yokogawai (강원도 양양군 광정친 및 남대친산 은어의 요꼬가와흡충 피낭유충 감염상)

  • This study was performed to observe the infection status of sweetfish, caught from Kwangjung- stream and Namdae-stream in Yangyang-gun, Kangwon-do in August 1989, with the metacercariae of Metagonimus yokogawai. All of 28 sweetish from Kwangjung-stream were infected with the metacercariae of M. yokogawai. The number of metacercariae in a fish ranked 89-521(224 on average), and the number of larvae/g of mesh was 12-55(22 on average). On the other hand, no metacercariae were detected in the fish from Namdae-stream. It has been confirmed that the Kwangjung-stream in Yangyang-gun, Kangwon·do is one of the endemic foci of metagonimiasis in Korea.

  • Four cases of human infection by Echinochasmus japonicus (Trematoda; Echinostomatidae) were proven by the recovery of adult worms after a treatment with praziquantel (10mg/kg in single dose) and purgation in February 1984 in Korea. The patients were 3 middle-aged men and 1 elderly woman residing in a riverside village of Kangjin-gun (Tamjin river), Jeonranam-do, an ever-known endemic area of metagonimiasis. The treatment revealed numerous M. yokogawai worms together with 1-43 (total 54) specimens of E. japonicus and/or a few number of other kinds of trematodes or cestodes. The patients had gastrointestinal troubles such as indigestion and abdominal discomfort, probably due to these fluke infections. They have been eating some raw flesh of fresh water fishes such as cyprinoid ones or sweet fish caught from the river. This is the first record of human E. japonicus infection in the literature.
